The purpose of this study is to evaluate the influence of sex, residence area, age group, school enrollment, poverty status, and income quintiles variables on the prevalence of stunting among Yemeni children and adolescents. The investigation was done on all children and adolescents (3004) aged 5-19 years and included in the last Yemeni Household Budget Survey (YHBS) data of 2005/2006. The data included a classification of the poverty status of surveyed households. The cutoff of -2 z scores of the height-for-age reference suggested by NCHS was used to calculate the prevalence rate of stunting. Descriptive, categorical testing, and logit modelling statistical analysis tools were used in the investigation. The statistical analysis shows the overall prevalence rate of stunting as 49.5% and the prevalence of stunting among males is higher than females. The prevalence of stunting among rural children and adolescents is higher than the urban children and adolescents, and it is higher among children and adolescents who were not enrolled than those enrolled. Children and adolescents of poor households were suffering from stunting (52.8%) as compared to children and adolescents of nonpoor households (47.7%). Children and adolescents living with the poorest, second, and middle-income households were 1.76, 1.73, and 1.46 times more likely to be stunted, respectively. The research provides an evidence that the childhood health situation in Yemen is chaotic and needs careful and effective cooperation and efforts both nationally and internationally to divert the foreseen danger looming.

Abstract Background Shortages and inequitable distribution of physicians is an obstacle to move towards Universal Health Coverage, especially in low-income and middle-income countries. In Brazil, expansion of medical school enrollment, curricula changes and recruitment programs were established to increase the number of physicians in underserved areas. This study seeks to analyze the impact of these measures in reduce inequities in access to medical education and physicians’ distribution. Methods This is an observational study that analyzes changes in the number of undergraduate medical places and number of physicians per inhabitants in different areas in Brazil between the years 2010 and 2018. Data regarding the number of undergraduate medical places, number and the practice location of physicians were obtained in public databases. Municipalities with less than 20,000 inhabitants were considered underserved areas. Data regarding access to antenatal visits were analyzed as a proxy for impact in access to healthcare. Results From 2010 to 2018, 19,519 new medical undergraduate places were created which represents an increase of 120.2%. The increase in the number of physicians engaged in the workforce throughout the period was 113,702 physicians, 74,771 of these physicians in the Unified Health System. The greatest increase in the physicians per 1000 inhabitants ratio in the municipalities with the smallest population, the lowest Gross Domestic Product per capita and in those located in the states with the lowest concentration of physicians occurred in the 2013–2015 period. Increase in physician supply improved access to antenatal care. Conclusions There was an expansion in the number of undergraduate medical places and medical workforce in all groups of municipalities assessed in Brazil. Medical undergraduate places expansion in the federal public schools was more efficient to reduce regional inequities in access to medical education than private sector expansion. The recruitment component of More Doctors for Brazil Program demonstrated effectiveness to increase the number of physicians in underserved areas. Our results indicate the importance of public policies to face inequities in access to medical education and physician shortages and the necessity of continuous assessment during the period of implementation, especially in the context of political and economic changes.

Kata Kunci: buruh tani, jumlah kepemilikan domba, kemiskinanABSTRACTThis study aims to determine the possibility of sheep farming as a livelihood activity for poverty alleviation and to determine the amount of sheep ownership that can bring farm laborers households out of poverty. Survey research has been carried out in Walangsari Village, Kalapanunggal District, Sukabumi Regency, involving 65 poor farmer households and 22 non-poor households. Factors affecting poverty were analyzed using a binary logistic regression model, while the number of sheep ownership needed to escape poverty was determined by a simple regression model. The results showed that the age of the head of the family, and experience of sheep farmers did not affect poverty, while the number of sheep ownership, number of household members, involvement in institutions, and sources of income from non-agriculture affected the poverty status of farm laborers' households. In the average number of household members as many as 4.45 people, to get out of poverty must maintain a minimum of 36.63 sheep per household. This research explains that sheep farming can be used as a means to reduce the poverty of farm laborers, and poverty alleviation programs will be effective if they involve local institutions. Keywords: farm labor, number of sheep ownership, poverty

Objectives: The carotid intima-media thickness (CIMT) is a reliable screening method for vascular alterations even in a pediatric cohort; however, reference values of CIMT established recently by LMS methods for childhood and adolescence are limited when comparing patients after Kawasaki disease (KD) and controls. We tested the hypothesis that there are significant differences between the values of CIMT expressed as absolute values and z-scores in children and adolescents after KD and controls. Methods: We reviewed 12 published articles regarding CIMT on patients after KD and controls. Absolute values (Ab) of the mean±1 SD of CIMT in patients after KD and controls were transformed to z-scores (Zs) using age-specific reference values established by Jourdan et al. (J: 247 Caucasian subjects aged 10-20 years) and our own data (O: 175 Asian subjects aged 6-20 years), and the results were compared between the two references. In this study, the mean age of the study population derived from each article was designated the representative age for transformation. Results: In either reference (J) or (O), there was no significant sex difference in CIMT at any given age. The mean CIMT of (Ab) and (Zs) transformed by (J) or (O) were significantly different between patients after KD and controls, at 41.6% (Ab), 66.6% (Zs) by (J), and 83.3% (Zs) by (O) among 12 articles, respectively. Therefore, patients after KD had significantly higher (Zs) by (O) than those of controls (0.66±0.71 vs. 0.03±0.68, p=0.006, respectively). Compared with reference values, the controls of (O) were within the normal range. However, there were no significant differences in (Zs) by (J) between the two groups (1.72±0.77 vs. 1.23±0.83, p=0.116, respectively). When we assessed 9 articles dealing with Asian subjects, the difference of (Zs) between the two groups remained significant only by (O) (p=0.015). In contrast, when we assessed 3 articles dealing with mainly Caucasian subjects, there was no significant difference in (Zs) between the two groups with both (J) and (O). Conclusions: These results indicate that age and race-specific reference values for CIMT are mandatory for performing an accurate assessment of the vascular status in healthy children and adolescents and particularly in those after KD.
