Objectives. Oral and maxillofacial surgery is a relatively newer and growing specialty of dentistry in Nepal whose scope is not yet estimated. The objective of this study was to estimate the scope and the factors influencing the scope of oral and maxillofacial surgery in Nepal. Study Design. In this cross-sectional study, all the oral and maxillofacial surgeons who were registered in the Nepalese Association of Oral and Maxillofacial Surgeons (NAOMS) were included (purposive sampling). The structured questionnaires were distributed to them, the collected data were entered in Microsoft excel 2010, and variables were analyzed using descriptive statistics (percentage) by SPSS 16.0. Results. Out of 46 questionnaires distributed, 35 were responded and returned (response rate = 76%). Majority of participants (77.1%) were in 30–39 years of age group. Male to female ratio was 4 : 1. More than half (68.6%) of the participants had practice experience of <5 years, and none had practice experience of >20 years. The ratio of the surgeons practiced in Medical/Dental Teaching Hospital to those in Government Hospital was 3 : 1. Sixteen (45.7%) participants practiced in Capital Valley and none in Far-Western Development Region. Traumatology was practiced by thirty-four (97.1%) participants although only twenty-three (65.7%) participants had primary interest in it. Each of oncology, orthognathic surgery, implantology, and cleft lip/palate surgery was performed by <8% of the participants. Common factors influencing the practice were inadequate training (71.4%) and insufficient facilities/infrastructures (45.7%). Conclusion. The scope of oral and maxillofacial surgery is limited in Nepal, and oncology, implantology, cleft lip/palate, and orthognathic surgery have received little attention.

Background: This paper reviews the types, prevalence and demographic distribution of maxillofacial tumors, cysts and tumor-like lesions in a Bangladeshiteaching Hospital.An observational descriptive study (January’ 2014 to December’ 2016) was performed at Oral and Maxillofacial Surgery depart- ment, Dental Unit,TMSS Medical College and Hospital. This study presents 55 cases of maxillofacial tumour & tumour like lesions collected over 3 years at a tertiary oral care centre in Bogra, Bangla- desh.Objectives:To find out the distribution & pattern of maxillofacial tumours and to find out the age, sex, site, clinical presentation and thehistological types of these tumors Method: A cross sectional descriptive study where the Histopathological types of the maxillofacial tumours were analyzed to indicate the numbers that occurred and also the pattern of occurrence according to age, gender, site and clinical presentation.Result: There were 55 maxillofacial tumour & tumour like lesions of which 44 (80.01%) were Benign tumours & 11 (19.81%) were malignant tumours. Among Eighteen histo- pathologic types were found of which Squamous cell carcinoma (18.18%), Ameloblastoma (16.36%), Odontogenic keratocystic tumour (10.90%), Dentigerous cyst (7.27%)& Radicular cyst (7.27%) were predominant. The male to female ratio was 1.29:1. Patients were between 10 years and 72 years with most patients (27.27%) in 51 to 60 years of life. Case presented with symptoms such as swelling (41.96%), pain (28.82%) and loosetooth (17.12%).Conclusion: Both malignant and benign tumours are seen. In the present study, SCC and ameloblastoma were the commonest malignant and benign odontogenic tumours seen respectively; the two representing more than 34.54% of all tumors.Tumour & tumour like lesions of the oral & maxillofacial region with late presentation still remains the main challenging factor in the early detection & management. Surgery was the main modality used for treat- ment while some patients had no treatment due to self discharge and late presentation.Update Dent. Coll. j: 2018; 8 (1): 22-28

Objectives: The basic aim of this study was to see the trend of usage of smart devices among dental undergraduates and their attitude towards e-learning. Study Design: Observational cross-sectional study. Setting: Oral and maxillofacial surgery department, PMC Dental Institute Faisalabad Medical University, Faisalabad. Materials and Methods: A questionnaire was developed by a team of dental experts and a literature review keeping in mind the trend of internet-connected devices and data was collected on specially designed Performa and submit it to the principal investigator. Results: The data were collected from 184 participants with a male to female ratio of 1:2.4 and the mean age was 22.5 years. From overall participants 99.5% have internet access, 97.3% use smart devices for education purposes and only 41.8% were comfortable reading e-book. The participants who find mobile phone beneficial are 92% and 28% have daily usage of more than 6-hours. When faced with a challenging scenario 23% said that they web resources and occasionally consult health-related videos. Conclusion: Within the limits of this study we concluded that students find usage of smart devices and the internet beneficial for study purposes and therefore, the study methods should consist of both books and media.

Abstract Background Oral and maxillofacial surgery specialty has grown rapidly in Kuwait in recent years. However, the general public and healthcare professionals remain unaware of its expanding scope of practice. The aim of the study is to assess public and professional (dental and medical) perception of the oral and maxillofacial surgical specialty in Kuwait. Methods This is a cross-sectional study evaluating responses of dental professionals, medical professionals, and general public in Kuwait toward the oral and maxillofacial surgical specialty using a previously validated survey instrument with 100 participants in each group. Participants were asked to choose the most appropriate specialist to treat certain procedures across 4 disciplines: reconstruction, trauma, pathology, and cosmetic. Statistical comparison was conducted between dentists and medical doctors using Fisher’s exact test with a p-value of < 0.05. Results Disparities were noted each group’s responses. Oral and maxillofacial surgery was preferred overall for most clinical scenarios in trauma (p < 0.001), pathology (p < 0.001), and reconstructive surgery (p < 0.001). Plastic surgery was preferred for cosmetic surgeries (p < 0.001). Conclusions This study indicates the need to increase awareness especially towards cosmetic surgery procedures, and conduct health campaigns regarding oral and maxillofacial surgery among healthcare professionals, especially medical doctors, and the general public.

Objective: To evaluate the prevalence of dental caries in patients with cleft lip and/or palate and their cleft-free sibling controls. Methods: The two subject groups (patient and control) comprised 106 participants. The former group consisted of 53 patients with cleft lip and/or palate, aged 12 to 29 years, who visited the Oral and Maxillofacial Surgery Hospital at Damascus University of Syria. The control group consisted of the patients’ siblings who had no clefts, and they were sex matched to the patient group. Dental caries were examined clinically and were reported using the decayed, missing, and filled permanent teeth (DMFT) index. The DMFT scores were compared between the two groups. Results: The author found an overall association of dental caries with the presence of cleft lip and/or palate (odds ratio  =  2.52; 95% confidence interval  =  1.389–4.574; p < .05). The DMFT index scores were proportionally higher in patients with cleft lip and/or palate compared with the control group (p < .001). Conclusion: Subjects with cleft lip and palate are susceptible to dental caries independently of socioeconomic status.

ABSTRACT Aim The aim of this paper was to report a case of orthognathic surgery successfully done in a patient with multiple sclerosis. Background Multiple sclerosis (MS) is a chronic, progressive inflammatory disorder of the central nervous system affecting young adults, characterized by lymphocytic infiltration of the brain and spinal cord leading to demyelination and focal axonal damage. Clinically, MS patients present with reversible neurological dysfunction in the early stages, which progresses to irreversible neurological disability and deficit. Oral manifestations of MS include facial numbness or pain, neuralgias, facial paralysis, dysarthria and dysphagia. While dental treatment is not contraindicated in MS patients, it is, however, limited to preventive and supportive dental care. Case description A 23-year-old Saudi male patient with a diagnosis of MS since 2008 reported to the oral and maxillofacial surgery (OMFS) department for correction of dentofacial deformity. The patient was under follow-up with the neurology department and was being treated with interferon beta-1a. Following consent from the neurologist and the patient, a Lefort 1 segmental osteotomy was done under general anesthesia. The patient was stable throughout the surgical procedure and during the postoperative period. The patient was discharged upon complete surgical recovery and no acute exacerbations of MS were reported during the perioperative period. Conclusion Based on our observations, orthognathic and maxillofacial surgical procedures can be safely carried out in patients with MS, provided a strict perioperative prophylactic regimen for stress reduction and prevention of acute attacks of MS is adhered to. Clinical significance Due to the stressful nature of dental treatment and oral and maxillofacial surgical procedures, acute exacerbations of MS are very much likely. Hence, it is imperative that dental and oral surgical practitioners are aware of the manifestations of MS and are able to manage such patients with suitable treatment modifications. Abstract Background: To evaluate the self-confidence of undergraduate dental students in relation to oral and maxillofacial surgery (OMFS) in order to assess the teaching curriculum at Kuwait University using a validated questionnaire originally developed by the Association of British Academic Oral Maxillofacial Surgeons (ABAOMS).Methods: A cross-sectional survey of 6th year (n=20) and 7th- year (n=19) dentistry students was conducted Kuwait University Faculty of Dentistry between the 1st and 15th May 2020. The ABAOMS questionnaire is composed of 17 questions assessing various areas of the undergraduate OMFS curriculum. The response options to the questionnaire utilised a Likert scale. Independent sample t-tests were performed to assess the difference in responses between the two year groups. Spearman’s Rho correlations were calculated to measure the strength of association between confidence in all aspects of surgical and forceps exodontia.Results: A total of 39 questionnaires were completed by the students. The majority of students expressed that they feel confident that they have enough knowledge to undertake independent practice (61%). General aspects of the questionnaire were answered favourably except for surgical extraction of teeth, in which both classes reported a lower level of self-confidence.Conclusions: This survey revealed the students’ self-confidence in undertaking independent practice and preforming basic oral surgery procedures. Students felt comfortable with exodontia using forceps and elevators, root removal, managing acute pericoronitis, managing haemorrhage from a socket, assessing impacted teeth, and recognising the clinical features of potentially malignant and malignant lesions of the oral cavity. They reported a lower level of confidence for performing surgical procedures. The ABAOMS survey instrument is a useful and thorough tool to assess the self-confidence of dental students according to their undergraduate oral and maxillofacial surgery teaching.

Objectives: The purpose of this study was to 3-dimensionally assess the airway characteristics of patients with unilateral cleft lip and palate (UCLP) and bilateral cleft lip and palate (BCLP) who underwent orthognathic surgery. Design: This was a retrospective study. Setting: The study took place at a national referral center for cleft lip and palate rehabilitation. Patients and Participants: The sample comprised cone-beam computed tomography (CBCT) scans obtained before and after orthognathic surgery of 15 individuals (30 CBCT scans), divided into 2 groups: UCLP group (n = 9 patients/18 CBCT scans) and BCLP group (n = 6/12 CBCT scans). All patients had a nonsyndromic UCLP or BCLP and a skeletal class III malocclusion at the preoperative period. Interventions: Airway volume, pharyngeal minimal cross-sectional area (mCSA), location of mCSA, sella-nasion-A point (SNA) and sella-nasion-B point (SNB) angles, and condylion-A point and condyloid-gnathion linear measurements were assessed in open-source software (ITK-SNAP and SlicerCMF). Main Outcome Measure: Airway dimensions of patients with UCLP and BCLP increase after orthognathic surgery. Results: After orthognathic surgery, UCLP group showed a significant 20% increase in nasopharynx volume. Although not significant, BCLP group also showed an increase of 18% in the same region. Minimal cross-sectional area remained dimensionally stable after surgery and was all located in the oropharynx region, on both groups. Additionally, a positive correlation was observed between volume and mCSA on both groups. Conclusion: Overall, individuals with UCLP and BCLP showed an increase in the upper airway after orthognathic surgery and this might explain the breathing and sleep improvements reported by the patients after the surgery.


Oral and maxillofacial surgery is largely a post-graduate subject. This chapter begins with a section on advanced trauma life support and goes on to include details of the diagnosis and management of facial trauma, orthognathic surgery, as well as head and neck surgery and reconstruction. Only basic aspects of this specialty have been included here as advanced techniques are beyond the scope of this chapter. However, this chapter provides enough information so as to ensure success for final year dental students but it can also be of use to those working in oral and maxillofacial units.
