Amelogenesis imperfecta is a group of conditions caused by over 15 different genes that affects the development of dental enamel and poses some challenges to dentists. An adult patient with amelogenesis imperfecta with severe changes in tooth color and reduction of occlusal vertical dimension sought dental treatment. Diagnostic wax-up was carried out to guide the stratification of a nanoparticulate resin for the restorative treatment. Direct composite resin restorations were applied on all teeth for modification of both esthetics and occlusion. After a 2-year follow-up, the findings appear to suggest that composite resin is a low-cost alternative when compared with indirect ceramic restorations, provides a good esthetic outcome, and offers considerable longevity for cases like the one reported herein.

ABSTRACT Hypodontia is the most prevalent craniofacial malformation in mankind. It may present a wide variety of manifestations and, depending on the number and location of missing teeth, it may affect the esthetics, mastication, speech and occlusal balance. This paper discusses the therapeutic approaches to solve this condition, describing a case report with hypodontia of one mandibular lateral incisor, which treatment option included space closure at the region of hypodontia associated with composite resin restorations in the mandibular central incisors. The three-year follow-up after treatment revealed occlusal stability, adequate intercuspation in Class I relationship and excellent micro and macroesthetics.

Background: Our aim was to compare salivary levels of secretory immunoglobulin A (s-IgA) in children with early childhood caries (ECCG) and those who are caries-free (CFG) and verify these levels in a follow-up period after restorative treatment. Materials and methods: We selected 46 systemically healthy children in the complete primary dentition period, who were allocated into two groups: CFG (n = 23) and ECCG (dmf-s > 0; n = 23). Unstimulated whole saliva was obtained at baseline from both groups and during the follow-up period (7 days, 1, 2 and 3 months) in the ECCG group. The s-IgA was measured using an ELISA assay, and total protein was assessed using the Bradford method. We also evaluated the flow rate (mL/min), Streptococcus mutans and Lactobacillus spp. counting using selective media plaques. The data were submitted to statistical analysis using the software SPSS 20.0 (SPSS Inc, IL, USA) with a confidence interval set at 95%. Results: Salivary s-IgA levels were higher in baseline of ECCG than in CFG (p<0.05). No statistically significant differences were observed between s-IgA salivary levels at baseline and the evaluations after dental treatment in ECCG (p>0.05). However, we observed two different changes in s-IgA levels among participants: one group presented s-IgA reduction, and the other group demonstrated its maintenance. It was shown that patients from the ECCG group who presented a reduction in s-IgA levels during follow-up also showed a decrease in Streptococcus mutans and Lactobacillus spp. count (p<0.05), in contrast to patients who did not present this reduction. The flow rate and total protein were similar between groups (p>0.05). Conclusions: The present data support the idea that children with early childhood caries present higher levels of s-IgA in saliva than caries-free children. The restorative dental treatment does not have a significant influence on salivary levels of this immunoglobulin during the follow-up period.

Inguinal hernia is a very common surgical problem for which mesh based technique particularly Lichtenstein repair is considered as standard. However, it is not free from some major and bothersome complications. Desarda technique of non-mesh hernia repair invented by the Indian surgeon is claimed as low cost tension free procedure with promising results. The objective of the study is to evaluate the feasibility of Desarda procedure in country like Bangladesh as a treatment of primary inguinal hernia in men by comparing with Lichtenstein repair in terms of various parameters. One hundred and sixty male patients between ages of 18-70 years with uncomplicated primary inguinal hernia were initially randomized to perform the one of the two procedures in every alternate patient. Outcome were measured and analyzed. A total of 100 patients were finally studied with a follow up of 2 years. Operative time and immediate postoperative pain were significantly less in Desarda arm. Cost and foreign body sensation were also in favour of Desarda group. There was no recurrence in either group. Desarda repair is easy to perform and takes less time. It is cost effective with a comparable clinical outcome to standard Lichtenstein repair at least in short term. Faridpur Med. Coll. J. Jan 2019;14(1): 20-26

INTRODUCTION: Orthodontic patients frequently present composite resin restorations, however there are few studies that evaluate the best way for orthodontic bonding in this situation. OBJECTIVE: The objective of this work was to evaluate the bond strength of orthodontic brackets in resin restorations with surface treatment. METHODS: Fifty one bovine lower incisors were randomly divided into three groups. On the control group (CG) the brackets were bonded to dental enamel; on experimental groups, brackets were bonded to resin restoration with diamond drill treatment (EGT) and with no treatment (EGN). The teeth were placed in PVC tubes with autopolymerized acrylic resin. The shear test was performed in EMIC universal testing machine. The groups were submitted to ANOVA analysis of variance with Tukey post test to verify the statistical difference between groups (α = 0.05). RESULTS: CG (6.62 MPa) and EGT (6.82 MPa) groups presented similar results, while EGN (5.07 MPa) obtained statistically lower results (p < 0.05). CONCLUSION: Therefore, it is concluded that the best technique for bonding of orthodontic brackets on composite resin restorations is the performance of surface detritions.

Case Report. An 8-year-old girl with amelogenesis imperfecta (AI) reported unsatisfactory aesthetics, difficulty in mastication, and dental hypersensitivity. The intraoral examination observed mixed dentition, malocclusion in anteroposterior relationships, anterior open bite, and dental asymmetry. A hypoplastic form of AI was diagnosed in the permanent dentition. A multidisciplinary planning was performed and divided into preventive, orthopedic, and rehabilitation stages. Initially, preventive treatment was implemented, with fluoride varnish applications, in order to protect the fragile enamel and reduce the dental sensitivity. In the second stage, the patient received an interceptive orthopedic treatment to improve cross-relationship of the arches during six months. Finally, the rehabilitation treatment was executed to establish the vertical dimension. In the posterior teeth, indirect composite resin crowns were performed with minimally invasive dental preparation. Direct composite resin restorations were used to improve the appearance of anterior teeth.Follow-Up. The follow-up was carried out after 3, 6, 12, and 18 months. After 18 months of follow-up, The restoration of integrity, oral hygiene, and patient satisfaction were observed .Conclusion. Successful reduction of the dental hypersensitivity and improvement of the aesthetic and functional aspects as well as quality of life were observed.

Enamel defects are described as changes in its structure characterized by local decrease or loss of its translucency from fluorosis, hypocalcifications, hypoplasias and inactive carious lesions with superficial and medium depth. The microabrasion of enamel selectively promotes the removal of its surface layer that presents color or structure modification through a mixture of abrasive and erosive agents mechanically. The association of these agents exposes a sub-layer of enamel with normal characteristics. This procedure can be used alone or associated with one or more aesthetic treatments, such as tooth whitening and composite resin restorations. Objective: to approach the microabrasion technique applied in dental enamel with changes in color and structure. Methodology: a non-systematic review of the scientific literature, published from 2009 to 2019, was carried out in the Virtual Health Library and in the academic Google. The inclusion criteria were articles in Portuguese and English, published in the last 10 years, that addressed the proposed theme. Conclusion: Dental microabrasion is a good alternative for aesthetic treatment for changes in the superficial structure of dental enamel, fluorosis and hypoplasia, because it wears little dental structure, restores aesthetics, does not cause damage to the pulp or periodontal tissues, requires little treatment time, does not require cavitary preparation, nor restorative material and is low cost. Aesthetic dissatisfaction is a concern of patients of all ages and the diagnosis of dental fluorosis should be the result of careful anamnesis and it should be taken into account that the etiology may be of various origins.

Background/Objective: To retrospectively analyze the characteristics, tendencies, and success rates of dental treatments for severe early childhood caries (SECC) under general anesthesia (GA) in in northwest China. Study design: Children diagnosed with SECC were included in this retrospective study. From January 2015 to December 2018, they received dental treatment under GA at Affiliated Stomatology Hospital of Xi’an Jiaotong University. Demographic information, caries status and treatment characteristics were collected from electronic medical record system. Success rates of different treatments at 6-month and 12-month follow up were also analyzed. Results: A total of 846 children (477 male, 369 female) received dental treatment under GA. The case number was increased from 148 in 2015 to 278 in 2018. There were 81.2% and 70.8% of the children participated the 6-month and 12-month follow up. SSC and pulpotomy was the most successful restorations and pulp therapy, with the success rate of 97.09% and 93.98% in 12-month follow up, respectively. The use of crown restorations (including composite resin crown and stainless steel crown) and pulp reservation therapies (including indirect pulp therapy and pulpotomy) were significantly increased while composite resin filling and pulpectomy decreased during 2015 to 2018. Conclusion: There has been an increasing demand for dental treatment under GA for children with SECC in northwest China, with a trend toward younger ages. With better clinical outcomes, crown restorations and pulp reservation therapies were the fastest-growing treatments under GA.

SUMMARY Oligodontia, or the congenital absence of teeth, can occur in isolation or as part of a syndrome. This study describes a case of isolated oligodontia associated with conical teeth and large diastemas in the anterior region. The patient was treated using direct composite resin restorations. Recent improvements in adhesive restorative materials allow practitioners to offer this low-cost, conservative esthetic treatment, which has the advantage of preserving sound dental structures, to patients with missing teeth.

SUMMARY Purpose To investigate clinical performance of a low-shrinkage silorane-based composite resin when used for repairing conventional dimethacrylate-based composite restorations. Background Despite the continued development of resin-based materials, polymerization shrinkage and shrinkage stress still require improvement. A silorane-based monomer system was recently made available for dental restorations. This report refers to the use of this material for making repairs and evaluates the clinical performance of this alternative treatment. Materials and Methods One operator repaired the defective dimethacrylate-based composite resin restorations that were randomly assigned to one of two treatment groups: control (n=50) repair with Adper SE Plus (3M/ESPE) and Filtek P60 Posterior Restorative (3M/ESPE), and test (n=50) repair with P90 System Adhesive Self-Etch Primer and Bond (3M/ESPE) and Filtek P90 Low Shrink Posterior Restorative (3M/ESPE). After one week, restorations were finished and polished. Two calibrated examiners (Kw≥0.78) evaluated all repaired restorations, blindly and independently, at baseline and one year. The parameters examined were marginal adaptation, anatomic form, surface roughness, marginal discoloration, postoperative sensitivity, and secondary caries. The restorations were classified as Alpha, Bravo, or Charlie, according to modified US Public Health Service criteria. Mann-Whitney and Wilcoxon tests were used to compare the groups. Results Of the 100 restorations repaired in this study, 93 were reexamined at baseline. Dropout from baseline to one-year recall was 11%. No statistically significant differences were found between the materials for all clinical criteria, at baseline or at one-year recall (p&gt;0.05). No statistically significant differences were registered (p&gt;0.05) for each material when compared for all clinical criteria at baseline and at one-year recall. Conclusions The hypothesis tested in this randomized controlled clinical trial was accepted. After the one-year evaluations, the silorane-based composite exhibited a similar performance compared with dimethacrylate-based composite when used to make repairs.
