There are some pathological conditions which, though of considerable interest to the student of morbid anatomy, appear to have very little importance from the point of view of clinical study. The reason for this, sometimes at least, lies in the fact that it is difficult, if not impossible, to diagnose the onset and follow the progress of the morbid process during the life of the patient. Pachymeningitis interna hemorrhagica is essentially a condition of this kind. It is a morbid state which has been named and described for years, and yet it is a condition the diagnosis of which during life is extremely difficult. As a rule, when it is demonstrated at the post mortem, the clinical records of the case furnish us with no definite information suggestive of its presence. Often when symptoms point to the likelihood of its presence no lesion is found, while it occurs in cases which during life showed nothing special to indicate the possibility of such a condition.

18 psychopharmacologically treated patients (7 schizophrenics, 5 schizoaffectives, 6 depressives) were studied using 99mTc-HMPAO-SPECT of the brain. The regional cerebral blood flow was measured in three transversal sections (infra-/supraventricular, ventricular) within 6 regions of interest (ROI) respectively (one frontal, one parietal and one occipital in each hemisphere). Corresponding ROIs of the same section in each hemisphere were compared. In the schizophrenics there was a significantly reduced perfusion in the left frontal region of the infraventricular and ventricular section (p < 0.02) compared with the data of the depressives. The schizoaffectives took an intermediate place. Since the patients were treated with psychopharmaca, the result must be interpreted cautiously. However, our findings seem to be in accordance with post-mortem-, CT- and PET-studies presented in the literature. Our results suggest that 99mTc-HMPAO-SPECT may be helpful in finding cerebral abnormalities in endogenous psychoses.

When seeking to uncover the brain correlates of language processing, timing and location are of the essence. Magnetoencephalography (MEG) offers them both, with the highest sensitivity to cortical activity. MEG has shown its worth in revealing cortical dynamics of reading, speech perception, and speech production in adults and children, in unimpaired language processing as well as developmental and acquired language disorders. The MEG signals, once recorded, provide an extensive selection of measures for examination of neural processing. Like all other neuroimaging tools, MEG has its own strengths and limitations of which the user should be aware in order to make the best possible use of this powerful method and to generate meaningful and reliable scientific data. This chapter reviews MEG methodology and how MEG has been used to study the cortical dynamics of language.

The purpose of this course is to introduce a new framework linking the humanities to public policy analysis as pursued in the government and the academy. Current efforts to link the particular contributions from the humanities to problems of public policy choice are often narrow either in terms of their perspective on the humanities or in terms of their selection of the possible means of influencing policy choice. Sometimes a single text from one of the humanities disciplines is selected to apply to a particular issue. At other times, arguments about the ethical dimensions of a single policy issue often are pursued with a single — or sometimes, no — point of access to the policy process in mind.

The following paper contains the record of an investigation into the degenerations which follow lesions of the gyrus marginalis and gyrus fornicatus in Monkeys. The work has been carried on under my direction by Mr. France, with the aid of a grant from the Government Grant Fund, and represents part of a long investigation into the degenerations which follow artificially produced cerebral lesions, the material for which has been furnished by cases operated upon in conjunction respectively with Professor V. Horsley and Dr. Sanger Brown. These cases and the physiological results of the operations have already been published in the ‘Philosophical Transactions.’ The experiments here dealt with, twelve in number, comprise only the lesions of the gyrus marginalis and gyrus fornicatus, and, with one exception (case 12), are taken from the series of experiments performed in conjunction with Mr. Horsley. Of the twelve cases, six were of removal, or attempted removal, of the gyrus marginalis, and six of removal, or attempted removal, of the gyrus fornicatus. But in only one or two instances was the lesion, as determined by post-mortem examination, exactly limited to the convolution which it was attempted to remove, for in most cases the adjacent gyrus was to a certain extent involved in the injury. This was especially the case when removal of the gyrus fornicatus had been attempted, on account of its deep situation, and the difficulty of getting at it without some manipulation of the superjacent gyrus. Nevertheless, the removal of one or the other gyrus was sufficiently complete in all the cases here selected to produce characteristic symptoms and characteristic descending degenerations.

Government fiscal positions in all the advanced economies suffered severe deteriorations during the financial crisis. Figure 1 illustrates the cumulative deterioration of the government budget ratio as a per cent of GDP between 2007 and 2009 in a selection of OECD economies. The sharpest declines materialised in Ireland, Spain and Finland, while public finances in Austria, Germany and Italy have held up better. Budget deficits have worsened in part because of the cyclical downturn, in part because of the policy response to the crisis, including both fiscal stimulus packages and certain fiscal costs related to government support of financial institutions, and in part because of a change in the relationship between revenue and production, which may prove longer-term.

Background Post-mortem studies demonstrate extensive grey matter demyelination in MS, both in the brain and in the spinal cord. However the clinical significance of these plaques is unclear, largely because they are grossly underestimated by MR imaging at conventional field strengths. Indeed post-mortem MR studies suggest the great majority of lesions in the cerebral cortex go undetected, even when performed at high field. Similar studies have not been performed using post-mortem spinal cord material. Aim To assess the sensitivity of high field post-mortem MRI for detecting grey matter lesions in the spinal cord in MS. Methods Autopsy material was obtained from 11 MS cases and 2 controls. Proton Density-weighted images of this formalin-fixed material were acquired at 4.7Tesla before the tissue was sectioned and stained for Myelin Basic Protein. Both the tissue sections and the MR images were scored for grey matter and white matter plaques, with the readers of the MR images being blinded to the histopathology results. Results Our results indicate that post-mortem imaging at 4.7Tesla is highly sensitive for cord lesions, detecting 87% of white matter lesions and 73% of grey matter lesions. The MR changes were highly specific for demyelination, with all lesions scored on MRI corresponding to areas of demyelination. Conclusion Our work suggests that spinal cord grey matter lesions may be detected on MRI more readily than GM lesions in the brain, making the cord a promising site to study the functional consequences of grey matter demyelination in MS.
