A lack of communication between social and health care staff has been recognised as one of the main obstacles in meeting the health needs of this group. The previous study of the introduction of a specialist nursing service in the west of Scotland showed that the rates of documentation and information-sharing between health and social care staff remain low but that a specialist nursing service could improve this. This article explores the reasons for the poor rates of information sharing and documentation by interviewing residential care workers (RCWs) and so may offer an explanation of why a specialist nursing service could help. The first theme to emerge was the conflict RCWs face between their corporate parent and health improvement roles. The need to protect the child from distress and the importance of maintaining a relationship with the child were suggested as important reasons for not promoting health interventions. Others included a lack of financial, training and staffing resources. The second theme was the difficulties involved in using the BAAF health record. There was seen to be a duplication of documentation between residential unit, health and other records and there were numerous examples of a new record being used when a child moves instead of the previous record being re-opened. Stigma experienced by the child using the record and a lack of understanding among health care professionals were also cited as reasons for the record going unused. The last theme concerned the barriers within the health service for children in residential care. This particularly referred to the inflexibility of the appointments systems in both primary and secondary care.

Abstract Background The coronavirus disease 2019 (COVID-19) may aggravate workplace conditions that impact health-care workers’ mental health. However, it can also place other stresses on workers outside of their work. This study determines the effect of COVID-19 on symptoms of negative and positive mental health and the workforce’s experience with various sources of support. Effect modification by demographic variables was also studied. Methods A cross-sectional survey study, conducted between 2 April and 4 May 2020 (two waves), led to a convenience sample of 4509 health-care workers in Flanders (Belgium), including paramedics (40.6%), nurses (33.4%), doctors (13.4%) and management staff (12.2%). About three in four were employed in university and acute hospitals (29.6%), primary care practices (25.7%), residential care centers (21.3%) or care sites for disabled and mental health care. In each of the two waves, participants were asked how frequently (on a scale of 0–10) they experienced positive and negative mental health symptoms during normal circumstances and during last week, referred to as before and during COVID-19, respectively. These symptoms were stress, hypervigilance, fatigue, difficulty sleeping, unable to relax, fear, irregular lifestyle, flashback, difficulty concentrating, feeling unhappy and dejected, failing to recognize their own emotional response, doubting knowledge and skills and feeling uncomfortable within the team. Associations between COVID-19 and mental health symptoms were estimated by cumulative logit models and reported as odds ratios. The needed support was our secondary outcome and was reported as the degree to which health-care workers relied on sources of support and how they experienced them. Results All symptoms were significantly more pronounced during versus before COVID-19. For hypervigilance, there was a 12-fold odds (odds ratio 12.24, 95% confidence interval 11.11–13.49) during versus before COVID-19. Positive professional symptoms such as the feeling that one can make a difference were less frequently experienced. The association between COVID-19 and mental health was generally strongest for the age group 30–49 years, females, nurses and residential care centers. Health-care workers reported to rely on support from relatives and peers. A considerable proportion, respectively, 18 and 27%, reported the need for professional guidance from psychologists and more support from their leadership. Conclusions The toll of the crisis has been heavy on health-care workers. Those who carry leadership positions at an organizational or system level should take this opportunity to develop targeted strategies to mitigate key stressors of health-care workers’ mental well-being.

Background Poor mental health and emotional well-being can negatively impact ability to engage in healthy lifestyle behavior change. Health care staff have higher rates of sickness and absence than other public sector staff, which has implications at both individual and societal levels. Individual efforts to self-manage health and well-being which add to the UK mental health prevention agenda need to be supported. Objective The objective of this study was to establish the feasibility and acceptability of the inclusion of a self-guided, automated, web-based acceptance and commitment therapy intervention in an existing health promotion program, to improve subjective well-being and encourage engagement with lifestyle behavior change. Methods For this 12-week, 4-armed, randomized controlled cluster feasibility study, we recruited participants offline and randomly allocated them to 1 of 3 intervention arms or control (no well-being intervention) using an automated web-based allocation procedure. Eligibility criteria were current health care staff in 1 Welsh health board, age≥18 years, ability to read English, and ability to provide consent. The primary researcher was blinded to cluster allocation. Feasibility outcomes were randomization procedure, acceptance of intervention, and adherence to and engagement with the wider program. We evaluated health and well-being data via self-assessment at 2 time points, registration and postintervention, using the 14-item Warwick-Edinburgh Mental Well-Being Scale, the 4-item Patient Health Questionnaire, and the 7-item Acceptance and Action Questionnaire—Revised. Results Of 124 participants who provided consent and were randomly allocated, 103 completed full registration and engaged with the program. Most participants (76/103) enrolled in at least one health behavior change module, and 43% (41/96) of those randomly allocated to an intervention arm enrolled in the well-being module. Adherence and engagement was low (7/103, 6.8%), but qualitative feedback was positive. Conclusions The procedure and randomization process proved feasible, and the addition of the well-being module proved acceptable to health care staff. However, participant engagement was limited, and no one completed the full 12-week program. User feedback should be used to develop the intervention to address poor engagement. Effectiveness should then be evaluated in a full-scale randomized controlled trial, which would be feasible with additional recruitment. Workplace stress is a world-wide concern and has been a subject of researchers, academicians, authorities and decision makers’ interest. It has evidently been considered to be causing poor performance, reduced employee morale, lack of autonomy, job insecurity and to greater extent the employee's wellbeing. Employees in healthcare and hospitality sector work in 24*7 work schedule due to the demanding nature of the industry. Empirical evidences have indicated that employee experience high stress on account of work overload and long working hours taking a toll on their mental and physical well-being. The purpose of this study was to investigate the impact of workplace stress on employee well-being among staff employees in the health care sector and hospitals in UAE. A total of 150 employees working at staff level in health care centers and hospitals in UAE were surveyed. The results showed that workplace has negative impact on employee well-being and the impact was found to be weak. The findings of the study suggest that an increase in workplace stresses will reduce wellbeing of employees. The researchers recommend that in order to reduce the impact of stress on employee well-being organizational support, family support and social support is essential.

Within the past decade, there has been a surge of interest in investigating the effects of clown intervention in a large variety of clinical settings. Many studies have focused on the effects of clown intervention on children. However, few studies have investigated clowning effects on adults. This paper presents an overview of the concept of medical clowning followed by a literature review conducted on the empirical studies drawn from three data bases (PubMed, PsycINFO, and Google Scholar), with the aim of mapping and discussing the evidence of clowning effects on non-children, namely adults. The following areas were investigated: Adult and elderly patients (mainly those with dementia), observers of clowning, namely non-hospitalized adults who are at the hospital as relatives of patients or health-care staff, and finally clowns themselves. The main results are that 1) clown intervention induces positive emotions, thereby enhancing the patient’s well-being, reduces psychological symptoms and emotional reactivity, and prompts a decrease in negative emotions, such as anxiety and stress; 2) clown doctors are also well-perceived by relatives and healthcare staff and their presence appears to be useful in creating a lighter atmosphere in the health setting; 3) few pilot studies have been conducted on clown doctors and this lacuna represents a subject for future research.

AIM: We aimed to understand the early warning signs and symptoms of occupational burnout as red flags among health care workers during the COVID-19 pandemic. METHODS: Based on the suggestions of the International Federation of Red Cross and Red Crescent Societies [8], health-care providers need to be trained to increase three components of resilience across the three levels of individual, team, and organization so that they can optimally manage their psychological responses to catastrophes. RESULTS: It seems that both targeted individual and organizational strategies are critical for the overall wellness of health care workers during the COVID-19 pandemic. CONCLUSION: Health care workers experience high levels of burnout during the COVID-19, which warrants attention and support from health policy-makers and practitioners. Current evidence demonstrated that health-care staff could gain significant benefits from interventions to modify burnout syndrome, especially from organization-directed interventions.

A sequence of pneumonia-causing diseases from the Novel coronavirus (COVID-19) appeared in Wuhan, Hubei, China in December 2019. The outbreak of COVID-19 spread quite rapidly. Just as we write this report, almost two and a half million verified cases were reported globally, and almost 180,000 people died.1 Experience from China reveals that COVID-19 outbreaks can be brought under control within 3 months, with highly efficient touch tracking and case isolation.2 Healthcare staff are at the forefront of treatment for COVID-19 cases and have a very large chance of exposure to the infection.3 Cuts in disposable gear and COVID-19 awareness are triggering infections in healthcare workers.4 As of February 11, 2020, China has contaminated more than 1,700 health-care workers. In Italy, 2026 (9 percent) of the COVID-19 incidents happened in healthcare professionals (as of March 15).5 But, 31 medical teams containing more than 42,000 nurses and doctors sent by the Hubei from other provinces did not have a reported infection. It indicates that sufficient information about the transmission of disease and the use of protective equipment and procedures to manage infections is necessary to avoid the spread of infection among health care staff.

Background Positive emotional well-being is associated with healthier lifestyle choices and overall health function, whereas poor mental health is associated with significant economic and psychological costs. Thus, the development of effective interventions that improve emotional well-being is crucial to address the worldwide burden of disease. Objective This study aims to develop a web-based emotional well-being intervention for use by health care staff using participatory design to consider adherence and engagement from a user perspective. Methods A 3-staged iterative participatory design process was followed, including multiple stakeholders: researchers, computer scientists, mental health experts, and health care staff. Stage 1 used document analyses, direct observation, and welcome interviews; stage 2 used focus group discussions, rapid prototyping, and usability tasks; and stage 3 evaluated a high-fidelity prototype. Results Different health care staff (N=38) participated during a sustained period. A structured, sequential, automated, 12-week, web-based emotional well-being intervention based on acceptance and commitment therapy was developed. Freely navigated psychoeducational resources were also included. Conclusions The iterative and collaborative participatory design process successfully met its objectives. It generated an in-depth understanding of well-being within the workplace and identified barriers to access. The 3-staged process ensured that participants had the opportunity to explore and articulate criteria relevant to their roles over time and reflect on decisions made at each stage.

The literature suggests that mental illness among UK health care staff is common. This study reports health care workers' knowledge and experience of mental illness. Medical staff, administration staff and other staff members employed in four NHS Trusts (n=2073) responded to a questionnaire survey. A proportion of health care workers in the NHS reported having personal (colleagues, family and self) experience of mental illness. Some health care workers held causal explanations of mental illness that are not evidence based. This study found that almost half of health care workers reported experiencing a mental illness, which could have significant implications for service delivery. Some health care workers held causal explanations of mental illness that were not evidence based; for example, some respondents reported that demonic possession or possession by evil spirits was a very good explanation for mental illness.

AbstractThe aim of this study was to explore staff perceptions of the impact of the COVID-19 pandemic on mental health service delivery and outcomes for women who were pregnant or in the first year after birth (‘perinatal’ women). Secondary analysis was undertaken of an online mixed-methods survey open to all mental health care staff in the UK involving 363 staff working with women in the perinatal period. Staff perceived the mental health of perinatal women to be particularly vulnerable to the impact of stressors associated with the pandemic such as social isolation (rated by 79.3% as relevant or extremely relevant; 288/363) and domestic violence and abuse (53.3%; 192/360). As a result of changes to mental health and other health and social care services, staff reported feeling less able to assess women, particularly their relationship with their baby (43.3%; 90/208), and to mobilise safeguarding procedures (29.4%; 62/211). While 42% of staff reported that some women engaged poorly with virtual appointments, they also found flexible remote consulting to be beneficial for some women and helped time management due to reductions in travel time. Delivery of perinatal care needs to be tailored to women’s needs; virtual appointments are perceived not to be appropriate for assessments but may be helpful for some women in subsequent interactions. Safeguarding and other risk assessment procedures must remain robust in spite of modifications made to service delivery during pandemics.

The SARS-CoV-2 virus infection is a rapidly spreading global pandemic. Recent media coverage has highlighted the importance of protecting health-care workers together with issues surrounding availability and suitability of Personal Protective Equipment (PPE). Around 20% of healthcare workers treating COVID19 cases in Italy have become infected which leads to staff absence at a critical point during the pandemic, and unfortunately in some cases mortality.PPE plays a major role in control programs. Standard PPE such as N95/FFP3 facemasks have limitations such as an ineffective seal during talking or after prolonged use, face shapes which cannot be adequately fitted, and logistical issues ensuring availability of the correct mask for each person. Furthermore, global stock is low, and issues around diagnostic testing specificity and turnaround time may lead to infectious patients receiving care from health care staff who are not wearing appropriate PPE. To address acute shortcomings in PPE availability, we have developed a simple pressurised air purified respirator unit, incorporating a combination of inexpensive and widely available components parts. The prototype was developed to minimise the number and complexity of manufacturing steps with the intention that derivative versions could be developed in many different parts of the world, including low resource settings with minor modification, where transmission could be rapid amongst high population densities.The “Personal Respirator – Southampton” (PeRSo) delivers HEPA filtered air from a battery powered fan-filter assembly through a lightweight hood/face mask that can be comfortably worn for several hours. Initial user feedback provided by doctors and nurses shows the PeRSo prototype was preferred to standard N95/FFP3 masks, being more comfortable, reducing time lost placing and removing PPE between patients, and allowing better communication. Preliminary tests indicate that the device removes microbes and passes the “fit tests” widely used to evaluate face masks. Full verification of the safety and the duration of effectiveness and durability of the device is required, as part of translation into use. Rapid upscale of production is required to protect healthcare workers from infection while the global situation accelerates, so that they can look after patients during the peak of the pandemic.
