Nursing service development or innovation projects, even small-scale ones, can be difficult to deliver and evaluate, due to a lack of resources and support. Results can also be difficult to disseminate, limiting transfer of learning. This paper presents findings from a realist evaluation of 10 small projects supported by the Queen's Nursing Institute Homeless and Inclusion Health Programme to deliver innovation in health care for people experiencing homelessness and other marginalised groups. These nurse-led projects were funded by the Queen's Nursing Institute and the Oak Foundation, and were largely successful in achieving outcomes to support the improved health of people experiencing homelessness and other marginalised groups. This realist evaluation explores the factors that contributed to the delivery of positive outcomes. All were impacted by the context and the response (mechanisms) of people experiencing homelessness and staff within these settings. It is hoped that the lessons learned will enable better support for nurse innovation projects in the future.

Pelayanan yang berpusat kepada pasien merupakan inti dari mutu pelayanan kesehatan di rumah sakit. Perawat memiliki peran penting dalam menjaga mutu kualitas asuhan pelayanan bagi pasien. Perawat Penanggung Jawab Asuhan (PPJA) harus memiliki kompetensi yang unggul agar dapat memberikan asuhan terbaik selama 24 jam bagi pasien. Pengabdian masyarakat ini bertujuan untuk meningkatkan pengetahuan perawat tentang konsep PCC dan peran PPJA. Metode yang digunakan dalam pengabdian masyarakat antara lain metode ceramah, tanya jawab dan pemodelan. Kegiatan pengabdian masyarakat ini dilakukan melalui Zoom CloudMeeting serta melibatkan 227 perawat dari beberapa rumah sakit. Melalui kegiatan abdimas ini diharapkan perawat PPJA mampu menerapkan asuhan keperawatan yang berpusat kepada pasien sehingga mutu layanan keperawatan semakin meningkat.Kata kunci: Mutu Keperawatan; Patient Centered Care; Perawat Penanggung Jawab Asuhan AbstractPatient-Centered Care (PCC) is the core of the quality of health services in hospital. Nurseshave an important role in maintaining the quality of service care for patients. Primary Nurseof care must have superior competence in order to provide the best 24 hour care for patients.This community service aims to increase nurse’s knowledge about the PCC concept and therole of Nurse in Charge. The methods used in community service include lecture, question andanswer and modelling methods. This community service though Zoom Cloud Meetings andinvolved 277 nurses from several hospital in Indonesia. The great value of home nursing services in the treatment of ailments in elderly patients has attracted increasing attention. This study describes a new mobile internet-based home nursing service system and investigates the reasons for its use among elderly patients. 520 cases of mobile internet-based home nursing services were investigated. The proportion of major reasons to use mobile internet-based home nursing services among the elderly was analyzed and the satisfaction rate was investigated. The constituent ratios of nursing care for pressure ulcers, peripherally inserted central catheter (PICC), subcutaneous injection, general stoma care, psychological care, and intramuscular injection were 61.35%, 28.85%, 6.15%, 1.92%, 1.35%, and 0.38%, respectively. The satisfaction rate with mobile internet-based home nursing services among elderly patients was 100%. Considering the demand for home nursing services for elderly patients, this is the first time that a new mobile internet-based home nursing service has been applied to provide home nursing services to elderly patients and meet their home nursing service needs. Treatment for pressure ulcers, PICC, subcutaneous injection, general stoma care, psychological care, and intramuscular injection were found to be the main reasons to use mobile internet-based home nursing services among the elderly. The new mobile internet-based home nursing service system provides convenient home nursing services to elderly patients and ensures that they get equal rights in home nursing. The results provide basis for healthcare policy makers to formulate new home nursing policies for elderly patients.
