Abstract Objective: To examine the effect of an intervention combining user fees removal with community-based management of undernutrition on the nutrition status in children under 5 years of age in Burkina Faso. Design: The study was a non-equivalent control group post-test-only design based on household survey data collected 4 years after the intervention onset in the intervention and comparison districts. Additionally, we used propensity score weighting to achieve balance on covariates between the two districts, followed by logistic multilevel modelling. Setting: Two health districts in the Sahel region. Participants: Totally, 1116 children under 5 years of age residing in 41 intervention communities and 1305 from 51 control communities. Results: When comparing children living in the intervention district to children living in a non-intervention district, we determined no differences in terms of stunting (OR = 1·13; 95 % CI 0·83, 1·54) and wasting (OR = 1·21; 95 % CI 0·90, 1·64), nor in severely wasted (OR = 1·27; 95 % CI 0·79, 2·04) and severely stunted (OR = 0·99; 95 % CI 0·76, 1·26). However, we determined that 3 % of the variance of wasting (95 % CI 1·25, 10·42) and 9·4 % of the variance of stunting (95 % CI 6·45, 13·38) were due to systematic differences between communities of residence. The presence of the intervention in the communities explained 2 % of the community-level variance of stunting and 3 % of the community-level variance of wasting. Conclusions: With the scaling-up of the national free health policy in Africa, we stress the need for rigorous evaluations and the means to measure expected changes in order to better inform health interventions.

Introducción. La distribución espacial de Aedes aegypti es heterogénea, y la interacción entre criaderos positivos y potenciales en el intradomicilio y el extradomicilio es uno de los aspectos más difíciles de caracterizar en los programas de control vectorial.Objetivo. Describir la relación espacial entre los criaderos potenciales y positivos de A. aegypti en el intradomicilio y en el extradomicilio en un sector de Cali, Colombia.Materiales y métodos. Se hizo una encuesta entomológica con el objetivo de recolectar datos de los criaderos en el intradomicilio y el extradomicilio. El análisis exploratorio de los datos espaciales incluyó la localización, la tendencia espacial, la autocorrelación espacial local, la continuidad espacial y la correlación espacial de los criaderos positivos y potenciales según el hábitat.Resultados. Se determinaron las tendencias espaciales. Mediante el análisis de autocorrelación espacial local se ubicaron los conglomerados de criaderos potenciales y positivos del extradomicilio. Se encontró una correlación positiva entre los criaderos potenciales y los positivos y una correlación negativa entre el intradomicilio y el extradomicilio.Conclusiones. La relación espacial entre criaderos positivos y potenciales de A. aegypti en el intradomicilio y el extradomicilio es dinámica y muy sensible a las características de cada territorio, por lo que establecer su distribución en el espacio contribuye a la priorización de recursos y acciones en los programas de control vectorial.

Abstract A spatial analysis is presented that aims to synthesize the evidence for climate and social dimensions of the “regreening” of the Sahel. Using an independently constructed archival database of donor-funded interventions in Burkina Faso, Mali, Niger, and Senegal in response to the persistence of drought in the 1970s and 1980s, the spatial distribution of these interventions is examined in relation to population density and to trends in precipitation and in greenness. Three categories of environmental change are classified: 1) regions at the northern grassland/shrubland edge of the Sahel where NDVI varies interannually with precipitation, 2) densely populated cropland regions of the Sahel where significant trends in precipitation and NDVI decouple at interannual time scales, and 3) regions at the southern savanna edge of the Sahel where NDVI variation is independent of precipitation. Examination of the spatial distribution of environmental change, number of development projects, and population density brings to the fore the second category, covering the cropland areas where population density and regreening are higher than average. While few, regions in this category coincide with emerging hotspots of regreening in northern Burkina Faso and southern central Niger known from case study literature. In examining the impact of efforts to rejuvenate the Sahelian environment and livelihoods in the aftermath of the droughts of the 1970s and 1980s against the backdrop of a varying and uncertain climate, the transition from desertification to regreening discourses is framed in the context of adaptation to climate change.
