We all aim for safe processes. However, providing safety is a complex endeavour. What is it that makes a process safe? And what is the contribution of humans? It is very common to consider humans a risk factor prone to errors. Therefore, we implement sophisticated safety management systems (SMS) in order to prevent potential "human failure". These SMS provide an impressive increase of safety. In safety science this approach is labelled "Safety-I", and it starts to be questioned because humans do not show failures only. On the contrary, they often actively contribute to safety, sometimes even by deviating from a procedure. This "Safety-II" perspective considers humans to be a "safety factor" as well because of their ability to adjust behaviour to the given situation. However, adaptability requires scope of action and this is where Safety-I and Safety-II contradict each other. While the former restricts freedom of action, the latter requires room for manoeuvring. Thus, the task of integrating the Safety-II perspective into SMS, which are traditionally Safety-I based, is difficult. This challenge was the main objective of our project. We discovered two methods that contribute to the quality of SMS by integrating Safety-II into SMS without jeopardizing the Safety-I approach.

This paper presents an integrated methodology that allows determining the probability of noncompliance for a given wastewater treatment plant. The methodology applies fault-tree analysis, which uses failure probabilities of individual components, to predict the overall system failure probability. The methodology can be divided into two parts : risk identification and risk quantification. In risk identification, the key components in the system are determined by analyzing the contribution of individual component failures toward system failure (i.e., noncompliance). In risk quantification, a fault-tree model is constructed for the particular system, component failure probabilities are estimated, and the fault-tree model is evaluated to determine the probability of occurrence of the top event (i.e., noncompliance). A list can be developed that ranks critical events on the basis of their contributions to the probability of noncompliance. Such a ranking should assist managers to determine which components require most attention for a better performance of the entire system. A wastewater treatment plant for treating metal-bearing rinse water from an electroplating industry is used as an example to demonstrate the application of this methodology.

Verdegem's (temporary) stay in Paris and his marriage to a Frenchwoman caused his ultimate estrangement from the Flemish nationalist environment. Moreover, his awkward characteristics contributed to his becoming "infamous rather than famous".

Empirical cumulative lifetime distribution function is often required for selecting lifetime distribution. When some test items are censored from testing before failure, this function needs to be estimated, often via the approach of discrete nonparametric maximum likelihood estimation (DN-MLE). In this approach, this empirical function is expressed as a discrete set of failure-probability estimates. Kaplan and Meier used this approach and obtained a product-limit estimate for the survivor function, in terms exclusively of the hazard probabilities, and the equivalent failure-probability estimates. They cleverly expressed the likelihood function as the product of terms each of which involves only one hazard probability ease of derivation, but the estimates for failure probabilities are complex functions of hazard probabilities. Because there are no closed-form expressions for the failure probabilities, the estimates have been calculated numerically. More importantly, it has been difficult to study the behavior of the failure probability estimates, e.g., the standard errors, particularly when the sample size is not very large. This paper first derives closed-form expressions for the failure probabilities. For the special case of no censoring, the DN-MLE estimates for the failure probabilities are in closed forms and have an obvious, intuitive interpretation. However, the Kaplan–Meier failure-probability estimates for cases involving censored data defy interpretation and intuition. This paper then develops a simple algorithm that not only produces these estimates but also provides a clear, intuitive justification for the estimates. We prove that the algorithm indeed produces the DN-MLE estimates and demonstrate numerically their equivalence to the Kaplan–Meier-based estimates. We also provide an alternative algorithm.
