Abstract Visitor restriction policies in pediatric wards during the novel coronavirus (COVID-19) outbreak are variable. Among 36 hospitals that responded to our survey, 97% allowed at least 1 visitor, with 67% restricting to 1 caregiver. Sixty-nine percent required the visitor to wear personal protective equipment and only 19% allowed non-household visitors.

Objective To review the appropriateness of vancomycin therapy, changes in vancomycin use, and the incidence of vancomycin-resistant Enterococcus (VRE) after implementation of a limited restriction policy requiring approval from the Infectious Diseases Approval service to continue vancomycin therapy beyond 72 hours. Design A prospective chart review was conducted in April 1995. Pharmacy billing data and infection control data were compared before and after policy implementation. Setting A 725-bed university teaching institution. Patients All patients receiving vancomycin during April 1995. Main outcome Measures Appropriateness of use was based on the Centers for Disease Control and Prevention (CDC) recommendations for prudent vancomycin use. Results A total of 333 courses of vancomycin therapy were reviewed. Vancomycin use was appropriate in 219 (66%) courses. Of the 114 courses that did not meet the CDC guidelines, 76 (67%) were for empiric use, 35 (31%) were for prophylactic use, and 3 (3%) were for therapeutic use. Overall, the total number of grams used decreased 9%, grams per 1000 patient-days decreased by 10, and the total number of patients exposed to vancomycin decreased 0.5%. Several services had large decreases in vancomycin use. Vancomycin expenditures decreased by $15 788 for the 7-month time period. The incidence of VRE remained unchanged, at 30% of all enterococcal isolates 2 years after policy implementation. Conclusions The limited restriction policy was effective in decreasing the total grams of vancomycin used. However, one-third of vancomycin therapy was inappropriate and the incidence of VRE was unchanged. A more stringent restriction policy could potentially increase appropriate use, further decrease the amount of vancomycin used, and decrease the incidence of VRE.

BACKGROUND Much has been published revealing concerns surrounding the use of meperidine due to associated toxicities, drug interactions, and lack of proven efficacy. Thus, many adult institutions have chosen to remove or limit the use of this agent, while little has been published about the restriction of meperidine in pediatrics. Many clinicians feel there are still clinical situations in which this agent may be useful. OBJECTIVE To describe methods taken in a pediatric hospital to restrict the use of meperidine and review literature describing uses of meperidine as a second-line agent. METHODS In our pediatric institution, a policy to restrict the use of meperidine was developed, approved, and implemented. An assessment of meperidine's use 6 months prior to policy implementation was done, along with a postinitiation review of use. RESULTS Data revealed that the use of meperidine dropped from 646 doses in 84 patients to 226 doses in 27 patients after restriction, as anticipated. Previous to implementation of these restrictions, orthopedics physicians ordered the majority of meperidine prescriptions, while the gastroenterology service ordered the majority of meperidine prescriptions after implementation of the restriction policy. However, the use of the required form was not widely adopted, with only 30% of practitioners utilizing it postrestriction. Widespread restriction of meperidine and education about use of the form at this institution are still under way. CONCLUSION Not only are there limited reasons for using meperidine, there are acceptable alternatives for every known indication. Limiting meperidine's use via a restriction policy and/or removal from the institution formulary can help limit the use of this potentially toxic agent in the pediatric patient.

This study tries to trace and find out the interpretation model of the tafsir Alahkam written by Abil Fadhol Alsenory, as an earlier tafsir of the pesantren in Indonesia. Based on the historical approach, philology, and analysis of critical discourse, by describing the socio-political context, as well as the author's response to the socio-religious conditions at the time, the results indicate that there are one primer script and two manuscript copies of his students. The primer script was written directly by Abil Fadhol, while the manuscript was a copy of the students' writing from Abil Fadhol's explanation during his teaching moment. All of the three texts show that there are two social conditions as its background, namely 'restriction' policy of the New Order regime with NU, and the debate over religious polemic between traditionalists (NU) and modernists in Indonesia in the second half of the 20th century. From this, it was revealed that Abil Fadhol's interpretation model was very critical but contextual and applicable to its surrounding social conditions.

Despite the fact that acute respiratory infections have viral etiology, the frequency of antibiotic prescriptions accounts for more than 70% in outpatient practice. However, the preventive administration of systemic antibiotics does not reduce the duration of the disease and the incidence of bacterial complications. In addition, the irrational use of antibiotic therapy can lead to the development of antibiotic resistance of infectious disease pathogens. The global problem of antibiotic resistance is seen as a serious threat to public health, and therefore the systemic antibiotic restriction policy is crucial, which helps to reduce the formation of antibiotic-resistant strains of infectious agents. The possibility of using local antibacterial drugs enables optimization of antibiotic therapy and reduces the risk of the development of antibiotic resistance. The article discusses the issues of use of thiamphenicol glycinate acetylcysteinate in various diseases in children.

Islam as a religion wants its people to always maintain a balance between religiosity (al din) and worldly problems (al dunya). The relationship between the two describes something that is separate but inseparable (harmony). However, for modern society, this condition often creates contradictions, this condition was seen at the time of the Covid 19 pandemic. The implementation of Large-Scale Social Restrictions by the government in an effort to overcome the spread of the impact of Covid 19 to the community by limiting activities in mosques and allowing activities in the market to continue in fact responded by the community differently. The methodology used is qualitative by using social policy analysis. This paper tries to examine social policies towards the application of large-scale social restrictions on mosques and markets. In this paper, the sociology of law theory is used, namely Law is a social engineering tool which emphasizes that law becomes the commander who must bring change to society. The results obtained in the research are that the community responds to the large-scale social restriction policy differently, where the purpose of the policy is for the community to be expected to make changes by complying with the rules that have been set by the government, the conditions for rejection and various responses are caused by disharmony between implementation of policies with public awareness of the law.Keyword : Policy, large-Scale Restrictions and the sociology of law. AbstrakIslam sebagai agama mengkhendaki umatnya agar senantiasa menjaga keseimbangan antara religiusitas (al din) dan masalah keduniaan (al dunia). Hubungan keduanya menggambarkan sesuatu yang terpisah namun tidak bisa dipisahkan (harmoni). Namun bagi masyarakat modern kondisi tersebut seringkali justru menimbulkan pertentangan, kondisi tersebut nampak pada saat terjadinya pandemi Covid 19. Penerapan Pembatasan Sosial Berskala Besar oleh pemerintah dalam upaya penanggulangan penyebaran dampak Covid 19 kepada masyarakat dengan membatasi aktivitas di masjid dan membiarkan kegiatan di pasar tetap berjalan nyatanya direspon oleh masyarakat secara berbeda. Metodologi yang digunakan adalah kualitatif dengan menggunakan analisis kebijakan sosial, Tulisan ini mencoba menelaah kebijakan sosial terhadap penerapan pembatasan sosial berskala besar terhadap masjid dan pasar. Dalam penulisan ini dipergunakan teori sosiologi hukum yaitu Law is a tool social engineering yang menegaskan bahwa hukum menjadi panglima yang harus membawa perubahan terhadap masyarakat. Hasil yang diperoleh dalam penelitian adalah Masyarakat merespon kebijakan pembatasan sosial berskala besar tersebut secara berbeda, dimana tujuan kebijakan tersebut adalah untuk masyarakat diharapkan dapat melakukan perubahan dengan mematuhi aturan yang telah di tetapkan oleh pemerintah, kondisi penolakan dan respon beragam tersebut di sebabkan oleh ketidak harmonisannya antara penerapan kebijakan dengan kesadaran masyarakat terhadap hukum.Kata Kunci : Kebijakan,  Pembatasan Sosial Berskala Besar dan Sosiologi Hukum
