Background: Aging is a natural process which happens to all living creatures. Aging is followed by declining physiological function. Elderly is a risk factor of micronutrient deficiency including zinc which has an important role in the immune system. This condition causes declining cellular immunity functions through reduced amount of CD4 .Objective: To identify the effect of zinc sulphate supply to serum zinc level and amount of CD4  among the elderly.Method: The study used clinical test research design, one group pre-post test design (before and after trial). Subject of the study were “healthy” elderly individuals of more than 60 years old, willing to be studied. Assessment on nutritional status, physical examination and zinc level were made to the subjects. Next, subjects were supplied with 15 mg of zinc supplement once a day for 28 days. Then, re-examination was made to identify zinc level and the amount of CD4  lymphocyte. Statistical analysis used paired t-test.Results: Average age of the elderly observed (n = 30) was 64.43 ± 3.10 years. Average zinc level before the study was 5.85 ± 1.95 μmol/L, as many as 26 subjects (86%) had zinc under normal level, 4 subjects (14%) had normal zinc level + + + and no subject had zinc above normal level. Zinc level at the end of the study increased in as many as 27 subjects (90%) with average as 4.54 ± 3.84 μmol/L (p < 0.001). The amount of CD4  lymphocyte increased in 11 subjects (36%) with + average increase 26.83 ± 137.46 (/mm) (p = 0.29). Factors affecting increase of CD4 lymphocyte were age, weight, score of mini nutritional assessment and upper arm circle, and preliminary zinc level.Conclusion: The supply of 15 mg zinc for 28 days among the elderly could elevate serum zinc level significantly and could increase the amount of CD4  lymphocyte only in 11 subjects (36%).

The aim was to assess the gut microbiota of long-livers from Moscow. This study included two groups of patients who signed their consent to participate. The group of long-livers (LL) included 20 participants aged 97–100 years (4 men and 16 women). The second group included 22 participants aged 60–76 years (6 men) without clinical manifestations of chronic diseases (healthy elderly). Gut microbiota was studied by 16S rRNA sequencing. Long-livers underwent a complex geriatric assessment as well as expanded blood biochemistry. Gut microbiota composition in the cohorts was also compared with microbiome in long-livers from Japan and Italy. Russian long-livers’ microbiome contained more beneficial bacteria than healthy elderly including Ruminococcaceae, Christensenellaceae, Lactobacillaceae families. Conditional pathogens like Veillonellaceae, Mogibacteriaceae, Alcaligenaceae, Peptococcaceae, Peptostreptococcaceae were more abundant in the healthy elderly. Compared with Italian and Japanese microbiome LL, the Russian LL appeared to be more similar to the Italian cohort. Bifidobacterium/Coprococcus and Faecalibacterium/Coprococcus balances were associated with femoral and carotid intima–media thickness, respectively. Bifidobacterium/Coriobacteriaceae balance was assessed with the folic acid level and Faecalibacterium/Coriobacteriaceae_u the with Mini Nutritional Assessment score. Long-livers’ microbiome appeared to be unexpectedly balanced. The high representation of beneficial bacteria in long-livers may prevent them from low-grade inflammation and thus protect them from the development of atherosclerosis and other aging-associated conditions.

Abstract. The aim of the study was to explore the effects of situational stress and anxiety in a group of healthy elderly, both in terms of psychophysiological correlates and cognitive performance. Eighteen participants ( Mage = 70 ± 6.3; range 60–85) were assessed for anxiety and were instructed to perform a computerized math task, under both a stressful and a control condition, while near-infrared spectroscopy (NIRS) signal and electrocardiography (ECG) were recorded. NIRS results evidenced an increased activation of right PFC during the entire procedure, even if effect sizes between left and right channels were larger during the experimental condition. The amount of right activation during the stressful condition was positively correlated with anxiety. Response times (RTs) were slower in more anxious than in less anxious individuals, both during the control and stressful conditions. Accuracy was lower in more anxious than in less anxious individuals, only during the stressful condition. Moreover, heart rate (HR) was not modulated by situational stress, nor by anxiety. Overall, the present study suggests that in healthy elderly, anxiety level has a significant impact on cerebral responses, and both on the amount of cognitive resources and the quality of performance in stressful situations.

ZusammenfassungDie Inzidenz von Mangelernährung geriatrischer Patienten wird im Allgemeinen mit über 50 % angegeben. Mangelernährung bei geriatrischen Traumapatienten rangiert im Kollektiv der proximalen Femurfrakturen zwischen 30 und 50 %. Insgesamt erscheinen sowohl die angegebenen Häufigkeiten als auch die angewandten Messinstrumente inhomogen. Malnutrition führt zu einer Verschlechterung der Wundheilung, einer längeren postoperativen Immobilität, einem verlängerten Krankenhausaufenthalt sowie zu einer Steigerung der Mortalität. Unter Hinzuziehung bestehender Leitlinien erreichen das Mini Nutritional Assessment (MNA) sowie das Nutritional Risk Screening (NRS) eine ausreichende prädiktive Validität in diesem geriatrischen Patientengut, um ein Screening auf Mangelernährung durchzuführen. Bezüglich möglicher therapeutischer Interventionen ist die Studienlage limitiert: Vorhandene Studien zeigen oft eine geringe Patientenzahl und demente Patienten, die besonders häufig mangelernährt sind, wurden häufig ausgeschlossen. Eine Leitlinie explizit für dieses spezielle Patientengut existiert aktuell nicht. Ein suffizientes Screening des Ernährungszustandes sowie Daten zur Durchführbarkeit und Effizienz einer kurzfristigen perioperativen Nahrungsergänzung könnten einen wichtigen Beitrag zur Stabilisierung dieser oft multimorbiden und fragilen Patienten leisten.
