There are times when we do not know which words to use. They throw themselves on paper, some or many of them without knowing exactly what will result, as a call for help on what they can show. So simple, so flexible, so hard, so complex they are. The request to write an editorial for the Rene Journal gave me the wheel of time. I’m retired of University teaching and research for a decade and a half. I was editor in chief and also section editor in the mental health area of this journal. So I have a lot to remember.

BACKGROUND The COVID-19 pandemic has had a significant impact on various sectors and industries around the world. Globally, healthcare workers and first responders have found themselves faced with unprecedented challenges, both within and outside of the workplace. OBJECTIVE The purpose of this research was to identify and explore the complex mental health outcomes resulting from the various new challenges experienced by healthcare workers and first responders during the COVID-19 pandemic. METHODS A phenomenological qualitative study was conducted in an effort to understand the mental health outcomes resulting from the various challenges and barriers faced while working during the pandemic. A total of 31 healthcare workers and first responders were interviewed. RESULTS The mental health outcomes described were not isolated to increased stress and anxiety; participants also described complex experiences, perspectives, and feelings related to guilt, stoicism, helplessness, fear, and anger. This paper adds to the current body of literature by further exploring the personal experiences and perspectives of healthcare workers and first responders related to these complex mental health outcomes. CONCLUSIONS Continued exploration, understanding, and awareness is needed in order to continue working towards addressing these outcomes and offering potential solutions.

Psychiatric disorders are common sequelae of diabetes mellitus and can further worsen the clinical state and quality of life of patients. The aims of this study were to determine the prevalence and pattern of psychological illnesses in patients with diabetes mellitus and to assess factors that predict these psychiatric disorders. The mental health of 114 eligible consenting adult patients with diabetes mellitus attending the diabetic clinic of Abubakar Tafawa Balewa University Teaching Hospital was assessed using Mini International Neuropsychiatric Interview (MINI). The prevalence of psychiatric disorders in the study respondents was 39.5% and the pattern of diagnoses found were Major Depressive Disorder (31.6%), Generalized Anxiety Disorder (6.1%) and Post-Traumatic Stress Disorder (1.8%). None of the sociodemographic variables was significantly associated with the presence of psychiatric disorders. The presence of diabetic complications (OR=2.519; 95% CI=1.056-6.001; P-value=0.037) was predictive of psychiatric disorders while good sugar control (OR=0.372; 95% CI=0.162-0.854; P-value=0.020) was a significant protective factor. A notable proportion of patients with diabetes mellitus develop mental health problems which can worsen the clinical outcome and further increase the burden of this illness. Efforts to promptly identify and manage these comorbid psychological problems can improve the prognosis of these patients.

This paper focuses on the use of Basque as the language of instruction. In the first part of the article the situation of Basque in the Basque Country is briefly described and the different possibilities regarding the language(s) of instruction are explained: model A with Spanish as the language of instruction and Basque as a subject; model B with both Basque and Spanish as languages of instruction and model D with Basque as the language of instruction and Spanish as a subject. Then, the results of research studies comparing these three models regarding achievement in Basque, Spanish and other areas of the curriculum are analysed. Finally the article considers the new challenges the Basque educational system is facing. One of these challenges is the need to go from bilingual education to multilingual education by teaching in a more effective way languages of wider communication. Another recent challenge is multiculturalism as a response to the increasing immigrant population which is a new phenomenon in the Basque educational system. The need for a more holistic approach towards multilingualism both in teaching and research is proposed so as to face these new challenges.
