A combined endodontic-periodontal lesion is characterized by the association of endodontic and periodontal infection in the same tooth, attacking and destroying its attachment system. This complicates the therapeutic management because the simultaneous control of the two infections would be inconvenient in clinical practice. It would therefore be essential to sequence the stages of both periodontal and endodontic treatment. The general objective of this article would be to shed light on the contribution of the endodontic approach in the treatment of combined endodontic-periodontal lesions and to specifically underline its influence on the prognosis of this condition.

Purpose In the field of audiology, auditory processing disorder (APD) continues to be a topic of ongoing debate for clinicians and scientists alike, both in terms of theory and clinical practice. In the current viewpoint, we first lay out the main issues that are central to the controversy surrounding APD, and then suggest a framework toward their resolution. Method The current viewpoint is informed by reviewing existing studies in the field of APD to better understand the issues contributing to the controversies in APD. Results We found that, within the current definition of APD, the two main issues that make the APD diagnosis controversial are (a) comorbidity with other disorders and (b) the lack of domain specificity. These issues remain unresolved, especially with the use of the existing behavioral APD test batteries. In this viewpoint, we shed light on how they can be mitigated by implementing the administration of an objective, physiological test battery. Conclusions By administering an objective test battery, as proposed in this viewpoint, we believe that it will be possible to achieve a higher degree of specificity to the auditory domain that will not only contribute towards clinical practice but also contribute towards strengthening APD as a theoretical construct.

Abstract Over the decades, research into placebo responses has shed light onto several endogenous (i.e. produced from within) mechanisms underlying modulation of pain perception initiated after the administration of inert substances (i.e. placebos). Chiropractors and manual therapists should embrace analgesic-placebo-research in an attempt to maximize clinical benefit. Historical views that placebo responses are fake, passive, undesirable, and require deception and therefore should be minimized and avoided in clinical practice are outdated. Further, statements that contend the placebo response represents a single mechanism are overly simplistic. This commentary will discuss research that shows that there are several active biological processes underlying modulation of pain perception involved in placebo analgesia and its counterpart nocebo hyperalgesia. We contend that it is highly likely that, to some extent, all of these biological processes are engaged, in varying degrees, following all interventions and represent endogenous pain modulating processes. Failure, of chiropractors and manual therapists, to embrace a more contemporary view of analgesic-placebo-research serves as a barrier to transferring knowledge into clinical practice and represents a missed opportunity to improve the delivery of current treatments.

The aim of this paper was to, on the basis of everyday clinical practice analize the effect of a homeopathic remedy, as a complementary remedy in endodontic therapy of endoperiodontal lesions and chronic apical periodontitis and to radiologically check its effect on the flow and speed of reparation of damaged periodontal tissues. Endodontic therapy was done according to the usual protocol for the therapy of infectious canals with root canal preparation, medication with calcium hydroxide points and hermetic obturation with AH-26 paste. During endodontic treatment patients were taking homeophatic remedy Hepar sulfuris per os three times daily. The remedy stimulates the natural defences mechanism of the organism. Radiological follow up after three months period showed obvious reductiont of radiologic radiolucency in periapical region.

Despite the importance placed upon the concept and act of formulation across multiple therapeutic approaches, there is a lack of literature from within the profession of counselling psychology directly on the role, use and practice of formulation, with existing literature predominantly emanating from the related yet distinct therapeutic fields of counselling, psychotherapy, clinical psychology or psychiatry. This, in conjunction with the controversies and lack of consensus on the subject, as well as the demands of the professional and regulatory bodies, have led to this paper. Our aim is to shed light on the role that formulation plays within the profession under the lens of counselling psychology’s philosophical underpinnings. More specifically, this systematic review investigates whether formulation may be considered as fact or opinion; whether formulations across therapeutic approaches may be cohesive or divisive within counselling psychology practice; whether formulations should be undertaken inclusively with clients and other professionals or exclusively by the practitioner. Such exploration uncovers key areas of debate and potential considerations for the profession regarding how formulation is approached, utilised in clinical practice and, taught by training institutions.

Daratumumab (Dara) is the first-in-class human-specific anti-CD38 mAb approved for the treatment of multiple myeloma (MM). Although recent data have demonstrated very promising results in clinical practice and trials, some patients do not achieve a partial response, and ultimately all patients undergo progression. Dara exerts anti-MM activity via antibody-dependent cell-mediated cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), complement-dependent cytotoxicity (CDC), and immunomodulatory effects. Deregulation of these pleiotropic mechanisms may cause development of Dara resistance. Knowledge of this resistance may improve the therapeutic management of MM patients.

Panobinostat represents a potent oral nonselective pan-histone deacetylase inhibitor (HDAC) with activity in myeloma patients. It has been approved by the FDA and EMA in combination with bortezomib and dexamethasone for the treatment of multiple myeloma, in patients who have received at least two prior regimens, including bortezomib and an immunomodulatory agent. In order to further explore its clinical potential, it is evaluated in different combinations in relapsed/refractory and newly diagnosed multiple myeloma. This review focuses on available data about panobinostat’s pharmacology and its role in clinical practice. This review will reveal panobinostat’s efficacy as antimyeloma treatment, describing drug evolution from preclinical experimental administration to administration in phase III trials, which established its role in current clinical practice. Based on the latest data, we will present its mechanism of action, its efficacy, and most important issues regarding its toxicity profile. We will further try to shed light on its role in current and future therapeutic landscape of myeloma patients. Panobinostat retains its role in therapy of multiple myeloma because of its manageable toxicity profile and its efficacy, mainly in heavily pretreated multiple myeloma patients. These characteristics make it valuable also for novel regimens in combination with second-generation proteasome inhibitors, IMiDs, and monoclonal antibodies. Results of ongoing trials are expected to shed light on drug introduction in different therapeutic combinations or even at an earlier level of disease course.

Often there is the need of moving teeth endodontically treated or teeth still in endodontic treatment. In order to collaborate with the comprehension and substantiation of the following subjects will be discussed: 1) Orthodontic movement in endodontically treated teeth without periapical lesion, 2) Orthodontic movement in endodontically treated teeth with inflammatory periapical lesion, and 3) Orthodontic movement in teeth endodontically treated due to aseptic pulp necrosis by dental trauma. In practically all situations, endodontically treated teeth to be orthodontically moved must be subjected to a careful evaluation by the endodontist about the conditions, adequate or not, of the endodontic treatment. Then, in this paper it was sought to induce an insight for new clinical researches about the theme that may definitely prove the information obtained by interrelations of information in parallel to clinical practice.

The introduction of modified bioceramic calcium silicate- based materials into clinical practice has changed the standards and strategies of endodontic treatment. The review article highlights modern calcium silicate- based cements that are used in various areas of endodontics, including vital pulp therapy, root canal obturation, management of endodontic complications and pulp regeneration. The classifications of bioceramic materials based on their chemistry and specific use in endodontics are presented. The physicochemical and biological properties of various types of materials, as well as the features of their clinical application, are described.
