Background: Transfusion of blood components and derivatives in day care unit is an eminent part of management of transfusion dependent patients. Day care transfusion service is an alternative to hospital admission and beneficial for those patients who receive blood more frequently for their survival. Objective: The aim of present study is to assess Transfusion Services provided in a Day Care Unit (DCU) of a tertiary care hospital. Methodology: This study was carried out in DCU of Transfusion Medicine Department, Bangabandhu Sheikh Mujib Medical University, (BSMMU) at Dhaka during January 2014 to December 2014. Data were collected from record registers. Recorded retrospective data were analyzed as percentage and proportion. Results: Total recipients were 718. Among those 424 (59.05%) were male and 294 (40.95%) were female and 562 (78.27%) were between 10 to 40 years. A total of 8587 units of blood components were used during this period. Red Cell Concentrate was most commonly utilized product 6388 (74.39%) followed by Fresh Frozen Plasma (FFP) 1360 (15.83%), Platelet Concentrate 544 (6.33%), Whole blood 260 (3.05%) and Cryoprecipitate 35 (0.40%). Transfusion was required more frequently in thalassaemic 365(50.88%) patients. Haemophilia 77(10.72%) and aplastic anaemia patients 49 (6.82%) were next high. The main transfusion reaction observed during transfusion was febrile non-haemolytic reactions. Conclusion: For increasing use of specific blood product and hassle-free transfusion services this kind of day care unit services should be strengthened. Long term study of this kind will help us to develop safe clinical transfusion practice.

Background: Diabetes mellitus is one of the important causes of morbidity and mortality. With a rise in non communicable diseases in India, diabetes has become a modern epidemic showing a rising trend in West Bengal also. A large number of diabetes patients come to the diabetes clinic of our tertiary care hospital in North Bengal Medical College and Hospital, Darjeeling, West Bengal. The large proportion of patients presenting with this condition prompted us to study the Sociodemographic and Anthropometric Prole of such patients. Methods: This was a descriptive hospital based cross sectional study involving a total of 344 diabetic patients enrolled in the diabetes clinic of a tertiary care teaching hospital from December 2019 to February 2020. A self-administered questionnaire was used to collect data and was analyzed in Microsoft excel sheet and using SPSS 20.0 software. Results: Among the 344 study participants, we found that 220 (63.95%) were males and 124 (36.05%) were females. Diabetes was more common among those aged 51-60 years 108 (31.40%). Family history was present among 188 (54.65%) of the diabetic patients. Almost 160 (46.51%) patients have diabetes 1.1 – 5 years. Hypertension was present among 216 (62.79%) patients. Almost 218 (63.37%) of the patients were overweight and 84 (24.42%) were obese. Conclusions: The diabetic patients presenting to this tertiary care hospital belong lower socioeconomic strata and having limited education in their age group. There is high proportion of obesity and hypertension among them.

Screening for Tuberculosis in people with Diabetes and screening for Diabetes in people with Tuberculosis will allow earlier detection of co-morbidities, leading to better health outcomes. This study carried out in a tertiary care hospital in South India consisted of 500 Diabetic patients and 250 patients of Tuberculosis. In 250 Tuberculosis patients screened for DM, the overall prevalence of DM in TB patients was 24.8%,out of which 17.2 % already had Diabetes and 7.6% was the additional yield during screening. About 500 Diabetes cases screened for tuberculosis, 7.6% had symptoms suggestive of tuberculosis. On further evaluation of these subjects none of them revealed evidence for tuberculosis. There exists a bidirectional relationship between TB and diabetes, and they both impact the presentation of each other. This study highlights the need of routine screening for dysglycemia for all TB patients, especially at the time of diagnosis, similar to HIV screening.

Objective: To assess the influence of pharmaceutical care activities on Knowledge, Attitude and Practice (KAP) among type 2 diabetic patients. Methods: A total of about 301 patients with type 2 Diabetes Mellitus were recruited from a tertiary care hospital. Patients were educated about their disease condition, lifestyle modifications and diet regimen to be followed and were given instructions for improving medication adherence. In the initial session, no pharmaceutical care was given, in the second session patient counseling was given verbally and in the last session patient counseling was given both verbally and non-verbally by means of pamphlet distribution. KAP questionnaire and glycemic levels (glycated hemoglobin, fasting blood sugar and post-prandial blood sugar) of participants were obtained at baseline and at follow up. Results: The baseline values of FBS in patients before intervention was found to be 176.5±74.46 mg/dL and it was reduced to 140.7±43.9 mg/dL after 3 mo. Henceforth, there was also an improvement in the PPBS values which was found to be 254±77.3 mg/dL at the baseline and was 212.9±54.2 mg/dL after intervention. The baseline values of HbA1c is 8.144±1.727% and after intervention, value reduced to about 7.173±2.768%. After the intervention, it was found to be that 30%, 16% and 12% reduction in FBS, PPBS and HbA1c values and the P-value (<0.0001) found to be statistically significant. KAP scores also found to be improved after the intervention and statistically significant. The results clearly indicate that through proper guidance and educating the patients by the pharmacist is beneficial in managing the glycemic levels and reduce further complications. Conclusion: The study concluded that the pharmaceutical care by the pharmacist is highly essentially and useful for the patients to manage the glycemic index and prevent further diabetic complications.
