A journal club is educational event where individuals meet regularly to critically evaluate recent articles in the scientific literature, generally any branch of science. Every participant of a journal club can take part in the debate and discussion. Journal clubs help people to learn and improve their critical appraisal skills and the ability to apply new knowledge [Esisi M., 2007; Swift G., 2004; Milbrandt E.B., Vincent J.L., 2004].  Journal clubs have often been cited as a bridge between research and practice [Kleinpell R.M., 2002]. Participants often differ widely in experience, knowledge and skills. An important role in this event plays a leader or chairman of a journal club [Esisi M., 2007]. One of these leaders in the field of neurology is the Head of the Department of Neurology and Reflexology, Shupyk National Medical Academy of Postgraduate Education, Prof. Natalia Svyrydova, who organized the first neurological journal club in Ukraine, which took place in the department of Neurology and Reflexology on June 30, 2016. Several modern European and American neurological articles were presented at the journal club. We present three articles related to cerebrovascular diseases: Fisher S. Primer on Diagnoses and Treatment of Symptomatic Neurogenic Orthostatic Hypotension. East European Journal of Parkinson’s Disease and Movement Disorders. 2015;1(1):45-53. Jolink WM, Klijn CJ, Brouwers PJ et al. Time trends in incidence, case fatality, and mortality of intracerebral hemorrhage. Neurology. 2015;85(15):1318-1324. Shah RS, Cole JW. Smoking and stroke: the more you smoke the more you stroke. Expert review of cardiovascular therapy. 2010;8(7);917-932. The journal club aroused great interest among practicing neurologists and staff of the Department of Neurology and Reflexology. Considering the popularity of the journal club, it was decided to hold this event on a regular basis.

Zielsetzung: Die Hausarztpraxis gilt als ideales Setting für die Tabakentwöhnung. Die bundesweite „ÄSP-kardio-Studie“ liefert aktuelle Daten zum Status quo der Tabakentwöhnung nach der „5 A-Strategie“ in deutschen Hausarztpraxen. Methodik: Zwischen 10/2011 und 03/2012 wurden über 4.000 Hausärzte repräsentativ befragt. Der Fragebogen wurde vorab einem Expert Review unterzogen, durch kognitive Interviews validiert und in einer Pilotstudie getestet. Ergebnisse: Routinemäßig erfassten 89 % der Hausärzte etwaigen Tabakkonsum („Assess“), 82 % rieten Rauchern zu einem Rauchstopp („Advise“) und 12 % vereinbarten schriftliche Ziele einer Entwöhnung („Agree“). Hilfestellungen („Assist“) in Form einer Kurzintervention fanden mit 72 % deutlich häufiger statt als in Form von Informationsmaterial (33 %) oder einer Entwöhnungstherapie (27 %). Etwa die Hälfte (54 %) vereinbarte Folgetermine zur Überprüfung des Rauchstopps („Arrange“). Das Angebot war von arzt-, praxis- sowie patientenspezifischen Faktoren abhängig und in den nordöstlichen Bundesländern unterdurchschnittlich. Acht von 10 Hausärzten (77 %) bewerteten ihre Maßnahmen als nicht erfolgreich. Schlussfolgerungen: Deutlich wird die suboptimale und ungleich verteilte Versorgung von Rauchern mit angemessenen Entwöhnungsmaßnahmen in deutschen Hausarztpraxen.

: The use of learning media is one of the critical success factors in higher education, therefore an educator must have innovation to use of learning media. The purpose of this research is to develop learning media based on the Inspiring Suit 8 on arbitration material courses of theoretical and practice for Volleyball 1 academic year 2018/2019 at IKIP Budi Utomo Malang. The existence of media will be a big successful support in learning process, so that students do not feel boredom because of the weaknesses of the lecturers, in which they are unable to provided good learning variation or even have difficulty in conveying or transferring knowledge. The research was descriptive qualitative design. The research subjects were students of Physical Education, Health, and Recreation Study Program who took volleyball theory and practice courses in the even semester academic year 2018/2019. The instruments used were the media expert review questionnaire, the learning expert questionnaire, and the trial analysis questionnaire. The questionnaire will be analyzed to see the feasibility of the developed media. Based on the data results review by media expert volleyball, media learning expert and field trials, it can be concluded that media developed is useful in learning process. In addition, it is used to provide independent motivation, which can be used in lesson for students.

Background:The rising incidence of Clostridium difficile infection (CDI) in the general population has been recognized by health care organizations worldwide. The emergence of hypervirulent strains has made CDI more challenging to understand and treat. Inflammatory bowel disease (IBD) patients are at higher risk of infection, including CDI.Objective:A diagnostic approach for recurrent CDI has yet to be validated, particularly for IBD patients. Enzyme immunoassay (EIA) for toxins A and B, as well as glutamate dehydrogenase EIA, are both rapid testing options for the identification of CDI. Without a high index of suspicion, it is challenging to initially differentiate CDI from an IBD flare based on clinical evaluation alone.Methods:Here, we provide an up-to-date review on CDI in IBD patients. When caring for an IBD patient with suspected CDI, it is appropriate to empirically treat the presumed infection while awaiting further test results.Results:Treatment with vancomycin or fidaxomicin, but not oral metronidazole, has been advocated by an expert review from the clinical practice update committee of the American Gastroenterology Association. Recurrent CDI is more common in IBD patients compared to non-IBD patients (32% versus 24%), thus more aggressive treatment is recommended for IBD patients along with early consideration of fecal microbiota transplant.Conclusion:Although the use of infliximab during CDI has been debated, clinical experience exists supporting its use in an IBD flare, even with active CDI when needed.

Coronary heart disease remains one of the leading causes of death in most countries. Healthcare improvements have seen a shift in the presentation of disease with a reducing number of ST-segment elevation myocardial infarctions (STEMIs), largely due to earlier reperfusion strategies such as percutaneous coronary intervention (PCI). Stents have revolutionized the care of these patients, but the long-term effects of these devices have been brought to the fore. The conceptual and technologic evolution of these devices from bare-metal stents led to the creation and wide application of drug-eluting stents; further research introduced the idea of polymer-based resorbable stents. We look at the evolution of stents and the multiple advantages and disadvantages offered by each of the different polymers used to make stents in order to identify what the stent of the future may consist of whilst highlighting properties that are beneficial to the patient alongside the role of the surgeon, the cardiologist, engineers, chemists, and biophysicists in creating the ideal stent.
