ABSTRACT Objectives: to report a clinical case of needle phobia which culminated in cardiac arrest and describe the outcome of a care plan based on fear and anxiety diagnoses, using the Roy adaptation model as the framework. Methods: case study conducted in a chemotherapy outpatient unit in Rio de Janeiro. Care was guided by the nursing process and the use of instruments to assess the venous network, anxiety and fear. Results: the Roy adaptation model enabled proposing nursing interventions that allowed the study subject to adapt to the external and internal stimuli triggered by vasovagal syndrome. The instruments indicated the choice of an adequate semi-implanted venous access device and led to improved levels of anxiety and fear. Final Considerations: after carrying out the nursing activities, anxiety was reduced, and the patient achieved greater control over fear.

RESUMOObjetivo: operacionalizar o processo de Enfermagem para adolescentes em cuidados paliativos, baseado no Modelo de Adaptação de Roy, utilizando NANDA, Classificação dos Resultados de Enfermagem e Classificação das Intervenções de Enfermagem. Método: estudo qualitativo, no qual se adotou, como estratégia metodológica, o estudo de caso, realizado com dois adolescentes e famílias, por meio de visitas domiciliárias. Como referencial teórico, foi utilizado o Modelo de Adaptação de Callista Roy. Resultados: observou-se que os cuidados aos adolescentes e suas famílias favoreceram a adaptação, pois os jovens apresentaram melhora significativa frente aos estímulos; as condições socioeconômicas identificadas interferiram no bem-estar da família e da adolescente do sexo feminino e, consequentemente, percebeu-se o impacto gerado. Todavia, deve-se propor intervenções como orientações sobre o manejo dos sintomas apresentados e sessões de musicoterapia, contribuindo para a melhora no ambiente familiar, além da redução da ansiedade. Conclusão: as intervenções implementadas foram importantes para a substituição de respostas ineficazes por respostas adaptativas. Results: it was observed that the care to the adolescents and their families favored the adaptation, since the young showed a significant improvement in front of the stimuli; the identified socioeconomic conditions interfered with the well-being of the female family and adolescent, and consequently, the generated impact was perceived. However, one should propose interventions as guidelines on the management of the presented symptoms and sessions of music therapy, contributing to the improvement in the environment reduction of anxiety. Introduction: Evidence suggests that end-stage renal disease (ESRD) significantly affects general health in the patients, causing their general health to be poorer compared to the general population. The Roy adaptation model (RAM) is the best one for ESRD patients. Objectives: The present study aimed to determine the effect of a RAM-based care plan on general health in hemodialysis patients. Patients and Methods: This randomized controlled clinical trial conducted on 60 hemodialysis patients in Iran. The data collected using a demographic questionnaire and the general health questionnaire-28 (GHQ-28). In the intervention group, the Roy assessment form was completed and the RAM-based care plan was then trained in four group sessions over 4 weeks. Individual sessions were also held if required and patients followed-up for 2 weeks. The control group received only routine care. At the end of the follow-up, general health was re-assessed in the patients. The findings were analysed using t test, the chi-square test and the McNemar test. Results: Despite observing no significant differences between the two groups in terms of general health levels before the intervention (P=0.530), the difference was significant after the intervention (P=0.028), since the mean score of general health decreased by 4.07 in the intervention group compared to before the intervention (P=0.003). The intervention significantly affected the subscales of somatic symptoms (P=0.013), anxiety and insomnia (P=0.006), social dysfunction (P=0.016) and depression (P=0.031). Conclusion: The findings suggested the positive effects of using the RAM on general health in hemodialysis patients. The RAM is therefore recommended that be used as a holistic care approach to improving general health in these patients

Objective: to identify the scientific evidence about the components of the Roy Adaptation Model in the population studiedin the light of this theory. Methods: this is an integrative literature review in databases of the Latin-American and CaribbeanCenter on Health Sciences Information, Medical Literature Analysis and Retrieval System Online, Spanish Bibliographic Indexon Health Sciences, Nursing Database, PubMed Central, Cumulative Index to Nursing and Allied Health Literature, Web ofScience, and SciVerse Scopus. The sample consists of 20 articles published between 2005 and 2013. Results: the three typesof stimuli, 38 of 82 adaptive problems, the four adaptive modes, and the six steps of the nursing process were identified.Conclusion: there is need for further studies on this theory and that address the entire nursing process, culminating in theincrease in specific nursing knowledge and affirmation of this science in health.

Nurses in a full range of settings from acute to home care are increasingly exposed to the use and maintenance of central VADs. Although the advantages of central VADs are well recognized, the incidence and potential for complications associated with these devices should not be minimized. The major complication of catheter occlusion has the potential of inducing significant morbidity and mortality. The purpose of the following care plan is to familiarize the nurse with the information needed to restore catheter patency efficiently, safely, and effectively, thereby reducing the potential complications and enhancing the quality of patient care.

A Sistematização da Assistência de Enfermagem é caracterizada por ser uma ferramenta utilizada pelo enfermeiropara aprimorar os saberes da prática, otimizando, aperfeiçoando e aprimorando as técnicas assistenciais a partirdas novas evidências científicas. O estudo teve como objetivo elaborar e validar um plano de cuidados parapaciente queimado. Trata-se de um estudo de caso realizado no Centro de Tratamento de Queimados de umhospital público no nordeste do Brasil, no período de abril a maio de 2016. As etapas metodológicas do estudoforam estas: coleta de dados; elaboração dos diagnósticos, resultados e intervenções de enfermagem; elaboraçãoe validação do plano de cuidados. Após a análise dos dados, foi realizada a elaboração e a validação de umplano de cuidados com quatro diagnósticos, resultados e 24 intervenções de enfermagem. Diante disso, o estudopermitiu identificar os diagnósticos, resultados e intervenções de enfermagem para a aplicação na prática clínica,corroborando para o cuidado científico e direcionado as necessidades prioritárias.Palavras-chave: Processos de Enfermagem; Enfermagem; Queimaduras. ABSTRACTThe Systematization of Nursing Care is characterized by being a tool that nurses use to improve the practicalknowledge, optimizing, enhancing and improving the technical assistance will from the new scientific evidence.The study aimed to develop and validate a plan of care for burn patients. This is a case study in the Burn TreatmentCenter of a public hospital in northeastern Brazil, from April to May 2016. The methodological stages of thestudy were these: data collection; preparation of diagnoses / outcomes and nursing interventions; preparationand care plan validation. After analyzing the data produced and validated a care plan with four diagnostic / 24results and nursing interventions. Thus, the study identified the diagnoses / outcomes and nursing interventionsto implement in clinical practice.Keywords: Nursing process; Nursing; Burns.
