Abstract The aim of the present study was to prospectively assess the frequency of teeth in which the Reciproc instruments were able to reach the full working length (WL). This case series study enrolled 277 consecutive teeth requiring primary endodontic treatment performed with the Reciproc system (VDW) without a previous glide path by 3 experienced endodontists. Instruments R25, R40 or R50 were selected, respectively, for narrow, medium and large canals, according to the preoperative radiographs, as per manufacturer recommendation. Canals were instrumented and classified as reaching the full WL (RFWL) and not reaching the full WL (NRFWL), according to the ability of the Reciproc instrument to reach without a glide path the full electronically determined WL. Finally, canals were filled using a single matched cone technique. Pearson's squared-chi test compared the frequency distributions of canals classified as RFWL and NRFWL. From a total pool of 673 root canals, Reciproc instruments reached the FWL in 96.28% of cases and 3 fractures occurred. For narrow canals the percentage of RFWL was 95.37% with 3 fractures. For medium and large canals, the R40 or R50 instruments reached the FWL in 98.51% and 97.62% of the cases respectively, with no fracture. Enlarging of root canals using the Reciproc instruments up to the FWL without a glide path is successful in a great percentage of cases. This case series suggests the possibility of using a single instrument to enlarge the root canal space of teeth by a simpler approach without an a prioriglide path procedure.

Background Reciproc instruments are the only contemporary root canal instruments where glide path preparation is no longer strictly demanded by the manufacturer. As the complete preparation of root canals is associated with success in endodontic treatment we wanted to assess the ability and find predictors for Reciproc instruments to reach full working length (RFWL) in root canals of maxillary molars in primary root canal treatment (1°RCTx) and retreatment (2°RCTx) cases. Methods This retrospective study evaluated 255 endodontic treatment cases of maxillary molars. 180 were 1°RCTx and 75 2°RCTx. All root canals were prepared with Reciproc instruments. The groups were compared and in a binary logistic regression model predictors for RFWL were evaluated. Results A total of 926 root canals were treated with Reciproc without glide path preparation. This was possible in 885 canals (95.6%). In 1°RCTx cases 625 of 649 (96.3%) canals were RFWL and in 2°RCTx cases 260 of 277 (93.9%). In second and third mesiobuccal canals (MB2/3) 90 out of 101 (89.1%) were RFWL with Reciproc in 1°RCTx and in the 2°RCTx treatment group 49 out of 51 cases (96.1%). In mesio-buccal (MB1) canals “2°RCTx” was identified as negative predictor for RFWL (OR 0.24 (CI [0.08–0.77])). In MB2/3 canals full working length was reached less often (OR 0.04 (CI [0.01–0.31])) if the tooth was constricted and more often if MB2/3 and MB1 canals were convergent (OR 4.60 (CI [1.07–19.61])). Discussion Using Reciproc instruments, the vast majority of root canals in primary treatment and retreatment cases can be prepared without glide path preparation.

<p><strong>Objective:</strong> To evaluate the effect of glide path creation on transportation promoted by NiTi and M-Wire instruments. <strong>Material and Methods: </strong>Sixty polyester resin blocks containing a simulated root canal were distributed into four groups (n=15), according to the protocols/systems used for root canal preparation: GPR group - glide path + Revo-S system; R group - no glide path + Revo-S system; GPPN group - glide path + ProTaper Next system and PN group - no glide path + ProTaper Next system. Root canals were photographed before and after preparation, and the images were superimposed to evaluate the transportation at the apical, middle and coronal thirds. The time spent to perform preparation was also measured (seconds). Data were submitted to the non-parametric Kruskal-Wallis test (p&lt;0.05) for statistical analysis. <strong>Results:</strong> In the apical third, there was no significant difference among groups (p&gt;0.05). GPR and R groups were similar in the middle third (p&gt;0.05). However, the transportation value in GPR group was statistically higher in comparison with GPPN and PN groups (p&lt;0.05). In the coronal third, GPR and R groups were similar (p&gt;0.05). Only R group presented significant difference in comparison with GPPN and PN groups (p&lt;0.05). There was no difference among groups about time spent to perform preparation (p&gt;0.05). <strong>Conclusion:</strong> None of the systems were capable of maintaining the original trajectory of the simulated root canal, and the glide path had no effect on the transportation promoted by instruments.</p><p><strong>Keywords</strong></p><p>Dental instruments; Endodontics; Root canal preparation.</p>

ABSTRACT Aim To evaluate the frequent use of ProTaper Next (PTN; Dentsply Maillefer, Ballaigues, Switzerland) systems on shaping ability of root canal utilizing Solidworks (2014, Dassault Systèmes) software. Materials and methods Thirty-six root canals in clear resin blocks (Dentsply-Maillefer) were allocated into six experimental groups (n = 36). Six new sets of PTN instruments (Dentsply Maillefer, Ballaigues, Switzerland) were used six times to shape the resin blocks. A #15 K-file was inserted to the working length (WL), followed by ProGlider (PG) to create a glide path. Sequential use of PTN instrumentation in a crown-down technique was used to reach size (30/07) apically. Macroscopic photos of the blocks were taken before and after instrumentation, layered by Paint Shop Pro 9 from JascSoftware®, and then canal transportation was measured using Solidwork 2014. The data were analyzed by SPSS software version 22. Multivariate statistical analysis general linear model (GLM) was also applied. Bonferroni correction test was used in multiple comparisons and the statistical significance was set to 0.05. Results There was no difference in canal transportation resulted from utilizing PTN files after six multiple uses; in addition, the PTN files showed ability to maintain the original canal anatomy, especially in the apical level, where lowest total mean value of canal center displacement was seen (3 mm level) (0.019 ± 0.017). Conclusion ProTaper Next files can be used to prepare single and multiple canals in a single furcated tooth. Clinical significance ProTaper Next nickel-titanium (NiTi) file system is a safe instrument that respects the canal shape, allows practitioners to treat difficult cases with good results, and low risk of separation. Background.This study purposed to compare the effect of new single glide path files on extruded apical debris and total preparation times during root canal preparation with the WaveOne Gold system. Methods. Thirty-six extracted human lower molar teeth with mesiobuccal canal curvature angles of 25‒35° were randomly splited to three groups. In group 1, the glide path was created with WaveOne Gold Glider (WGG) file at working length (WL); in group 2, the glide path was created with ProGlider file (PG); in group 3, the glide path was not performed. In all the groups, the root canals were shaped with WaveOne Gold Primary (WOG) reciprocating files at WL. Apically extruded debris during instrumentation was picked up into pre-weighed Eppendorf tubes. The weight of the dry extruded debris was calculated by subtracting the pre- and post-instrumentation weights of the tubes in each group. The total time elapsed during the canal preparation was calculated with a chronometer. The data were analyzed using one-way ANOVA and post hoc Tukey tests. Results. The WGG/WOG group extruded significantly fewer debris than the WOG and PG/WOG groups (P<0.05). There was no significant difference between the WOG and PG/WOG groups (P>0.05). The WGG/WOG and PG/WOG groups were significantly faster than the WOG group. Conclusion. The amount of debris extruded apically significantly diminished when conventional WGG was implemented before using WOG. The total preparation time significantly diminished when the WOG file was used in combination with reciprocating and rotary glide path preparation techniques.

A male elderly patient came with buccal mucosa swelling associated with tooth 46 and 47. Periapical radiograph confirmed the presence of periapical lesion and narrow root canals on both teeth. Challenging impediments were sensed upon negotiating most of the narrow root canals which were common in elderly patients. Glide path creation involved pre-flaring, K-file bending, watch winding movement, and very gentle strokes. After achieving patency, shaping, cleaning, and sealing phase can be performed.

Aims. A glide path is created prior to root canal instrumentation by nickel-titanium (NiTi) rotary files to increase the efficiency and safety of cleaning and shaping. This study aimed to assess root canal transportation in use of different glide path files in curved canals. Materials and Methods. 30 sound mesiobuccal root canals of mandibular molars with 20° to 40° curvature were selected and randomly assigned to 3 groups of EdgeGlidePath (EGP, EdgeEndo), One-G (Micro-Mega), and Neolix (Neoniti). The specimens were scanned before and after glide path creation by microcomputed tomography (micro-CT). The pre- and postoperative micro-CT scans were superimposed, and the degree of canal transportation and centering ratio were measured at 1, 3, 5, and 7 mm distances from the apical foramen. Statistical Analysis. The data were analyzed by two-way and one-way ANOVA. Results. The effects of distance from the apical foramen and instrument type and the interaction effect of the two were not significant on the centering ability of the files or canal transportation. Conclusion. EdgeGlidePath, One-G, and Neolix files fabricated from the conventional NiTi alloy or heat-treated M-Wire alloy showed similar performance regarding centering ability and canal transportation in glide path preparation in curved canals.

SummaryA retrospective case series study was done to determine the long-term outcome of operations upon dogs treated for canine hip dysplasia by means of a triple pelvic osteotomy (TPO). Twentyfour dogs with bilateral hip dysplasia, that received a unilateral TPO between January 1988 and June 1995, were re-examined at the Ontario Veterinary College. The assessment included physical, orthopedic and lameness examinations, standard blood work, pelvic radiographs and force plate gait analysis. They were compared to bilaterally dysplastic dogs that had not been treated, and also to normal dogs. Force plate data analysis demonstrated a significant increase in peak vertical force (PVF) and mean vertical force over stance (MVF) in the limb that underwent surgical correction by means of a TPO, when compared to the unoperated hip. It was determined that performing a unilateral TPO on a young dysplastic dog resulted in greater forces and weight bearing being projected through the TPO corrected limb when compared to the unoperated limb.Dogs with bilateral hip dysplasia treated with a unilateral triple pelvic osteotomy (TPO) were assessed by force plate gait analysis, radiographs and orthopedic examination. There was a significant increase in hip Norberg angles over time, although degenerative changes did progress. Limbs that had been operated upon had significantly greater peak and mean ground reaction forces than limbs that had not received an operation.

AbstractThe purpose of this study was to evaluate the outcomes of isolated medial patellofemoral ligament (MPFL) reconstruction, regardless of the presence of predisposing factors. A total of 21 knees that underwent isolated MPFL reconstruction from March 2014 to August 2017 were included in this retrospective series. Radiographs of the series of the knee at flexion angles of 20, 40, and 60 degrees were acquired. The patellar position was evaluated using the patellar tilt angle, sulcus angle, congruence angle (CA), and Caton-Deschamps and Blackburne-Peel ratios. To evaluate the clinical outcome, the preoperative and postoperative International Knee Documentation Committee (IKDC) and Lysholm knee scoring scales were analyzed. To evaluate the postoperative outcomes based on the predisposing factors, the results were separately analyzed for each group. Regarding radiologic outcomes, 20-degree CA was significantly reduced from 10.37 ± 5.96° preoperatively to −0.94 ± 4.11° postoperatively (p = 0.001). In addition, regardless of the predisposing factors, delta values of pre- and postoperation of 20-degree CA were not significantly different in both groups. The IKDC score improved from 53.71 (range: 18–74) preoperatively to 94.71 (range: 86–100) at the last follow-up (p = 0.004), and the Lysholm score improved from 54.28 (range: 10–81) preoperatively to 94.14 (range: 86–100) at the last follow-up (p = 0.010). Isolated MPFL reconstruction provides a safe and effective treatment for patellofemoral instability, even in the presence of mild predisposing factors, such as trochlear dysplasia, increased patella height, increased TT–TG distance, or valgus alignment. This is a Level 4, case series study.
