ABSTRACT Objective: The aim of the present study was to determine whether the side of application of the plate itself affects the mechanical stability of the fixation. The specific question addressed is whether or not a lateral or medial plate application is biomechanically better, for the treatment of distal diaphysis fractures of the femur. Methods: Stability and stiffness of medial sided plating relative to the conventional lateral sided plating in distal diaphysis of the femur were measured by analyzing axial loading forces leading to implant failure. Sixty synthetic femurs were tested in physiological bending, to calculate the yield and ultimate load to displacement following fixation of distal diaphysis fractures of the femur by either medial or lateral sided plating. Axial loading was applied to samples using a uniaxial testing machine. Results: There was more implant deformation in the lateral sided plating group – a difference with statistical significance. Conclusion: Medial sided plating was found to be as stiff as lateral plating. Medial plating may be a reasonable treatment option that can be used safely in selected cases. Purpose: This study aims to assess the biomechanical performance of different tying techniques of a double-stranded looped suture (DSLS). Methods: Loop and knot security of DSLS tying techniques (nice knot (NK), modified nice knot (MNK), double-twist knot (DTK), and double-barrel knot (DBK)) were compared. The square knot of DSLS (SKD) and the square knot of single-stranded suture (SKS) had been used as references. Twenty-four loops of each configuration were created using No. 2 Fiberwire (Arthrex, Naples, Florida, USA) and tested with a material testing machine. Samples were loaded with 10 N preloads for loop security assessment. Knot security was subsequently evaluated. Twelve loops of each knot were loaded to failure. The rest were subjected to cyclic load testing and the elongation at the 50th and 1000th cycles were measured. Knot bulkiness was determined by measuring knot height before testing. Data were compared with analysis of variance and post hoc tests. Statistical significance was p < 0.05. Results: All knots showed no statistically significant difference in displacement with preload. The load-to-failure was highest in NK, followed by MNK, DTK, DBK, SKD, and SKS. The cyclic loading test at the 50th cycle and the 1000th cycle demonstrated that NK has significantly less displacement than the others except MNK. DTK provided a minimal average knot height followed by NK, SKS, DBK, MNK, and SKD. Conclusion: The different tying techniques in DSLS provided the similar loop security but different knot security and knot bulkiness. NK and MNK are biomechanically superior to the other knots, whereas DTK is the least bulky. The findings in the present study may help set the guide for the surgeons to select the tying technique of DSLS to best suit their requirement.

Purpose: The aim of this study was to compare the mechanical stability of two methods of fixation for arthrodesis of the hallux metatarsophalangeal joint: a technique using an intramedullary screw and a standard technique using crossed interfragmentary compression screws. Methodology: The metatarsophalangeal joint was mechanically evaluated in cantilever bending using a servohydraulic testing machine. Differences in stiffness and strength parameters between the two techniques were checked for significance (P<0.05) using a paired t-test. Results: Fixation provided by the intramedullary screw was stiffer and stronger than that from crossed compression screws. Conclusions: The stronger and stiffer intramedullary screw technique offers mechanical advantages over the crossed interfragmentary screw technique.

Abstract Purpose Attention has recently been paid to Clinical Linguistics for the detection and support of clinical conditions. Many works have been published on the “linguistic profile” of various clinical populations, but very few papers have been devoted to linguistic changes in patients with eating disorders. Patients with Anorexia Nervosa (AN) share similar psychological features such as disturbances in self-perceived body image, inflexible and obsessive thinking and anxious or depressive traits. We hypothesize that these characteristics can result in altered linguistic patterns and be detected using the Natural Language Processing tools. Methods We enrolled 51 young participants from December 2019 to February 2020 (age range: 14–18): 17 girls with a clinical diagnosis of AN, and 34 normal-weighted peers, matched by gender, age and educational level. Participants in each group were asked to produce three written texts (around 10–15 lines long). A rich set of linguistic features was extracted from the text samples and the statistical significance in pinpointing the pathological process was measured. Results Comparison between the two groups showed several linguistics indexes as statistically significant, with syntactic reduction as the most relevant trait of AN productions. In particular, the following features emerge as statistically significant in distinguishing AN girls and their normal-weighted peers: the length of the sentences, the complexity of the noun phrase, and the global syntactic complexity. This peculiar pattern of linguistic erosion may be due to the severe metabolic impairment also affecting the central nervous system in AN. Conclusion These preliminary data showed the existence of linguistic parameters as probable linguistic markers of AN. However, the analysis of a bigger cohort, still ongoing, is needed to consolidate this assumption. Level of evidence III Evidence obtained from case–control analytic studies.

In this paper, the experimental investigation of polyurethane (PU) foam-filled into circular aluminum tubes subjected to axial crushing was presented. The purpose of this study is to improve the energy absorption of aluminium tube under axial quasi-static load. The aluminium tube was made from the AA6063-T5 aluminium alloy tubes. Each tube was filled with polyurethane foam. The density of foam was varied from 100, 150 and 200 kg/mP3P including with empty tube. The range of diameter/thickness (D/t) ratio of tube was varied from 15-55. The specimen were tested by quasi-static axial load with crush speed of 50 mm/min using the 2,000 kN universal testing machine. The load-displacement curves while testing were recorded for calculation. The mode of collapse of each specimen was analyzed concerning on foam density and the influence of D/t ratio. The results revealed that the tube with foam-filled provided significantly increment of the energy absorption than that of the empty tube. While the density of foam and D/t ratios increase, the tendency of collapse mode is transformed from asymmetric mode to concertina mode.

Abstract Background The optimal treatment strategy for the surgical management of femur fractures and non-unions remains unknown. The aim of this study is to assess union rates, complications and outcome after femoral double plating. Treatment of shaft, distal, periprosthetic fractures and pathological proximal femur fractures as well as femoral non-unions with double plating were evaluated. Methods A systematic review according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) statement was conducted. Published literature reporting on the treatment and clinical outcome of femoral fractures and non-unions with double plating was identified. In total, 24 studies with 436 cases of double plating, 64 cases of single plating, 84 cases of intramedullary nailing (IM), and 1 interfragmentary screw treatment met the inclusion criteria of this systematic review. The evaluated literature was published between 1991 and 2020. Results Double plating of femoral fractures achieved high healing rates and few complications were reported. It displayed significantly less intraoperative haemorrhage, shorter surgery time reduced risk of malunion in polytraumatised patients when compared to IM. Fracture healing rate of double-plating distal femoral fractures was 88.0%. However, there were no significant differences regarding fracture healing, complication or functional outcome when compared to single plating. Treatment of periprosthetic fractures with double plating displayed high healing rates (88.5%). Double plating of non-unions achieved excellent osseous union rates (98.5%). Conclusions The literature provides evidence for superior outcomes when using double plating in distal femoral fractures, periprosthetic fractures and femoral non-unions. Some evidence suggests that the use of double plating of femoral fractures in polytraumatised patients may be beneficial over other types of fracture fixation. Level of evidence IV.

Abstract Background The aim of our study was to compare the long-term outcome after perichondrium transplantation and two-component surface replacement (SR) implants to the metacarpophalangeal (MCP) and the proximal interphalangeal (PIP) joints. Methods We evaluated 163 joints in 124 patients, divided into 138 SR implants in 102 patients and 25 perichondrium transplantations in 22 patients. Our primary outcome was any revision surgery of the index joint. Results The median follow-up time was 6 years (0–21) for the SR implants and 26 years (1–37) for the perichondrium transplants. Median age at index surgery was 64 years (24–82) for SR implants and 45 years (18–61) for perichondium transplants. MCP joint survival was slightly better in the perichondrium group (86.7%; 95% confidence interval [CI]: 69.4–100.0) than in the SR implant group (75%; CI 53.8–96.1), but not statistically significantly so (p = 0.4). PIP joint survival was also slightly better in the perichondrium group (80%; CI 55–100) than in the SR implant group (74.7%; CI 66.6–82.7), but below the threshold of statistical significance (p = 0.8). Conclusion In conclusion, resurfacing of finger joints using transplanted perichondrium is a technique worth considering since the method has low revision rates in the medium term and compares favorable to SR implants. Abstract Rotator cuff (RC) tears cause pain and functional disability of the shoulder. Despite advances in suture anchors, there are still reports about the incidence of surgical-related injuries to RC mainly associated with sutures. The purpose of this study was to design and evaluate the mechanical behavior of sutureless implants to repair RC tears. We hypothesized that the implants present mechanical characteristics suitable for the surgical treatment of RC tears as suture anchors. Three different implants (T1,T2,T3) were designed and fabricated with titanium: T1 has two rods and rectangular head; T2 has two rods with a small opening and enlarged rectangular head and T3 has three rods and a circular head. The implants were fixed in rigid polyurethane foam blocks by a series of blows, and the applied mechanical loads along with the number of blows were quantified. Pullout tests using tapes fixed between the implant head and testing machine grip were conducted until implant failure. The maximum pullout strength and displacement of the implant relative to the rigid foam block were computed. Statistical significance was set at p &lt; 0.05. Owing to its geometric configuration, implant T2 presented the best characteristics related to stability, strength, and ease of insertion. Implant T2 confirms our hypothesis that its mechanical behavior is compatible with that of suture anchors which could lead to the reduction of RC repair failures and simplify the arthroscopic procedure.

Background: High-level athletes (HLAs) have been shown to have better short-term outcomes than nonathletes (NAs) after hip arthroscopy. Purpose: (1) To report midterm outcomes of HLAs after primary hip arthroscopy and (2) to compare their results with a propensity-matched cohort of NA patients. Study Design: Cohort study; Level of evidence, 3. Methods: Data were prospectively collected and retrospectively reviewed between February 2008 and November 2015 for HLAs (professional, college, or high school) who underwent primary hip arthroscopy in the setting of femoroacetabular impingement syndrome (FAIS). HLAs were included if they had preoperative, minimum 2-year, and minimum 5-year follow-up data for the modified Harris Hip Score (mHHS), Non-Arthritic Hip Score (NAHS), and Hip Outcome Score Sports-Specific Subscale (HOS-SSS). Radiographic and intraoperative findings, surgical procedures, patient-reported outcomes (PROs), patient acceptable symptomatic state (PASS), minimal clinically important difference (MCID), and return to sport were reported. The HLA study group was propensity-matched to a control group of NA patients for comparison. Results: A total 65 HLA patients (67 hips) were included in the final analysis with mean follow-up time of 74.6 ± 16.7 months. HLAs showed significant improvement in all PROs recorded, achieved high rates of MCID and PASS for mHHS (74.6% and 79.4%, respectively) and HOS-SSS (67.7% and 66.1%, respectively), and returned to sport at high rates (80.4%). When compared with the propensity-matched NA control group, HLAs reported higher baseline but comparable postoperative scores for the mHHS and NAHS. HLA patients achieved MCID and PASS for mHHS at similar rates as NA patients, but the HLA patients achieved PASS for HOS-SSS at higher rates that trended toward statistical significance (66.1% vs 48.4%; P = .07). NA patients underwent revision arthroscopic surgery at similar rates as HLA patients (14.9% vs 9.0%, respectively; P = .424). Conclusion: Primary hip arthroscopy results in favorable midterm outcomes in HLAs. When compared with a propensity-matched NA control group, HLAs demonstrated a tendency toward higher rates of achieving PASS for HOS-SSS but similar arthroscopic revision rates at minimum 5-year follow-up.
