<b><i>Background:</i></b> The very low-calorie ketogenic diet (VLCKD) has been recently proposed as an appealing nutritional strategy for obesity management. The VLCKD is characterized by a low carbohydrate content (&#x3c;50 g/day), 1–1.5 g of protein/kg of ideal body weight, 15–30 g of fat/day, and a daily intake of about 500–800 calories. <b><i>Objectives:</i></b> The aim of the current document is to suggest a common protocol for VLCKD and to summarize the existing literature on its efficacy in weight management and weight-related comorbidities, as well as the possible side effects. <b><i>Methods:</i></b> This document has been prepared in adherence with Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) guidelines. Literature searches, study selection, methodology development, and quality appraisal were performed independently by 2 authors and the data were collated by means of a meta-analysis and narrative synthesis. <b><i>Results:</i></b> Of the 645 articles retrieved, 15 studies met the inclusion criteria and were reviewed, revealing 4 main findings. First, the VLCKD was shown to result in a significant weight loss in the short, intermediate, and long terms and improvement in body composition parameters as well as glycemic and lipid profiles. Second, when compared with other weight loss interventions of the same duration, the VLCKD showed a major effect on reduction of body weight, fat mass, waist circumference, total cholesterol and triglyceridemia as well as improved insulin resistance. Third, although the VLCKD also resulted in a significant reduction of glycemia, HbA1c, and LDL cholesterol, these changes were similar to those obtained with other weight loss interventions. Finally, the VLCKD can be considered a safe nutritional approach under a health professional’s supervision since the most common side effects are usually clinically mild and easily to manage and recovery is often spontaneous. <b><i>Conclusions:</i></b> The VLCKD can be recommended as an effective dietary treatment for individuals with obesity after considering potential contra-indications and keeping in mind that any dietary treatment has to be personalized. <b><i>Prospero Registry:</i></b> The assessment of the efficacy of VLCKD on body weight, body composition, glycemic and lipid parameters in overweight and obese subjects: a meta-analysis (CRD42020205189).

Antidepressant drugs are frequently prescribed for women and have various side effects, including potential effects on body weight. This experiment examined the effects of information about the weight-related side effects of antidepressants on women's attitudes toward the drugs. 60 college women were randomly assigned to read about one of two drugs, fluoxetine (Prozac) or Imipramine (Tofranil). Participants were either told or not told about veridical weight-related side effects, namely, weight loss for Prozac and weight gain for Tofranil. As hypothesized, weight-gain information lowered the personal acceptability of Tofranil, and weight-loss information enhanced the acceptability of Prozac. Although research with clinical populations is required, undergraduate women's decisions about the use of antidepressant medications may be influenced by societal body-image ideals.

Chemotherapy causes various side effects that can affect the patient's physical or non-physical condition. Patients' perceptions or ratings of these side effects vary. This study aimed to identify cancer patients' perceptions of the side effects of chemotherapy they are undergoing. The descriptive study was conducted on 84 samples, namely cancer patients aged 20 years who had undergone chemotherapy at least once before. Data collection using a questionnaire that has been tested for validity and reliability. Data analysis was carried out descriptively with frequency distribution. The results showed that the perception of the physical side effects of chemotherapy that was considered very disturbing by the patient (severe symptoms) was nausea (59.5%), feeling of weakness (36.9%), hair loss (35.7%), vomiting. (29.8%), loss of appetite (28.6%). %), weight loss (27.4%), insomnia and skin discoloration (19% each), headache (16.7%) and fever (15.5%). When the perception of non-physical side effects was fear of death, affecting work/household duties and feeling fear (15.5% each), feeling anxious about my life, and the presence of family members I have to take care (14.3% each). The most disturbing side effects of chemotherapy are physical problems. The health service is expected to increase interventions to minimize the side effects of chemotherapy.

OBJECTIVES: The systemic and cognitive side effects of hepatitis C virus (HCV) therapy may be incapacitating, necessitating dose reductions or abandonment of therapy. Oral cannabinoid-containing medications (OCs) ameliorate chemotherapy-induced nausea and vomiting, as well as AIDS wasting syndrome. The efficacy of OCs in managing HCV treatment-related side effects is unknown.METHODS: All patients who initiated interferon-ribavirin therapy at The Ottawa Hospital Viral Hepatitis Clinic (Ottawa, Ontario) between August 2003 and January 2007 were identified using a computerized clinical database. The baseline characteristics of OC recipients were compared with those of nonrecipients. The treatment-related side effect response to OC was assessed by χ2analysis. The key therapeutic outcomes related to weight, interferon dose reduction and treatment outcomes were assessed by Student’sttest and χ2analysis.RESULTS: Twenty-five of 191 patients (13%) initiated OC use. Recipients had similar characteristics to nonrecipients, aside from prior marijuana smoking history (24% versus 10%, respectively; P=0.04). The median time to OC initiation was seven weeks. The most common indications for initiation of OC were anorexia (72%) and nausea (32%). Sixty-four per cent of all patients who received OC experienced subjective improvement in symptoms. The median weight loss before OC initiation was 4.5 kg. A trend toward greater median weight loss was noted at week 4 in patients eventually initiating OC use (−1.4 kg), compared with those who did not (−1.0 kg). Weight loss stabilized one month after OC initiation (median 0.5 kg additional loss). Interferon dose reductions were rare and did not differ by OC use (8% of OC recipients versus 5% of nonrecipients). The proportions of patients completing a full course of HCV therapy and achieving a sustained virological response were greater in OC recipients.CONCLUSIONS: The present retrospective cohort analysis found that OC use is often effective in managing HCV treatment-related symptoms that contribute to weight loss, and may stabilize weight decline once initiated.

In recent years antihypertensive therapy has evolved from treatment for a relatively small number of patients with severe hypertension to treatment for millions of people with mild to severe hypertension. We now treat not only patients at high risk for future cardiovascular morbidity and mortality, of whom nearly all are benefitting from antihypertensive therapy, but also much larger groups of patients each individually at low risk. In this latter group only a small percentage actually benefits from antihypertensive therapy. For example, in the Australian trial in subjects with mild hypertension and no other evidence of cardiovascular disease, only two excess deaths were prevented at the expense of 1000 patient-years of drug treatment (Australian Therapeutic Trial 1980). For most individual members of this group normalizing their mild blood pressure elevation appears to offer no benefit, yet all of them are exposed to antihypertensive therapy and its side effects. When instituting antihypertensive pharmacotherapy in patients with mild hypertension one has to be concerned about these side effects, not just the objective ones (e.g., effects on plasma lipoproteins or glucose which may offset any gains to be obtained by lowering blood pressure), but also subjective ones (e.g., fatigue, impotence) which may markedly affect the quality of life.Nonpharmacologic, in particular nutritional, management of mild hypertension has intuitively major appeal for "lowering blood pressure without side effects." Many studies have evaluated the effects of dietary changes on blood pressure. Several recent symposia have addressed the issue of nutrition and hypertension. Despite this, the report from the first Consensus Development Conference of the Canadian Hypertension Society ("on the management of mild hypertension in Canada") states "Because of conflicting evidence and problems with patient compliance, the conference had difficulty reaching consensus on the effectiveness of salt restriction and, for the obese, of weight reduction in lowering blood pressure" (Logan 1984).This issue of the Canadian Journal of Physiology and Pharmacology contains the proceedings of a workshop "Nutritional Management of Hypertension: Controversies and Frontiers," held in Harrison Hot Springs, British Columbia, September 6–7, 1985. This workshop was organized under the auspices of the Canadian Hypertension Society and made possible by generous financial support from ICI Pharma, Canada (general sponsorship) as well as from the National Institute of Nutrition for the obesity session, and from the Dairy Bureau of Canada for the sodium–calcium session.To define more clearly the controversies and uncertainties, this workshop was organized in a different way than previous meetings dealing with this issue. A clinical scientist working in a particular area was invited to outline the evidence in favour of a given dietary manipulation for the treatment of hypertension, and another one to outline the evidence against. This evaluation would particularly concern evidence regarding "efficacy" and "effectiveness". A discussant then presented an evaluation of the two position papers, followed by a general discussion and a summary by the session chairman. This type of scrutiny of our current knowledge was done for sodium restriction, calcium supplementation, and weight loss. As part of this evaluation two speakers addressed the closely related issues of practical aspects of diet management (e.g. compliance) and the consequences–risks of weight loss in relation to the pathophysiology of obesity.In the last part of the workshop possible future developments in nutrition and hypertension were reviewed, such as "nutrition in the young, early intervention?," vegetarian approach to hypertension, role of dietary fats, and proteins and precursors.The organizing committee very much appreciated that Dr. David Sackett was willing to serve as the scientific chairman of this workshop, to summarize the present "state of the art" on diet modulation in the management of hypertension as well as to propose recommendations for treatment of hypertension in clinical practice and for future research directions.It was a pleasure for me to serve as chairman of the organizing committee. The endocannabinoid system has emerged as a significant player in the control of energy balance and metabolism, through its direct central and peripheral effects, as well as via its interaction with other appetite-regulating pathways. There is mounting evidence that the endocannabinoid system is overactive in obesity and were it possible to safely dampen-down the elevated endocannabinoid tone, lipid and carbohydrate profiles could be improved and weight loss induced. The series of randomised clinical trials showed reproducible beneficial effects on weight, HbA1c and lipid parameters, in addition to other cardiovascular risk factors. However, to date, clinical developments have been halted because of psychiatric side effects. Although recent evidence has highlighted the importance of an appetite-independent, peripheral mode of action, it is still unclear whether selectively blocking the peripheral system could potentially solve the problem of the central side effects, which thus far has led to the demise of the cannabinoid antagonists as useful pharmaceuticals. In this concise review, we summarise the data on the metabolic effects of the cannabinoid pathway and its antagonists.
