Human enamel veneer restoration: an alternative technique to restore anterior primary teeth

Restoration of severely decayed primary teeth is a clinical challenge in Pediatric Dentistry. Among the restorative treatment options, the use of prefabricated crowns and resin composite restorations, either by means of direct or indirect techniques is mentioned in the literature. The purpose of this article is to describe the rehabilitation of primary anterior teeth in a 5-year-old patient. Dental treatment consisted on an anterior space maintainer prosthesis made with natural primary teeth, plus human dental enamel veneer (facet) restorations. The advantages of this technique are better esthetics and the natural enamel has physiologic wear and offers superficial smoothness and cervical adaptation compatible with those of the surrounding teeth.

Introduction: the esthetic factor should be considered for every patient who seeks a comprehensive dental treatment, especially for women. The anterior teeth deformities will diminish the levels of confidence and decrease self-esteem of the patients. The tooth development disturbances, such as enamel hypoplasia, may present an esthetic problem. Enamel hypoplasia is an alteration of organic matrix formation which characterized by the defect or porosity on the enamel surface. It can be restored with a variety of restorative treatment options including indirect porcelain veneers. Case: a 30 years old female patient was referred with chief complaint of anterior teeth discoloration. Clinical examination on the maxillary left central and lateral incisors revealed the defects on nearly half of the labial surface. Case management: the incisal lapping technique were chosen to prepared teeth #21 and #22 for indirect porcelain veneer fabrications. Conclusion:  indirect porcelain veneers established an excellent esthetic result and restored the size, shape and color of anterior teeth with long term stability.

<p><strong><em>Background: </em></strong><em>Nursing bottle carries  is  a problematic condition in children. On one side, parents try to supply enough nutrition to their children, though on the other side this night-time bottle feeding habit might cause negative effect on the childrens dental health. Esthetic dental treatment on children have improved greatly over the past few years, as parents expect an almost-complete restoration of their child’s dental condition. The alternatives for pediatrics esthetic dental restoration may depend on the cooperativeness and habit of the child themselves</em><em>, while also taking the child’s development into consideration. <strong>Aim:</strong> to report evaluation of aesthetic restoration on primary teeth. <strong>Case:</strong> A 4 year-old female child was presented by her parents, with damaged four anterior teeth, which made her uncomfortable especially on feeding due to the sharp edges. From history taking we found a night-time bottle-feeding habit, which usually occurred until patient was asleep. <strong>Case Management:</strong> restoration of the four primary incisive teeth was performed using a resin composite crown. Outcome of the treatment was evaluated until the primary teeth exfoliateand permanent teeth arise. <strong>Conclusion:</strong> cooperation between patient, parents, and dentist is important in the dental restorative treatment on paediatric patient. Moreover, a good communication with parents is encouraged, especially on follow-up schedule, treatment cost, success rate and possibility of treatment failure is also encouraged. Abstract The early loss of anterior primary teeth is frequent in the pediatric dentistry clinic and it is associated with extensive caries or dental trauma. The purpose of this study was to report two cases of aesthetic-functional rehabilitation due to early loss of anterior primary teeth, with the aid of fixed space maintainers of the tube-bar type. In the first case, a 4-year-old female child referred for dental treatment is reported. During clinical examination, several restorations in the posterior teeth and absent upper anterior teeth were observed. The treatment plan consisted of installing a tube-bar type upper fixed space maintainer, considering the ease of installation and adaptation for young children. The second case reports a 5-year-old male patient with the main complaint of dental caries. Clinical examination revealed extensive coronary destruction in the upper central incisors, as well as in the posterior teeth that presented carious lesions in several teeth. For the case planning, it was determined the need for  the posterior teeth restorations and the exodontia of the anterior dental remnants. After oral rehabilitation, an aesthetic-functional space maintainer of the tube-bar type was made and installed. In both cases, prosthetic rehabilitation was fundamental to maintain the space until the eruption of the permanent successors, avoiding future damages, as well as to restore the children's self-esteem and introversion  behavior. Keywords: Deciduous. Mouth rehabilitation.  Resumo A perda precoce dos dentes decíduos anteriores é frequente na clínica odontopediátrica e está associada a processos de cáries extensas ou traumas dentários. O objetivo do presente estudo foi relatar dois casos clínicos de reabilitação estético-funcional após perda precoce de dentes decíduos anteriores, com auxílio de mantenedores de espaço fixo do tipo tubo-barra. O primeiro caso refere-se a uma criança do sexo feminino, quatro anos de idade, encaminhada para atendimento odontológico. Durante exame clínico, observou-se restaurações nos dentes posteriores e ausência dos dentes anteriores superiores. O plano de tratamento consistiu na instalação de um mantenedor de espaço fixo superior do tipo tubo-barra, levando em consideração a facilidade de instalação e adaptação para crianças com pouca idade. O segundo caso reporta um paciente de sexo masculino, cinco anos de idade, tendo como queixa principal a presença de cáries dentárias. Ao exame clínico, foi notado extensas destruições coronárias nos incisivos centrais superiores, assim como nos dentes posteriores. Para o planejamento do caso, determinou-se a necessidade de restaurações dos dentes posteriores e as exodontia dos remanescentes dentários anteriores. Após adequação do meio bucal foi confeccionado e instalado um mantenedor de espaço estético-funcional do tipo tubo-barra. Em ambos os casos, a reabilitação protética foi fundamental para a manutenção do espaço até a irrupção dos sucessores permanentes, evitando prejuízos futuros, devolvendo a autoestima e a mudança do comportamento de introversão das crianças.  Palavras-chave: Dente decíduo. Trauma to deciduous anterior teeth, frequently occur in children, and the treatment is a big challenge for the pediatric dentistry. In these cases, besides the pain and discomfort provoked by the injury, both child and parents/persons responsible were eager to reconstruct the damage, as soon as possible. In modern operative restorative dentistry, no restorative material is able to substitute for the human dental enamel in quality, color and resistance. The aim of this paper is to relate the treatment of esthetic veneer (facet) of human dental enamel in a three-year-old child after trauma that caused concussion and accentuated color alteration. Clinical results showed an efficient esthetical resolution, revealing it to be a good alternative for treatment of traumatized anterior deciduous teeth.

AbstractThis study aimed to describe the survival trajectory of dental restorations placed in an outpatient population of geriatric and adult special needs patients over a 15-year span, with particular interest in longevity of subsequent restorations in teeth that received multiple restorations over time. Dental restorations of different types and sizes in patients age ≥65 years treated between 2000-14 at the University of Iowa, College of Dentistry were followed until they incurred an event (i.e., restoration replacement, extraction of the tooth, or endodontic treatment of the tooth). Survival analysis and extended Cox regression models were used to generate hazards ratios for selected predictor variables. A total of 9184 restorations were followed in 1551 unique patients. During the follow-up period, 28.7% of these restorations incurred an event; and overall the restorations had a median lifespan of 6.25 years. In multivariable regression models, after controlling for gender and age, composite restorations and greater number of restoration surfaces were associated with higher risks of failure; and the initial restoration recorded in the database for each subject tended to have lower risk of failure than restorations placed later that included any of those same surfaces. This information potentially could be helpful to elderly patients considering various restorative treatment options during the dental treatment planning and informed consent process.

Space maintainers are used to minimise undesirable drifting of teeth in the dental arch. Using space maintenance after the loss of primary teeth restricts the creation of spaces. Space maintainers also prevent the development of malocclusion due to space. The primary teeth loss can lead to different dental malocclusions like midline shifts, space loss and also crowding of teeth. Gropers appliance is a space maintainer used for children who have lost their primary anterior due to accident or caries. Gropers appliance is a fixed partial denture used for aesthetic purposes. This is used to restore mastication and speech to prevent abnormal oral habit development and also for aesthetics. The aim of the study is to find the prevalence of Gropers appliance in Pediatric Dentistry. This was a retrospective study done under a University setting. All the patients' records were collected from June 2019 to March 2020 and were reviewed. The data included patients who required space maintainers. The data was tabulated and entered in excel and the data were analysed using SPSS package software. p<0.05 was considered to be the level of statistical significance in the study. The prevalence of Gropers appliance in Pediatric Dentistry was found to be 39.6%. There was a male prevalence of 55.6% in receiving a Gropers appliance. The common age who received it was 3 years old. Within the limitation of current study, the prevalence of Gropers appliance was 39. 6% with the common age of children being 3 yrs.

Background. Esthetic appearance of primary anterior teeth is one of the major demands in the dental field. Destructed anterior primary teeth due to caries is considered a major issue due to the difficulty in regaining the esthetic crowns and attaching them until the normal exfoliation time. There have been many attempts and tools used to attach the composite crowns to the treated canals of primary anterior teeth. The study evaluates endodontic treatment for destructed primary maxillary incisors with a glass fiber-reinforced post as a retentive tool to hold the esthetic composite crowns until the normal exfoliation time of primary incisors. Case Presentation. A four-year-old child attending a dental clinic complained of pain of maxillary incisors. Dental examination showed irreversible pulpitis of four maxillary incisors indicated for root canal treatment and crown placement. Endodontic treatment was carried out, and a glass fiber-reinforced post was used to get successful retention for the composite crowns. Follow-up was carried out for 3 years. The 3 crowns were retained successfully until replaced by permanent incisors. One crown fell during the treatment course. Conclusion. Retention of primary teeth is one of the challenges in pediatric dentistry. Restoration of primary decayed incisors is important for child medical, physical, and psychological conditions. A glass fiber-reinforced post and core is a strong retentive tool for composite crown retention for primary incisors. This procedure opens the door for a strong tool to retain composite crowns for a long time. Objective: To compare the effect of Papacarie and Atraumatic Restorative Treatment (ART) on pain and discomfort during caries removal among children. Study Design: Fifty healthy, 4–8 year-old children were equally and randomly allocated to Papacarie and ART to remove caries from decayed primary teeth. A randomized, controlled, blinded, two parallel-arms clinical trial was conducted in the clinic of the Pediatric Dentistry and Dental Public Health Department, Alexandria University, Egypt in March 2014. Pain and discomfort were assessed blindly by two independent investigators watching videotaped treatment sessions using the Sound, Eye and Motor scale (SEM). Their reliability was assessed using Kappa statistics. The effect of caries removal methods, time spent to remove caries and other confounders on SEM score was assessed using regression analysis. Results: Mean time to remove caries using Papacarie and ART was 5.8 and 4.8 minutes, P= 0.005. Median Paparie and ART scores for the S, E and M components were 1, 1, 1 and 3, 2, 3. Adjusted mean SEM score= 3.6 and 7.8, P &lt;0.0001. Method of caries removal was the only factor significantly affecting pain and discomfort. Conclusion: Papacarie is associated with minimal pain during caries removal from primary teeth compared to ART, although it has longer working time.

Aesthetic requirement of severely mutilated primary anterior teeth in the case of early childhood caries has been a challenge to pediatric dentist. Among restorative treatment options, prefabricated crown and biological and resin composite restoration either by means of direct or indirect technique are mentioned in the literature. This paper presents the clinical sequence of rehabilitation of maxillary anterior primary teeth. Endodontic treatment was followed by the placement of a glass fibre-reinforced composite resin post. The crown reconstruction was done with composite restoration. Resin glass fibre post has best properties in elasticity, translucency, adaptability, tenaciousness, and resistance to traction and to impact. Along with ease of application, fiber can be used as an alternative to traditionally used materials in the management of early childhood caries.

Mini implants have seen an evolutionary change in the phase of implant placement. The most common use for mini implant is the stabilization of over denture and orthodontic treatments, now they are used in pediatric dentistry for congenitally missing teeth and tooth loss due to trauma. Mini implants as a prosthetic replacement gives a psychological advantage to the child as it provides a feeling of his own teeth. A thirteen year old boy came to the department of pedodontics, KVG dental college, Sullia, with a chief complaint of spacing in the lower front tooth region since 4 years. On radiographic examination absence of 41, 31 were found. On model analysis, space deficiency was found in relation to the lower anterior and he was diagnosed with angle’s class I malocclusion with proclined maxxilary anterior teeth and congenitally missing lower central incisors. The patient’s main concern was on esthetics. Hence the patient was planned for a fixed provisional functional space maintainer using mini implant followed by crown prosthesis. The approach comes up with positive aesthetic and functional results that may reflect on self-esteem and social well-being of children and adolescents.
