Trauma to deciduous anterior teeth, frequently occur in children, and the treatment is a big challenge for the pediatric dentistry. In these cases, besides the pain and discomfort provoked by the injury, both child and parents/persons responsible were eager to reconstruct the damage, as soon as possible. In modern operative restorative dentistry, no restorative material is able to substitute for the human dental enamel in quality, color and resistance. The aim of this paper is to relate the treatment of esthetic veneer (facet) of human dental enamel in a three-year-old child after trauma that caused concussion and accentuated color alteration. Clinical results showed an efficient esthetical resolution, revealing it to be a good alternative for treatment of traumatized anterior deciduous teeth.

Restoration of severely decayed primary teeth is a clinical challenge in Pediatric Dentistry. Among the restorative treatment options, the use of prefabricated crowns and resin composite restorations, either by means of direct or indirect techniques is mentioned in the literature. The purpose of this article is to describe the rehabilitation of primary anterior teeth in a 5-year-old patient. Dental treatment consisted on an anterior space maintainer prosthesis made with natural primary teeth, plus human dental enamel veneer (facet) restorations. The advantages of this technique are better esthetics and the natural enamel has physiologic wear and offers superficial smoothness and cervical adaptation compatible with those of the surrounding teeth.

Impaction of the anterior teeth, which is less frequent in central incisors, can cause serious esthetic and subsequent psychological problems for patients during the mixed dentition period. Traumatic injury to deciduous teeth is the most common etiologic factor. Thus, treatment of maxillary incisor impaction is highly important. Nowadays, application of laser has been suggested in orthodontics and pediatric dentistry for different treatments, such as surgical exposure of impacted teeth and application of low-level laser therapy (LLLT) for acceleration of orthodontic tooth movement. In this paper, the authors present treatment of an impacted and dilacerated maxillary central incisor with laser application for its surgical exposure and LLLT for acceleration of its orthodontic traction and eruption.

Keeping in mind the final result is the basis of any type of treatment, especially those in which the morphology, size and proportion of the anterior teeth will be changed. This is where a good treatment plan based on a diagnostic wax-up tested with a mock-up and approved by the patient becomes crucial. This case report aims to exemplify how to transfer the diagnostic information to the patient’s mouth and direct it not only to the restorative dentistry, but also to the surgeon when performing the crown length. Diagnostic waxing was performed by the laboratory technician, obeying anterior posterior incisal and gingival criteria and curvatures, which were transported to an aesthetic guide through mock up to mark the surgical points. Then, surgery to increase the clinical crown with bone access and after healing, direct venners in composite resin also guided by waxing. In conclusion, a workflow can be established using the wax-up / mock-up that serves as a guide for the periodontist in the approach to surgical lengthening of the crown and for the rehabilitator who uses it to produce changes in the shape of dental dimensions .

Abstract This case report describes an esthetic treatment to improve the shape and alignment of the anterior teeth, reestablishing smile harmony, using feldspathic porcelain veneers. Results of clinical follow up after 36 months are also presented. The advantages, disadvantages and limitations of the technique are detailed with reference to the relevant literature. This suggests that the success of treatment depends on adequate conditions of bonding between the veneers and the tooth complex, which involves parameters such as the strength and durability of the bond interface. Therefore, the clinical success of feldspathic porcelain veneers depends on the accurate selection of cases and correct execution of clinical and laboratory procedures. The rehabilitation involved from first right premolar to the left with feldspathic porcelain veneers made on refractory dies. After the 3-year follow up, excellent clinical results and patient satisfaction were achieved.

AbstractStable isotope analysis of human tissues has become a valuable tool for mapping human geolocation. This study adds to the existing knowledge of the relationship between oxygen stable isotopes in human enamel and drinking water by presenting enamel oxygen values in clinic-extracted human dental enamel with known provenance. The results from this study indicate that the theoretical isotopic relationship between enamel and drinking water oxygen is weak at the city and country-level. Differences of up to 15‰ were observed between predicted drinking water oxygen values using existing models and observed values, highlighting the complexity of using water/enamel conversion equations. The lower isotopic boundary of enamel oxygen values is now understood for Metro Vancouver at δ18Oc(VPDB) = – 11.0‰ and presents the possibility of using stable isotope analysis as an exclusionary tool where individuals falling below threshold value can be identified as non-local. Overall, this study’s results support the development of geographical reference maps for human enamel oxygen.

ABSTRACTSalivary pellicle is an organic biofilm formed by the physisorption of proteins and carbohydrates onto the surface of dental enamel exposed to the oral environment. The pellicle has several key roles in oral physiology including lubrication and reduction of friction between teeth during mastication, as well as chemical protection of the enamel against acidic solutions. However, pellicle proteins are known to react with dietary compounds to cause extrinsic staining on the tooth surface.In this study, nanoindentation and AFM have been used in vitro to examine the acquired salivary pellicle formed in vivo on dental enamel. The mechanical properties, growth, structure and morphology of pellicle grown in vivo on human enamel surfaces have been analysed. In addition, the effects of dietary agents such as polyphenols on the pellicle's morphology and properties have been studied.It was found that initial adsorption of proteins on the enamel surface occurred within 30 seconds of exposure to the oral cavity, with full growth achieved within 2 hours. Differences in the properties of the pellicles such as surface adhesion, and time dependent effects due to polyphenol interaction were measured using nanoindentation. It was seen that the polyphenol interaction has a significant effect on these properties. These results suggest that the stained pellicle is mechanically stiffer, but also less viscous and more fluid like. This could explain why traditional tooth brushing techniques do not efficiently remove this layer.

AbstractBackgroundNanosilver fluoride (NSF) was developed as an alternative in the prevention of dental caries.PurposeThe aim of this study was to test the remineralizing action of NSF on incipient enamel caries and its antimicrobial action on the acid production and adhesion of Streptococcus mutans.MethodsDeciduous enamel fragments were treated with sodium fluoride (NaF), NSF and deionized water. Microhardness, fluorescence spectroscopy and optical coherence tomography imaging were performed on each specimen before chemical caries induction, after caries induction and after 14 days of pH cycling. The treated enamel fragments were also placed into test tubes containing bacterial suspension and saliva. The pH readings and quantification of the adhered microorganisms to the dental enamel were determined. Analysis of variance, Kruskal-Wallis, Mann-Whitney, Tukey and mixed linear regression model were applied.ResultsNSF and NaF were effective in enamel remineralization, with a statistically significant difference (p<0.001) to deionized water, and they had no statistically significant difference between themselves (p>0.005). NSF had greater effectiveness compared to NaF in preventing decreases of pH and adhesion of S. mutans to the enamel surface, with statistically significant (p<0.001) differences.ConclusionNSF may be more effective than conventional fluorides in treating incipient caries lesions due to its remineralization and antibacterial actions.

AbstractComposite resins represent a class of material widely used in restorative dentistry, not only for anterior aesthetics but also as the first choice to restore posterior teeth. However the key limitation in the use of composite resins as a restorative material is related to shrinkage during polymerization which leads to poor marginal seal, marginal staining, restoration displacement, tooth fracture and recurring caries [1].Polymerization shrinkage may affect negatively the clinical outcome of the restoration. Hence the present study evaluates the Polymerization shrinkage of 2 different posterior composites; Filtek Z350(3M) and everX Posterior(GC), using a pycnometer.Independent Sample T-Test was used to determine statistically significant difference in volumetric shrinkage among the tested composite resins. everX Posterior showed comparatively less shrinkage than Filtek Z350; which can be attributed to the presence of silanated e-glass fibres.

Curcumin-mediated Photodynamic Inactivation (PDI) has shown great potential to disinfect specific sites on tooth enamel but may involve contact with restorative materials. Thus, before use in dentistry, it is necessary to investigate whether the PDI protocol causes undesirable changes in the surfaces of aesthetic restorative materials and dental enamel. This study investigated the effect of PDI mediated by curcumin (CUR) in a liquid crystal precursor system on color stability (ΔE), surface roughness (Ra), and microhardness (kgf) of three different composite resins and bovine dental enamel specimens. The microhardness and roughness readings were performed 60 days after the treatments while the color readings were performed immediately, 24, 48, and 72 h, 7, 14, 21, 30, and 60 days after the treatments. Results showed that CUR mediated-PDI does not seem to have the potential to promote any esthetic or mechanical changes to the surface of tooth enamel and can be applied safely in clinical practice. However, the results on color, roughness, and hardness obtained for composite resins show that some negative effects can be produced, depending on the type of restorative material; more experiments must be performed with different formulations and, perhaps, with lower concentrations of CUR.
