Staphylococcus Aureus Contamination in a Pediatric Dental Clinic

Staphylococcus aureus strains can be disseminated during dental treatment and occasionally lead to contamination and infection of patients and dentists. The objective of this study was to determine the frequency and compare the number of S.aureus colonies isolated from the nose, hands and tongue of students and patients, as well as from the clinical environment, before and after dental treatment. Staphylococcus species were isolated from the tongue, nose and hands of 30 students and 30 patients and from the environment of a Pediatric Dentistry Clinic. The samples were incubated in SMA plates at 37o C for 48 hours. Results: The colonies that showed the presence of mannitol fermentation were collected as identification for Staphylococcus aureus, using CHROMagar and the coagulase test. The highest amount of S.aureus was found in the nose and tongue of children. In relation to dental students, more contamination was observed on gloved hands, followed by the tongue and hands without gloves, before clinical attendance. At the end of dental treatment, S. aureus colonies isolated from the gloved hands of students decreased significantly. Considering the clinical environment, the most contaminated areas were the auxiliary table and the storeroom, which was located at the center of the clinic. Conclusion: The dental clinic can be considered an environment for S. aureus cross-transmission. Preventative measures should be used to avoid the dissemination of pathogenic microorganisms.

SUMMARYOur hypothesis was that a method of caries activity evaluation based on the clinical features of the lesions would be less time consuming but more influenced by the examiner's experience than the scoring system used in association with the International Caries Detection and Assessment System (ICDAS). Thus, the aim of this study was to evaluate the performance of three groups of examiners with different levels of experience using two different methods to assess the activity status of caries lesions by visual inspection. A cross-sectional study in a dental office setting was performed selecting 18 children, aged three to eight years, who had sought dental treatment at a dental school. Examinations to detect caries lesions were performed using visual inspection by six examiners with different levels of experience: two undergraduate dental students, two specialists in pediatric dentistry, and two graduate students. The examiners used ICDAS and two different methods to assess caries activity: using an additional score system or considering the examination of clinical features. Two benchmark examiners examined the children in a joint session, and their consensus was considered to be the reference standard. The sensitivity, specificity, and reproducibility were calculated for different thresholds: all, cavitated, and active caries lesions. Multilevel analyses were performed to compare the different methods and examiners. No differences were observed among the examiners, either in detecting all lesions and cavitated lesions or regarding the activity assessment. The methods of assessing activity status performed similarly, but the time spent on examinations was shorter for the method evaluating clinical features. In conclusion, the experience of examiners does not significantly influence the performance of visual inspection, and both methods of assessing activity status result in similar diagnostic accuracy.

ABSTRACT Objective Strategies to prepare the child with play activities in order to establish an initial friendly relationship between patient and dentist are widely used as a means to soften the inconsistent behaviors and negative emotions during dental treatment. Thus, the objectives of this study were to evaluate the emotions of children before and after performing play activities and at the end of dental treatment, as well as assessing child behavior. Methods A total of 74 children, both boys and girls, aged 4 to 10 years old, participated in this study and attended the Pediatric Dentistry Clinic for two consecutive semesters. The children responded as they felt through the Emotions Deck during the three stages. In addition, data on behavior and procedures were also collected. Results A total of 468 emotions were evaluated, of which 69 (14.75%) were negative and 399 (85.25%) were positive; the joy was the one of greater frequency in the three evaluated moments. A total of 128 (82.05%) positive emotions were observed before, 133 (85.25%) after the play activities and 136 (87.18%) of the positive emotions were after the dental treatment. As for negative emotions, 28 (17.94%) were before, 23 (14.74%) after play activities and 20 (12.82%) after treatment. Regarding behavior, only 9 (5.77%) were not registered as cooperative, while 147 collaborated (94.23%). Conclusion Therefore, it was possible to conclude that the majority of the patients presented positive emotions at the different moments evaluated and the negative emotions were observed mainly before the ludic activities, having a reduction after the dental care. Most patients demonstrated cooperative behavior.

In the year 2020 Covid-19 disease caused an extensive pandemic world-wide. It arose from Wuhan city in China and spread to whole world resulting in many deaths. Immediately, researchers conducted many studies to assess the etiology, epidemiology, ways of transmission and protective instructions and materials against Covid-19, but still there are not enough studies about challenges which dentists, dental students and patients referring to dental clinics encounter regarding their social, economic and psychological status. In this review, authors tried to focus on the literature regarding these problems. The literature search was conducted using natural language keywords and controlled vocabulary using MeSH through PubMed, Scopus and Google Scholar from 1st of January 2020 to 21st of November 2020. In addition, Gray literature and manual search of references were also performed. Articles with low quality methodology were excluded based on critical appraisal. The articles about the effects of Covid-19 on "dental education", "pediatric dentistry", "geriatric dentistry" and "dental professions and practice" were included in the study. The results showed that due to the practical nature of the dental profession and education and the importance of social distance during this pandemic (Covid-19), the dental community and even patients in need of dental treatment have been affected by this pandemic in different ways that should be considered by health officials for facilitating feedback to this pandemic.

Ketamine has been used as a safe and effective sedative to treat adults and children exhibiting high levels of anxiety or fear during dental treatment. Pediatric dentistry often involves patients with high levels of anxiety and fear and possibly a few positive dental experiences. Patient management can involve behavioral approaches, as well as the use of sedation or general anesthesia with a variety of agents, including ketamine. The aim of this study was to assess the knowledge and awareness of ketamine as an anesthetic among dental students. A questionnaire survey was carried out online containing 10 questions that were sent to 100 dental students through survey planet software and the data was collected and statistically analysed. 74% of the students were aware about the uses and adverse effects of ketamine .Within the limits of this study, it may be concluded that most of the students were aware of ketamine as an anesthetic.

Background: The nature of dental profession requires a dentist to be in a close proximity to the patient’s mouth to perform various operative treatments including procedures such as generating aerosols. As such, dentistry is considered as one of the riskiest profession for the cross-transmission of SARS-CoV2. Although the prevalence and incidence of SARS-CoV2 continue to increase worldwide; the dental services and treatment of non-emergency cases have resumed in many countries including Sudan. Moreover, the use of smartphone technology in communication with patients can play an important role in controlling the spread of SARS-CoV2. Therefore, the aim of this short review was to draw a summary on using smartphone technology for telescreening dental patients prior to their visit to the clinics and to set guidelines and rules for dental treatment during SARS-CoV2 pandemic. Methods:  For the purpose of this study, telescreening is referred as a virtual contact between patients and healthcare personnel used to provide health services during the highly contiguous SARS-CoV2 pandemic. Using a smartphone, dental patients can be screened and questioned about their medical history and particularly for SARS-CoV2 (for symptoms such as fever, headache, coughing, breath shortness, loss of smell or taste). They should also be asked about their travel history to epidemic area, visiting hospital or coming in contact with a confirmed SARS-CoV2 patient during the last 14 days. On arrival to a dental clinic, the patient's temperature should be measured, instruction for wearing masks, hand hygiene, and keeping distances from others inside the waiting zone. Conclusion: Telescreening and triaging will minimize the time of patients’ exposure to dental clinic environments, hence decreasing the risk of getting SARS-CoV2 infection for both patients and dental healthcare workers.  Keywords: COVID-19, dentistry, procedure generating aerosols, smartphone, social distance, surgical masks.

Objective: To assess self-reported stress during the performance of different procedures in pediatric dentistry, according to the professional experience of the dentists. Study design: During the years 2010 to 2011, an anonymous survey was administered by means of an internet link, and by distribution at professional meetings of dentists . Results: No statistically significant differences in stress were reported for maxilla and mandibular procedures. Placement of a rubber dam was rated as the most stressful procedure among dental students. For general practitioners and specialists, injection of local anesthesia to an anxious child was the most stressful procedure, regardless of age, sex, or years of professional experience. A negative correlation was found between years of experience and level of stress for all the procedures surveyed, but not for the use of nitrous oxide. No differences were found between male and female dentists in stress scores for any of the procedures. Conclusion: Higher rates of stress during operative procedures were reported among dental students than among experienced dentists. Anxiety of the pediatric patients, but not the location of the procedure: maxillary or mandibular, affected the dentists' reported level of stress.

Introduction: In dental schools, the clinical students are closely supervised to ensure a good quality of care delivery. However, little is known about the relation between the patients' satisfaction and the quality of care they received, therefore the aim of this study was to assess the relation between parents' satisfaction and the quality of dental care provided by dental students at IIUM students' polyclinic during paediatric clinical session. Materials and Methods: A self-administered questionnaire on service quality, interpersonal communication, and efficacy and treatment outcome dimensions were distributed to parents who attended their children dental treatment from September 2016 until December 2016. Meanwhile, the students' performances were assessed for their clinical and soft skills by the allocated paediatric dentistry lecturer. Data collected were analysed by using X² test. Results: 42 questionnaires and assessment forms were completed and analysed. The result showed 71.1% of the parents were satisfied with the quality of dental care provided despite the students' performance outcomes were moderate (84.2%). There is no significant difference between parents' satisfaction and the quality of dental care provided by the dental students (p>0.05). Mother showed higher percentage of satisfaction (60%) compared to father (36.4%). However, there is no significant relation between parents' satisfaction and the gender of the parents (p=0.076). Conclusion(s): Generally, parents are satisfied with the dental care provided by the dental students at IIUM Paediatric Dental Clinic regardless of the quality. Hence, parents' satisfaction could not be the best tool to assess the quality of care provided by the operators.

Цель исследования - охарактеризовать микробиологическую картину стоматита до и после его лечения высокоактивными культурами лактобацилл, дать оценку эффективности их применения при экспериментальном стоматите. Методика. Опыты проводились на 32 самках беспородных белых крыс массой 230 г. Моделирование стоматита включало 2 этапа: моделирование травматического стоматита и воспроизведение бактериального стоматита на базе травматического. У крыс контрольной и опытных серий на всех этапах эксперимента брали мазки с поверхности десен для характеристики микробиоценоза ротовой полости. Результаты. После обработки слизистой оболочки полости рта (СОПР) культурой патогенного штамма Staphylococcus aureus существенно снизились частота встречаемости и количество условно-патогенных микроорганизмов. Лечение стоматита у крыс культурами лактобацилл ( Lactobacillus 11 зв., Lactobacillus 2 п.рта, Lactobacillus 24 д.ст.) и их комбинацией приводило к снижению распространенности S. aureus вплоть до его исчезновения, а также к нормализации количества условно-патогенной микробиоты. Заключение. Исследования показали эффективность использования отдельных высокоактивных штаммов лактобацилл или их комбинации при лечении заболеваний СОПР, в частности бактериального стоматита. Aim. To characterize the microbiological picture of stomatitis in intact rats before and after the treatment with highly active cultured lactobacilli and to evaluate the effectiveness of this treatment in experiments on white rats. Methods. Experiments were carried out on 32 mongrel female white rats weighing 230 g. Smears were taken from the gum surface of control and experimental rats at all stages of the experiment to characterize the oral cavity microbiocenosis. Results. After treatment of the oral cavity with cultured Staphylococcus aureus , the occurrence and quantity of opportunistic microorganisms significantly decreased. The treatment of stomatitis in rats with cultured lactobacillus ( Lactobacillus 11 zv., Lactobacillus 2 p.r., Lactobacillus 24 d.st. and their combination) led to a decrease in S. aureus prevalence up to its extinction and to normalization of the quantitative composition of opportunistic microbiota. Conclusions. The study showed the effieacy of highly active lactobacillus strains individually or their combinations in the treatment of oral diseases, in particular, bacterial stomatitis.

The article is devoted to the issue of early orthopaedic rehabilitation of youngest preschool children with ectodermal dysplasia and congenital edentulism. The essence and details of children’s psychophysiological development at this age are revealed, as well as main pedagogical tools making it possible to adapt the children to the specific environment of the dental clinic and motivate them for undergoing long-term multistage orthopaedic dental treatment. Clinical approaches and peculiarities in the dentist’s work with children of different psychological types as well as particular aspects in their adaptation to removable dentures are described. Two clinical cases of effective orthopaedic rehabilitation of 3-years-old children with ectodermal dysplasia and congenital edentulism using full removable lamellar dentures.
