An urgent task in modern restorative therapeutic dentistry is to make a decision in each specific clinical case, which is justified not only from the esthetic and medical, but also from the economic, as well as the biomechanical and ergonomic points of view. According to many authors, the key to the successful restorative treatment of hard tissue defects of any genesis is to understand the etiological causes and conditions of their occurrence, as well as to assess the size of the defect and the level of material and technical base of modern dentistry.Long-term results of esthetic restoration of teeth using the direct method with various composite materials indicated the admission of multiple errors and complications, such as inconsistency in the color and transparency of the filling; violation of the marginal adherence; and the appearance of marginal staining along the border of the composite material with the tooth tissues, both during and in various terms after treatment.In contrast to the direct method of restoration, the ceramic inlay is made indirectly. This allows reducing polymerization shrinkage, which in turn improves marginal adherence and reduces the frequency of caries recurrence.

Aesthetic composite restoration is the most common method for dental hard tissue defects reconstruction. Despite the active development and use of ceramic restorations for dental rehabilitation, the introduction of new nanotechnological filling materials has opened up fundamentally new possibilities in restorative dentistry. This article represents a clinical case of direct composite restoration in teeth 25, 26 and 27 using new nanoceramic materials.

AbstractThis study aimed to describe the survival trajectory of dental restorations placed in an outpatient population of geriatric and adult special needs patients over a 15-year span, with particular interest in longevity of subsequent restorations in teeth that received multiple restorations over time. Dental restorations of different types and sizes in patients age ≥65 years treated between 2000-14 at the University of Iowa, College of Dentistry were followed until they incurred an event (i.e., restoration replacement, extraction of the tooth, or endodontic treatment of the tooth). Survival analysis and extended Cox regression models were used to generate hazards ratios for selected predictor variables. A total of 9184 restorations were followed in 1551 unique patients. During the follow-up period, 28.7% of these restorations incurred an event; and overall the restorations had a median lifespan of 6.25 years. In multivariable regression models, after controlling for gender and age, composite restorations and greater number of restoration surfaces were associated with higher risks of failure; and the initial restoration recorded in the database for each subject tended to have lower risk of failure than restorations placed later that included any of those same surfaces. This information potentially could be helpful to elderly patients considering various restorative treatment options during the dental treatment planning and informed consent process.

The development of an adequate treatment method for urethral obstruction, based on the pathogenesis of the disease and taking into account the anatomical structure of the cat's urinary sys-tem, remains an urgent task of modern veterinary medicine. Cystotomy with suturing of the catheter and subsequent detoxification therapy can achieve complete recovery, as the cause of blockage of the urethra is eliminated.

The aim of this paper is to report a clinical case with bilateral missing mandibular second premolars that were restored by metal-ceramic inlay-retained fixed dental prostheses. The first prosthesis was of a traditional design and the second was of a modified design. The suggested design is created by modifying the retainer wings of the traditional resin-bonded inlay-retained fixed dental prosthesis and covering the wings with composite resin. The modified design is relatively conservative, esthetic and provides an extra element for the retention of posterior metal-ceramic inlay-retained fixed dental prostheses.

Within the dental sphere it is advantageous to be able to bond the restoration to dentine in order to avoid clinical problems such as marginal leakage with subsequent caries. Dentine bonding agents are normally employed to create an effective interface between the restoration and hard tissue with the formation of chemical links. The measurement of the bond strength between dental restorations or implants and surrounding hard tissue is generally achieved through the use of an experimental push-out test. However, these bond strengths have been found to be quite variable between laboratories even when similar chemical bonding agents have been used. This variability may be due to a number of factors including experimental differences such as, material moduli effects, specimen loading and changes in the thickness and properties of the interdiffusional zone (IDZ) that forms following polymerization of the bonding agent. In this work, these variables have been explored using the finite element method initially through a 2-D description of the geometrical set up and subsequently a 3-D discretization. The results indicate that the loading and specimen support during testing have a significant effect on the measured push-out values. Furthermore the effects of a gradually changing modulus in the uppermost rings of the IDZ can have a significant influence on the support bearing property of the bonding agent. These changes in modulus of the IDZ may be the result of fluoride leaching to bring about increases in properties and modulus. A mixed moduli IDZ where the uppermost layer is approximately double the value of the remaining material can lead to increases in push-out values of 10% or more.

For solving the problem of diagnosis and treatment of hard tissue defects the significant role belongs to the choice of tactics for dental treatment of hard tissue defects located in the gingival line of any tooth.This work aims to study the problems of diagnosis and classification of gingival line defects of the teeth hard tissues. That will contribute to the objectification of differentiated diagnostic and therapeutic approaches in the dental treatment of various clinical variants of these defects localization.The objective of the study – is to develop the anatomical-functional classification for differentiated estimation of hard tissue defects in the gingival part, as the basis for the application of differential diagnostic-therapeutic approaches to the dental treatment of hard tissue defects disposed in the gingival part of any tooth.Materials and methods of investigation: There was conducted the examination of 48 patients with hard tissue defects located in the gingival part of any tooth. To assess the magnitude of gingival line destruction the periodontal probe and X-ray examination were used.Results. The result of the performed research the classification of the gingival line defects of the hard tissues was offered using exponent power. The value of this indicator is equal to an integer number expressed in millimeters of distance from the epithelial attachment to the cavity’s bottom of defect.Conclusions. The proposed classification fills an obvious gap in academic representations about hard tissue defects located in the gingival part of any tooth. Also it offers the prospects of consensus on differentiated diagnostic-therapeutic approaches in different clinical variants of location.  This classification builds methodological “bridge of continuity” between therapeutic and prosthetic dentistry in the field of treatment of the gingival line defects of dental hard tissues.

AbstractLess traumatic alternative methods for removal of decayed tissue has been developed. A non-invasive technique, which consists of applying a proteolytic substance that softens the infected dentin tissue and preserves healthy tooth tissue was released in the market. Thus, this work intended to describe a clinical case of caries removal using BRIX 3000 papaine-based enzymatic gel. A 6-year-old male patient attend to a Univesity Dental Clinic complaining of painful sensitivity during mastication. After examination, acute deep carious lesion was found in dental elements 64 and 65. The caries removal procedure was performed for both teeth without anesthesia, under relative isolation, following the principles of atraumatic restorative treatment - ART. The gel was taken into the cavity and leaved to act for 2 minutes and the infected tissue was excavated. Caries lesion was then detected below the OM cusp with undercut enamel, needing to enlarge the cavity in the occlusal region with rotating cutting instruments and reapply the gel. After the second application, all the infected tissue had been removed and the teeth restoration with chemical ionomer cement was performed. At the end of the treatment, the discomfort during the performed procedure was asked to the patient using the Wong-Baker Face Scale. The child pointed to score 1, indicating mild pain for both treatments. It can be concluded that the use of enzymatic papaine gel-based was an efficient alternative for caries removal, easy to use, and preserve to the maximum the dental structure healthy, showing promise to be used as atraumatic caries removal technique.Keywords: Dental Caries. Papain. Pediatric Dentistry.ResumoMétodos alternativos menos traumáticos para remoção de cárie vem sendo desenvolvidos. Uma técnica não invasiva, que consite na aplicação de uma substância proteolítica que amolece a dentina infectada e preserva o tecido dental hígido foi lançado no mercado. Sendo assim, este trabalho visa relatar um caso clínico de remoção da lesão de cárie através do gel enzimático a base de papaína, BRIX-3000. Paciente do gênero masculino, 6 anos, compareceu à Clínica Odontológica Universitária com queixa de sensibilidade dolorosa durante a mastigação. Após avaliação, constatou-se lesão cariosa profunda aguda nos elementos dentais 64 e 65. O procedimento de remoção de cárie foi realizado sem anestesia e sob isolamento relativo, seguindo os princípios do tratamento restaurador atraumático (ART). O gel foi levado à cavidade, deixado agir por 2 minutos e realizado a escavação do tecido infectado. Foi detectado lesão de cárie abaixo da cúspide OM com esmalte socavado, necessitando de ampliação da cavidade na região oclusal com instrumentos cortantes rotatórios e reaplicação do gel. Após a segunda aplicação, todo o tecido infectado foi removido e realizada a restauração dos dentes com cimento de ionômero químico. Ao final do tratamento, foi aplicado a escala de faces de Wong-Baker. A criança apontou score 1, indicando dor leve para ambos os tratamentos. Pôde se concluir que a utilização do gel enzimático a base de papaína é uma alternativa eficiente para remoção do tecido cariado, de fácil utilização, preservando ao máximo a estrutura dental sadia, sendo promissor para ser utilizado na técnica atraumática de remoção da cárie.Palavras-chave: Cárie Dentária.
