The clinical concept "gastritis" has existed for a long time. Nevertheless, the need to create a clinical picture of chronic gastritis has matured, and this is recognized by many authors. For example, they say: There is no plastic characteristic clinical picture of gastritis.

The use of applications of dimethyl sulfoxide in patients with ulcerative disease and chronic gastritis leads to suppression of increased acid secretion, ambivalence with normal acid secretion and has no effect in patients with atrophy of the gastric mucosa. The normalizing effect of dimethyl sulfoxide (DMSO) applications in persons with acidic, decompensated ^ and subcompensated states of the stomach by stimulating the neutralizing function of the antral glands was established. The use of dimethyl sulfoxide as a transporter of various drugs in physiotherapy can be indicated in patients with HC1 hypersecretion in the interdigestive phase of ventricular secretion.

The use of repeated breakfasts to determine the secretory function of the stomach has been known for a long time. Back in 1901, Zimnitsky reported on his experiments with repeated feeding of dogs operated on according to the Heidenhain-Pavlov method, which were given 200 grams of white bread for the first test breakfast and after 2 hours again 200 grams of white bread; later Zimnitsky reported on re-feeding two patients and two healthy people in parallel, who were given one breakfast, consisting of 35 grams. white bread, 250 k. from. tea and after an hour the same breakfast was given again. In 1922 and 1923, Vandorfy published his data on the use of refreshed breakfasts as Belastungsprobe *. In 1922 prof. Zimnitsky reports on his method of studying the gastric cell and during the last decade the method of re-breakfast (meat broth) was promoted by his students to determine the types of gastric cells (normal type, asthenic, inert, isosecretory and torpor) and pointed out the reversibility of these types.

The aim of this work was to study the peculiarities of changes in the levels of cyclic nucleotides (cAMP and cGMP) in peripheral blood during oral administration of solutions with different pH in patients with peptic ulcer disease and chronic gastritis. Studies were conducted in 55 patients, 23 of them had duodenal ulcer, 16 - chronic gastritis with decreased secretory function of the stomach.

An unfavourable prediction of syphilitic aortic lesion is due to its late diagnosis. The key to the success of therapy is in early diagnosis of s. a., when there are no persistent anatomical changes and the main clinical signs have not yet developed. The initial forms of s. a. have been disguised for a long time, distinguishing them from other vascular lesions which symptoms are usually quite definite.

The clinical presentation of paracoccidioidomycosis is spectral. Spontaneous cure, state of latency or active disease with different levels of severity can occur after the hematogenous dissemination. The morphology and number of skin lesions will depend on the interaction of host immunity, which is specific and individual, and fungus virulence. Some individuals have natural good immunity, which added to the low virulence of the fungus maintain the presence of well-marked granulomas with no microorganism and negative serology for a long time, making the diagnosis a challenge. Factors inherent to the fungus, however, may modulate the immune response and modify the clinical picture over the time. We present a sarcoidosis-like clinical presentation and discuss the immunological factors involved.

Tuberculosis osteomyelitis of foot bones is uncommon and may evade the diagnosis for long time because of atypical presentation, paucibacillary nature and relatively normal picture on laboratory investigations. As tuberculosis of bone mimic like chronic osteomyelitis, clinical picture with confection by other organism lead to misdiagnosis. A case of tubercular osteomyelitis of talus reported here for its rarity and atypical presentation. Clinical suspicion and histological confirmation of diagnosis is recommended avoiding undue delay and for early chemotherapy.Kathmandu University Medical Journal Vol.12(4) 2014; 298-300

SUMMARY Twenty-one dogs with spontaneous hyperadrenocorticism presented a clinical picture characterized by an insidious onset of abdominal enlargement, muscular weakness, obesity and alopecia. Slight diabetes mellitus was present in some. In the bitches usually there was absence of oestrus for a long time. In addition to polydipsia and polyphagia, features indicative of heat intolerance were observed frequently. The signs and symptoms found are compared to those of human Cushing's syndrome. Determinations of the excretion of 17-hydroxycorticosteroids in 24-hr. urines were of great diagnostic significance. There was a considerable overlap of levels of plasma 11β-hydroxycorticosteroids in normal dogs and those with hyperadrenocorticism. Results of dexamethasone suppression tests are given. In the sera of four exophthalmic animals and in one pituitary obtained by necropsy, elevated levels of exophthalmos-producing substance were found. Pathological findings have become available in a number of cases. Preliminary results with hypophysectomy in four dogs are encouraging.

Recently increased interest in the issues of syphilis of the nervous system has led to significant advances in the pathogenesis and clinical picture of neurology. In connection with the study of the problems of syphilis, many questions of neuropathology, which remained in the shadows for a long time, received a different explanation, although sometimes, perhaps, not entirely sufficient and still raising doubts. It is precisely such questions that have received a new interpretation that include cases of muscle atrophy in the presence of syphilis.

The paper describes the case of a girl with recurrent cough attributable to the so-called Cough Hypersensitivity Syndrome (CHS). The cough was misdiagnosed as drug resistant asthma for long time. CHS should be considered in all cases where recurrent, spasmodic cough, more than wheezing, predominates in the clinical picture.
