This article describes a rare case of congenital anomalies: giant melanocytic nevus of the face. Errors in the choice of treatment strategy of children with this disease and their complications can lead to poor aesthetic and functional results. When choosing a method of plastic eliminate defects formed after removal of nevi, it is necessary to take into account anatomical features of the maxillofacial region. The department developed the scheme of complex treatment of these patients. Surgical treatment in combination with massage and myogymnastics, regular medical observation, provides a good aesthetic results. Integrated approach in the treatment of children with giant nevi of the face allows for an early medico-social rehabilitation of children with this pathology.

Aim: To present the first report of a large congenital melanocytic nevus with satellite nevi in an apparently healthy child from Sokoto, North-Western Nigeria. Presentation of Case: A three year old girl was brought to the paediatric out-patient clinic of Paediatrics department of Usmanu Danfodiyo University Teaching Hospital (UDUTH) Sokoto with complaints of darkened skin colour on the left side of the face and scalp, the left arm, lower back, buttocks, and thighs, and excessive hair growth over the same side of the face since birth. There were no neurological symptoms Physical examination findings revealed a well-nourished, not ill looking child. She had a hyper pigmented patch on the left side of the face extending from the lower jaw to the scalp, measuring 21 cm in its longest length, with hypertrichosis on the same site, and two distinct, firm, painless nodular lesions on the left temporal region measuring 3 mm and 4mm respectively. On the lower one-third of the left arm was a hairy, velvety area of hyperpigmentation measuring 2X3 cm in diameter. Other affected sites were the lower back, the gluteal region and the thighs. Her neurologic and other systemic examinations were normal. A diagnosis of large congenital facial melanocytic hairy nevus with multiple satellite nevi was made. Discussion: Congenital melanocytic nevi are benign proliferations of melanocytic cells said to be present at birth or in the first two years of life. Large lesions are rare, they measure 20 cm or more and are said to occur more commonly on the trunk and thighs. The exact pathogenesis of congenital melanocytic nevi is yet, unknown. It is thought to occur as a result of a morphological error in the neuroectoderm during embryogenesis. Treatment of patients with large congenital melanocytic nevus may include surgical or non-surgical procedures as well as psychological interventions. Large lesions, multiple satellite lesions or paravertebral and axial locations are sometimes associated with the risk of neurological complications and malignant transformation. Conclusion: Large congenital melanocytic nevi are uncommon skin lesions that can occur in apparently healthy children. Individualization of the patients with regards to treatment options and long term monitoring are imperative.

<p>Congenital melanocytic naevi are neuroectodermal lesions that are mainly composed of melanocytes. They are present in 1% to 6% of all newborns. These lesions carry the risk of transforming into melanomas; however, the psychological effect of such disfiguring naevi is potentially of greater concern to both the child and parent. Several classifications for congenital melanocytic naevi have been proposed, the most common of which is the sub-classification according to their size as this affects the choice of treatment. Many treatment modalities have been utilized including surgical excision followed by reconstruction, curettage, dermabrasion, laser therapy and chemical peels. In this report, we present a case of an otherwise healthy eight-year-old girl with a giant congenital melanocytic naevus on the central face. The lesion was mostly excised with remaining satellite lesions dermabraded. The defect was then reconstructed with a full thickness skin graft harvested from the expanded supraclavicular skin, in addition to the ReCell (non-cultured epithelial autograft) technique. Within six months post-operation, excellent skin pigmentation and texture was achieved.</p>

Every year there is a significant increase in combined anomalies and deformities of the maxillofacial region, congenital pathologies, malignant tumors, severe inflammatory diseases of the face and neck. Almost every patient who underwent surgery on the maxillofacial region develops secondary deformities of the maxillofacial region, adentia of the teeth of the jaws, impaired occlusion and speech, skeletal anomalies, which requires a comprehensive interdisciplinary approach of doctors of different profiles. The aim of the work was to improve the provision of specialized care to patients with pathology of the maxillofacial region through the introduction of an interdisciplinary approach in the treatment of patients in the center of maxillofacial surgery at the N.N. Burdenko Penza Regional Clinical Hospital (N.N. Burdenko ). Material and methods. The study was carried out on the basis of the Center for Maxillofacial Surgery of the State Budgetary Healthcare Institution "Penza Regional Clinical Hospital named after V.I. NN Burdenko ". A retrospective analysis of outpatient patient records, case histories, electronic databases, regulatory and legal documentation from 2010-2020 was carried out. Results and its discussion. Until November 2020, in the Penza region, medical care for patients with diseases of the maxillofacial region was provided in the department of maxillofacial surgery of the N.N. Burdenko State Budgetary Clinical Hospital. When analyzing the reporting and regulatory documents, it was concluded that it is necessary to improve the quality of specialized care in the field of maxillofacial surgery. We propose to employ the following specialists on the staff of the department: dentist-therapist for adults and children, gnathologist, dentist-orthodontist, otorhinolaryngologist, speech therapist. Conclusion. The materials presented in the article reflect the need for an integrated approach and close interaction between the doctors of the center. This stage begins during the preoperative preparation and planning of the operation. Rehabilitation of patients with pathology of the maxillofacial region is aimed at maximum aesthetic and anatomical recovery and requires an individual approach. To solve this problem, it is necessary to introduce interdisciplinary interaction of specialists in the center of maxillofacial surgery.

Introduction: Currently the problems of treating patients with advanced carcinomas of the oral cavity, and especially the reconstructive surgery for this causes, are very relevant. Materials and methods: We analyzed 127 reconstructive operations performed for patients with oral cancer. We used free (90 cases - 71%) and pedicle (37 cases - 29%) flaps for the reconstruction. Results: Good functional results were achieved in most cases (85%). Adequate mobility of the tongue was restored by using skin-fascial flaps. In cases of reconstruction of the maxillofacial region (mandible and maxilla) with the use of bone flaps it was possible to restore the continuity of mandible and maxilla, the natural contour of the face and the opening of the mouth. Conclusions: To achieve good functional and cosmetic results as well as to reduce postoperative complications of reconstructive surgery in patients with oral cavity tumors an adequate reconstructive material should be selected depending on the prevalence of the tumor process, the volume of reconstruction and previous treatment.

Background. Congenital melanocytic nevus is a benign pigmented neoplasm composed of nevus cells that clinically manifest at birth. When choosing a treatment for nevi, the possibility of recurrence as well as the threat of tumor malignancy should be considered. The aim of this work is to justify the surgical removal of a large and giant nevi of the face as a method of treatment justified by the pathomorphological structure. Materials and Methods. In 40 children of different ages born with large and giant nevi of the face, we used various types of plastic surgery to eliminate any defects formed after the excised nevi. We accounted for the features of the maxillofacial area: local plasty, expander dermotension, and transplantation of free skin grafts. We performed a total of 68 surgical interventions. Sixteen patients underwent the surgery once and 24 patients underwent secondary surgery (from 2 to 4). We also developed a scheme of the staged surgical treatments and conservative methods. Results. All patients had stable positive results that were studied by comparing the outcomes of different surgical treatment options and accounting for various morphological characteristics of the removed nevi.

Rapid Maxillary expansion or palatal expansion as it is sometimes called, occupies unique niche in dentofacial therapy. Rapid Maxillary expansion (RME) is a skeletal type of expansion that involves the separation of the mid-palatal suture and movement of the maxillary shelves away from each other. RME effects the maxillary complex, palatal vaults, maxillary anterior and posterior teeth, adjacent periodontal structures to bring about an expansion in the maxillary arch. Morphogenesis and anatomical features of the upper jaw determine the choice of the method of rapid palatal expansion with narrowing of the upper dentition, as an effective method for eliminating congenital deformities of the maxillofacial region. The majority of dental transverse measurements changed significantly as a result of RME. The maturity of the maxillofacial structures determines the timing and degree of success of rapid palatal dilatation treatment.

Relevance. Cleft lip and palate is one of the severe malformations of the face and jaw, requiring a comprehensive approach to the rehabilitation of the patients, including doctors of various specialties, one of which is orthodontists. A feature of orthodontic treatment is difficulty of fixing bracket systems, as well as lowering the level of oral hygiene, caused by deformation and displacement of fragments of the maxillofacial region.Purpose. Improving the effectiveness of orthodontic treatment and hygiene of the oral caviti in patients with cleft lip and palate in permanent bite period.Materials and methods. A comparison was made of the effetctiveness of fixing brackets systmes with direct and indirect bonding techniques and the effectiveness of oral hygiene during orthodontic treatment using irrigators.Results. The results of the study showed a difference in the effectiveness of using direct and indirect methods of fixing bracket systems in patients with cleft and palate. The use of irrigators as additional means of oral hygiene has demonstrated a positive dynamic of hygiene indices.Conclusions. Results of the study demonstrate the advantages of fixation the brackets by indirect bonding and use additional hygiene products irrigator for improving of oral hygiene.
