Introduction. The modern approach to the treatment of unstable fractures of the thoracolumbar and lumbar spine in children is surgical stabilization at the early stages after trauma by using metal structures that quickly restore vertical functionality to the patient and shorten the period of inpatient treatment. However, the issues related to restorative treatment have not been sufficiently addressed. Aim. To develop an algorithm for restorative treatment of children at the inpatient stage after surgical treatment of unstable uncomplicated fractures of the thoracolumbar and lumbar spine. Material and methods. Based on the results of treatment of 73 patients aged 9 to 17 years with unstable uncomplicated vertebral fractures, an algorithm of stage-by-stage rehabilitation by means of therapeutic gymnastics depending on the severity of the injury, method of surgical stabilization of the spine, physical condition of the child, and time passed after the operation was developed. Results and discussion. The use of differentiated groups of respiratory gymnastics and isometric and dynamic exercises for muscle groups restored vertical functionality to patients in 1–3 days after surgery, restored spine and motor functions, and shortened the duration of inpatient treatment to a range of 10–14 days. Conclusion. The developed algorithm for physical rehabilitation of children after surgical treatment of unstable injuries of the thoracic and lumbar spine by using metal structures at the inpatient stage contributed to the selection of the most rational and effective program of restorative treatment.

Surgical treatment of patients with spinal injuries, accompanied by traumatic stenosis of the spinal canal, is an urgent and discussed problem of modern vertebrology. Surgical treatment of 111 patients with various injuries of the thoracolumbar spine was performed. In 40 patients, post-traumatic stenosis was eliminated using ligamentotaxis using transpedicular osteosynthesis. The immediate results of treatment were followed up in all patients: good results were obtained in 33 (82.5%) cases; satisfactory — in 6 (15.0%), unsatisfactory — in 1 (2.5%). Long-term results with a follow-up of more than 1 year were followed up in 27 (67.5%) patients; good results were obtained in 20 (74.0%), satisfactory in 7 (25.0%).

Objective. To evaluate the efficacy of using porous TiNi implants for interbody fusion in the treatment of lumbar spine degenerative diseases.Material and Methods. A total of 570 patients with degenerative lumbar spine disease were operated on with porous TiNi implants. Surgical treatment included stabilization and decompression and stabilization operations through posterior and anterior approaches and endoscopic transabdominal operations. Results of surgical treatment were followed-up for the period of 3 to 24 months. The change of pain syndrome was evaluated using a Visual Analogue Scale (VAS) and Oswestry Disability Index. The degree of interbody bone block formation was assessed based on X-ray and spiral CT data.Results. Functional results of treatment were evaluated at 18-24 months after surgery as good and satisfactory in 94.1% of cases, and the formation of interbody bone-metal block was noted in 94.8% of cases.Conclusion. Nikelid Titanium exhibits good osseointegration properties and can be used as osteoplastic material without additions of bone tissue, which simplifies operation, reduces its traumaticity, and provides good and satisfactory treatment results in 94.1% of cases.

The application of the principles of the integrated use of endoscopic hemostasis and the use of eradication, antisecretory therapy in patients with gastroduodenal bleeding who are inpatient treatment at the Central City Clinical Hospital of Almaty, made it possible to improve the results of treatment of patients, significantly reduce the number of complications after surgery, and reduce the incidence of both general and postoperative mortality.

The literature review summarizes the scientific information devoted to the study of auxiliary methods used in restorative treatment after surgery for fractures of the femoral neck. The restoration of motor functions in patients with cervical hip fractures is a priority task of the comprehensive restorative treatment program. Despite the achievements in the field of osteosynthesis and endoprosthetics, many patients receive an unfavorable outcome of surgery and a group of disabilities. As a result of an improperly selected program of complex rehabilitation treatment or neglect of auxiliary methods, in most cases, patients gradually develop secondary complications from the damaged segment: the formation of articular contractures, muscle hypo- and atrophy, violation of the walking stereotype, ineffective consolidation, fracture and migration of metal structures with excessive load on the limb, etc., as well as the general somatic condition of the patient worsens: secondary hypostatic complications join and chronic diseases a exacerbated. The purpose of the literature review is to systematize data on the use of auxiliary treatment methods in the recovery period of patients who have undergone surgical treatment of hip fractures and their effectiveness. The article discusses the general characteristics of the applied auxiliary methods of restorative treatment, the timing and scope of their implementation, and also presents the results of individual randomized controlled trials on their effectiveness.

Objective To study the radiologic characteristics of facet joints at different stages of the degenerative process in the lumbar spine to determine the indications for various methods of fixing the affected segment and to evaluate the results of treatment.Material and Methods To determine the radiologic aspects of facet joints in the affected area and in  adjacent segments of the lumbar spine, two groups were formed, including 136 patients who underwent multispiral computed tomography in two-energy mode before surgery and 12 months after surgery. Group I included patients who underwent rigid fixation of the spine (360°), and group II included patients who underwent dynamic fixation using nitinol rods (180°).Results Based on a comprehensive instrumental study, it was found that the degeneration of the intervertebral disc according to Pfirrmann II and III revealed an increase in the density of the cartilaginous plate in facet joints (HU). These digital indicators confirm the preservation of joint functionality, both in the affected area and in adjacent segments. With severe degrees of disk degeneration in Pfirrmann IV and V and facet joints, deep pathological changes occurred, directed towards the loss of facet joints functionality.Conclusion The obtained digital indicators of dual-energy computed tomography for the state of facet joints  in combination with the results of magnetic resonance imaging can be used as criteria in a complex of patient studies to assess the degree of degeneration of the vertebral motion segment in the affected area and adjacent segments. We recommend using these criteria as a diagnostic component for finding optimal methods of surgical treatment.

The objectiveis to study the results of different methods of surgical treatment of patients with infected pancreonecrosis and to conduct a comparative analysis of minimally invasive and traditional surgical interventions.Material and methods.The results of treatment of 206 patients with infected pancreonecrosis were analyzed. In accordance with the used method of surgical treatment, the patients were divided into 2 groups: 105 (51.0%) patients with “traditional” open interventions were included in the first group (comparison), and 101 (49.0%) patients treated with various miniinvasive technologies, or a combination of minimally invasive and “open” interventions were included in the second group.Results.It was found that the mortality rate in the second group was less than in the first group by 12.8% (p<0.05).Conclusion.Minimally invasive surgical techniques are the method of choice for delimited pancreatogenic ulcers. The use of combined surgical interventions leads to a significant reduction of postoperative mortality and duration of inpatient treatment.

Summary. The results of treatment of 108 patients with unstable fractures of the pelvic bones and internal organs lesions were provided for the period from 2009 to 2017, 56 of them were victims of the proposed tactic, which allowed to reduce the postoperative mortality by 17,3 %, the frequency of purulent-septic complications by 14,7 %.

Medical, social and economic relevance of inflammatory diseases of the woman's reproductive organs requires a very careful attitude to the diagnosis and treatment of this pathology. The number of patients with genital infections and inflammatory diseases of the pelvic organs can takes the first place in structure of gynecological morbidity, and is 60.4-65.0%, and this fact is not unique to Russia, but all over the world. Incidence rate of inflammatory diseases of the pelvic organs in the first decade of the twenty-first century is increased at 1.4 times in patients who are from 18 to 24 years old and at 1.8 times in patients aged 25-29 years. At the same time, the cost of diagnosis and treatment has increased, reaching 50-60% of the total cost of providing gynecological care for population. The inflammatory diseases of pelvic organs are a collective concept. It includes of various nosological forms. There are numerous contradictions in the views on diagnostic approaches and treatment tactics, the nature of screening and control over the long-term results of treatment, the etiological and pathogenetic significance of various microorganisms found in the genital tract in patients with inflammatory diseases of the pelvic organs. Currently, there are many opinions among specialists about diagnostic approaches and treatment tactics, the type of screening and monitoring the long-term results of treatment, the etiological and pathogenetic role of various microorganisms which can be found in the genital tract in patients with inflammatory diseases. This review presents the results of a modern approach to the diagnosis, management and rehabilitation of patients with inflammatory diseases of the pelvic organs.
