Background: Daytime sleepiness impairs academic performance in college students. Napping is a counter to daytime sleepiness, but often causes sleep inertia on waking up. Caffeine absorption from beverages peaks 30 minutes after their ingestion presenting a window of opportunity to have a short nap such that the time of waking up is in synchrony with onset of action of caffeine; thereby abolishing post-nap inertia and achieving synergistic mitigation of fatigue.Objective of this study to assess effect of nap, coffee, ‘coffee and nap’ and ‘wakeful break without coffee’ on daytime sleepiness using Psychomotor Vigilance Tests (PVTs) and Karolinska Sleepiness Scale (KSS) score.Methods: After Institutional Review Board clearance, 10 subjects (aged 19-21 years) were selected using their Epworth Sleepiness Scale score (ESS >5) and called to the study site 8 times on different days to be exposed to these four conditions twice - only coffee (standardized), only nap (30min), coffee immediately followed by 30min nap, wakeful break (30min) without coffee or nap. Pre and post scores were recorded for electronic PVT (Reaction Time and Motor Responsiveness) and KSS for each attempt.Results: Test outcome was associated with intervention used (p=0.00001). ‘Nap only’ group was associated with deterioration in outcomes (p=0.00001), accounting for highest percentage (41%) of all deteriorated test outcomes. ‘Coffee only’ group was associated with improvement in test scores (p=0.00001), responsible for highest share (38.8%) of all improved test outcomes. ‘Nap only’ and ‘Coffee-nap’ group showed improvement in 11.67% and 21.67% of outcomes respectively. Conclusions: Pre-nap coffee is a proactive counter-measure to post nap sleep inertia.

Abstract Introduction Overweight and obesity are a major public health concern in the United States, including among active duty military personnel. Approximately 51% of active duty personnel are classified as overweight and 15% are classified as obese. This may impact military readiness. The current study aimed to determine if a weight loss intervention impacted fitness test scores among Air Force personnel. Materials and Methods From 2014 to 2016, 204 Air Force members with overweight/obesity were randomized into either a Self-paced or counselor-initiated arm in a weight loss program. Study procedures were approved by the Institutional Review Board of the 59th Medical Wing in San Antonio and were acknowledged by the Institutional Review Board at the University of Tennessee Health Science Center. Fitness test scores from before, during, and after the intervention were used to determine if the intervention resulted in improvements in overall fitness test ratings and scores on individual components of the test. Results Participants who lost at least 5% of their weight had better fitness ratings during the intervention compared to individuals who did not lose 5%. However, in the overall sample, fitness ratings worsened from preintervention to during the intervention, and from during to postintervention. Participants with overweight had better aerobic scores pre- and postintervention as well as better abdominal circumference scores and better fitness test ratings preintervention, during the intervention and postintervention compared to participants with obesity. Conclusions Behavioral weight management interventions that achieve 5% weight loss may help improve military fitness test ratings.

Abstract Introduction We evaluated the effectiveness of New Orientation for Reducing Threats to Health from Secretive-problems That Affect Readiness (NORTH STAR), a community assessment, planning, and action framework to reduce the prevalence of suicidality, substance problems, intimate partner violence, and child abuse. Materials and Methods One-third of U.S. Air Force bases worldwide were randomly assigned to NORTH STAR (n = 12) or an assessment-and-feedback-only condition (n = 12). Two Air Force-wide, cross-sectional, anonymous, web-based surveys were conducted of randomly selected samples assessing risk/protective factors and outcomes. This study was reviewed and approved by the institutional review board at the investigators’ university and by the institutional review board at Fort Detrick. Results NORTH STAR, relative to control, bases experienced a 33% absolute risk reduction in hazardous drinking rates and cumulative risk, although, given the small number of bases, these effects were not statistically significant. Conclusions Given its relatively low cost, use of empirically supported light-touch interventions, and emphasis on sustainability with existing resources, NORTH STAR may be a useful system for prevention of a range of adult behavioral health problems that are difficult to impact.

AbstractObjectivesThis study aimed to assess the sleep quality (habits and disorders) and the daytime sleepiness among medical students.MethodsA cross-sectional questionnaire-based study was conducted during September 2018, through November 2018 at the Faculty of Medicine, Tanta University, Egypt. The study recruited undergraduate Egyptian and Malaysian students and applied a modified form of two questionnaires, namely the Sleep Habits and Life Style and the Epworth Sleepiness Scale (ESS)”. Statistical analysis was done using SPSS. The results were expressed as frequency, percentage, and mean ± standard deviation (SD). Chi-square test was used to explore associations between categorical variables. An independent sample t-test was used to detect the mean differences between groups. Ordinal regression analyses were done on the ESS findings in relation to demographics and sleep habits. p-values<0.05 were accepted as statistically significant.ResultsThe study included 899 medical students. Most of the participants were Egyptians (67%), rural residents (57.4%), and in the preclinical stage (79.5%). Males represented 66.0% of the study participants and participants average age (SD) was 21.98 (1.13) years. The average durations (SD) of night sleep were 7.3 (1.6) hours in work days and 8.7 (2.1) hours during the weekends. Both were significantly longer among young (<21 years-old) and preclinical students (p<0.05). Students had on average (SD) 1.33 (0.29) hours duration of napping, but 60% of the participants never or rarely scheduled for napping. Larger proportion of male and Malaysian students sometimes scheduled for napping more significantly than their peers (p<0.05). Only 16.24% of students reported that the cause of daytime napping was no enough sleep at night. The students reported sleep disorders of insomnia in the form of waking up too early, trouble falling asleep, or waking up at night with failure to re-sleep (31, 30, and 26%, respectively). Snoring (22.2%) and restless legs (22.0%) were also reported by the students. High chances of dozing off was reported by 22.02% of the participants, of which 10% used sleeping pills, 41.4% suffered psychological affection, and 34.8% reported life pattern affection. We found an increased chance of daytime sleepiness among males (0.430 times) and Egyptian (2.018 times) students. There was a decreased chance of daytime sleepiness in students from rural areas and those below 21-years-old (0.262 and 0.343 times, respectively). Absence of chronic diseases suffering was significantly associated with 5.573 more chance of daytime sleepiness or dozing off. In addition, enough and average sleep at night significantly decreased the chance of daytime sleepiness by 6.292 and 6.578, respectively, whereas daytime consumption of caffeinated beverages significantly decreased the chance of daytime sleepiness by 0.341.ConclusionThere was unbalanced sleep duration in work days and weekends as well as lack of scheduling for napping among the students. Sleep disorders as insomnia, snoring, and restless legs were associated with excessive daytime sleepiness. Some students who suffered daytime sleepiness also underwent psychological and life pattern affection including taking sleeping pills. Enough and average sleep duration at night as well as daytime consumption of caffeinated beverages decreased the chance of daytime sleepiness.

Background Coronavirus-2 (COVID-19) has caused a worldwide pandemic since December 2019. Since then, clinical trials with vaccines have been started and completed, and at present, 3 COVID-19 vaccines have been approved for use in the United States. Healthcare providers were among the first to get vaccinated, but the precise attitudes of healthcare workers toward vaccination are uncertain. Objective To understand residents and fellows’ attitudes toward vaccination and record any side effects after vaccination. Methods We conducted an anonymous survey that was open from 3-1-2021 to 3-12-2021 using distribution lists from the Graduate Medical Education office on the Lubbock campus of the Texas Tech University Health Sciences Center after getting approval from the Institutional Review Board (L21-088). Results Eighty-one residents and fellows (26.6% out of 304) responded to our survey. Among those who responded, 63 (77.8 %) were between 25 and 35 years old, and 41 (50.6%) were males. Seventy-seven (95.1%) received the vaccine (Pfizer-BioNTech), 78 (96.3%) reported that they supported vaccination, and 3 (3.7%) reported that they did not want vaccination. Eight members (9.8%) had tested positive for COVID-19 infection before vaccination, but only 1 (1.23%) had tested positive for COVID-19 antibodies. All residents and fellows reported side effects after the vaccination, including pain at the injection site (77; 100%), local redness (9; 11.6%), local swelling (13; 16.8%), fever (25; 32.5%), fatigue (25; 32.5%), chills (34; 44.1 %), headache (38; 49.4%). Conclusions Most medical trainees have a high interest in COVID-19 vaccination; however, a few reported that they did not want vaccination.
