Background Coronavirus-2 (COVID-19) has caused a worldwide pandemic since December 2019. Since then, clinical trials with vaccines have been started and completed, and at present, 3 COVID-19 vaccines have been approved for use in the United States. Healthcare providers were among the first to get vaccinated, but the precise attitudes of healthcare workers toward vaccination are uncertain. Objective To understand residents and fellows’ attitudes toward vaccination and record any side effects after vaccination. Methods We conducted an anonymous survey that was open from 3-1-2021 to 3-12-2021 using distribution lists from the Graduate Medical Education office on the Lubbock campus of the Texas Tech University Health Sciences Center after getting approval from the Institutional Review Board (L21-088). Results Eighty-one residents and fellows (26.6% out of 304) responded to our survey. Among those who responded, 63 (77.8 %) were between 25 and 35 years old, and 41 (50.6%) were males. Seventy-seven (95.1%) received the vaccine (Pfizer-BioNTech), 78 (96.3%) reported that they supported vaccination, and 3 (3.7%) reported that they did not want vaccination. Eight members (9.8%) had tested positive for COVID-19 infection before vaccination, but only 1 (1.23%) had tested positive for COVID-19 antibodies. All residents and fellows reported side effects after the vaccination, including pain at the injection site (77; 100%), local redness (9; 11.6%), local swelling (13; 16.8%), fever (25; 32.5%), fatigue (25; 32.5%), chills (34; 44.1 %), headache (38; 49.4%). Conclusions Most medical trainees have a high interest in COVID-19 vaccination; however, a few reported that they did not want vaccination.

Objective:To collect information about the safety of taking antiretroviral drugs for human immunodeficiency virus (HIV) postexposure prophylaxis (PEP).Design:A voluntary, confidential registry.Setting:Hospital occupational health clinics, emergency departments, private physician offices, and health departments in the United States.Results:492 healthcare workers (HCWs) who had occupational exposures to HIV, were prescribed HIV PEP, and agreed to be enrolled in the registry by their healthcare providers were prospectively enrolled in the registry. Three hundred eight (63%) of 492 of the PEP regimens prescribed for these HCWs consisted of at least three antiretroviral agents. Of the 449 HCWs for whom 6-week follow-up was available, 195 (43%) completed the PEP regimen as initially prescribed. Forty-four percent (n=197) of HCWs discontinued all PEP drugs and did not complete a PEP regimen. Thirteen percent (n=57) discontinued ≥1 drug or modified drug dosage or added a drug but did complete a course of PEP. Among the 254 HCWs who modified or discontinued the PEP regimen, the two most common reasons for doing so were because of adverse effects attributed to PEP (54%) and because the source-patient turned out to be HIV-negative (38%). Overall, 340 (76%) HCWs with 6-week follow-up reported some symptoms while on PEP: nausea (57%), fatigue or malaise (38%), headache (18%), vomiting (16%), diarrhea (14%), and myalgias or arthralgias (6%). The median time from start of PEP to onset of each of the five most frequently reported symptoms was 3 to 4 days. Only 37 (8%) HCWs with 6-week follow-up were reported to have laboratory abnormalities; review of the reported abnormalities revealed that most were unremarkable. Serious adverse events were reported to the registry for 6 HCWs; all but one event resolved by the 6-month follow-up visit. Fewer side effects were reported by HCWs taking two-drug PEP regimens than by HCWs taking three-drug PEP regimens.Conclusions:Side effects from HIV PEP were very common but were rarely severe or serious. The nature and frequency of HIV PEP toxicity were consistent with information already available on the use of these antiretroviral agents. Clinicians prescribing HIV PEP need to counsel HCWs about PEP side effects and should know how to manage PEP toxicity when it arises.

While the new Health Insurance Privacy and Accountability Act (HIPAA) research rules governing privacy, confidentiality and personal health information will challenge the research and medical communities, history teaches us that the difficulty of this challenge pales in comparison to the potential harms that such regulations are designed to avoid. Although revised following broad commentary from researchers and healthcare providers around the country, the HIPAA privacy requirements will dramatically change the way healthcare researchers do their jobs in the United States. Given our reluctance to change, we risk overlooking potentially valid reasons why access to personal health information is restricted and regulated. In an environment of electronic information, public concern, genetic information and decline of public trust, regulations are ever-changing. Six categories of HIPAA requirements stand out as transformative: disclosure accounting/tracking, business associations, institutional review board (IRB) changes, minimum necessary requirements, data de-identification, and criminal and civil penalties.

Interviewing is a means of engaging an individual in dialogue to reflect upon and share his or her life experience. For health humanities, this method accesses the lived reality of patients and healthcare providers. Asking people to share their personal narratives can allow for emic—from the subject’s perspective—and etic—from the researcher’s point of view—interpretation. Health humanities interviews consist of six steps: define the research question, design the interview, apply for Institutional Review Board approval, conduct the interviews, analyze the data, and distribute the findings. This chapter examines best practices for conducting interview studies including format (structured, unstructured, semi-structured), question type (closed- or open-ended), sampling (convenience, snowball), and notetaking. The author uses a study on collecting death histories to demonstrate this process and how to apply narrative, thematic, and frequency analyses.

OBJECTIVES: The objective of this study was to evaluate the safety and efficacy of polyethylene glycol-electrolyte solution vs polyethylene glycol-3350 for the treatment of fecal impaction in pediatric patients. METHODS: A retrospective, observational, institutional review board–approved study was conducted over a 1-year time period. Patients were included in the study if they were admitted to the hospital with a diagnosis of fecal impaction or constipation and were treated with either polyethylene glycol-electrolyte solution (PEG-ES) or polyethylene glycol-3350 (PEG-3350). Patients were excluded if they were discharged prior to resolution of treatment and/or did not receive PEG-ES or PEG-3350. RESULTS: Fifty-one patients (ranging in age from 1 month to 15 years) were evaluated: 23 patients received PEG-ES and 28 patients received PEG-3350. Sex, race, age, and weight were not statistically different between the 2 groups. Resolution of fecal impaction was not significantly different between PEG-ES vs PEG-3350 (87% and 86%, respectively; p = 0.87). There was only 1 reported side effect with PEG-3350, vs 11 reported side effects with PEG-ES (p < 0.01). CONCLUSIONS: Theses results suggest that PEG-3350 is as effective as PEG-ES for the treatment of fecal impaction in pediatric patients and is associated with fewer side effects.

The institutional review board (IRB) process is often protracted and can be a source of frustration, especially when you want your research and publications to move apace. However, because of historical events, the IRB is an important requirement for conducting research with human participants and is regulated by federal oversite. When conducting research as part of a pedagogical study, it is important to identify which level of IRB review (exempt, expedited, or full board) is required. The purpose of this article is to highlight IRB basics within the United States for pedagogy research. Although there are guidelines internationally, this article specifically focuses on U.S. IRBs, including a brief history of the IRB, pedagogical and community-based participatory research, IRB review, tips for IRB submissions, and example case studies.

Background: The novel coronavirus disease-2019 (COVID-19) has become a significant worldwide problem since January 2019. Hospitals have spent most of their time and logistics on patients with COVID-19. During this crisis, many healthcare providers have been infected with the disease, and occasionally, some wards and operating rooms were shut down as a result. Here, we explain our experience with the healthcare staff involvement with COVID-19 in our hospital. Methods: As a referral tertiary center, Imam Khomeini Hospital (Tehran, Iran) has 4,200 health-care workers (HCWs). From February 20, 2020 to August 21, 2020, we investigated the hospital database for COVID-19 involvement among the staff. Results: During the study period, 973 (23%) hospital HCWs were detected with COVID-19, 378 (9%) of whom were involved between June 21 and July 21, 2020. In the orthopedic department, 20 of 43 (46%) HCWs were infected with COVID-19. Conclusion: We believe that the increase in the incidence of the disease and higher risk of exposure is a highly noticeable factor which should be addressed by the administrative health officials.

IntroductionA cohort of 12 000 children in the Philippines who had enrolled in a 2000–2004 (current ages 16 to 20 years) Phase 3 11-valent pneumococcal conjugate vaccine for the prevention of radiographically confirmed pneumonia are now being asked to participate in a separate study (expected completion date September 2021) to assess the cohort’s current long-term audiometric and otologic status. This new study would allow assessments of the utility of the pneumococcal vaccine in conferring its protective effects on the long-term sequelae of otitis media (OM), if any. Lack of trained local healthcare providers in otolaryngology/audiology and testing equipment in Bohol, Philippines, necessitates the development of a distinct methodology that would lead to meaningful data analysis.Methods and analysisReliable data collection and transfer are achieved by a US otolaryngologist/audiologist team training local nurses on all procedures in a didactic and hands-on process. An assortment of portable otolaryngologic and audiologic equipment suitable for field testing has been acquired, including an operating otoscope (Welch-Allyn), a video-otoscope (JedMed), a tympanometer with distortion product otoacoustic emission measurements (Path Sentiero) and a screening audiometer (HearScreen). Data will then be uploaded to a Research Electronic Data Capture database in the USA.Tympanometric and audiologic data will be codified through separate conventional algorithms. A team of paediatric otolaryngology advanced practice providers (APPs) have been trained and validated in interpreting video otoscopy. The protocol for classification of diagnostic outcome variables based on video otoscopy and tympanometry has been developed and is being used by APPs to evaluate all otoscopy data.Ethics and disseminationThe study was approved by the Research Institute of Tropical Medicine, Alabang, Manila, Philippines, and the institutional review board and the Colorado Multiple Institutional Review Board of the University of Colorado School of Medicine, Aurora, Colorado, USA.Research results will be made available to children and their caregivers with abnormal audiologic outcomes, the funders and other researchers.Trial registration numberISRCTN 62323832; Post-results.

Background: Healthcare workers’ have increased risk of contracting the deadly COVID-19 pandemic due to their exposures to infected persons and their specimens. Ghana, like many other countries, has felt the devastating effects of this virus; it was therefore prudent to assess the levels of knowledge, perception and practice of Ghanaian healthcare providers on the current pandemic.Methods: This descriptive cross-sectional study enlisted 979 healthcare workers’ in Ghana from 1st Apr to 20th Nov, 2020, via self-reported questionnaire. SPSS version 22.0 was used for the analysis. Responses were summarized using frequency and proportions. A chi-square test was utilized to test for association at significant level of p <0.05.Results: Out of the 979 healthcare workers recruited, 56.4% had good knowledge and 59.5% displayed good perception on the novel COVID-19 while 63.1% of them effectively practiced the expected precautionary measures. Age (p=0.001), gender (p=0.034) and profession (p<0.001) significantly affected the practice of precautionary measure among the health personnel. The respondents’ perceptions towards the pandemic were significantly linked to their age (p<0.001), gender (p=0.043), profession (p<0.001), type of occupation (p=0.001) and source of information (p=0.006). Also, knowledge significantly related to good practices but did not influence the perception of HCWs.Conclusions: The current study identifies that more than half of HCWs in Ghana have sufficient knowledge, perception, and practice of precautionary measures; however, it elucidates some significant concerns about the knowledge gap in this COVID-19 outbreak. There is an obvious need for progressive in-service training programs for the health workers to broaden their scopes on the risks and preventive measures.

The purpose of this study was to investigate school administrators’ knowledge of appropriate instructional practices in physical education. For this study 130 k-12 school administrators from two states in the United States were surveyed regarding their knowledge of appropriate instructional practices in physical education. University Institutional Review Board granted approval to conduct this study. Surveys were sent electronically to the school administrators to take. At the completion of the survey the school administrators were able to click a submit button and have the surveys returned to the researchers. It was found that school administrators identified dodge ball, relay races and elimination tag as an appropriate instructional practice. Also, full-sided games (30 students divided into two teams) were appropriate for students to participate in. These instructional practices are considered inappropriate for students to participate in. The implications of this study are to inform school administrators what practices are appropriate and inappropriate. It is hoped that physical educators and school administrators will work together to see that appropriate instructional practices are taught in physical education, thus benefitting students to become competent in their participation in physical activity. 本研究的目的是探討學校管理者對體育的教學實踐知識。在這項研究中,邀請兩個州份的130位 K-12學校行政人員進行調 查,並獲大學機構審查委員會准許進行這項研究。調查以電子方式發送給學校管理者採取。結果發現,學校的管理者確定躲避球和接力賽作為適當的教學實踐。希望體育教師和學校管理者將共同努力,看到合適的體育教學實踐,從而惠及學生參與體能活動。
