Background: The objective of the study was to evaluate surgical and aesthetic outcomes of bilateral reduction mammoplasty (RM) through the inferior pedicle with inverted-T approach (PO).Methods: 23 women with bilateral symptomatizing macromastia underwent preoperative breast measurements, calculation of breast volume (BV), determination of quality of life (QoL) and self-esteem scorings using the breast reduction assessed severity scale (BRASS) and the Rosenberg self-esteem scale (RSES). RM was performed and intraoperative and PO data were collected. At 3-m PO, aesthetic outcome was evaluated using the ABNSW score, which assesses breasts asymmetry and shape, nipple deformation, skin condition and wound scar, and QoL outcome and overall satisfaction were determined. Results: Mean operative time was 240.4±39.7 min and mean weight of excised breast tissue was 2778.3±307.7 gm. Minor PO complications were managed conservatively and no patient required revision surgery. PO breast measurements were significantly improved with decreased BV and increased breast projection. Median ABNSW score was 10 and PO median BRASS and RSES scores were 9 and 17 with significant change in comparison to preoperative data. Preoperative BRASS score was negatively correlated age and body mass index, while RSES score was positively correlated with age. Nine women were highly satisfied by aesthetic outcome, 10 women were satisfied and 16 women were highly satisfied with overall outcome. Conclusions: RM significantly improved women QoL, aesthetic appearance and psychological status. Inferior pedicle with inverted T-mammoplasty is a safe, and applicable pattern of mammoplasty, allow significant reduction of BV with improved breast measurements and projection and provide satisfactory aesthetic outcome.

Aim This study aimed to review the literature with regard to the psychological aspects in patients with pectus excavatum (PE) and pectus carinatum (PC). Methods The literature was reviewed by analyzing articles on PubMed using the search terms “psychology” and “pectus.” Results The literature search revealed 22 articles that offered a total of 2214 patients for analysis. Regarding chest wall deformities (CWD), there were 15 articles referring to PE, 4 articles on PC, and 3 pertaining to both PE and PC. Authors used various types of questionnaires and 14 studies which also included a parent questionnaire. There are 14 reports which analyzed both the preoperative and postoperative psychological status and 4 reports in which a control group was used. Treatment of CWD was shown to have consistently positive results. There is no consensus regarding the correlation between severity of CWD and psychological issues. Only 2 studies found no statistically significant results after treatment. Conclusions The literature reveals a great concern for psychosocial issues in CWD patients. Most articles describe improvement in overall quality of life (QOL) after surgery. An increase in social and physical function, body image, and self-esteem was observed in CWD patients, which is supported by parent responses.

The high prevalence of dental anomalies and deformities requires a careful approach to orthodontic treatment, especially to improve quality of life. The purpose of the study was to determine the effectiveness of orthodontic care by index assessment of the results of correction of dental anomalies and determination of the psychosocial profile in adolescents of the Transcarpathian region. Material and methods. The object of the study were 68 adolescents from the Transcarpathian region. Determination of the correction of dental and maxillary apparatus anomalies was performed by calculating the relevant part of the ICON index, calculating the evaluation of the results. The quality of life of adolescents was assessed using the OHIP-14 quality of life questionnaire. Statistical analysis was performed after consolidation of results using the application package Statistica 10.0 (StatSoft, Inc., USA) and Microsoft Office Excel 2010. Results and discussion. Many studies in the field of psychology have determined that self-esteem is a core characteristic of the individual and together with the level of claims is a regulator of human activity and significantly affects its development. The result of self-esteem depends on how a person evaluates his/her success in joint activities where he/she is a member. This is especially true for teenagers and adolescence. The most important aspect of impaired quality of life in patients of this subgroup were problems of psychological and social nature, issues of external attractiveness, which prevailed even over the physical aspects of the impact, which is quite typical of adolescence. In patients with moderate orthodontic treatment there was a statistically significant improvement on the scales of psychological discomfort (from 6.4±0.6 points to 3.96±0.6 points; p <0.05), psychological disability (from 6.5±0.5 points to 2.18±0.5 points; p <0.05) and social disability (from 5.4±0.5 points to 3.36±0.5 points; p <0.05). Patients in this group after solving problems related to the aesthetics of the smile, as well as discomfort during communication, felt greater psychological comfort, relief in interaction with others and peers. There was a noteworthy shift of accentuation in such patients from problems with appearance to study and active social life. Among patients with severe orthodontic treatment, the most pronounced dynamics was observed on all scales of dental quality of life: on the scale of masticatory dysfunction (from 5.7±1.0 points to 3.12±1.0 points; p <0.05), physical pain (from 7.5±0.7 points to 2.01±0.7 * points; p <0.05), psychological discomfort (from 7.9±0.6 points to 3.48±0.6 points; p <0.05) physical disability (from 7.7±0.5 points to 3.20±0.5 points; p <0.05), psychological disability (from 7.5±0.4 points to 3.41±0.5 points; p <0.05). Conclusion. According to the study results, the success of orthodontic treatment (indicators of severity and effectiveness of orthodontic treatment according to the ISO index) directly correlated with the level of quality of life (r = 0.62; p <0.05)

Abstract Background Reduction mammoplasty effectively improves quality of life for women with macromastia. However, little is known whether surgical- or patient-related factors affect satisfaction. Objective To investigate factors associated with altered patient satisfaction following reduction mammoplasty. Methods A cross-section study was performed by sending the BREAST-Q Reduction module to all patients, whom had undergone reduction mammoplasty between January 2009 and December 2018 at two tertiary Danish hospitals. Demographics, pre-, per- and postoperative details were gathered from electronic medical records. Results 393 patients returned the questionnaire and were eligible for the study. Increasing age at the time of surgery was associated with higher satisfaction with breasts (p<0.001), nipples(p<0.001), headache(p <0.05), psychosocial well-being(p<0.001), and outcome(p<0.05). Increased BMI at the time of surgery negatively affected satisfaction with breasts(p<0.05) and psychosocial well-being(p<0.05). Increase in BMI after surgery was further associated with lower satisfaction with breasts(p<0.05), nipples(p<0.05), sexual well-being(p<0.05), and more pain in the breast area(p<0.05). Postoperative scar revision and wound infection was more common following inferior pedicle technique than superomedial technique(p<0.05) and negatively affected satisfaction with outcome(p<0.05) and pain in the breast area(p<0.05). Conclusion Patients should be motivated to optimize their weight prior to reduction mammoplasty to achieve optimal satisfaction. Furthermore, reduction mammoplasty can benefit obese patients by facilitating subsequent weight loss for additional satisfaction. Age was associated with improved patient satisfaction and this should considered when operating on younger patients. Postoperative complications affect patient’s satisfaction and the superomedial technique seems to be a better choice than the inferior pedicle technique in medium-large breasts.

Hidradenitis suppurativa (HS) is a chronic, debilitating skin condition that negatively affects patients’ quality of life. Indirect self-destructiveness refers to activities extended over time, such as addictions, risky behaviors, neglects, resignation, helplessness. These can be an additional factor impeding the achievement of positive clinical effects in the treatment of HS patients, therefore the objective of the study was to assess the indirect self-destructive behaviors in patients suffering from HS. The study group involved 100 adult HS patients with 59 males and 41 females. Indirect self-destructiveness was investigated with the Polish version of the Kelley’s Indirect Self-Destructiveness Scale (CS-DS). The study revealed that the average total score of indirect self-destructiveness in HS population was 130.16 ± 21.3 (median 128 points). The CS-DS scores were significantly higher in smoking patents (p = 0.006). The most expressed class of indirect self-destructiveness was A5 (Helplessness and Passivity). The indicated results pointed out a strong domination of passive forms of indirect self-destructiveness over its active forms. Due to related low self-esteem, social isolation and exclusion, HS patients are more prone to behave in a self-destructive manner, which may lead to poor health maintenance in a form of leaving appointments and non-adherence.

BackgroundThere is little knowledge of the predictors of objective and subjective quality of life.AimsTo describe changes at 2 and 6 years in objective and subjective quality of life in 261 individuals attending a community mental health service and to identify predictors of change in each life domain.MethodProspective study of demographic, diagnostic and service utilisation characteristics, psychopathology, functioning, disability, self-esteem, affect balance and service satisfaction.ResultsFemale gender, unmarried status, older age, less education and greater disability predicted a worsening of objective quality of life over time, but explain a small amount of variance. The variance in subjective quality of life was higher (>40%). Greater clinician-rated anxiety and depressive symptoms had a negative effect on satisfaction with health and general well-being. Psychological status, self-esteem and satisfaction with service were the most important predictors in almost all subjective domains; these variables should be important targets for treatment.ConclusionsThis study is the first to provide information to enable clinicians to make prognostic judgements about quality of life and plan effective therapeutic strategies to improve quality of life.
