In spite of scientific achievements in diagnostics and treatment of the diseases of nose and paranasal sinuses in children, this problem remains important. Acute bacterial rhinosinusitis occupies one of the leading positions among ENT diseases in terms of seeking medical care, and often results in the development of orbital and intracranial complications. Rhinogenic orbital complications in children occur more frequently than in adults, where 10–22% are purulent-septic orbital processes with a possible outcome of blindness. The article presents a clinical case of acute purulent rhinosinusitis complicated with a right orbit phlegmon in a four months old infant. Timely diagnosis, early surgical treatment made it possible to stop the inflammatory process, to avoid the development of more severe complications. The specific features of diagnostics and treatment of acute rhinosinusitis in infants, occurring with orbital complications, predetermines the need for an interdisciplinary approach of doctors of different specialties.

Purpose: The purpose of this study was to investigate the epidemiology and microbiology of severe acute bacterial rhinosinusitis in patients admitted to a Danish tertiary hospital. Methods: A retrospective study was performed including all cases of acute rhinosinusitis admitted to the ENT-department of Aarhus University Hospital, in the period 2001 to 2011. Results: In total, 143 cases of acute rhinosinusitis were identified of which 51% were children. The most prevalent bacterial strains were Staphylococcus aureus followed by group A streptococcus, Haemophilus influenzae and Streptococcus pneumonia. Morexella catarrhalis was only rarely cultured. Anerobes were not assessed routinely. Of all patients, 47.8% presented with orbital complications and 2.1% developed intracranial complications. Patients infected with group A streptococcus had significantly higher leukocyte and neutrophil counts than other patients. All cultured S. aureus were resistant to penicillin, but sensitive to methicillin. Conclusion: Our results imply that S. aureus and group A streptococcus are important pathogens in severe and complex cases of ABRS, in addition to the accepted pathogens S. pneumoniae and H. influenzae. On the contrary M. catarrhalis appears less significant. These findings have important implications regarding the selection of relevant treatment strategy in secondary care, which may currently be underestimating the role of S. aureus.

Рецензируемая книга Александра  Михайловича  Терехина «Сумасшествия в  музыкальном театре: опера, балет», изданная в  2020 году, уникальна во  многих отношениях. Во-первых, интерес вызывает личность самого автора. Профессиональная и  творческая деятельность Терехина связана с  медициной и  музыкой. Врач-психиатр с  сорокалетним стажем, Терехин более четверти века работал в Мариинском театре в качестве артиста миманса. Во-вторых, тема сумасшествия (безумия) еще не  становилась самостоятельным предметом изучения в российских музыковедческих исследованиях. В-третьих, оригинальность книги составляют медицинские заключения. В центре внимания врача — тексты оперных и балетных либретто, на основе которых автор раскрывает причины помешательства и  ставит различные диагнозы персонажам музыкального театра. Рассматривая конкретные клинические случаи, Терехин прибегает к профессиональным медицинским терминам. Среди диагнозов отметим такие, как реактивный параноид у Лючии («Лючия ди Ламмермур» Г. Доницетти), шизофрения у Мельника («Русалка» А. С. Даргомыжского, интоксикационный (гашишный) психоз у Солора («Баядерка» Л. Минкуса). Книга А. М. Терехина, не претендуя на всеохватность освещения темы сумасшествия (безумия), открывает музыковедам новые грани междисциплинарного подхода в изучении оперного и балетного репертуара. Alexander Mikhailovich Terekhin’s peer-reviewed book Madness in Musical Theater: Opera, Ballet, published in 2020, is unique in many ways. Firstly, the author’s personality is of interest. Terekhin’s professional and creative activity is connected with medicine and music. A psychiatrist with forty years of experience, Terekhin worked for more than a quarter of a century at the Mariinsky Theater as a mimance artist. Secondly, the theme of madness (insanity) has not yet become an independent subject of study in Russian musicological studies. Thirdly, the originality of the book is based on medical reports. The doctor focuses on the texts of opera and ballet librettos, on the basis of which the author reveals the causes of insanity and makes various diagnoses to the characters of the musical theater. Considering a specific clinical case, Terekhin resorts to professional medical terms. Among some diagnoses we can mention: reactive paranoid in Lucia (Lucia di Lammermoor by Donizetti), schizophrenia in Melnik (Rusalka by Dargomyzhsky, intoxicational (hashish) psychosis in Solor (La Bayadere by Minkus). Without claiming to cover the topic of insanity (insanity) comprehensively the book by Terekhin, opens up new facets of the interdisciplinary approach in studying opera and ballet repertoire to musicologists.

Dryness of the mucous membranes (in mouth, nose, genitals in women) and skin are the most common complaints of patients with Sjogren’s disease — a multisystem immune‑mediated disorder that causes anxiety and concern associated with the difficulties of initial diagnosis. The authors present a clinical case of a patient with primary Sjogren’s syndrome, a large number of symptoms of digestive diseases, complaints of dryness of mucous membranes, in particular the oral cavity. The diagnosis was confirmed by allied specialists such as ophthalmologist, gynecologist, rheumatologist, dentist. Detailed dental examination revealed the main dental complaints (constant dry mouth, galitos, changes in taste), clinical condition (dryness of the red border of the lips, swelling of the oral mucosa, changes in the relief of the gums), decreased secretory activity and significant reduction of unstimulated salivation with a moderate level of stimulated salivation. It is noted that a significant decrease in sialometry (0.01 ml per minute) and functional activity of the small salivary glands (up to 18) confirms the presence of true xerostomia. Micro‑crystallization of saliva revealed disorganization of irregularly shaped structures, as well as a significant number of organic inclusions, which indicates a violation of homeostasis in the oral cavity. Given the results of ultrasound examination of the large salivary glands and the absence of enlargement and swelling of the glands during clinical examination, we can assume that the destructive damage to the glands is not so pronounced, evacuation function of the gland is not reduced, but the amount of secretion is significantly reduced. To provide medical aid to patients with Sjogren’s syndrome, the accessibility of the qualified consultation is important and required, with conduction of comprehensive examinations by allied specialists, including a dentist.

Acute rhinosinusitis is one of the most common diseases in the world. According to statistics, about 10 million cases are registered in Russia every year.Acute bacterial rhinosinusitis (ABRS) in most cases develops as a result of an acute respiratory viral disease, proceeds easily and it does not require antibiotic therapy. However, it significantly reduces the patient's quality of life in cases of moderate and severe disease, and in the absence of timely treatment, can lead to intracranial complications (meningitis, encephalitis, intracranial abscesses, etc.) and orbital (reactive edema of the eyelid, phlegmon of the orbit, periorbital abscesses, etc.). To avoid complications, with exacerbation of rhinosinusitis, it is necessary to choose the right antibiotic therapy, taking into account the resistance of the microorganisms that caused the disease.The article describes the differential diagnosticprinciples of bacterial rhinosinusitis according to the latest recommendations and research in this area, the most common antibiotic-resistant pathogens in the practice of an ENT doctor are presented, the issue of choosing the correct systemic antibiotic therapy for the treatment of patients with moderate and severe acute bacterial rhinosinusitis is considered according to modern data on the sensitivity of bacterial pathogens to antibacterial drugs prescribed in the otorhinolaryngologist's practice. The article presents data on a new universal cephalosporin of the 3rd generation (the active ingredient is cefditoren), as a second- and third-line drug, due to its high activity against most causative agents of acute bacterial rhinosinusitis, and clinical observation using the above antibacterial agent within the framework of the considered topics.
