Рецензируемая книга Александра  Михайловича  Терехина «Сумасшествия в  музыкальном театре: опера, балет», изданная в  2020 году, уникальна во  многих отношениях. Во-первых, интерес вызывает личность самого автора. Профессиональная и  творческая деятельность Терехина связана с  медициной и  музыкой. Врач-психиатр с  сорокалетним стажем, Терехин более четверти века работал в Мариинском театре в качестве артиста миманса. Во-вторых, тема сумасшествия (безумия) еще не  становилась самостоятельным предметом изучения в российских музыковедческих исследованиях. В-третьих, оригинальность книги составляют медицинские заключения. В центре внимания врача — тексты оперных и балетных либретто, на основе которых автор раскрывает причины помешательства и  ставит различные диагнозы персонажам музыкального театра. Рассматривая конкретные клинические случаи, Терехин прибегает к профессиональным медицинским терминам. Среди диагнозов отметим такие, как реактивный параноид у Лючии («Лючия ди Ламмермур» Г. Доницетти), шизофрения у Мельника («Русалка» А. С. Даргомыжского, интоксикационный (гашишный) психоз у Солора («Баядерка» Л. Минкуса). Книга А. М. Терехина, не претендуя на всеохватность освещения темы сумасшествия (безумия), открывает музыковедам новые грани междисциплинарного подхода в изучении оперного и балетного репертуара. Alexander Mikhailovich Terekhin’s peer-reviewed book Madness in Musical Theater: Opera, Ballet, published in 2020, is unique in many ways. Firstly, the author’s personality is of interest. Terekhin’s professional and creative activity is connected with medicine and music. A psychiatrist with forty years of experience, Terekhin worked for more than a quarter of a century at the Mariinsky Theater as a mimance artist. Secondly, the theme of madness (insanity) has not yet become an independent subject of study in Russian musicological studies. Thirdly, the originality of the book is based on medical reports. The doctor focuses on the texts of opera and ballet librettos, on the basis of which the author reveals the causes of insanity and makes various diagnoses to the characters of the musical theater. Considering a specific clinical case, Terekhin resorts to professional medical terms. Among some diagnoses we can mention: reactive paranoid in Lucia (Lucia di Lammermoor by Donizetti), schizophrenia in Melnik (Rusalka by Dargomyzhsky, intoxicational (hashish) psychosis in Solor (La Bayadere by Minkus). Without claiming to cover the topic of insanity (insanity) comprehensively the book by Terekhin, opens up new facets of the interdisciplinary approach in studying opera and ballet repertoire to musicologists.

Dryness of the mucous membranes (in mouth, nose, genitals in women) and skin are the most common complaints of patients with Sjogren’s disease — a multisystem immune‑mediated disorder that causes anxiety and concern associated with the difficulties of initial diagnosis. The authors present a clinical case of a patient with primary Sjogren’s syndrome, a large number of symptoms of digestive diseases, complaints of dryness of mucous membranes, in particular the oral cavity. The diagnosis was confirmed by allied specialists such as ophthalmologist, gynecologist, rheumatologist, dentist. Detailed dental examination revealed the main dental complaints (constant dry mouth, galitos, changes in taste), clinical condition (dryness of the red border of the lips, swelling of the oral mucosa, changes in the relief of the gums), decreased secretory activity and significant reduction of unstimulated salivation with a moderate level of stimulated salivation. It is noted that a significant decrease in sialometry (0.01 ml per minute) and functional activity of the small salivary glands (up to 18) confirms the presence of true xerostomia. Micro‑crystallization of saliva revealed disorganization of irregularly shaped structures, as well as a significant number of organic inclusions, which indicates a violation of homeostasis in the oral cavity. Given the results of ultrasound examination of the large salivary glands and the absence of enlargement and swelling of the glands during clinical examination, we can assume that the destructive damage to the glands is not so pronounced, evacuation function of the gland is not reduced, but the amount of secretion is significantly reduced. To provide medical aid to patients with Sjogren’s syndrome, the accessibility of the qualified consultation is important and required, with conduction of comprehensive examinations by allied specialists, including a dentist.

Purpose Projects involving collaborations between different institutions require data security via selective de-identification of words or phrases. A semi-automated de-identification tool was developed and evaluated on different types of medical reports natively and after adapting the algorithm to the text structure. Materials and Methods A semi-automated de-identification tool was developed and evaluated for its sensitivity and specificity in detecting sensitive content in written reports. Data from 4671 pathology reports (4105 + 566 in two different formats), 2804 medical reports, 1008 operation reports, and 6223 radiology reports of 1167 patients suffering from breast cancer were de-identified. The content was itemized into four categories: direct identifiers (name, address), indirect identifiers (date of birth/operation, medical ID, etc.), medical terms, and filler words. The software was tested natively (without training) in order to establish a baseline. The reports were manually edited and the model re-trained for the next test set. After manually editing 25, 50, 100, 250, 500 and if applicable 1000 reports of each type re-training was applied. Results In the native test, 61.3 % of direct and 80.8 % of the indirect identifiers were detected. The performance (P) increased to 91.4 % (P25), 96.7 % (P50), 99.5 % (P100), 99.6 % (P250), 99.7 % (P500) and 100 % (P1000) for direct identifiers and to 93.2 % (P25), 97.9 % (P50), 97.2 % (P100), 98.9 % (P250), 99.0 % (P500) and 99.3 % (P1000) for indirect identifiers. Without training, 5.3 % of medical terms were falsely flagged as critical data. The performance increased, after training, to 4.0 % (P25), 3.6 % (P50), 4.0 % (P100), 3.7 % (P250), 4.3 % (P500), and 3.1 % (P1000). Roughly 0.1 % of filler words were falsely flagged. Conclusion Training of the developed de-identification tool continuously improved its performance. Training with roughly 100 edited reports enables reliable detection and labeling of sensitive data in different types of medical reports. Key Points:  Citation Format

The family, as the basic microstructure of social life, constitutes, at the very least by supposition, the first and foremost educational environment of a child; an environment which should correspond to the child’s natural needs, especially psychological ones, such as the need for love, unconditional acceptance, the need for respect and recognition, activity, independence, and self-realization [9]. This why M. Lukšienė states, “A good family home is the basis of human physical and spiritual life; it guarantees one’s efficient, creative activity”. The parents’ worldview plays an important role in education, providing an answer to the fundamental questions concerning our existence: Who am I? What sense does my life have? What is the goal of my life? What ideals and rules of action do I represent? What is the world and my place within it? Undoubtedly, a variety of factors affect the shaping of a young person’s worldview, among them school, religion, ideology, the level of social development, social organization, the particular historical period in which the person lives, as well as certain individual psychological predispositions. Nevertheless, parents have always played the pivotal role in the shaping of children’s worldview. Parents prepare their children for an independent life in society by teaching them values, norms, models of behavior, and cultural customs. In this way, parents fulfil their function as a microstructure, fulfilling aims that support the macrostructure. Beginning with the second half of the 19th century, the notion of the family becomes one of the main themes in Polish literature, appearing in the works of such authors as Bolesław Prus, Henryk Sienkiewicz, Eliza Orzeszkowa, Stefan Żeromski, Władysław Reymont, and Maria Konopnicka. Moreover, the literary representation of the family as an educational environment of a child is also often evoked in the theoretical musings of professionals in the field of education. Thse professionals include Helena Radlińska, Jerzy Ostrowski, Janusz Korczak, and Aleksandra Kamiński. In conclusion, the broadly defined notion of family is, without question, universal and timeless. As an area of scholarly inquiry, it requires an interdisciplinary approach that encompasses all possible perspectives.

Background. Primary tumoral calcinosis is an orphan disease. There are few data in the literature on the incidence of this disease, as well as clinical recommendations for treatment. Clinical case. This report presents the case of an 11.5-year-old boy with primary tumoral calcinosis and equinus deformity of the foot. The patient had multiple foci of the subcutaneal calcification, cannot walk, experienced fatigue, and had high fever and equinus deformity of the left foot. Immunological and genetic studies were performed, but any specific mutations were not found. After the diagnosis was verified and interleukin-1 inhibitor therapy was prescribed, there was a significant positive trend observed in the patient: a significant improvement in the patients general condition, a decrease in the number of calcinates, and a reduction in inflammation. Calcification of the Achilles tendon and gastrocnemius muscle was the cause of the deformity of the left foot. Discussion. Significant improvement was achieved during treatment: the boy started walking, fatigue was decreased, no new calcificates were formed, and inflammation was under the control. Using an inhibitor of interleukin-1 as a permanent therapy of primary tumoral calcification allowed performsurgical treatment without complications from an operation site, as well as a relapse of deformity. Conclusion. The clinical case presented here demonstrated the application of an interdisciplinary approach to the treatment of an extremely rare disease.

The article presents a rare clinical case of hamartoma of the tongue in combination with a cleft palate, diagnosed in a newborn child, as a manifestation of a severe congenital malformation of the maxillofacial region. The preparation and successful implementation of the first stage of treatment of this combined pathology and subsequent rehabilitation in a multidisciplinary children's hospital are described in detail on the basis of continuity in the work of specialists and an interdisciplinary approach.

Abstract The limited accountability achieved to date for sexual violence in conflict has largely reflected one specific form: the rape of women and girls. Investigation, prosecution and convictions for other forms of sexual violence have lagged behind significantly. This is the case for sexual violence against men and boys. Using detailed data from over 130 expert medical reports of Syrian male former detainees, the article contrasts known typology of male sexual violence in conflict with penal codes and case law across a range of jurisdictions. It also considers the broader challenges for access to justice, reparation and recovery for such victims and survivors of these crimes. The article then turns to examine how these gaps might be bridged to provide better access and outcomes for justice to male survivors of conflict-related sexual violence. It explores a local Syrian interdisciplinary approach using medical expert documentation which has opened up investigation and awareness of male sexual violence. Lastly, it sets out concluding recommendations for approaching male sexual violence in national investigations and prosecutions.

In spite of scientific achievements in diagnostics and treatment of the diseases of nose and paranasal sinuses in children, this problem remains important. Acute bacterial rhinosinusitis occupies one of the leading positions among ENT diseases in terms of seeking medical care, and often results in the development of orbital and intracranial complications. Rhinogenic orbital complications in children occur more frequently than in adults, where 10–22% are purulent-septic orbital processes with a possible outcome of blindness. The article presents a clinical case of acute purulent rhinosinusitis complicated with a right orbit phlegmon in a four months old infant. Timely diagnosis, early surgical treatment made it possible to stop the inflammatory process, to avoid the development of more severe complications. The specific features of diagnostics and treatment of acute rhinosinusitis in infants, occurring with orbital complications, predetermines the need for an interdisciplinary approach of doctors of different specialties.

This study aimed to test the algorithm of complex clinical, functional, and instrumental analysis in the context of treatment for patients with a need of complete dental reconstruction. Materials and methods. A rehabilitation program for patients with adentia was proposed, which was based on the objective examination data obtained comprehensively with multiple diagnostic methods. Particular attention was paid to the temporomandibular joint function and symptoms of maxillofacial muscle abnormalities. Results. The clinical case report covers the results of the examination and treatment of a patient who previously received implants in one of the clinics without an appropriate examination and an operating template. A set of diagnostic and therapeutic procedures was defined that included clinical, functional, and instrumental analysis of condylographic and cephalometric data. The outcomes from treatment and rehabilitation allowed the optimal occlusal load distribution, and the oral hygiene improved. The approach that was used here enabled a timely correction of functional and esthetic disorders. Conclusions. The algorithm to treat patients who need complete dental reconstruction should include asking the medical history, clinical instrumental analysis (condylography), recording and evaluating the static and dynamic occlusal characteristics, and assessing cephalometric and esthetic characteristics. The algorithm is anatomically and pathogenetically justified, since it takes into account all alterations and interconnections between dentofacial structures and other body systems that underlie the clinical manifestations in patients with adentia who have a need for complete dental reconstruction.

Представлен клинический случай коморбидной патологии у пациента с оказанием специализированной помощи в многопрофильной медицинской организации города Сургута, первично диагностированного как COVID-инфекция. Описанный клинический случай, требовал междисциплинарного подхода в диагностике и лечении пациента и представляет пример эффективности работы многопрофильной больницы. A clinical case of comorbid pathology in a patient with specialized care in a multidisciplinary medical organization of the city of Surgut, initially diagnosed as a COVID infection, is presented. The described clinical case required an interdisciplinary approach in the diagnosis and treatment of the patient and is an example of the effectiveness of a multidisciplinary hospital.
