The aim of this study was to evaluate the accuracy on the diagnosis of secondary caries-like lesions simulated on esthetic restorations of different materials, changing the incidence vertical angle (IVA) of the x-ray beam. Twenty human teeth received MOD inlay preparations. In the experimental group (n=10), a round cavity was made in the floor of the proximal box to simulate the caries-like lesion. All teeth were restored with 3 composite resins (Charisma, Filtek-Z250 and TPH-Spectrum) at 3 moments. Two radiographic images were acquired with 0º and 10º IVA. Ten observers evaluated the images using a 5-point confidence scale. Intra- and interobserver reliability was analyzed with the Interclass Correlation Coefficient and the diagnostic accuracy was evaluated using the area under the ROC curve (Az), Friedman test and Wilcoxon test (α=0.05). Higher accuracy values were obtained with 10º IVA (Az=0.66, Filtek-Z250>Az=0.56, TPH-Spectrum) compared to 0º (Az=0.55, Charisma>Az=0.37, TPH-Spectrum), though without statistically significant difference (p>0.05). The detection of secondary caries-like lesions simulated on esthetic restorations of different materials suffered no negative influence by changing the IVA of the x-ray beam.

SUMMARY This study aimed to objectively evaluate the radiopacity of different dental composites and their subjective influence on diagnosing secondary caries–like lesions and how these results correlate. For objective analysis, three resin specimens (1 mm thick, with a 4-mm internal diameter) were made with four composites: 1) Charisma; 2) Filtek Z250; 3) Prisma AP.H; and 4) Glacier. Three human teeth were selected and then mesio-distally sectioned (1 mm thick) to make the dental specimens. An aluminum (Al) wedge (12 steps, 1 mm thick, 99.8% purity) was used as an internal standard to calculate the radiopacity. For subjective analysis, 20 human teeth were selected and then prepared with a mesio-occluso-distal (MOD) inlay cavity, with half the teeth receiving a round cavity to simulate the carious lesion. The MOD was restored using the composites at four different times. Standardized radiographs were acquired and then digitized (300 dpi and eight-bit TIFF) for both analyses. A histogram objectively measured the pixel intensity values of the images, which were converted into millimeters of Al using linear regressions. Eight observers subjectively evaluated the images using a five-point rating scale to diagnose the caries. The data were statistically analyzed using the Student t-test, the Kappa test, diagnostic testing, and the Pearson correlation coefficient (α=0.05). All materials showed radiopacity values compatible with dental tissues (p>0.05); Glacier was similar to dentin and Prisma AP.H was similar to enamel, while the remaining materials showed a middle radiopacity. Prisma AP.H and Glacier differed (p<0.05) in relation to their accuracy to caries diagnosis, with Glacier having greater accuracy. There was a correlation between objective and subjective analyses with negative linear dependence. An increase in the material's radiopacity could have a subjectively negative influence on the diagnosis of secondary caries; thus, an ideal radiopacity for a dental composite is closer to the dentin image and produces similar attenuation to X-rays than does dentin.

Objective. The chewing group of teeth is intended for equally crusher of food and forming food lump. This group is presented by two kinds of teeth: premolars and molars, and more often affected by caries, because a lot of retention zones are contribute to the active delay of plaque and the appearance of carious cavities. It is happening before 18 years. In the absence of these teeth or badly damaged of the crown part, food is chewed badly and as a result, stomach contain extremely large pieces. But its size is impossible for physiological digestion by the gastrointestinal tract and as a consequence the occurrence of gastrointestinal diseases. According to modern data, to work with lesions of hard tissues of the teeth, namely the chewing group of teeth, it is advisable to use composite light-curing microhybrid materials, which are characterized by particles close to the spherical shape. The aim of this study is determination of the possibility using a budgetary aesthetic composite for restorative restoration of the masticatory group of teeth in patients with a burdened anamnesis. Material and methodology. Was examined 22 people with aesthetic and anatomical disorders of the vestibular and occlusal surface of the hard tissues of the chewing group of teeth (n=154), followed by their recovery. The stability of filling materials was assessed according to Ryge-criteria: color coincidence, discoloration of the marginal segment, marginal adaptation, anatomical shape integrity, restoration surface, secondary caries. Results. Clinical and dental examination showed all the above violations of existing restorations (according to Ryge classification) in patients before restoration manipulations. After 1 week after the restoration, the examination showed that both groups of composites completely restore the anatomical and aesthetic chewing group of teeth and meet all the criteria. After 3 months, the assessmen t of the restoration by the composite of group A and group B did not reveal evidence of secondary caries on the perimeter of the restorations. Clinical and dental examination after 6 months showed minor changes in the restorations of composites of both groups in assessing the stability of color (90.9% and 83.3% in groups A and B) and the stability of surface polishing (89.6% and 81.8%, respectively). Discussion. The main signs of the restoration, which are undergoing changes, are discoloration, as well as the preservation of the smoothness of the surface regardless of the cost and branding of the base material.

Introduction. Critical review of the available medical scientific sources demonstrates that despite the wide range of advantages, photopolymer composite materials have certain disadvantages. The main of those are contraction during polymerization and incomplete polymerization of the filling volume. These disadvantages cause the crevice between the direct restoration and the tooth wall, the possible discoloration can reduce tooth hardness. Some researchers recommend the application of inlays in order to prevent complications while restoring caries cavities in posterior teeth. Complete polymerization and reliable adjacency to dental tissues ensure sustainability of indirect photopolymer restoration and prevent secondary caries in restored areas. Objective. The study is aimed at raising the effectiveness of dental treatment by clinical evaluation of the quality of direct and indirect restorations made from composite materials. Methods and Materials. In order to conduct comparative clinical evaluation 90 patients with defects of coronal part of lateral teeth, aged 19-54 have been examined, who have orthognathic occlusion, don’t have dental and facial malformations or muscular and joint dysfunctions. They underwent 189 restorations of lateral teeth with cavities of O, MO, OD, MOD types due to medium or severe caries or because of dissatisfying condition of the previously inserted fillings. The patients were divided into two groups according to the method of treatment. The I (basic) group included 42 patients, who underwent 76 indirect restorations for reconstructing the defects of teeth crowns. The II (test) group included 54 patients, who underwent 113 direct restorations for reconstructing the defects of teeth crowns. Each group was subdivided into two subgroups. The patients who underwent 38 indirect restorations with photopolymer composite material Filtek Z 250 (3M ESPE) were placed in the subgroup I A, the patients who underwent 38 indirect restorations with material Charisma (Heraeus Kulzer) were placed in the subgroup I B. The patients who underwent 57 direct restorations with photopolymer composite material Filtek Z 250 (3M ESPE) were placed in the subgroup II A, the patients who underwent 56 direct restorations with material Charisma (Heraeus Kulzer) were placed in the subgroup II B. Clinical efficiency of both direct and indirect restorations was evaluated immediately after their application and then after 12 and 24 months with the help of visual and instrumental examination. When secondary caries was suspected, the target X-rays were taken with Heliodent Vario (Sirona). The evaluation was conducted according to the Ryge criteria (1998), approved by USPHS (United States Public Health Service). They include the following indicators: anatomic contour, marginal integrity and marginal coloration, secondary caries, surface texture, preservation of the restoration, restoration fracture, crevice at the edge of restoration, restoration abrasion, tooth fracture, no approximal adjacency, overhanging restoration, localized gingivitis. Results. The comparative analysis of the clinical efficiency of lateral teeth restorations made from photopolymer materials under study revealed significantly higher quality of indirect restorations (composite inlays), which demonstrated good clinical results even in long-term perspective. Only 5,3% of patients with indirect restorations (with both Filtek Z 250 and Charisma) underwent partial restoration in order to prevent further destruction. By contrast, the patients with direct restorations required the partial restoration in 7% with Filtek Z 250 and 8,9% with Charisma and needed immediate replacement of fillings in 10,5% and 17,8% correspondingly.
