Data of the Federal Information Fund for Social Hygiene Monitoring conducted on the basis of the Federal Center for Hygiene and Epidemiology of Rospotrebnadzor have been analyzed to assess environmental risks, as well as to develop environmental safety system as a factor for sustainable development of the territory in the regions of the Russian Federation. Atmospheric air pollution in Russian regions was evaluated by content of priority pollutants. Ranking of Russian regions according to the quality of drinking water supply was carried out. The possibility of using Federal Information Fund for Social Hygiene Monitoring as an integral part of the model for optimizing the social and environmental conditions of populated areas is estimated.

There are described an algorithm and methods for collecting the evidential base for causing hazard to the health of the population of Zakamensk (the Republic of Buryatia) in the zone of influence of the waste of the past activities of the Dzhidinsky tungsten-molybdenum combine. The source of danger is lying sands, containing a complex of toxic heavy metals - lead, cadmium, manganese, nickel, chromium, etc. The algorithm for the formation of the evidential base included: a general analysis of the sanitary and hygienic situation; assessment in the dynamics of the quality of atmospheric air, drinking water, soil, food; assessment of the risk to the health of the population; epidemiological studies; in-depth clinical surveys of a representative group of city residents (280 people). In the blood of the examinees, heavy metals were identified as biological markers of the exposure. There was evaluated a complex of the response (effect) adequate to effects of laboratory markers. By methods of mathematical statistics there was established the presence of reliable relationships in the system “source of hazard - the quality of the environment - exposure to risk factors - health status.” The hazard to the health of a citizen, due to the negative impact of the factor, was considered to be proven if it was possible to build a continuous step-by-step logical chain of relationships between the source of the harmful effect, the quality of the habitat and the establishment of the fact of the disease. The quality of the habitat of the population of Zakamensk was shown to give rise in unacceptable risks to the health of residents. The main influencing factor is the pollution of atmospheric air, local food products and drinking water from public and private wells. Risks are realized in terms of a level of the primary morbidity rate of the population of the respiratory diseases, endocrine system, digestive organs that is relatively high compared to the matched area. The relationship between health hazard and environmental factors has been fully proved in 14% of the surveyed persons. At the same time, the territory was shown not be characterized as a zone of “ecological disaster” or “an emergency environmental situation”.

Minor towns with population size of 50-100 thousand people are much rarely studied in regards of the ecological-hygienic assessment of human environment, in spite of the fact that they are characterized by specific ecological problems. The objective of the study was the hygienic assessment of the impact of urban environment on population health status in Tikhvin and Gatchina towns of the Leningrad region. Morbidity rate in the population of Tikhvin and Gatchina in 2005-2015, atmospheric emissions of industrial enterprises, findings on monitoring for both atmospheric air pollution and centralized drinking water supply, as well as motor transport noise levels were studied. Population health risk stipulated by industrial emissions and noise exposure was calculated. Statistical data treatment with correlation analysis of the relationship between environmental pollution indices and population morbidity was carried out. The study revealed increased atmospheric air pollutant concentrations in Gatchina, which might be connected with motor transport effect. Acute non-carcinogenic risk levels resulting from air pollutants are on the borderline of the admissible level, while in Tikhvin town those values are significantly lower. Estimated concentrations of atmospheric pollutants produced by industrial emissions are by 1.5-2 times lower than the actual levels shown by monitoring. Noise levels and predicted health risks in Gatchina are on the borderline of the admissible level, while in Tikhvin they are significantly reduced. The statistically significant gain in the general primary morbidity rate indices due to respiratory diseases in Gatchina town, correlated with atmospheric air pollution, was revealed. Our study showed that despite the presence of large industrial enterprises in studied towns, motor transport proved to be the key factor that forms air pollution and noise load, more typical of large cities.

Background: Vertigo is defined as an illusion of motion caused by a mismatch of information between the visual, vestibular and somatosensory systems. The most common diagnosis associated with whirling vertigo is benign paroxysmal positional vertigo (BPPV), which affects approximately 3.4% of patients older than 60 years of age. Objective: This paper aims to educate primary care providers on how to diagnose BPPV by performing canalith repositioning maneuvers at the initial point of care. Timely treatment of BPPV in the primary care office is believed to reduce healthcare costs by way of limiting unnecessary diagnostic testing and lowering referrals for specialty care. Immediate treatment is also believed to improve the quality of healthcare delivery for the vertigo patient by reducing morbidity and resolving the condition without the need for referrals or imaging. Population Health: A review of the literature finds that delayed diagnosis and treatment of BPPV is associated with a host of deleterious effects on patients. Population health impacts include increased rates of anxiety and depression; loss of work and/or change of career paths; inappropriate use of medications or emergency care resources; decreased access to healthcare services; increased healthcare costs; and reduced quality of care. Diagnosis: A history of positional vertigo and evidence of nystagmus with Dix-Hallpike positioning confirms the diagnosis. A detailed description of the performance of this test is elucidated. Treatment: The observed nystagmus is analyzed and classified based on directionality. Treatment can be initiated immediately with canalith repositioning maneuvers.

. Environmental pollution affects population health depending on the extension and the degree of exposure to environmental factors. In most cases it is difficult to obtain an accurate situation of exposure of population to harmful factors. Health status is determined by: human biology, ecological factors, the socio-economic situation of each person, and the quality of medical services. In according to regional peculiarities the interdependence of those factors could vary, but not significantly. The environmental risks are everywhere but diminishing them may improve the health status of the population.
