A clinical case of a combination of ulcerative colitis with cirrhosis in the outcome of primary sclerosing cholangitis in a twelve-year-old child is presented. The uniqueness of the clinical observation lies in the atypical onset of ulcerative colitis in the form of complaints of weakness and headache, as well as detected anemia of 3 severity. It is important that the child lacked diarrhea, blood in the stool, tenesmus, weight loss, and fever. Of the most characteristic signs of liver damage, only itching of the skin of the lower extremities was noted. As a result, the child was diagnosed with cirrhosis of the liver at the end of the extraintestinal manifestation of ulcerative colitis — primary sclerosing cholangitis.

We studied the prognostic significant clinical, hemodynamic, hemostasiological, immunological characteristics, markers of endothelial dysfunction, changes in lipid profile in patients with atherosclerosis obliterans of the femoral-femoralpopliteal-tibial localization. As a result, multivariate analysis the pathogenetically substantiated prediction system with the calculation of the index of the disease. When the index of less than 13 points for obliterative atherosclerosis assessed as non progressive, at index 13 points, and more-progressive.

Ahterosclerosis of vessels of the lower extremities is widely distributed in the population. Open surgical revascularization is an efficient method for treatment of this pathology. One of the rare postoperative complications is an anastomosis aneurysm development. We report about a clinical case of a 74-year-old male with a giant (25 cm in diameter) aneurysm of the proximal anastomosis after femoropopliteal bypass. Open aneurysmectomy was performed, and the postoperative period was uneventful.

This article presents the results of a study of the immune status of 46 patients with lower extremities atherosclerosis obliterans before and after reconstructive surgery. To assess the nature of the immunologic changes the content of the markers were determined in systemic circulation and subcutaneous veins of the rear foot serum. It was established that patients suffering obliterating atherosclerosis of lower extremities had dysimmunoglobulinemia between the characteristics of systemic and local blood flow. The high content of markers of chronic inflammation - immunoglobulins of classes A, G, anti-inflammatory cytokines IL-1, IL-6 was detected especially in local blood flow both in pre and post-operative periods. The marked decrease was found in immunoglobulin class A in local blood flow compared to the pre-operative values by 6.87% (p=0.02), to 11.52% (p<0.05), 12,4% (p<0.05) after femoral-popliteal, aorta-femoral and interventional treatment respec-tively. The authors observed a decrease in circulating immune complexes after the reconstructive surgery in the systemic and local blood flow. A statistically significant decrease in circulating immune complexes was revealed in local blood flow after femoral-popliteal bypass surgery in 2 times (p=0.00001) and after angioplasty with stenting on 37,03% (p=0.0005).

Relevance. Trophic ulcers on the background of systemic diseases are characterized by long flow and cause greater difficulties in treatment due to the reduced regenerative capabilities of the body. As a result of taking hormones muscles and skin become atrophic changes. Thinning and dry skin are particularly susceptible to trauma. Trophic ulcers in systemic connective tissue diseases often have no specific signs. Indicate systemic disease can atypical localization (thighs, buttocks, torso, upper extremity, head, mucosa of the oral cavity) wound, long for ulcers, with no tendency to regenerate, the big help in diagnostics renders the identification of syndromes, evidence of systemic autoimmune lesions of organs and tissues (polyarthritis, polyserositis, damage internal organs). Purpose: description of the case of the treatment of the patient with Sjogren disease. Materials and methods. In this report a clinical case of treatment of ulcers of the tibia on the background of long-term hormonal therapy. Patient more than 20 years of suffering from Sjogren's disease. Diagnosis of Sjogren’s disease (chronic, moderate activity (II) expressed stage), dry keratoconjunctivitis, parenchymatous parotitis, xerostomia, Raynaud's Syndrome, trophic ulcer of left tibia and varicose disease of the lower extremities, 2. Sjogren’s disease - a systemic autoimmune disease characterized by involvement of exocrine glands, mainly salivary and lacrimal, with the gradual development of secretory failure, combined with various systemic manifestations. Local treatment of trophic ulcers took place against the background therapy of the underlying disease. In the first stage for cleansing the surface of trophic ulcers used for the contact controlled cryodestruction was carried out hydropressing treatment of the wound. The result - on the 5th day the wound was completely cleansed from necrotic tissue. Further, the local treatment consisted in the creation of optimal conditions for regeneration. After 4 weeks the wound is almost completely epithelization. Conclusions. When Sjogren's disease treatment of ulcerative defects of the skin of the lower extremities takes place on the background of long-term hormonal medications. An important condition for wound healing is to decrease the dose of hormones to the minimum necessary. In this clinical observation in local treatment was applied the technique of contact controlled cryodestruction of pathological tissues on the surface of the ulcer.

This article presents a clinical case demonstrating the correction treatment of the lower extremities in a patient with Issacs syndrome, as well as the results of ankle surgery and successful post-surgery rehabilitation. The high efficiency of surgery as a treatment method was confirmed.

Capillary hemangiomas are benign, vascular lesions of skin and mucous membranes that often occur in infancy and childhood. Capillary hemangiomas are most commonly found in the head and neck region. Capillary hemangiomas that occur in adults and on the lower extremities are uncommon. A clinical case involving surgical treatment of the lesion on an adult foot is presented.
