IntroductionSeveral studies have argued a causal relationship between diabetes and depression, while others have highlighted that their association is a result of common risk factors. Because Mexico is a country with a high prevalence of diabetes, and diabetes and depression are a frequent comorbidity, we chose this country to investigate the longitudinal relationship of these two conditions, focusing on the influence of demographic, health, and socioeconomic factors which could act as common risk factors for both conditions.Research design and methodsUsing the harmonized Mexican Health and Aging Study, a nationally representative sample of adults older than 50 with a response rate of 93%, we analyzed the longitudinal relationship of diabetes and depressive symptoms using ‘between-within’ random-effects models, focusing on the effect of demographic, socioeconomic and health factors.ResultsWhile older adults with diabetes reported a higher prevalence of depressive symptoms in the four waves of the study, there was no causal longitudinal association between them once controlling for demographic, socioeconomic and health factors (between-effect OR=0.88, 95% CI 0.77 to 1.01; within-effect OR=0.87, 95% CI 0.69 to 1.11).ConclusionsThere is no causal longitudinal association between diabetes and depression; the higher prevalence of depression among older adults with diabetes seems a result of socioeconomic and health factors that are not exclusive to respondents with diabetes but are more frequent in this group. Our results highlight the importance of prevention and control of chronic conditions as well as the role of socioeconomic inequalities in mental health.

Abstract Background: To discern if prevalence of depressive symptoms in adults aged 65 and above in Taiwan changed during the past decade and to identify if protective and risk factors of depressive symptomatology differ by gender. Method: Data of nationally representative older adults (65+) interviewed from the 2005, 2009 and 2013 National Health Interview Survey (NHIS) in Taiwan was analyzed (n=8,832). The Center for Epidemiologic Studies Depression Scale (CES-D) was used for the measurement of depressive symptomatology. Results: Age adjusted prevalence rate of depressive symptomatology among older adults in Taiwan reduced from 20.6% to 13.3% (X2=-7.5, p<.05) in the community. The most significant factors associated with higher depressive symptomatology in both gender was too much carbohydrates intake, which was significantly associated with 8.8 (95%CI=5.1-15.2) and 7.9 (95%CI=5.2-11.8) times depressive symptomatology in men and women respectively. Factors associated with lower depressive symptomatology in both gender include advanced age (over the age of 85), exercise and social participation. The advanced age for men and women reduced about 63% (AOR over the age of 85=0.4, 95%CI=0.2-0.9) and 62% (AOR over the age of 85=0.4, 95%CI=0.2-0.8) of depressive symptomatology for men and women respectively; Exercise reduced about 50% (AOR exercise=0.5, 95%CI=0.3-0.6) and 58% (AOR exercise=0.5, 95%CI=0.3-0.6) of depressive symptomatology for men and women respectively; social participation reduced about 55% (AOR social participation=0.5, 95%CI=0.3-0.7) and 36% (AOR social participation=0.6, 95%CI=0.5-0.9) of depressive symptomatology for men and women respectively.Conclusions: For adults aged over 65, advanced age, which is 85+ for men and 75+ for women, is a significant protective factor guarding against depressive symptoms. Carbohydrates, cognitive disorder, heart disease and falls were associated with higher depressive symptomatology in both gender. The pulmonary disease, underweight and educational level were risk factors for men; metabolic disease and milk intake were risks for women. Common factors associated with lower depressive symptomatology in both gender includes advanced age, exercise and social participation. Tea and coffee intake and married status were associated with lower depressive symptomatology for women.

Abstract A growing body of literature suggests that early life circumstances can influence mental health throughout the lifespan. However, how these early life circumstances cumulatively contribute to depression in old age is not completely understood. The present study examined the associations of eight factors representing multifaceted early life experience at individual, family, and community levels with depression among community-dwelling older adults. Data were from the China Health and Retirement Longitudinal Study. We included 8,239 community-dwelling individuals who were ≥60 years, completed the life history questionnaire, and had assessment of depression. Chi-square test was used to examine the unadjusted associations between each of the eight early life risk factors and depression. An early life disadvantage index was established using risk factors that were significantly associated with depression. Logistic regression was used to examine the association of each early life risk factor and the index with depression. Of 8,239 individuals included, 2,055 (24.9%) had depression. In bivariate analysis, each of eight early life risk factors was significantly associated with depression. Except for maternal and paternal education, all risk factors persisted to be associated with depression after multivariable adjustment. In the multivariable-adjusted model, a one-point higher in the early life disadvantage index (range: 0-6) was associated with a 45% (95% CI: 37%, 53%) higher odds of depression. There was a strong association between early life environments and depressive symptoms among Chinese community-dwelling older adults. Adverse early life circumstances could contribute cumulatively to depression in old age.

Abstract Background Few studies have examined the relationship between falls and pain, insomnia and depressive symptoms which are common and risk factors in older adults. We aimed to examine the independent and synergistic effects of these risk factors on future falls among older adults. Methods We used data of 2558 community-dwelling older adults from 2011 (Y1) to 2015 (Y5) of the National Health and Aging Trends Study (NHATS). Pain was determined by whether participants reported bothersome pain in the last month. Insomnia was assessed by two questions about how often the participants had trouble falling asleep and maintaining sleep. Depressive symptoms were assessed by Patient Health Questionnaire-2. Generalized estimation equation (GEE) models were used to examine the independent effects of pain, insomnia and depressive symptoms at prior-wave (period y-1) on falls at current wave (period y) adjusting for covariates (age, sex, education, race/ethnicity, living arrangement, BMI, smoking, vigorous activities, number of chronic illnesses and hospitalization). The significance of the three-way interaction of these factors (pain*insomnia*depression) was tested using the aforementioned GEE models to determine their synergistic effects on falls. Results Overall, the participants were mainly 65–79 years old (68%), female (57%) and non-Hispanic White (70%). At Y1, 50.0% of the participants reported pain, 22.6% reported insomnia and 9.9% reported depressive symptoms. The incidence of falls from Y2 to Y5 was 22.4, 26.0, 28.3, and 28.9%, respectively. Participants with pain (Odds ratio [OR], 95% confidence interval [CI] = 1.36, 1.23–1.50) and depressive symptoms (OR, 95% CI = 1.43, 1.23–1.67) had high rates of falling adjusting for covariates. After further adjustment for insomnia and depressive symptoms, pain independently predicted falls (OR, 95% CI = 1.36, 1.22–1.51). Depressive symptoms also independently predicted falls after further adjusting for pain and insomnia (OR, 95% CI = 1.40, 1.20–1.63). After adjusting for pain and depression, the independent effects of insomnia were not significant. None of the interaction terms of the three risk factors were significant, suggesting an absence of their synergistic effects. Conclusions Pain and depressive symptoms independently predict falls, but synergistic effects seem absent. Further research is needed to develop effective strategies for reducing falls in older adults, particularly with pain and depressive symptoms.

Abstract The coronavirus disease 2019 (COVID-19) pandemic adversely impacted the mental health of older adults. This study aims to explore the associations between protective/risk factors of depression during the pandemic and to examine the differences in these associations by marital status. Data from the Health and Retirement Study 2020 COVID-19 module, released in February, 2021, were used. The level of resilience during the pandemic was selected as a protective factor. The level of COVID-19 pandemic concern was selected as a risk factor. Among older adults aged 51 years and older, the weighted regression model found that higher levels of COVID-19 concern were associated with higher levels of depressive symptoms (p&lt;0.05), whereas higher levels of resilience were associated with lower levels of depressive symptoms (p&lt;0.05). Marital status moderated the association between COVID-19 concern and depressive symptoms. Never-married people were at higher risk of depressive symptoms than married people when COVID-19 concerns increased. It is important to enhance support for never-married people during the pandemic to protect their psychological well-being.

OBJECTIVE: To evaluate the incidence and persistence of fear of falling in older adults and the clinical/functional, psychosocial and lifestyle-related risk factors. METHODS: A longitudinal study with 393 community-dwelling older adults aged 65 years and over (110 men/ 283 women) resident in the North Zone of the city of Rio de Janeiro, Brazil. The fear of falling was assessed by the Falls Efficacy Scale-I-BR. The explanatory variables assessed were: number of comorbidities and medicines, history of falls, fracture from falling, use of walking aids, functional dependence in basic and instrumental activities of daily living, hearing and visual impairment, hand grip strength, walking speed, self-rated health, body mass index, depressive symptoms, cognitive impairment, living alone and activity level. Incidence, persistence and risk factors were estimated. Multivariate analysis was performed using Poisson Regression, obtaining relative risks (RR) and corresponding to 95% confidence intervals. RESULTS: Among the 393 participants, fear of falling occurred in 33.5% and was persistent in 71.3%. Incidence was found to associate with using seven or more medicines and reporting worse activity level than the prior year. Risk factors for persistent fear were: using seven or more medicines, a history of one or two falls, reduced walking speed, hearing impairment, cognitive impairment, depressive symptoms and poor or very poor self-rated health. CONCLUSION: Fear of falling is a frequent and persistent condition. Many factors related to persistent fear showed no association with the incidence of fear, emphasizing the need for focused strategies to reduce risk factors that may be associated with the chronification of fear of falling.

Abstract Study Objectives Growing evidence demonstrates pronounced alterations in rest–activity functioning in older adults at-risk for dementia. White matter degeneration, poor cardiometabolic functioning, and depression have also been linked to a greater risk of decline; however, limited studies have examined the white matter in relation to rest–activity functioning in at-risk older adults. Methods We investigated associations between nonparametric actigraphy measures and white matter microarchitecture using whole-brain fixel-based analysis of diffusion-weighted imaging in older adults (aged 50 years or older) at-risk for cognitive decline and dementia. The fixel-based metrics assessed were fiber density, fiber cross-section, and combined fiber-density, and cross-section. Interactions between rest–activity functioning and known clinical risk factors, specifically body mass index (BMI), vascular risk factors, depressive symptoms and self-reported exercise, and their association with white matter properties were then investigated. Results Sixty-seven older adults were included (mean = 65.78 years, SD = 7.89). Lower relative amplitude, poorer 24-h synchronization and earlier onset of the least active 5-h period were associated with reductions in markers of white matter atrophy in widespread regions, including cortico-subcortical and cortical association pathways. Preliminary evidence was also found indicating more pronounced white matter alterations in those with lower amplitude and higher BMI (β = 0.25, 95% CI [0.05, 0.46]), poorer 24-h synchronization and more vascular risk factors (β = 0.17, 95% CI [–0.02, 0.36]) and earlier onset of inactivity and greater depressive symptoms (β = 0.17, 95% CI [0.03, 0.30]). Conclusions These findings highlight the complex interplay between rest–activity rhythms, white matter, and clinical risk factors in individuals at-risk for dementia that should be considered in future studies.
