Abstract Objectives To investigate the factors influencing primary tooth survival after pulpectomy and to elucidate age-related morphological changes in the root canal of primary molars. Materials and Methods This retrospective study included 592 primary anterior teeth and 583 primary molars after pulpectomy. Survival analysis was performed to determine the predictors of pulpectomy failure. Cone-beam computed tomography (CBCT) data of the maxillary first primary teeth from 63 children aged 3–8 years were collected, and Spearman’s correlation and binary logistic regression were performed to assess age-related changes in canal morphology. Results Periapical periodontitis, Vitapex® filling, and non-general anesthesia treatment increased the failure risk of anterior primary teeth pulpectomy. Older age at treatment increased the primary molar pulpectomy failure risk. On CBCT analysis, the ratio of canal length to root length for the mesiobuccal (MB), distobuccal (DB), and palatal (P) roots negatively correlated with age (P<0.05). The ratio between the root canal wall area with inner diameter lesser than 0.15 mm and the whole root canal wall area (R.15) at plane D of the MB canals positively correlated with age (P<0.05). The number of discontinuous canals (Ndis) for the MB and DB roots increased with age and the canal area at plane D for MB and P canals negatively correlated with age (P<0.05). Conclusions Older age at treatment for primary molar pulpectomy and periapical lesions, non-general anesthesia treatment, and Vitapex® filling for anterior primary teeth pulpectomy may predict a higher failure risk. Age-related morphological changes in the primary molar root canal were evident and could be a reason for treatment failure.

Microbes are considered as the primary etiologic agents in endodontic diseases.Disinfection of the root canal is obtained by the combined effect of biomechanicalpreparation, irrigation and intra canal medicament. The aim of the present study wasto assess the antimicrobial activity of intracanal medicaments (formocresol andEndosepton) against two micro organisms (Streptococcus mutans and staphylococcusaureus) isolated from 15 necrotic pulps of primary molars indicated for pulpectomyprocedure. The samples were cultured, and purified using microbiological evaluation.Broth dilution test was performed in our study by preparing test tubes containing10 ml of BHI broth (pH. 7) which then inoculated with strains of the tested bacteriaand incubated at 37 C° for 24 h. After over night incubaction, ten fold dilution weremade in test tubes containing 9 ml of normal saline by adding 1 ml of the inoculum tothe first tube . Then from dilution 10-1 , 0.1 ml of cell suspension was added to 9.9 mlof formocresol and endosepton, then 0.1 ml was taken and spread on duplicates ofBHI agar plates at different intervals and incubated aerobically for 24 h. at 37 C°.Colonies on the plates were counted after incubation and CFU/mL (colony formingunit) was calculated. Our results indicating that there were no significant differencesbetween the intracanal medicaments, but there were high significant differencesbetween the intervals time of the study. We concluded that both materials had greatantibacterial effect against the pathogens commonly isolated from necrotic pulpaltissue of primary teeth.

Relevance. To study the incidence of different types of resorption of multirooted primary teeth, to specify indications for deciduous molar extraction to prevent eruption abnormalities of permanent posterior teeth in mixed dentition.Materials and methods. Root resorption of 375 multirooted primary teeth (166 first primary molars and 209 second primary molars) was studied on panoramic X-rays of 60 children (30 girls and 30 boys) aged between 7 and 15. Illustrated classification by T.F. Vinogradova (1967) improved by authors was used to determine type and degree of root resorption of multi-rooted primary teeth. Received data were described with absolute values of number of cases and percentage. Chi-square was used to detect differences in sign incidence rate between groups, p<0.05 was considered statistically significant.Results. There were no statistically significant gender differences (p>0,05) in type and degree of root resorption of multirooted primary teeth. Type A resorption prevailed and constituted 53.3% of all primary molars. Disturbances in root resorption of multirooted primary teeth in mixed dentition were related to health condition of primary teeth. Transition of even resorption to unven was considered a risk factor of delayed eruption and aberrant position of permanent teeth, and indication for extraction of a primary molar in question. Conclusions. 1) Even root resorption (type A) was detected in 53.3% of primary molars in mixed dentition by orthopantomography. 2) Transition from even resorption of primary molar roots to uneven resorption was associated with eruption deviations and delayed premolar eruption. 3) Timely extraction of primary molars with uneven root resorption facilitated correct eruption of premolars and increased effectiveness of secondary prevention of malocclusion in children.

Objective: PTFE tape, which is commonly used as plumber's tape is an inorganic, non-fibrous, ribbon like material. The aim of this study was to evaluate PTFE tape as endodontic spacer material and to compare it with commonly used spacer material that is cotton, in primary teeth. Study Design: Seventeen children undergoing pulpectomy of lower second primary molar bilaterally were included in the study. Cotton and PTFE tape were placed as spacers on each side randomly. Samples were taken from the access cavity at baseline and after seven days to check for microbial leakage. Spacer materials were also checked for microbial contamination. Results: The results revealed that there was a significant increase in the bacterial colony count after seven days in cotton group. The access cavities were also positive for microbial leakage in the cotton group where the spacers showed positive growth. In PTFE group only two samples showed microbial contamination of spacer and out of two only one sample showed contamination of access cavity along with spacer. Conclusion: Within the limitations of this study, it can be concluded that PTFE tape performed better than cotton as endodontic spacer material. Thus, PTFE tape can be recommended as an endodontic spacer material as an alternative to cotton in primary teeth.

The primary goal of endodontic treatment in primary teeth is to eliminate infection, and to retain the tooth in a functional state until their normal exfoliation time without endangering the permanent dentition or the general health of the child. The complexity of the pulp canal system in primary teeth presents a discerning problem for chemo-mechanical preparation. One of the factors determining the success of endodontic treatment in infected primary teeth is the sealing material that should encompass among other factors a potent bactericidal effect and the capacity to resorb along with the roots of primary teeth. This study evaluated the antimicrobial effectiveness of 5 root canal filling materials and a negative control agent against 23 strains of bacteria isolated from infected root canals of primary molar teeth and 3 non standard bacterial strains using agar diffusion assay. The materials were Zinc oxide and Eugenol (ZOE), Zinc oxide-Eugenol and Formocresol (ZOE+FC), Calcium hydroxide and sterile water (CAOH+H2O), Zinc oxide and Camphorated phenol (ZO+CP), Calcium hydroxide and Iodoform (Metapex) and Vaseline (Control). All the materials except Vaseline showed varied antimicrobial activity against the test bacteria. The zones of inhibition were ranked into 4 inhibition categories based on the proportional distribution of the data. All the 26 bacterial isolates were classified under 4 groups based on Aerobic/Anaerobic and Gram positive/Gram negative. Statistical analysis was carried out to compare the antimicrobial effectiveness between materials tested with each of the bacterial groupings. ZOE+FC produced strong inhibition against most bacteria when compared to ZOE, ZO+CP and CAOH+H2O. Metapex and Vaseline were found to be non inhibitory

AIM: The aim of this study is to present severe root resorption following obturation of the primary molar with zinc oxide eugenol (ZOE) at different follow-up periods. CASE REPORT: We present the case of a 6-year-old boy with decayed lower primary molars. Pulpectomy for pulpally involved primary molars was performed. ZOE has been used as root canal filling material and follow-up was done at 3, 6, and 9 months. CONCLUSION: External root resorption was detected at 3 months and the resorptive process continued till 9 months. This finding emphasizes the need for alternative filling material with less irritating properties.

Aim: To evaluate and compare the instrumentation time, obturation time, quality of obturation, and clinical and radiological success of pulpectomized teethfollowing root canal preparation of primary molars with rotary and manual file system. Methods: A total of 150 primary molars requiring pulpectomy were selected from children aged four to seven years. These teeth were divided into three groups of 50 teeth each. In Groups 1, 2, and 3, cleaning and shaping were carried out with Kedo-S pediatric rotaryfiles, HERO Shaper rotary files, and manual NiTi K-files, respectively. Obturation was carried out with zinc oxide eugenol cement and an engine-driven Lentulo spiral. The instrumentation and obturation times were recorded. A radiographic assessment of thequality of the root filling was carried out immediately after obturation. Finally, the pulpectomized teeth were clinically and radiographically evaluated over a two-year period. Results: The mean instrumentation times for Groups 1, 2, and 3 were 14.56 ± 2.89 min, 17.93 ± 3.51 min, and 29.00 ± 2.08 min, respectively. The mean obturation times for Groups 1, 2, and 3 were 8.11 ± 1.7 min, 7.93 ± 1.3 min, and 9.64 ± 17.61 min, respectively. The mean difference in the quality of obturation was not statistically significant in primary molars instrumented with Kedo-S pediatricand HERO Shaper rotary file systems (p = 0.16). However, this mean difference was significant when compared between primary molar instrumented with rotary file systems and manual NiTi files (p = <0.001). At two years, the clinical success rate was 100% and the radiological success rates were 95.3%, 97.9%, and 89.5% in Groups 1, 2, and 3, respectively. Conclusions: The rotary file systems took significantly less instrumentation and obturation time than the manual NiTi files. There were no significant differences in obturation quality or success rates after two years.

The purpose of this study is to evaluate the morphological characteristics of the roots and roots canals of primary molar teeth using Cone-Beam Computed Tomography (CBCT).<br/>A total of 68 CBCT images of children aged 4 - 5 years was used for this study. A total of 160 molar teeth were analyzed. Various parameters such as the number of roots and canals, length of root and root canal, and the angulation and shape of the roots were analyzed.<br/>All maxillary primary molars had 3 roots. The presence of 2 root canals in 1 root was only observed in the mesiobuccal root of maxillary primary second molars. Most mandibular primary molars had 2 roots, and most mesial roots had 2 root canals.<br/>Concerning the length of the roots, the palatal root of the maxillary primary molar was found to be longest whereas the distobuccal root was shortest. In mandibular primary molars, the mesial root was longer than the distal root. In maxillary primary molars, the palatal root had the greatest angulation whereas the distal root has the greatest in mandibular molars. The root and root canals of maxillary primary molars were more curved in shape whereas mandibular primary molars were straight.

Pulpectomy in deciduous teeth involves endodontic access opening, root canal debridement and obturation with an appropriate filling material. EndoREZ (ER) is the urethane dimethacrylate-based filler, which can be used for root canal obturation in permanent and primary teeth. This observation aimed to evaluate the behavior of the ER as a filler in root canals of two primary molars after the physiological resorption process using the scanning electron microscopy (SEM) and micro-computed tomography (µCT) in second lower molars after their natural exfoliation. The SEM analysis revealed a non-uniform, porous and lacunary structure of ER, visually similar to the resorbed surface of the dentine. The µCT observations demonstrated the differences in the resorption level of the root and material surfaces. The preliminary observations suggest that ER is resorbed faster than root tissues and can therefore be a suitable material for the root canal filling in primary teeth. However, more investigations are needed to support these preliminary findings.

Abstract Introduction Pulpectomy helps in preserving a pulpally involved primary tooth by eliminating bacteria and their products and ensures hermetic seal of the root canals, so that the primary teeth can complete their function until normal exfoliation. For the success of endodontic treatment in primary teeth, the resorption of obturating material plays a vital role. The materials used for obturation in this study included zinc oxide propolis (ZOP) as a new paste, Endoflas, Metapex, and zinc oxide eugenol (ZOE). Objective To evaluate the success rate of pulpectomy of primary molars using four different root canal filling materials: ZOP as a new paste, Endoflas, Metapex, and ZOE. Materials and Methods A total of 48 primary molars were divided into four equal groups of 12 molars according to the type of root canal filling paste, that is, ZOE, Endoflas, ZOP, and Metapex. Results When Tukey’s test was applied, out of the four materials, ZOP was the most significant material as its p-value was < 0.05, whereas other materials showed nonsignificant results as their p-value was > 0.05. Conclusion Among the four groups, ZOP showed greater clinical success and its resorbability was similar to that of root.
