Introduction. The article discusses methods of screening for hearing impairments in patients of the older age group in the frame-work of primary health care.Aim of the study. Conduct a comparative analysis of the effectiveness of different hearing assessment protocols in primary health care.Materials and methods. Сlinical and demographic data were collected in 585 elderly and senile patients (mean age 76.43 ± 9.83), tonal threshold audiometry was performed in the frequency range from 250 Hz to 12000 Hz, hearing was studied using the web application “Automated primary hearing assessment” (patent No. 2019664671) and analyzed the self-assessment of hearing with the HHIE questionnaire.Results. Most of the patients were elderly people (57.44%) with a high percentage of concomitant diseases (up to 89.23%). When interviewing a geriatrician about a complaint of hearing loss, a sensitivity of 91.5% and a specificity of 82.2% for detecting mild hearing loss, a sensitivity of 95.5% and a specificity of 71.8% for screening for moderate to severe hearing loss were obtained. When assessing the total score of the HHIE questionnaire (>17 points) and moderate hearing loss, the sensitivity was 84.7% and the specificity was 88.7%. The sensitivity index of the web application «Automated primary hearing assessment» for detecting moderate hearing impairment was 90.6% for the left ear and 88.5% for the right ear, and specificity – 88.5% for the left ear and 97.5% for the right ear.Discussion. Raising awareness of hearing problems through the introduction of feasible methods of assessing hearing function should lead to an increase in the number of older adults receiving adequate hearing rehabilitation.Conclusions. The authors conclude that it is important to take preliminary account of data on the sensitivity and specificity of assessment protocols for detecting hearing impairments of varying severity at the stage of examination of an older patient by a geriatrician and an otorhinolaryngologist.

ABSTRACT Objective: to reflect on how nursing may employ the bioethics knowledge inside their practice to assistance and humanize the embracement on primary health care to Lesbian, Gay, Bisexual, and Transgender people (LGBT). Methodology: a reflective essay based on extensive literature review and consolidated through discussions during classes, as also seminars and Bioethics readings extracted from the discipline  "Fundamentals of Ethics and Bioethics in Health Care” from Masters of Science in Nursing’s Associate Program of Pernambuco State University and Paraiba State University, Brazil. Results: the bioethics knowledge can subscribe the real recognition of citizenship for all people, especially the most vulnerable ones, providing a space for reflections in the direction of harmony and equal opportunity among LGBT and health professionals. Conclusion: the theoretical framework of bioethics can help and support registered Nurses to promote humanized care for embracement of LGBT people in primary health services, in that it gives them sense of dignity and respect through the tolerance and acceptance of the right of diversity. RESUMO Objetivo: refletir sobre como a Enfermagem pode utilizar o referencial da Bioética para fundamentar sua prática e assistência em relação à garantia de um acolhimento humanizado a Lésbicas, Gays, Bissexuais, Travestis e Transexuais (LGBT) nos serviços de saúde da atenção básica. Metodologia: ensaio reflexivo baseado em revisão bibliográfica e consolidado por meio de discussões e seminários realizados na disciplina Fundamentos da Ética e Bioética na Saúde e Enfermagem do Programa de Mestrado Associado de Pós-Graduação em Enfermagem UPE/UEPB. Resultados: a Bioética contribui para o reconhecimento da plena cidadania de todos os seres humanos principalmente dos mais vulneráveis, deste modo, proporcionando um espaço de reflexão na busca da convivência e da oportunidade entre usuários LGBT e profissionais da saúde. Conclusão: o referencial teórico da Bioética pode ajudar a respaldar e promover o acolhimento humanizado de LGBT pela Enfermagem nos serviços da atenção básica por meio da tolerância com a diversidade e respeito à dignidade humana destes cidadãos. Metodología: ensayo reflexivo basado en  revisión de la literatura y consolidado através de discusiones y seminarios  durante las clases, así como seminarios y lecturas de Bioética extraídos de la disciplina "Fundamentos de la Ética y Bioética en la Atención Sanitaria" del Programa Master en Enfermeria Asociado de las Universades de los estados de Pernambuco y Paraiba, Brazil. resultados: la Bioética puede sostener um reconocimiento real de la ciudadanía de las personas, especialmente los más vulnerables, proporcionando un espacio de reflexión en el sentido de la armonía y la igualdad de oportunidades entre los LGBT y profesionales de la salud. Conclusión: el marco teórico de la bioética puede sustener a las enfermeras  para una atención más humanizada  de la acogida a los LGBT dentro de los servicios de salud primaria, garantizando a estas personas un sentido de dignidad y respeto a través de la tolerancia y la aceptación del derecho de la diversidad. Primary Health Care (PHC) is expected to serve as a basis for the country's health sector, of which it is the primary responsibility and priority, as well as the community's overall collective and economic prosperity. Promotion of food supply and proper nutrition are among eight elements of PHC. Protein deficiency is one of the most important health concerns in some parts of the world. A huge numbers of malnourished or hungry people have been reported particularly in the less developed countries. Advantages of aquaculture in nutrition are varied. However, there are some zoonotic microbial illnesses occurred due to consumption of infected seafood. Some of these challenges will be cor- rected by good aquaculture practices. With the right reforms in the aquaculture industry, progress can be made toward solving some of the challenges facing PHC delivery. In this mini-review, the benefits and risks of aquaculture foods in PHC are briefly discussed.

In the developed nations physiotherapist are autonomously participating in the first line and already they are working in the second line treatment alongside with other professionals and the right for the mass population similar to the goal of the public health. The aim of the study was to sort out the real scenario of the development of physiotherapy services as a part of primary health care services including the knowledge and attitude of the general people. A multistage sampling method was selected for conducting the study in districts hospitals of the Barisal division. Result depicted that among the respondents mean age 36.86 ± 4.558 years and most 33.3% (n= 73) of them were younger age range 21-30 years and interestingly highest 41.55% (n= 91) were graduated with highly significant (x2= 1.196, P= 0.000). 65.3% (n= 143) of patients had knowledge about physiotherapy. Majority of case 41.1% (n= 90) orthopedic (x2= 86.175, P= 0.000) and medical condition stroke ware highest 30.1% (n= 66) statically significant (x2= 1.561, P= 0.000). Due to the awareness maximum, 39.70% (n= 87) came from self-reference and 88.4% (n= 176) received exercise therapy including other electrical modalities. Lowest 8.70% (n= 19) of patient expense > 300 BDT. Treatment right time 60.3% (n= 132). Treatment success rate 69.4% (n= 152). Difficulty to come physiotherapy center 43.8% (n= 96). Some limitation that causes insufficiency in treatment provide; 10.6% patient noted poor timing, 30.7% long witting time, 68.3% (n= 136) unpleasant, 78.9% (n= 157) absence of physiotherapist, 9.5% (n= 19) lack of cooperation. Although that 99.5% patient demand physiotherapy center as close as their range and 60.3% (n=132) satisfied with the treatment. Therefore it is strongly suggested to set up a modern physiotherapy department within the primary health care system with the qualified physiotherapist in community-level also District level hospital

Objective: justifying the possibility of using frequency-based hearing research via the web application ‘Automated System for Initial Hearing Assessment’. Materials and methods. The study was carried out at the Department of Otorhinolaryngology of the Samara State Medical University clinics in October 2019. It involved 91 patients 17 to 73 years old (average age 48±14.6 years). All study participants were divided into two age groups: Group I 17-59 years old), and Group II (over 60 years old). Patients were required to undergo two hearing assessment tests: using the Interacoustics AC-40 clinical audiometer and using our web application ‘Automated System for Initial Hearing Assessment’ (patent No. 2019664671). Results. The maximum difference in average hearing thresholds between pure-tone threshold audiometry and the web application in Group I was 3.3 dB at a frequency of 2 kHz on the right and 3.2 dB at frequencies of 2 and 4 kHz on the left; in Group II, it constituted 4.7 and 3.5 dB at 1 and 2 kHz, on the right, correspondingly, and 7.2 dB at 4 kHz on the left. The presented data were evaluated using the Cohen’s kappa coefficient, which confirmed high level of agreement between the results obtained using the clinical audiometer vs. our original web application. Conclusion. Based on the obtained results, a conclusion was made about the possibility of using the original web application for the initial hearing express assessment in general practice in conditions of limited availability of audiological care.

Abstract Background Worldwide, workers' health is still a Public Health concern, given the high number of hazardous occupational activities, as well as workers affected by work-related diseases. Among these diseases, occupational Noise-Induced Hearing Loss-NIHL is considered the second commonest occupational disease, and in Brazil, its reporting is compulsory. However, occupational diseases are usually underreported, hindering the knowledge of their actual magnitude, and the elaboration of prevention-oriented public policies. Objective To analyze the perception and knowledge of Primary Health Care professionals in Curitiba-Parana State, Brazil, on the compulsory NIHL reporting. Methods quantitative and qualitative approach conducted in three steps: analysis of a series of NIHL case histories reported in the official database between 2007 and 2018; application of a questionnaire to Primary Health Care Network professionals; collective interview with Primary Health Care Network nurses, speech therapists and doctors. Results NIHL reporting evidenced 82 cases, 67 (81.7%) among males, age ranging 50-64 years, 10 (12.2%) had complete high school level, 27 (32.9%) were formally employed, and 12 (14.6%) worked in the processing industry. Regarding the result of the applied questionnaire among the Primary Health Care professionals, 48 (68.7%) stated that they were prepared to identify workers' health problems, to 33 (68.7%) professionals, guidance to reassure users' comprehensive care is more important than the compulsory reporting, 21 (43.7%) claimed that they were qualified to identify NIHL cases, and 25 (52.1%) did not report the cases. Difficulties in reporting NIHL cases are: not understanding their flow, being afraid of the legal implications, which may generate difficulties in requesting audiometric testing, not having the management support, etc. Conclusions Most health care professionals are knowledgeable on NIHL, but they do not report the suspected cases due to lack of guidance. Key messages Primary care health professionals does not consider occupational health as part of the service to be provided. Primary care services are not prepared to establish the relationship between the injury and the job.

Objective: To evaluate the performance of self-reported hearing loss alone and in combination with pure tone audiometry screening in primary health care clinics in South Africa. Design: Nonprobability purposive sampling was used at 2 primary health care clinics. A total of 1084 participants (mean age 41.2 years; SD 15.5 years; range 16-97 years, 74.0% female) were screened using self-report and audiometry screening. Those failing audiometric screening and a sample of those who passed audiometric screening were also assessed by diagnostic pure time audiometry, to confirm or negate the finding of a hearing loss. Results: Four hundred and thirty-six participants (40.2%) self-reported a hearing loss with no significant association with gender or race. One hundred and thirty-six participant (12.5%) self-reported hearing loss and failed audiometry screening (35 dB HL at 1, 2, and 4 kHz). Combining self-report with a second stage audiometry screening revealed a high test accuracy (81.0%) for hearing loss, being most accurate (86.1%) to identify high-frequency hearing loss. Conclusion: While self-report of hearing loss is an easy and time-efficient screening method to use at primary health care clinics, its accuracy may be limited when used in isolation and it may not be sufficiently sensitive to detect hearing loss. Combining a simple audiometry screening as a second-stage screen can significantly improve overall performance and efficiency of the screening protocol.
