The article reports an up-to-date classification of complications of cosmetic procedures and two clinical cases where multicomponent gel based on troxerutin, heparin sodium and depanthenol was used to rehabilitate patients after cosmetic procedures complicated by ecchymosis, hematomas and edema. In the first case a 30-year-old patient had a procedure to give greater volume to her lips and to correct their asymmetry with hyaluronic acid filler. The procedure was complicated by the formation of hematomas and edema. To minimize the edema and hematomas and to avoid the risk of infection the patient was prescribed 0.05% aqueous solution of chlorhexidine bi-gluconate once per day and taking of multicomponent gel based on troxerutin, heparin sodium and depanthenol two times per day for seven days. By the fifth day there was observed a significant decrease in edema of the lip area and almost complete resorption of hematomas. In the second case a 42-year-old patient had a procedure of a biorevitalization with unstabilized hyaluronic acid complicated by the appearance of ecchymosis up to 12 mm in diameter and edema in the periorbital zone. A 0.05% aqueous solution of chlorhexidine bi-gluconate once per day and a multicomponent gel based on troxerutin, heparin sodium and dexpanthenol twice per day for seven days were prescribed as post-procedural rehabilitation. By the fourth day clinical improvement was noted. The edema disappeared, the color changed, the size of the ecchymosis decreased to 4 mm. The ecchymosis disappeared completely on the sixth day. In addition, the main characteristics of troxerutin, heparin sodium and depanthenol as components of a multicomponent gel were considered. A multicomponent gel based on troxerutin, heparin sodium and depanthenol as supportive aid helped to get complications completely cleaned out and significantly reduced the rehabilitation period for these patients. The authors consider it possible to recommend it as a means of post-procedural rehabilitation.

Objective: Many authors recommended that reliable and clinically significant change (RCSC) should be calculated when reporting results of interventions. To test the reliability of the Health of the Nation Outcome Scales (HoNOS) in identifying RCSC, we applied the Jacobson and Truax model to two HoNOS assessments in a large group of people evaluated in 10 community mental health services in Lombardy, Italy, in 2000. Method: The HoNOS was administered to 9817 patients; of these, 4759 (48%) were re-assessed. Reliable change (RC) was calculated using Cronbach's alpha ( α), as a parameter of the reliability of the measure. Clinical significance cut-offs were calculated using a classification of severity based on HoNOS items. Results: In the whole sample, the clinical improvement cut-off was 11 and the remission cut-off was 5. Considering the severe patients, the clinical improvement cut-off was 12. The RC index calculated on the whole group and on the subgroup of severe patients indicated that eight-point and seven-point changes, respectively, were needed to be confident that a real change had occurred. Longitudinal changes were depicted on two-dimensional graphs as examples of reporting RCSC on HoNOS total scores in a routine data collection: 91.6% of the whole sample (4361) was stable, 5.6% (269) improved and 1.8% (129) worsened. Conclusion: Our study proposes a methodological framework for computing RCSC normative data on a widely used outcome scale and for identifying different degrees of clinical change.

Abstract Introduction. Leprosy is a disease that predominantly affects the skin and peripheral nerves, resulting in neuropathy and associated long-term consequences, including deformities and disabilities. According to the WHO classification, there are two categories of leprosy, paucibacillary (PB) and multibacillary (MB). The standard treatment for leprosy employs the use of WHO MDT (Multi Drug Treatment) regimen, despite its multiple downsides such as clofazimine-induced pigmentation, dapsone-induced haematological adverse effects, poor compliance due to long therapy duration, drug resistance, and relapse. Multiple studies and case reports using ROM regimen have reported satisfactory results. Nevertheless, there are still insufficient data to elucidate the optimum dosage and duration of ROM regimen as an alternative treatment for leprosy. Previous experience from our institution revealed that ROM regimen given three times weekly resulted in a satisfactory outcome. Case Reports. We report two cases of leprosy treated with ROM regimen from our institution. The first case was PB leprosy in a 64-year-old male who presented with a single scaly plaque with erythematous edge on the right popliteal fossa. Sensibility examination showed hypoesthesia with no peripheral nerve enlargement. Histopathological examination confirmed Borderline Tuberculoid leprosy. ROM regimen was started three times weekly for 6 weeks and the patient showed significant clinical improvement at the end of the treatment with no reaction or relapse until after 6 months after treatment. The second case was MB leprosy in a 24-year-old male patient with clawed hand on the 3rd-5th phalanges of the right hand and a hypoesthetic erythematous plaque on the forehead. Histopathology examination confirmed Borderline leprosy. The patients received ROM therapy 3 times a week with significant clinical improvement after 12 weeks. Conclusion. ROM regimen given three times weekly for 6 weeks in PB leprosy and 12 weeks in MB leprosy resulted in a significant clinical improvement. Thus, ROM regimen could be a more effective, safer, faster alternative treatment for leprosy.

The study was carried out in order to evaluate clinically and laboratory cachectic animals suffering from anemia. Animal examined were 50 cow and calf. The study include clinical, hemato and biochemical test for accurate diagnosis of cachexia in cows and calves . Blood smears were conducted for detection of blood parasites , fecal examination for gastrointestinal parasites and Different parameters were applied for classification of cachexia , depending on bony projection specially ribs and pelvic and generalized muscular atrophy. However , The study revealed an incidence of cachexia and anemia of blood parasites was including Theileria, Anaplasma, gastrointestinal parasites, ten cases were shown foreign body syndrome while other tens were diagnosed as other clinical cases. A significant decrease ( P < 0.05) of RBCS, Hb, PCV and WBC count in debility animals were observed . However , a significant lymphocytosis were seen in blood parasites infection and other clinical cases , Neutrophilia in foreign body syndrome and other clinical cases , Esenophilia in gastrointestinal parasites infestation ,Monocytosis in all clinical cases were detected . Different parameters were applied for classification of anemia mainly morphological classification including macrocytic hypochromic anemia mainly recorded in blood parasites infection and other clinical cases while Normocytic hypochromic anemia appeared in gastrointestinal parasites infestation, and foreign body Syndrome.Fecal examination of cachectic animal indicate the identification of (7) species of gastrointestinal parasites including: Cooperia onchophora, Haemonchus contortus, Bunostomum phlebtomum, Ostertagia spp., Oesophagostomum radiatum, Trichostrongylus axei and Strongyloides papillosus. However , a mixed infestation parasite was dominant and recorded in Animal.Biochemical changes revealed a. Hypoprotenemia appears in all cachectic animal except in foreign body syndrome cases .

Heparin (Hep), hyaluronic acid, chondroitins (sulfate) A, B, and C, and heparitins (sulfate) A, B, C, and D were subjected to microelectrophoresis in barbital–agarose gel, fixed with cetylpyridinium chloride and stained with toluidine blue. The optical densities of the resulting bands were compared with optical densities obtained upon reaction with azure A in aqueous solution and with the carbazole reagent. A linear relation was obtained between optical density and concentration of purified sulfated mucopolysaccharide (SMP). Less than 1 μg of Hep and 2 μg of other SMPs are required for measurement by electrophoresis, while about 30 μg of each is required with the carbazole reagent. The optical density of a mixture of SMPs was equal to the sum of the densities for the individual SMPs upon microelectrophoresis. It was demonstrated that the individual SMPs in mixtures were distinguished by reaction with specific enzymes and by changes in migration in agarose with barbital, phthalate, ethylenediamine, or propane-diamine buffers, permitting ready demonstration and quantitation of various SMP species. Examples are shown of the application of the procedure to measure the total SMPs and individual SMPs in tissue extracts. The method is sensitive, reproducible, flexible, and measures quantities [Formula: see text] of those measured colorimetrically, yet is relatively unaffected by protein, carbohydrate, or inorganic electrolytes.

Abstract Background Occurrences of suprasellar central nervous system (CNS) embryonal tumors in adults are extremely rare. Hemorrhagic onset is further uncommon, with only anecdotic cases reported in the literature. The authors describe the case of a 57-year-old man affected by a suprasellar CNS embryonal tumor, with hemorrhagic onset and a unique diffusion pattern along the optic pathways. Material and Methods A 57-year-old man presenting with acute visual acuity worsening and left homonymous hemianopia was referred to our hospital. Neuroradiologic studies demonstrated an infiltrating, high-grade lesion involving the optic chiasm and right retrochiasmatic pathways with a hemorrhagic area in the ipsilateral pulvinar. Results The patient underwent microsurgical biopsy. Pathologic assessment confirmed the diagnosis of CNS embryonal tumor, not otherwise specified (NOS) according to the 2016 World Health Organization (WHO) classification of CNS tumors. The patient was referred to a multimodal adjuvant treatment; he eventually died 4 months after surgery. Competent literature has been systematically reviewed in the light of the relevant changes made in the last version of the WHO classification. Conclusion Embryonal tumors should be considered in the differential diagnosis for sellar and suprasellar space-occupying lesions, despite the rarity of the disease and the uncommon features at time of presentation. As per our knowledge, this is the first case ever described of hemorrhagic suprasellar embryonal tumor with a diffusion pattern along white matter fibers. Histogenesis, biomolecular and neuroradiologic features, and classification of embryonal tumors are an open field of research, with considerable implications for the definition of better diagnostic pitfalls and therapeutic regimens.

Double filtration plasmapheresis (DFPP) is an emerging semi-selective apheretic method for treating immuno-mediated neurological diseases. Here we report the first case of steroid-refractory relapsed multiple sclerosis (MS) on Fingolimod (FTY), treated effectively by this technique, in a 37-year-old woman. This condition is thought to be caused by soluble inflammatory species, but its demyelinating pattern is unknown; moreover, despite megadoses of intravenous 6-methyl prednisolone, it induces severe neurological deterioration, but dramatically responded to DFPP in our patient. The clinical improvement was driven by a strong DFPP-induced anti-inflammatory effect, with significant reduction of C3/C4 components, total gamma globulin concentrations (IgG), and gamma-fibrinogen (FGG), resulting in a brain pseudoatrophy phenomenon. Our findings are: first, the steroid-refractory relapsed MS on FTY, however serious, can be treated with DFPP; second, given the good clinical improvement due to the DFPP-induced neuroinflammatory components removal, this clinical condition can be associated with a Lucchinetti pattern II of demyelination.
