Abstract Sustainable treatment and management of fecal sludge in rural areas require adapted solutions. Rustic and simple operating processes such as sludge treatment reed beds (STRB) have been increasingly considered for this purpose. The biggest full scale (2,600 m2 of STRB) septage treatment unit in France had been built in Nègrepelisse with the final objectives of reusing treated sludge and leachates for agriculture spreading and tree irrigation, respectively. The aim of this investigation was to validate the treatment chain of this installation. The obtained field data showed firstly that the overall removal efficiencies of STRB were satisfactory and stable. Removal rates higher than 98% for chemical oxygen demand and suspended solids and a 95% for Kjeldahl nitrogen represented so far a beneficial septage treatment by STRB. The highlighted necessity of a suitable complementary leachate treatment (before tree irrigation) justified the presence of the second stage of vertical flow constructed wetland. The sludge deposit drying and mineralization efficiencies were on the right track. According to hydrotextural diagram analysis, surface deposit was however found to have high deformability probably due to the youth of the installation. An in-depth understanding of STRB system needs continuous long-term studies.

Reed beds are an alternative technology wastewater treatment system that mimic the biogeochemical processes inherent in natural wetlands. The purpose of this project was to determine the effectiveness of a reed bed sludge treatment system (RBSTS) in southern New England after a six-year period of operation by examining the concentrations of selected metals in the reed bed sludge biomass and by determining the fate of solids and selected nutrients. Parameters assessed in both the reed bed influent and effluent: total suspended solids, biochemical oxygen demand, nitrate-nitrogen and total phosphorus. In addition, the following metals were studied in the reed bed influent, effluent and Phragmites plant tissue and the sludge core biomass: boron, cadmium, chromium, copper, iron, lead, manganese, molybdenum, nickel, and zinc. The removal efficiencies for sludge dewatering, total suspended solids and biochemical oxygen demand were all over 90%. Nitrate and total phosphorus removal rates were 90% and 80% respectively. Overall metals removal efficient was 87%. Copper was the only metal in the sludge biomass that exceeded the standards set by the Massachusetts Department of Environmental Protection for land disposal of sludge. The highest metal concentrations, for the most part, tended to be in the lower tier of the sludge profile. The exception was boron, which was more concentrated in the middle tier of the sludge profile. The data and results presented in this paper support the notion that reed bed sludge treatment systems and the use of reed beds provide an efficient and cost effective alternative for municipal sludge treatment.

Sentiment about fibre-to-the-premises (FTTP) runs hot in Australia, fuelled by the NBN initiative for Australia's next generation of broadband.  Unfortunately most of the debate is ill-informed, focusing on plumbing rather than the uses to which the network may be put. The base of users subscribing to the higher speeds made possible by FTTP is limited right now, and much of their communication will be speed-limited by constraints outside of the NBN FTTP segment.  In addition, a growing proportion of Australians choose to operate entirely on mobile connections.  For all these reasons, the number of users who would benefit from FTTP in the short term is modest. The Coalition has proposed greater use of existing infrastructure – in particular, using fibre-to-the-node (FTTN)/DSL technology and potentially hybrid-fibre-coax (HFC) technology – to deliver an upgrade earlier and at lower cost.  In the face of cost pressures, it is likely that the original 93% FTTP target would be adjusted with greater use of wireless and satellite technologies in rural areas and the use of copper for the final link in multi-dwelling complexes.  More generally, there is no question that FTTN/DSL solutions can deliver good quality broadband, but further work will be needed to determine where this can be done cost-effectively.  Similarly, with appropriate upgrades, HFC networks can deliver next-generation broadband speeds.  When practical factors are taken into consideration, the gap between Labor and Coalition plans closes somewhat. Developing the best strategy for Australia needs to take cost into consideration, and there is scope for NBN Co to improve its performance in this area.  Any major cost blowout would have significant ramifications for broadband users and use in Australia.  If the rollout of FTTP is scaled back, users with a need may still get access on a user-pays principle – though the practicality of this has yet to be demonstrated.  Widespread FTTP remains the right long-term goal for Australia, but the approach for getting there needs to be finessed.

Lorong Halus, Singapore's first landfill leachate treatment system, consists of a pre-treatment system (8,000 m2), five constructed reed beds (38,000 m2), five polishing ponds (13,000 m2), an education centre and a learning trail for visitors. Eight species of wetland plants (total 160,000 plants) were selected for their ability to uptake nutrients, tolerance to low phosphorus concentrations and resistance to pest infestations. The wetland was launched in March 2011 and water quality monitoring started in April 2011. The removal efficiencies of the pre-treatment system from April 2011 to August 2012 are biochemical oxygen demand (BOD5) 57.4%; chemical oxygen demand (COD) 23.6%; total suspended solids (TSS) 55.1%; ammoniacal nitrogen (NH4-N) 76.8%; total phosphorus (TP) 33.3% and total nitrogen (TN) 60.2%. Removal efficiencies of the reed beds are BOD5 47.0%; COD 42.2%; TSS 57.0%; NH4-N 82.5%; TP 29.3% and TN 83.9%. Plant growth is generally satisfactory, but the lower than designed volume of leachate has adversely affected some sections of plants and resulted in uneven flow distribution in reed beds. The plant management programme includes improving plant regrowth by harvesting of alternate strips of plants and replanting. The treated effluent meets water quality limits for discharge to the public sewer and is subsequently treated by the NEWater treatment system, which recycles water for industrial and indirect potable use.

One scenario put forward by researchers, political commentators and journalists for the collapse of North Korea has been a People’s Power (or popular) rebellion. This paper analyses why no popular rebellion has occurred in the DPRK under Kim Jong Un. It challenges the assumption that popular rebellion would happen because of widespread anger caused by a greater awareness of superior economic conditions outside the DPRK. Using Jack Goldstone’s theoretical expla-nations for the outbreak of popular rebellion, and comparisons with the 1989 Romanian and 2010–11 Tunisian transitions, this paper argues that marketi-zation has led to a loosening of state ideological control and to an influx of infor-mation about conditions in the outside world. However, unlike the Tunisian transitions—in which a new information context shaped by social media, the Al-Jazeera network and an experience of protest helped create a sense of pan-Arab solidarity amongst Tunisians resisting their government—there has been no similar ideology unifying North Koreans against their regime. There is evidence of discontent in market unrest in the DPRK, although protests between 2011 and the present have mostly been in defense of the right of people to support themselves through private trade. North Koreans believe this right has been guaranteed, or at least tacitly condoned, by the Kim Jong Un government. There has not been any large-scale explosion of popular anger because the state has not attempted to crush market activities outright under Kim Jong Un. There are other reasons why no popular rebellion has occurred in the North. Unlike Tunisia, the DPRK lacks a dissident political elite capable of leading an opposition movement, and unlike Romania, the DPRK authorities have shown some flexibility in their anti-dissent strategies, taking a more tolerant approach to protests against economic issues. Reduced levels of violence during periods of unrest and an effective system of information control may have helped restrict the expansion of unrest beyond rural areas.

First an overview of the systems currently in use and being discussed for sludge treatment is presented will) particular emphasis on distinguishing between the object of the system (conditioning objective of the various phases in the system) and a system concept (concept of various phases of the system in sequence to attain the disposal objective). More detailed information is given as to the salient systems as used with smaller sewage treatment plants in rural areas, such as digestion, dewatering, hygienization, composting and thermal drying. A further item of discussion is how sludge treatment influences the sewage treatment process. For the critical emissions (nitrogen, phosphorus) demanded in Germany, and thus for the degree of sewage treatment required, the load of the sewage treatment system resulting from sludge treatment needs to be taken into account. Accordingly, operation of sludge treatment and sewage purification must always be harmonized. The extent of these return loads also limits the spatial centralization of the system phases; this applies in particular to smaller sewage treatment plants in rural areas. In conclusion, an attempt is made to present a perspective for the agricultural utilization of such sludge in Germany. Since the critical values for emissions have been further tightened by new regulations, thus considerably elevating the associated sophistication of monitoring techniques, it is to be expected that the use of sewage sludge in agriculture will also be further reduced in rural areas, especially since public awareness of emission control has considerably reduced the acceptance of sewage sludge as fertilizer.

AbstractWe conducted a retrospective analysis of the data of 107 cases of peritonsillar abscess treated at our hospital between January 2014 and December 2018. Data on age, sex, affected site, duration of hospitalization, method of drainage, presence/absence of laryngeal edema, antibacterial drugs used, and isolated bacteria were analyzed. Of the 107 patients, 71 were males and 36 were females; the median age was 44 years (range: 18–88 years).The left side was affected in 55 patients, the right side in 50 patients, and both sides in two patients. The abscess was localized in the superior pole in 71 patients, and in the inferior pole in 36 patients. Thirty-five patients had laryngeal edema, of which three underwent tracheotomy. Recurrence of the abscess was observed in 15 cases, with the recurrence developing within 3 months in 7 cases, and over a period of 3 years in 4 cases. As for the sensitivity of the causative bacteria to antibacterial drugs, 17% of the causative bacteria showed resistance to clindamycin, while none showed resistance to ampicillin/sulbactam (ABPC/SBT). We concluded that ABPC/SBT might be suitable for the initial treatment of peritonsillar abscess, and that we need to bear in mind the possibility of long-term recurrence.

Anatomy and physiology are the basis of human body functioning and as we have progressed in management of various diseases, we have understood that physiological intervention is always better than an anatomical one. For more than 50 years, a standard approach to permanent cardiac pacing has been an anatomical placement of transvenous pacing lead at the right ventricular apex with a proven benefit of restoring the rhythm. However, the resultant ventricular dyssynchrony on the long-term follow-up in patients requiring more than 40% ventricular pacing led to untoward side effects in the form of heart failure and arrhythmias. To counter such adverse side effects, a need for physiological cardiac pacing wherein the electrical impulse be transmitted directly through the normal conduction system was sought. His bundle pacing (HBP) with an intriguing alternative of left bundle branch pacing (LBBP) is aimed at restoring such physiological activation of ventricles. HBP is safe, efficacious, and feasible; however, localization and placement of a pacing lead at the His bundle is challenging with existing transvenous systems due to its small anatomic size, surrounding fibrous tissue, long-learning curve, and the concern remains about lead dislodgement and progressive electrical block distal to the HBP lead. In this article, we aim to take the reader through the challenging journey of HBP with focus upon the hardware and technique, selective versus nonselective HBP, indications and potential disadvantages, and finally the future prospects.

Abstract INTRODUCTION Treatments for pediatric brain tumors (PBT) are neurotoxic and lead to long-term deficits that are driven by the perturbation of underlying white matter (WM). It is unclear if and how treatment may impair WM connectivity across the entire brain. METHODS Magnetic resonance images from 41 PBT survivors (mean age: 13.19 years, 53% M) and 41 typically developing (TD) children (mean age: 13.32 years, 51% M) were analyzed. Image reconstruction, segmentation, and node parcellation were completed in FreeSurfer. DTI maps and probabilistic streamline generation were completed in MRtrix3. Connectivity matrices were based on the number of streamlines connecting two nodes and the mean DTI (FA) index across streamlines. We used graph theoretical analyses to define structural differences between groups, and random forest (RF) analyses to identify hubs that reliably classify PBT and TD children. RESULTS For survivors treated with radiation, betweeness centrality was greater in the left insular (p < 0.000) but smaller in the right pallidum (p < 0.05). For survivors treated without radiation (surgery-only), betweeness centrality was smaller in the right interparietal sulcus (p < 0.05). RF analyses showed that differences in WM connectivity from the right pallidum to other parts of the brain reliably classified PBT survivors from TD children (classification accuracy = 77%). CONCLUSIONS The left insular, right pallidum, and right inter-parietal sulcus are structurally perturbed hubs in PBT survivors. WM connectivity from the right pallidum is vulnerable to the long-term effects of treatment for PBT.
