The present paper is about the research conducted on the patients with metabolic syndrome and its results of endoscopic treatment of benign prostatic hyperplasia in these patients. It is generally accepted that the size and volume of the prostate gland is the main prognostic factor affecting the intraoperative and postoperative outcomes of endoscopic treatment of BPH. However, with the rapid development of endoscopic instruments and surgical techniques, the size of the prostate tends to have a lesser influence on the outcome of endoscopic BPH treatment. Currently, MS is becoming an increasingly common disease, which in most cases predicts a poor prognosis for treatment of patients with LUTS.

Objective: Benign prostatic hyperplasia (BPH) is a common disease prevalent in elderly men but the genetic determinants of BPH is still remain unclear. Since metabolic syndrome, especially the diabetes, maybe influences the progression of benign prostatic hyperplasia, we investigated whether susceptibility loci for the diabetes would increase the risk of BPH development and progression in Chinese elderly men. Material and Methods: Fifteen SNPs associated with the diabetes risk in a Chinese population were genotyped in 377 BPH cases (152 aggressive and 225 non-aggressive BPH cases) and 1,008 controls. The association between the SNPs and risk of BPH development was evaluated through logistic regression. Additionally, effects of the 15 SNPs on BPH related clinical parameters, including body mass index (BMI) International Prostate Symptom Score (IPSS), Quality of Life (QoL) and prostate volume (PV) were also evaluated. Results: SNP rs9864104 in IGF2BP2 at 3q27 (OR=1.24, P =0.0148) was significantly associated with BPH development. In addition, SNP rs9863780, rs9864104, rs10229583 and rs17727841 were significantly associated with baseline clinical parameters in BPH patients. Moreover, the risk allele of rs6763887 (C) and rs17727841 (C) were significantly associated with the change of storage score and voiding score after treatment. No SNPs was associated with the risk of BPH progression. Conclusions: This is a systematic investigation on the contributions of diabetes susceptibility loci to risk of BPH development and progression. Our findings advance the understanding of the genetic basis of BPH and provide new insights into the genetic determinants shared between BPH and metabolic syndrome.

Introduction. Benign prostatic hyperplasia (BPH) is a neoplasm and clinically occurred by progressive enlargement of the prostate. However, prostate neoplasm can also be malignant, which is come out from high-grade prostatic intraepithelial neoplasia (PIN). It is a proven precancerous condition. Purpose. Тo evaluate the data published on September and October 2020 on the prevalence, pathogenesis of BPH and PIN and methods of their treatment. Materials and methods. The search results in scientific databases PubMed, MEDLINE, Embase were analyzed for the queries «BPH», «PIN», «epidemiology of BPH», «pathogenesis», «treatment of PIN». Results. BPH is a very common disease in the elderly population. The pathogenesis of BPH includes age, genetics or hormones disorders, growth factors, inflammation, and lifestyle factors. PIN is the only common precursor of prostatic cancer. The main treatment methods are 5α-reductase inhibitors, phosphodiesterase-5 inhibitors, and surgical methods. In addition, plant active molecules are also widely used in the treatment of BPH and PIN. Conclusions. While 5α-reductase and phosphodiesterase-5 inhibitors treatment, as well as surgical methods have a lot of adverse events and complications, a unique herbal complex Gardaprost was developed, which suppresses hyperplasia of prostate. Curcumin, genistein and epigalocatechin-3- gallate, which are part of the preparation, form a safe agent aimed at combating the growth of the prostate gland in BPH, and also has a significant effect in the prevention of exacerbations of chronic prostatitis and malignancy in patients with PIN.

Abstract Background Benign prostatic hyperplasia (BPH) is a common disease which causes various health problems for elderly men such as urinary retention, recurring urinary tract infection and bladder stones. The aim of this study is to evaluate the therapeutic effects of Juniperus communis L. seed extract (JCS) on BPH in male Wistar rats. Methods To this end, 30 rats were divided into 5 groups (N = 6): group 1 (vehicle), group 2 (disease control), group 3 (standard medicine; 10 mg/kg finasteride), and groups 4 and 5 were treated with 300 mg/kg and 600 mg/kg of the hydroalcoholic JCS seed extract, respectively. Groups 2, 3, 4 and 5 received testosterone enanthate to induce prostatic hyperplasia. At the end of experimental period (28 days), prostate glands were cut off under anesthesia. Histopathological examination was done and biochemical parameters such as Malondialdehyde, Glutathione and protein carbonyl were also measured. Their body weights were also observed during the study. At the end of the experiment, prostate weights and prostate specific antigen (PSA) levels were measured. Prostate index, inhibition prostate weight and inhibition prostate index were also calculated. Results Both histopathological examination and biochemical parameter results showed significant improvements in rats treated with finasteride and 600 mg/kg JCS extract (p < 0.01). In addition, PSA levels showed significant decrease in comparison with the disease group. But acute toxicity test indicated that using JCS extract resulted in an increase in liver enzymes (ALP, LDH, SGOT, SGPT). As a result, the extract should be used with caution. Conclusions Oral administration of JCS extract is effective on preventing testosterone-induced benign prostatic hyperplasia.

Objectives: Bladder neck contracture (BNC) is a bothersome complication following endoscopic treatment for benign prostatic hyperplasia (BPH). The objective of our study was to give a more realistic insight into contemporary endoscopic BNC treatment and to evaluate and identify risk factors associated with inferior outcome. Material and Methods: We identified patients who underwent transurethral treatment for BNC secondary to previous endoscopic therapy for BPH between March 2009 and October 2016. Patients with vesico-urethral anastomotic stenosis after radical prostatectomy were excluded. Digital charts were reviewed for re-admissions and re-visits at our institutions and patients were contacted personally for follow-up. Our non-validated questionnaire assessed previous urologic therapies (including radiotherapy, endoscopic, and open surgery), time to eventual further therapy in case of BNC recurrence, and the modality of recurrence management. Results: Of 60 patients, 49 (82%) and 11 (18%) underwent transurethral bladder neck resection and incision, respectively. Initial BPH therapy was transurethral resection of the prostate (TURP) in 54 (90%) and holmium laser enucleation of the prostate (HoLEP) in six (10%) patients. Median time from prior therapy was 8.5 (IQR 5.3–14) months and differed significantly in those with (6.5 months; IQR 4–10) and those without BNC recurrence (10 months; IQR 6–20; p = 0.046). Thirty-three patients (55%) underwent initial endoscopic treatment, and 27 (45%) repeated endoscopic treatment for BNC. In initially-treated patients, time since BPH surgery differed significantly between those with a recurrence (median 7.5 months; IQR 6–9) compared to those treated successfully (median 12 months; IQR 9–25; p = 0.01). In patients with repeated treatment, median time from prior BNC therapy did not differ between those with (4.5 months; IQR 2–12) and those without a recurrence (6 months; IQR 6–10; p = 0.6). Overall, BNC treatment was successful in 32 patients (53%). The observed success rate of BNC treatment was significantly higher after HoLEP compared to TURP (100% vs. 48%; p = 0.026). Type of BNC treatment, number of BNC treatment, and age at surgery did not influence the outcome. Conclusions: A longer time interval between previous BPH therapy and subsequent BNC incidence seems to favorably affect treatment success of endoscopic BNC treatment, and transurethral resection and incision appear equally effective. Granted the relatively small sample size, BNC treatment success seems to be higher after HoLEP compared to TURP, which warrants validation in larger cohorts.

Benign enlargement of the prostate gland does not always cause symptoms or obstruction to the flow of urine. Old terminology, for example, ‘prostatism’ can therefore be misleading, and the British Association of Urological Surgeons (BAUS), the International consultation on BPH and the International Continence Society accept the definitions given below:Benign prostatic hyperplasia (BPH) is a histological diagnosis. The first pathological signs appear under the age of 40 years, followed by a rapid increase in prevalence with age; 80% of 80-year-olds have evidence of BPH. The onset of BPH is dependent on the presence of functioning testes and increasing age. It is characterized by a combination of atrophy and proliferation in both glandular and stromal tissue. Although BPH is detectable in most elderly men, it does not always cause enlargement of the prostate, symptoms, or obstruction to the flow of urine.

Background The medical armamentaria for the treatment of lower urinary tract symptoms (LUTS) secondary to benign prostatic hyperplasia (BPH) have been extensively implemented over the past decade. Nevertheless, the timeliest moment for a possible treatment has not been fully established. Evidence Acquisition A systematic literature search in January 1996 until June 2016 was performed to answer the following question: in men with LUTS due to BPH, does early treatment result in better outcome? An ad hoc Population/patient Intervention/indicator Comparator/control Outcome (PICO) was developed. The Medline, PubMed and Scopus databases were searched. Each article title and abstract were reviewed for relevance and appropriateness with regard to the topic of this review. Evidence Synthesis Overtime, the introduction of novel medications and the implementation of surgical techniques have significantly improved the treatment outcomes and markedly reduced the rate of BPH surgery. Early treatments in patients at risk of disease progression may result in better clinical outcomes than a delayed approach. However, the evidence to support early intervention remains weak and criteria to identify the patient phenotype that could best benefit from immediate treatment remain ill-defined. On the contrary, the patients who ultimately undergo surgery following prolonged pharmacological treatment present with larger prostates, older age and comorbidities. At the same time, the technological progress has partly compensated this critical scenario, and commonly, a nonpejorative trend has been recorded in perioperative complications. Conclusions The ideal moment to begin a treatment in LUTS/BPH patients is still uncertain, and surprisingly, rare good quality studies are available on this topic.
