This research work is conceived as the analysis of the nursing care process in a user with a diagnosis of Acinetobacter baumannii, for this the methodology used is descriptive, with a non-experimental qualitative approach, since a study is carried out on the records of the nursing care applied to a polytraumatized patient with a history of a car accident, admitted to the Intensive Care Unit from the operating room, and later diagnosed with infection by Acinetobacter Baumannii, for which a care plan based on NANDA taxonomies was developed , NOC and NIC emphasizing the hemodynamic and respiratory improvement, which allowed the patient to achieve a positive evolution, managing to discharge her to general hospitalization without ventilatory support.

<p class="AbstractContent"><strong>Objective:</strong> Schizophrenia is a combination of various clinical symptoms in which the patients suffer cognitive, emotional, perceptional, and behavioral disorders. Nursing care for patients with schizophrenia who are hospitalized is of importance in order to restore patient’s health into the highest possible level. This study aimed to explore the implementation of nursing care for schizophrenia patients in inpatient units of a psychiatric hospital in Yogyakarta.</p><p class="AbstractContent"><strong>Methods: </strong>This was a descriptive quantitative study. Data were collected by observing 78 medical records which were selected using total sampling method. We used checklist form to measure the implementation of nursing care process from assessment, diagnosis, plan, implementation, and evaluation. Data were analyzed using frequency test.</p><p class="AbstractContent"><strong>Results:</strong> Over half of the patients was diagnosed with undetailed schizophrenia (67.9%) with the length of hospitalization was more than 14 days (59%). The implementation of nursing care was good in general (76.9%). The implementation of nursing care was fair (48.7%) for assessment and intervention (94.9), and good for diagnosis (71.8%), care plan (98.7%), evaluation (100%), and documentation (100%).<strong></strong></p><p class="AbstractContent"><strong>Conclusion: </strong>The implementation of nursing assessment and intervention was lower compared to other parts of nursing process. Nurse managers need to encourage their nursing staffs to provide accurate and complete documentation of nursing care, particularly in the assessment and intervention parts.<strong></strong></p><strong>Keywords: </strong>medical records, nursing care, schizophrenia

ABSTRACT Objective: To reflect on autonomy in health care, its perspective on the lives of the elderly, and its meaning for the practice of nursing care. Methods: Theoretical essay on the practice of nursing care to promote autonomy and self-care for the elderly. Results: It is necessary to qualify the practice of nursing care to consider the aging process and maintain the autonomy of the elderly. Three parts emerged: “The autonomy of the elderly and the care plan”, “Nursing in the self-care process”, and “Challenges to the practice of caring for the elderly in nursing”. Final considerations: There is still a deficiency in promoting autonomy for the elderly due to the need for a review of the practice of nursing care, which, considering aging in Brazil, needs to undertake new actions in order to encourage self-care and autonomy in this population.

BACKGROUND The integration of clinical practice guidelines (CPGs) into the nursing care plan and documentation systems aims to translate evidence into practice, improve safety and quality of care, and standardize care processes. OBJECTIVE This study aimed to evaluate nurses’ perceptions of the usability of a nursing care plan solution that includes 234 CPGs. METHODS A total of 100 nurses from 4 adult intensive care units (ICUs) responded to a survey measuring nurses’ perceptions of system usability. The survey included 37 rated items and 3 open-ended questions. RESULTS Nurses’ perceptions were favorable with more than 60.0% (60/100) in agreement on 12 features of the system and negative to moderate with 20.0% (20/100), to 59.0% (59/100) in agreement on 19 features. The majority of the nurses (80/100, 80.0% to 90/100, 90.0%) agreed on 4 missing safety features within the system. More than half of the nurses believed they would benefit from refresher classes on system use. Overall satisfaction with the system was just above average (54/100, 54.0%). Common positive themes from the narrative data were related to the system serving as a reminder for complete documentation and individualizing patient care. Common negative aspects were related to duplicate charting, difficulty locating CPGs, missing unit-specific CPGs, irrelevancy of information, and lack of perceived system value on patient outcomes. No relationship was found between years of system use or ICU experience and satisfaction with the system (P=.10 to P=.25). CONCLUSIONS Care plan systems in ICUs should be easy to navigate; support efficient documentation; present relevant, unit-specific, and easy-to-find information; endorse interdisciplinary communication; and improve safety and quality of care.

Abstract Introduction/Objective Acute interstitial pneumonia (AIP) is a rare disease clinically characterized by rapidly progressing respiratory failure in individuals with no history of respiratory illness or other inciting factors. While most often diagnosed in middle-aged adults, it may present in any age group. Initial presentation is described as influenza- like, and respiratory failure requiring ventilatory support often progresses within weeks to months. Prognosis is poor, with an estimated mortality rate approaching 80% without treatment. Methods We present the case of a 44-year-old male nonsmoker with no significant medical history, who presented in 2018 with 1.5 months of dyspnea and headache initially diagnosed as atypical pneumonia. Chest imaging revealed bilateral opacities; however, microbial workup revealed no evidence of infectious etiology. Autoimmune serology studies were likewise unrevealing. Despite aggressive supportive and medical management, he deteriorated to respiratory failure and succumbed. Results At autopsy, the lungs were symmetrically congested and edematous (combined weight 2,340 g) but free of evident consolidation or discrete lesions. Microscopic examination revealed diffuse alveolar damage with extensive hyaline membrane formation, interstitial edema, and fibroblastic proliferation. The vasculature was severely congested, and the alveoli contained hemorrhage and scattered macrophages. No fungal or mycobacterial elements were identified by staining. Based on the histologic features and clinical context, the diagnosis of AIP was made. Conclusion AIP is a rare, aggressive, and diagnostically challenging disease that includes a broad range of both clinical and histologic differentials. Timely recognition and intervention with aggressive respiratory support and high- dose glucocorticoids are the mainstays of clinical management. The diagnostic role of histology is significant, but hinges on early clinical consideration of AIP as disease progression may later preclude the biopsy procedure. We share this case to raise awareness of this rapidly progressive and diagnostically troubling interstitial lung disease while emphasizing the importance of clinicopathologic correlation.
