Background Many people use the internet to access health care information to support health care decisions, and hospital websites can be the first point of contact to provide health care information services for consumers. However, little is known about the current information services provided by the websites of large Chinese hospitals. Objective The aim of this study is to evaluate and compare the information services of the websites of large hospitals in China and the United States. We hope that our findings will benefit hospital managers worldwide in providing service information on the web. Methods This study adopted a cross-sectional analytical approach to evaluate the websites of large hospitals in China and the United States in 2020. A total of 300 large hospitals were randomly selected, of which half were in China and half were in the United States. Based on the 7Ps marketing mix, we identified 39 items that represent typical hospital website information services, covering the following seven dimensions: product, price, place, propagation, people, process, and physical evidence. Results Most of the items (34/39, 87%) related to information services offered by hospital websites were less covered in China than in the United States; however, 5 items (appointments by a third-party platform, mobile payment, hospital value, hospital environment display, and physicians’ profiles) had higher coverage in China. The average scores for hospital websites in China and the United States were 13.25 (SD 2.99) points and 23.16 (SD 2.76) points, respectively. Generally, high scores were given to the south areas of China and north areas of the United States. Conclusions Hospital websites in China lagged behind those in the United States with regard to information services offered. We recommend that hospital managers in China place more emphasis on the people, product, and propagation dimensions of the 7Ps marketing mix in the construction of information services on hospital websites. Through the comparison of the websites of large hospitals in China and the United States, our study findings can provide suggestions for forming standard hospital website construction guidelines worldwide.

Objective To determine the prevalence of dysphagia, reported etiologies, and impact among adults in the United States. Study Design Cross-sectional analysis of a national health care survey. Subjects and Methods The 2012 National Health Interview Survey was analyzed, identifying adult cases reporting a swallowing problem in the preceding 12 months. In addition to demographic data, specific data regarding visits to health care professionals for swallowing problems, diagnoses given, and severity of the swallowing problem were analyzed. The relationship between swallowing problems and lost workdays was assessed. Results An estimated 9.44 ± 0.33 million adults (raw N = 1554; mean age, 52.1 years; 60.2% ± 1.6% female) reported a swallowing problem (4.0% ± 0.1%). Overall, 22.7% ± 1.7% saw a health care professional for their swallowing problem, and 36.9% ± 0.1.7% were given a diagnosis. Women were more likely than men to report a swallowing problem (4.7% ± 0.2% versus 3.3% ± 0.2%, P < .001). Of the patients, 31.7% and 24.8% reported their swallowing problem to be a moderate or a big/very big problem, respectively. Stroke was the most commonly reported etiology (422,000 ± 77,000; 11.2% ± 1.9%), followed by other neurologic cause (269,000 ± 57,000; 7.2% ± 1.5%) and head and neck cancer (185,000 ± 40,000; 4.9% ± 1.1%). The mean number of days affected by the swallowing problem was 139 ± 7. Respondents with a swallowing problem reported 11.6 ± 2.0 lost workdays in the past year versus 3.4 ± 0.1 lost workdays for those without a swallowing problem (contrast, +8.1 lost workdays, P < .001). Conclusion Swallowing problems affect 1 in 25 adults, annually. A relative minority seek health care for their swallowing problem, even though the subjective impact and associated workdays lost with the swallowing problem are significant.

Guidelines aimed at slowing the spread of COVID-19 resulted in major changes in people’s lives. A cross-sectional online survey, completed by 1,405 adults in Canada and the United States in June 2020, found respondents from rural areas/small towns reported better coping and adjustment (i.e., less use of substances for support), less personal impact, less life disruption, and fewer challenges with transportation and health care, than urban and suburban respondents. Those in rural areas were less likely to use the newspaper, but more likely to use social media, for information. Finally, rural respondents reported higher levels of support for their national leadership’s response to the pandemic. The needs and strengths of rural areas, as well as approaches to serve rural areas are discussed.

Abstract Context.—Changes in health care economics and organization have resulted in increased use of nonphysician providers in most health care settings. Attitudinal acceptance of nonphysician providers is important in the current health care environment. Objectives.—To obtain descriptive information regarding pathology resident attitudes and opinions about pathologists' assistants in anatomic pathology practice and to assess the implications of resident attitudes and opinions for pathology practice and training. Design.—A self-administered, mailed, voluntary, anonymous questionnaire was distributed to a cross-sectional sample of pathology residents in the United States (2531 pathology residents registered as resident members of one of the national pathology professional organizations). The questionnaire contained (1) items relating to resident demographics and program characteristics, (2) Likert-scale response items containing positive and negative statements about pathologists' assistants, (3) a multiple-choice item related to pathologists' assistants scope of practice, and (4) an open-ended item inviting additional comments. Both quantitative and qualitative analysis of responses was performed. Results.—The overall response rate was 19.4% (n = 490); 50% of the respondents were women, and 77% reported use of pathologists' assistants in their program. Most respondents were 25 to 35 years old and in postgraduate years 3 through 5 of their training, and most were located in the Midwestern United States. The majority of residents expressed overall positive attitudes and opinions about pathologists' assistants and felt that pathologists' assistants enhanced resident training by optimizing resident workload. A minority (10%–20%) of residents expressed negative attitudes or opinions about pathologists' assistants. Additionally, some residents reported a lack of knowledge about pathologists' assistants' training or roles. Conclusions.—Increased resident education and open discussion concerning pathologists' assistants may be beneficial for optimizing resident attitudes about and training experiences with pathologists' assistants.

According to the Ponemon (2012) Third Annual Benchmark Study on Patient Privacy & Data Security, 94 percent of healthcare organizations surveyed suffered at least one data breach; 45 percent experienced more than five in the past two years. Data breaches cost the U.S. healthcare industry an average of $7 billion annually (Ponemon, 2012). Electronic health records are becoming more pervasive at hospitals and clinics in the United States. Meanwhile, healthcare organizations are taking small steps toward meaningful exchange and secure data security of patient information. This has created a need for new expertise in health data security from a newly degreed and young in information security professionals from the “Millennial Generation”. This chapter explores the attraction, recruitment, and retention of younger-generation professionals with critical and emerging health information security skills.

Background: Homelessness is a significant public health issue in the United States. Living in rural locations has been associated with an increase in poverty. Additionally, it has been found that veterans are at greater risk for homelessness than the general population. The aim of this research was to characterize rural homeless veterans and non-veterans living in Nebraska, United States. Methods: A cross-sectional study was conducted comprising 50 veterans and 64 non-veterans recruited from rural locations in Nebraska. Fully structured interviews were conducted by the research staff that consisted of questions regarding participant sociodemographics, housing, clinical characteristics, psychosocial factors, and utilization of health care and social services. Results: In comparison to non-veterans, rural homeless veterans were found to be older, more qualified, and more likely to have ever been married. Veterans spent fewer nights in a shelter and more nights in a halfway house. Regarding clinical features, veterans were more likely to report posttraumatic stress disorder and alcohol misuse. Veterans also reported shorter travel times to reach health care services and used them more often compared to non-veterans. Conclusion: These findings suggest that homeless veterans and non-veterans within rural settings have unique needs to be addressed when it comes to providing health care and social services, as well as in attempts to eliminating homelessness. Further research will help in the development of improved methods to support rural veterans and non-veterans.

Purpose: Recent cancer drug approvals are lauded as being more effective with relatively fewer adverse effects, but these treatments come with a great cost to the US health care system. There is little information on recent trends in actual antineoplastic expenditures representative of the whole US health care system or by sector. Therefore, the objective of this study was to describe antineoplastic expenditures in the United States by year and sector. Methods: This was a retrospective, cross-sectional study of IQVIA (formerly QuintilesIMS) National Sales Perspective data for the period of January 1, 2011, to December 31, 2016. Actual expenditures were totaled by health care sector and calendar year, then adjusted for medical-cost inflation to 2016 dollars. Growth was calculated as the percentage increase from the previous year. Results: Total expenditures of antineoplastic agents across all channels grew from $26.8 billion in 2011 to $42.1 billion in 2016. Antineoplastic spending increased 12.2% in 2016 (compared with the previous year), followed by 15.6% in 2015, 13.4% in 2014, 6.3% in 2013, and 0.4% in 2012. Throughout the study period, 96.5% of total antineoplastic expenditures occurred within clinics, mail-order pharmacies, nonfederal hospitals, and retail pharmacies. Conclusion: Antineoplastic expenditures are expected to increase because of continuing development and approval of costly targeted cancer therapies. Cost containment and utilization management strategies must be balanced so as not to restrict access or disrupt innovation. Future policies should focus on ensuring safe and appropriate use of antineoplastics while balancing long-term drug costs.
