The major problem encountered in recent days due to the advent of emerging antimicrobial resistance strains is Hospital Acquired Infections (HAIs). The spread of HAI is mainly through the hospital staff and the hospital settings’ inanimate surfaces. The study includes the surveillance of hospital surfaces, including wards and ICUs, by taking swabs dipped in normal saline. Pseudomonas aeruginosa (53.8%) was found to be the most common pathogen isolated, followed by Klebsiella species (18%), Methicillin-Sensitive Staphylococcus aureus (16%), Coagulase-Negative Staphylococci (6%), Methicillin-Resistant Staphylococcus aureus (3%). The widespread presence of bacterial sensitivity to antimicrobials and the modifications insensitivity forms the basis for designing antibacterial therapy’s practical recommendations and rational use of antimicrobials. Proper display of all Information, Education, and Communication (IEC) materials at appropriate places play a significant role in preventing nosocomial infections. Audiovisual aids and training to the staff play utmost importance in preventing the spread of HAIs. All these can reduce the occurrence and outbreak of nosocomial conditions. Overall, these minimize health care costs.

The main objective of the study was to find out the contamination of different types of microbial organisms and their resistance pattern on the dresses and stethoscopes of the health care workers in order to evaluate the risk of transmission of microbial organisms and its consequences on safety of the patient and control of hospital acquired infections (HAIs). Swabs were taken from the dresses of 100 different healthcare workers from 4 different areas namely collar, pocket, sleeves and sides and 100 stethoscopes (diaphragm) were tested for bacteriological analysis using standard techniques. Then each health care worker were given a structured questionnaire and requested to fill the form which includes his or her speciality/unit, cadre, practice of hand hygiene, white-coat or uniform usage (Example: duration of usage, frequency of washing, type of washing etc). The collar and pockets were found to be the most contaminated areas. Gram-positive cocci such as Staphylococcus aureus and Coagulase-negative Staphylococci (CONS) were the important organisms isolated from the dresses and stethoscopes of health care workers followed by Gram-negative bacilli such as Pseudomonas aeruginosa and Klebsiella species. Among the microbial organisms, Staphylococcus aureus and CONS were resistant to the drugs like Co-trimoxazole and Penicillin-G. Organisms isolated from the dresses and stethoscopes of health care workers could be a source of infection to immunocompromised patients. Hence, frequent washing of the dresses and strict disinfectant practices of the stethoscopes will minimize the contamination with microbial organisms and the patient safety is improved in the hospital environment.

Background and Objectives: Staphylococcus aureus infections especially MRSA infections are an important cause of nosocomial infections worldwide and multidrug resistance is also very commonly seen in these infections. In a healthcare setup, these infections can be acquired through the hands, clothes, and equipments of health care workers. Hence, this study was conducted to see the percentage of MRSA carriage among healthcare workers and to study their antibiotic susceptibility pattern. Materials and Methods: Nasal swabs collected from various categories of Healthcare workers were subjected to processing in the Bacteriology section of Microbiology lab, GMC Jammu. Staphylococcus aureus identification was done according to the standard procedures of the lab. Cefoxitin disc was used for the identification of MRSA strains. Results: A total of 240 volunteers participated in the study. Among the samples screened 38 (16%) were MRSA, 28 (12%) were MSSA. In 172 (72%) no colonization was seen. Highest prevalence of MRSA was seen among Nurses 18 (27%), followed by Laboratory Technicians 10 (15%), Doctors 6 (9%) and Paramedics 4 (6%). Vancomycin and Linezolid were the most sensitive drugs showing 100 % sensitivity. Penicillin was 100 % resistant. Conclusion: A study of MRSA carriage among healthcare workers can help in framing proper and timely antibiotic policies in our hospital which would further stop the emergence of multidrug resistant organism and also guide us in decreasing the prevalence of nosocomial infections. Abstract Background Blood stream infections (BSIs) due to Gram-positive pathogens such as methicillin-resistant Staphylococcus aureus (MRSA) are associated with high mortality ranging from 10 to 60%. The current anti-MRSA agents have limitations with regards to safety and tolerability profile which limits their prolonged usage. Levonadifloxacin and its oral prodrug alalevonadifloxacin, a novel benzoquinolizine antibiotic, have recently been approved for acute bacterial skin and skin structure infections including diabetic foot infections and concurrent bacteremia in India. Methods The present study assessed the potency of levonadifloxacin, a novel benzoquinolizine antibiotic, against Gram-positive blood stream clinical isolates (n = 31) collected from January to June 2019 at a tertiary care hospital in Mumbai, India. The susceptibility of isolates to antibacterial agents was defined following the Clinical and Laboratory Standard Institute interpretive criteria (M100 E29). Results High prevalence of MRSA (62.5%), quinolone-resistant Staphylococcus aureus (QRSA) (87.5%), and methicillin-resistant coagulase-negative staphylococci (MR-CoNS) (82.35%) were observed among bacteremic isolates. Levonadifloxacin demonstrated potent activity against MRSA, QRSA, and MR-CoNS strains with significantly lower minimum inhibitory concentration MIC50/90 values of 0.5/1 mg/L as compared with levofloxacin (8/32 mg/L) and moxifloxacin (2/8 mg/L). Conclusion Potent bactericidal activity coupled with low MICs support usage of levonadifloxacin for the management of BSIs caused by multidrug resistant Gram-positive bacteria.

Objectives: This study was performed to access the knowledge of health care professionals regarding health care associated infections, nosocomial pathogens, fomites and their role in transmission of nosocomial pathogens. Study Design: Descriptive, questionnaire based, cross-sectional study. Setting: Tertiary care hospital of Lahore, Punjab. Period: From October 2017 to January 2018. Material & Methods: Questionnaires were carefully formulated to access basic knowledge of physicians, surgeons and nurses. Responses given were analyzed and recorded as frequency and percentage. Results: Regarding health care associated infections, respiratory tract infections were identified by 72% surgeons, 65% physicians and 59% nurses. Surgical wound infections were identified by 76% of surgeons and 81% nurses. 45.7% physician identified bacteraemia as nosocomial infection. Rate of identification for rest of nosocomial infections was sub optimum (<50%) by health care workers. Regarding identification of nosocomial pathogens, Methicillin Resistant Staphylococcus aureus was marked by 65% of physicians, 83.8% of nurses, 76% of surgeons. Pseudomonas nosocomial pathogen was identified by 40%, 46% and 64% of physicians, nurses and surgeons respectively. The rate of identification for rest of the nosocomial organisms was again sub optimum (<40%) by health care workers. Regarding fomites, mattresses and pillows, thermometer, stethoscopes were identified by 75.7%, 59.2 and 50% of Health care professionals respectively. Conclusion: This survey identified positive attitude among Health care workers towards infection control but low level of knowledge regarding health care associated infections and nosocomial pathogens. Therefore, to prevent nosocomial infections, there is strong need to develop strategies for improving knowledge of Health care professionals.

 Objective: The aim of this study was to find the prevalence of methicillin-resistant Staphylococcus aureus (MRSA) carrier status in anterior nares and hands of health-care professionals working in orthopedic wards of a tertiary care hospital and to decolonize them to reduce spread of MRSA to their patients.Methods: The study was conducted in a super specialty, tertiary care teaching hospital. The samples were collected from anterior nares, palm, web spaces, and fingertips of 140 health-care professionals (48 doctors, 74 nurses, and 18 technicians) working in orthopedic wards using sterile pre-moistened swabs. MRSA carrier status was identified by Kirby-Bauer disc diffusion method.Result: Most (76.4%) of the health-care professionals were <30 years of age and 51% were male. MRSA in anterior nares of doctors was 4.3%, nurses 1.4%, and technicians 0.7% and none had MRSA in their hands. Methicillin-sensitive Staphylococcus aureus (MSSA) growth was more among nurses (nurses - 5.7%, doctors - 2.1%, and technicians - 1.4%) in anterior nares. In addition, 1.4% nurses’ hands were colonized with MSSA. Both MRSA and MSSA carriers were decolonized effectively and repeat sampling showed no growth.Conclusion: Health-care professionals have a greater chance of transmitting MRSA to patients and orthopedic patients are more susceptible for infection. Although MRSA carrier status was not very high among orthopedic health-care professionals compared to previous studies, it cannot be ignored. Nasal mupirocin and bath with chlorhexidine soap were effective in decolonization. Periodic screening and treatment of colonizers would help in elimination of MRSA carriage.
