Latar Belakang. Disfungsi tiroid terjadi jika fungsi kelenjar tiroid terganggu, ditandai dengan kadar Thyroid Stimulating Hormone (TSH) dan free Thyroxine (fT4) yang lebih tinggi atau lebih rendah dari normal. Perubahan kadar hormon tiroid menyebabkan perubahan basal metabolic rate (BMR) yang menyebabkan perubahan keseimbangan energi dan berat badan. Tujuan. Penelitian bertujuan untuk menganalisis hubungan TSH dan fT4 dengan indeks massa tubuh (IMT) pada pasien dewasa baru di Klinik Litbangkes Magelang. Metode. Penelitian ini dilakukan dengan menggunakan metode cross-sectional dan menggunakan data sekunder dari rekam medis pasien. Sampel penelitian adalah pria dan wanita dewasa (>17 tahun) yang pertama kali mengunjungi Klinik Litbangkes Magelang pada tahun 2019 sebanyak 173 orang. Variabel penelitian adalah TSH, fT4, dan IMT. Pengujian hipotesis dengan uji statistik Korelasi Rank Spearman. Hasil. Rata-rata kadar TSH responden adalah normal yaitu 0,91±2,17 µIU/mL, sedangkan rata-rata kadar fT4 responden di atas normal yaitu 3,20±5,83 ng/dL. Sebagian besar subjek memiliki IMT normal (55,5%) dengan rata-rata 23,74±4,57. Uji Korelasi Rank Spearman menunjukkan bahwa terdapat hubungan yang signifikan antara TSH dan IMT (p=0,003) dengan korelasi positif (r=0,228) dan ada hubungan yang signifikan antara fT4 dan IMT (p=0,000) dengan korelasi negatif (r=-0.323). Kesimpulan. Terdapat hubungan antara kadar TSH dan fT4 dengan nilai IMT pasien. Semakin tinggi kadar TSH maka semakin tinggi pula nilai IMT pasien, begitu juga sebaliknya, serta semakin tinggi kadar fT4 maka nilai IMT pasien semakin rendah, dan sebaliknya.

Latar belakang. Talasemia β Mayor merupakan kelainan herediter yang disebabkan gangguan produksi rantai globin. Transfusi rutin menyebabkan kelebihan besi yang tertimbun dalam jaringan sehingga menyebabkan kardiomiopati, gangguan liver, dan komplikasi endokrin. Tiroid merupakan kelenjar endokrin yang berperan penting bagi anak. Pengendapan besi di kelenjar tiroid dapat menyebabkan gangguan fungsi tiroid. Pemberian kelasi besi deferiprone dan deferasirox dan pengaruhnya pada kadar TSH dan FT4 perlu dievaluasi lebih lanjut.Tujuan. Menganalisis perbedaan kadar TSH dan FT4 pada pasien anak dengan talasemia β mayor menggunakan kelasi besi deferiprone dan deferasirox Metode. Penelitian analitik dengan rancangan potong lintang (cross sectional) terhadap 43 pasien talasemia β mayor anak berusia 9-18 tahun pada bulan April sampai Juni 2017. Pemilihan subjek dilakukan secara consecutive sampling. Data di analisis dengan SPSS 20 mengunakan uji t independen dan uji man whitney.Hasil. Rerata usia pasien 12,5+3,12 tahun. Rerata kadar TSH kelompok deferiprone dan deferasirox adalah 3.051,78 IU/ml dan 2.351,29 IU/ml. Sedangkan rerata kadar FT4 untuk kelompok deferiprone dan deferasirox 15.424,12 mmol/l dan 15.822,75 IU/ml. Tidak terdapat perbedaan kadar TSH dan FT4 pada kelompok yang mendapatkan deferasirox dan deferiprone berturut-turut nilai TSH (p=0,148; p>0,05) dan FT4 (p=0,836; p>0,05). Kesimpulan. Tidak terdapat perbedaan kadar TSH dan FT4 pada pasien talasemia beta mayor yang mendapatkan kelasi deferasirox maupun deferiprone.

AbstractThyroid-stimulating hormone (TSH) and free thyroxine (FT4) reference intervals are essential for screening and diagnosing thyroid dysfunction during pregnancy. The aim of this study was to establish method- and trimester-specific TSH and FT4 reference intervals in pregnant Chinese women using the Beckman Coulter UniCel™ DxI 600.A cross-sectional dataset analysis was performed. A total of 3507 participants were recruited, and 2743 were eligible for analysis to set reference intervals. TSH, FT4, and thyroid peroxidase antibody (TPOAb) levels were analyzed with the Beckman Coulter UniCel™ DxI 600 AccessThe calculated reference intervals for the first, second, and third trimesters were TSH: 0.06–3.13, 0.07–4.13 and 0.15–5.02 mIU/L, respectively, and FT4: 8.72–15.22, 7.10–13.55 and 6.16–12.03 pmol/L, respectively.Our reference intervals for TSH and FT4 are distinct from the ranges reported in the DxI 600 instruction manual and previously reported data, confirming the importance of method-specific reference intervals.

Background: Hyperthyroidism is a thyroid disorder caused by increased synthesis and secretion of thyroid hormone by the thyroid gland. Hyperthyroidism is characterized by low thyroid stimulating hormone (TSH) and high free thyroxine (FT4) levels. The prevalence of hyperthyroidism in Indonesia was 0.6% in female and 0.2% in male. Zinc acts as an enzyme cofactor that converts thyroxine into triiodothyronine and it affects TSH synthesis. Adequacy of dietary zinc intake and serum zinc levels in hyperthyroid patients indirectly affects TSH and FT4 hormones.Objective: to analyze the correlation of dietary zinc intake and serum zinc with TSH and FT4 levels in adult hyperthyroid patients.Materials and Methods: This cross sectional study sampling based on inclusion and exclusion criteria which obtained 50 adult hyperthyroid patients at the Clinic of the Magelang Health Research and Development Center. Data of TSH and FT4 levels were measured by the ELISA method, serum zinc was measured with the ICP-MS method, dietary zinc intake’s data was collected using the 24 hour dietary recall and it was analyzed based on the table of Indonesian food composition 2017. Statistical analysis used in this study were spearman correlation and multivariate linear regression with 95% confidence level.Results : Dietary zinc intake did not correlate with both TSH and FT4 levels (p>0.05). Serum zinc had no correlation with TSH level (p>0.05), while serum zinc had a positive correlation with FT4 level (r=0.327; β=0.054; p<0.05).Conclusion: Serum zinc concentrations in adult hyperthyroid patients must be maintained because abnormal zinc status in the body’s metabolism will affect FT4 level.International Journal of Human and Health Sciences Vol. 05 No. 03 July’21 Page: 341-346

AbstractThyroid hormone reference intervals are crucial for diagnosing and monitoring thyroid dysfunction during early pregnancy, and the dynamic change trend of thyroid hormones during pregnancy can assist clinicians to assess the thyroid function of pregnant women. This study aims to establish early pregnancy related thyroid hormones models and reference intervals for pregnant women. We established two derived databases: derived database* and derived database#. Reference individuals in database* were used to establish gestational age-specific reference intervals for thyroid hormones and early pregnancy related thyroid hormones models for pregnant women. Individuals in database# were apparently healthy non-pregnant women. The thyroid hormones levels of individuals in database# were compared with that of individuals in database* using nonparametric methods and the comparative confidence interval method. The differences in thyroid stimulating hormone and free thyroxine between early pregnant and non-pregnant women were statistically significant (p<0.0001). The reference intervals of thyroid stimulating hormone, free thyroxine and free triiodothyronine for early pregnant women were 0.052–3.393 μIU/ml, 1.01–1.54 ng/dl, and 2.51–3.66 pg/ml, respectively. Results concerning thyroid stimulating hormone and free thyroxine reference intervals of early pregnancy are comparable with those from other studies using the same detection platform. Early pregnancy related thyroid hormones models showed various change patterns with gestational age for thyroid hormones. Early pregnancy related thyroid hormones models and reference intervals for pregnant women were established, so as to provide accurate and reliable reference basis for the diagnosing and monitoring of maternal thyroid disfunction in early pregnancy.
