The aim of this study was to assess the accuracy of visual-tactile inspection (VTI) and laser fluorescence (LF) in detection of initial carious lesions on occlusal surfaces. Total of 120 premolars and molars, without any macroscopic cavitation on occlusal surface, planed to be extracted for orthodontic, prosthetic or surgical reasons, were included in this study. The agreement between histological diagnosis and diagnostic and treatment decisions of evaluated methods was assessed. The obtained results showed that specificities and sensitivities were for VTI=83 and 36%, and LF-77 and 96%. The percentage of correct treatment decisions were: VTI=54% and LF-80%. Diagnosing caries was significantly dependent on the fissure morphology and LF showed statistically better results than VTI. It can be concluded that laser fluorescence was significantly efficient and more accurate in diagnosing initial carious lesions on occlusal surfaces.

Posterior composite restorations are difficult to contour and polish due to their occlusal anatomy and opposing occlusion. Our study describes a technique for duplicating occlusal surface anatomy, using different dental materials that are able to copy anatomic details. The aim of the study was to evaluate the efficiency of occlusal matrix technique, by comparing technical ease of different dental materials. The study was conducted on extracted teeth which had intact occlusal surfaces or initial carious lesions at this level. An impression of the occlusal surface was made prior to cavity preparation, with four different dental materials: a flow composite, a dual-cure resin cement, a temporary acrylic resin (powder and liquid) and a temporary crown and bridge two-component material in a Unidose®. When the last layer of composite has been placed, the occlusal matrix was forced into the uncured composite to replicate the original occlusal surface, instead of performing manual curing and shaping as in the standard approach. Time needed for complete finishing the case, the hardness of each occlusal matrix and the final morphology obtained were evaluated. The main benefits of the occlusal matrix technique are the technical ease of use due to its simplicity and its high accuracy in reconstructing occlusal morphology. Although the shortest time needed for occlusal matrix preparation was for the dual-cure resin cement, the best oclusssal matrix was obtained with the temporary acrylic resin (powder and liquid).

Abstract The diagnostic management is a very important and integral part of the entire treatment process and has a direct influence on the decision-taking on the choice of the most appropriate form of therapy consistent with current knowledge. Knowledge of the morphology of hard dental tissues lesions has led to the development of quantitative methods for diagnosis and monitoring of dental caries, which enabled the implementation of appropriate treatments aimed at repairing than replacing damaged tissue. The aim of the study was to compare selected diagnostic methods: visual (ICDAS), impedance spectroscopy (CarieScan PRO) and laser fluorescence (Diagnodent Pen) in detecting caries in grooves on the chewing surfaces of molars and premolars. The obtained results indicated a high concordance of measurements performed with the Diagnodent Pen with the results of visual examination and a lower compliance of visual examination with the results obtained using the CarieScan PRO. A combination of visual and tactile method with tests using advanced technology provides greater opportunity to confirm the diagnosis of carious lesions requiring medical intervention.

Introduction: Dental operating microscopes (DOM) enable dentists to examine, with the aid of magnification and photodocumentation in clinical conditions, the occlusal anatomy of newly erupted permanent molars and to detect any early carious lesions more precisely. Aim: To determine the advantages of magnifying technology in detecting early occlusal carious lesions in newly erupted permanent first molars. Materials and methods: We examined 176 first molars of 44 children divided into two age groups: 7-8 and 9-10 years. The surfaces of each of the teeth were examined after they were cleaned with a brush without paste. The diagnoses were first made using only illumination and a clinical examination. These were followed by a visual examination of the occlusal surface using a DOM at ×8 magnification. A photo of the occlusal surface was then taken with a camera at the respective magnification for the purposes of photodocumentation. Results: This study demonstrated that dental operating microscopes increased by 7% the rate of successful detection of early caries lesions on the occlusal surfaces. There was also a significant increase of the number of diagnosed cavitated caries in the dentine in comparison with those detected with naked eye. Using DOMs in pediatric dentistry facilitates the more detailed and precise diagnosing of early stage carious lesions and/or cavitations in the fissures. Conclusion: The use of a DOM facilitates differential diagnosing between deep fissures and early carious lesions, which is a key point in the assessment of occlusal surfaces of newly erupted permanent molars, necessary for their preventative sealing and micro-invasive treatment.

Regenerative medicine-based approaches for caries treatment focus on biomimetic remineralization of initial carious lesions as a minimal invasive therapy. In vitro, self-assembling peptide P11-4 enhances remineralization of early carious lesions. To investigate the safety and clinical efficacy of P11-4 for treatment of initial caries, a randomized controlled single-blind study was conducted on children aged >5 y with visible active early caries on erupting permanent molars. Subjects were randomized to either the test group (P11-4 + fluoride varnish) or control group (fluoride varnish alone). Caries were assessed at baseline and at 3 and 6 mo posttreatment per laser fluorescence, a visual analog scale, the International Caries Detection and Assessment System, and Nyvad caries activity criteria. Intention-to-treat analyses were performed, and safety and clinical feasibility of the treatment approaches were assessed. Compared with the control group, the test group showed clinically and statistically significant improvement in all outcomes at 3 and 6 mo. The laser fluorescence readings (odds ratio = 3.5, P = 0.015) and visual analog scale scores (odds ratio = 7.9, P < .0001) were significantly lower for the test group, and they showed regression in the International Caries Detection and Assessment System caries index (odds ratio = 5.1, P = 0.018) and conversion from active to inactive lesions according to Nyvad criteria (odds ratio = 12.2, P < 0.0001). No adverse events occurred. The biomimetic mineralization facilitated by P11-4 in combination with fluoride application is a simple, safe, and effective noninvasive treatment for early carious lesions that is superior to the presently used gold standard of fluoride alone. By regenerating enamel tissue and preventing lesion progression, this novel approach could change clinical dental practice from a restorative to a therapeutic approach. This could avoid additional loss of healthy hard tissue during invasive restorative treatments, potentially enabling longer tooth life and thereby lowering long-term health costs ( ClinicalTrials.gov NCT02724592).

Objective: This study aimed to investigate the association of two fluorescent dyes and Laser Fluorescence (LF) device in detecting smooth and occlusal natural caries in primary teeth in vitro.Methods: Measurements were performed with the LF and with LF associated with tetrakis (N-methylpyridyl)porphyrin (LF TMPyP) and protoporphyrin IX (LF PPIX) in 72 smooth (63 primary molars) and 134 occlusal sites (81 primary molars). For validation, surfaces were sectioned and sections obtained were evaluated under stereomicroscope. Smooth surfaces were also evaluated using polarized light microscopy and Knoop microhardness. For both smooth and occlusal surfaces, ROC analyses were performed, and sensitivities, specificities and accuracies were assessed. In smooth surfaces, Pearson’s correlation coefficients between LF values and lesions hardness or lesions depth were calculated.Results: LF TMPyP presented higher correlation with hardness and lesion depth than other methods in smooth surfaces. No differences were observed in other parameters among the methods, in both smooth and occlusal surfaces.Relevance: The LF TMPyP might improve performance in quantifying smooth-surface caries lesions in primary teeth. However, the sensitivity is improved at D2 (caries extending into inner half of the enamel but not to amelodentinal junction) threshold when using PPIX in smooth caries lesions. The association of LF with fluorescent dyes does not improve the performance on occlusal caries.

AbstractPlacodontia were a group of marine reptiles that lived in shallow nearshore environments during the Triassic. Based on tooth morphology it has been inferred that they were durophagous, but tooth morphology differs among species: placodontoid placodonts have teeth described as hemispherical, and the teeth of more highly nested taxa within the cyamodontoid placodonts have been described as flat. In contrast, the sister taxon to the placodonts, Palatodonta bleekeri, like many other marine reptiles, has tall pointed teeth for eating soft-bodied prey. The goals of this paper are to quantify these different tooth morphologies and compare tooth shape among taxa and with a functionally “optimal” tooth. To quantify tooth morphology we measured the radius of curvature (RoC) of the occlusal surface by fitting spheres to 3D surface scans or computed microtomographic scans. Large RoCs correspond to flatter teeth, while teeth with smaller RoCs are pointier; positive RoCs have convex occlusal surfaces, and a negative RoC indicates that the occlusal surface of the tooth is concave. We found the placodontoid taxa have teeth with smaller RoCs than more highly nested taxa, and palatine teeth tend to be flatter and closer to the optimal morphology than maxillary teeth. Within one well-nested clade, the placochelyids, the rearmost palatine teeth have a more complex morphology than the predicted optimal tooth, with an overall concave occlusal surface with a small, medial cusp. These findings are in keeping with the hypothesis that placodonts were specialized durophagous predators with teeth modified to break hard prey items while resisting tooth failure.

This study validates a case-based survey method and analyzes the extent to which Danish dental professionals apply current concepts and strategies for occlusal caries management in children, adolescents, and young adults. A case-based, precoded questionnaire consisting of 10 clinical cases/patients with 26 teeth/occlusal surfaces was developed. The cases were set up in a PowerPoint presentation and color printed as a booklet illustrating patients with different patterns of caries activity, severity, and risk. A total of 69 dental professionals participated. Content and face validity of the survey method was established using a panel of experts. The panel also assessed the reliability of the method using a test-retest procedure (κ ≥ 0.80) and acting as benchmark. Measurements of agreement between dental professional and benchmark assessments showed substantial agreement for overall caries activity and risk assessment of patients and for clinical and radiographic severity of occlusal lesions (κ = 0.61-0.67). For assessment of caries lesion activity on occlusal surfaces, the agreement was moderate (κ = 0.50). Regarding treatment decisions, dental professionals showed substantial agreement when indicating restorative treatments (κ = 0.68). Multivariate logistic regression analysis showed a significant influence of various patient-, lesion-, and participant-related variables in the assessment of caries risk, caries activity and severity, and treatment decision. In conclusion, Danish dental professionals participating in the study apply reasonably well current concepts on overall caries activity and risk assessment, clinical and radiographic severity of occlusal lesions, and, to a certain extent, assessment of caries lesion activity on occlusal surfaces. Nonoperative treatment decisions had a high priority among Danish professionals.

The present study aimed to compare the micro-tensile bond strength of new and conventional glass-ionomers and Z350 flowable composite resin to sound and caries-affected dentin. Eighty extracted third molars were selected. Standard Cl I cavities were prepared on occlusal surfaces. Forty teeth were assigned to the sound tooth group, and in the remaining 40 teeth, carious lesions were produced using a laboratory technique. Then each group (n=40) was subdivided into 5 groups (n=8) in terms of the restorative material used (EQUIA® Forte, Ketac Molar, encapsulated and hand-mixed GC Fuji II LC glass-ionomers and Z350 flowable composite resin). The cavities were restored. The highest micro-tensile bond strength was recorded with Z350 flowable composite resin in sound dentin (29.65), and the lowest was recorded with GC Fuji II LC (powder, liquid) in affected dentin (7.88). Significant differences were detected in the micro-tensile bond strength between the 5 restorative material groups. The composite resin bond strength was the maximum and was significantly different from all the other groups. The micro-tensile bond strength was affected by the type of dentin; in this context, in all the restorative groups, the bond strength in sound dentin was significantly higher than that in affected dentin. The most frequent failure mode was adhesive in the sound and affected dentin.
