Introduction: Dental operating microscopes (DOM) enable dentists to examine, with the aid of magnification and photodocumentation in clinical conditions, the occlusal anatomy of newly erupted permanent molars and to detect any early carious lesions more precisely. Aim: To determine the advantages of magnifying technology in detecting early occlusal carious lesions in newly erupted permanent first molars. Materials and methods: We examined 176 first molars of 44 children divided into two age groups: 7-8 and 9-10 years. The surfaces of each of the teeth were examined after they were cleaned with a brush without paste. The diagnoses were first made using only illumination and a clinical examination. These were followed by a visual examination of the occlusal surface using a DOM at ×8 magnification. A photo of the occlusal surface was then taken with a camera at the respective magnification for the purposes of photodocumentation. Results: This study demonstrated that dental operating microscopes increased by 7% the rate of successful detection of early caries lesions on the occlusal surfaces. There was also a significant increase of the number of diagnosed cavitated caries in the dentine in comparison with those detected with naked eye. Using DOMs in pediatric dentistry facilitates the more detailed and precise diagnosing of early stage carious lesions and/or cavitations in the fissures. Conclusion: The use of a DOM facilitates differential diagnosing between deep fissures and early carious lesions, which is a key point in the assessment of occlusal surfaces of newly erupted permanent molars, necessary for their preventative sealing and micro-invasive treatment.

Posterior composite restorations are difficult to contour and polish due to their occlusal anatomy and opposing occlusion. Our study describes a technique for duplicating occlusal surface anatomy, using different dental materials that are able to copy anatomic details. The aim of the study was to evaluate the efficiency of occlusal matrix technique, by comparing technical ease of different dental materials. The study was conducted on extracted teeth which had intact occlusal surfaces or initial carious lesions at this level. An impression of the occlusal surface was made prior to cavity preparation, with four different dental materials: a flow composite, a dual-cure resin cement, a temporary acrylic resin (powder and liquid) and a temporary crown and bridge two-component material in a Unidose®. When the last layer of composite has been placed, the occlusal matrix was forced into the uncured composite to replicate the original occlusal surface, instead of performing manual curing and shaping as in the standard approach. Time needed for complete finishing the case, the hardness of each occlusal matrix and the final morphology obtained were evaluated. The main benefits of the occlusal matrix technique are the technical ease of use due to its simplicity and its high accuracy in reconstructing occlusal morphology. Although the shortest time needed for occlusal matrix preparation was for the dual-cure resin cement, the best oclusssal matrix was obtained with the temporary acrylic resin (powder and liquid).

Abstract Proximal surfaces are the second in turn after the occlusal surfaces, as being the most exposed places to the carious process. The specificity of changes and location makes the diagnostic of approximal cavities difficult. The aim of the study was to collect information on possibilities of detection, monitoring and assessment of carious lesions located on proximal surfaces, starting from the early stage. On the base of current literature, chosen methods of diagnosis, from conventional, to advanced technologies were presented. Current literature also suggests that none of the diagnostic methods exclude the possibility of misdiagnosis of proximal caries. Therefore, during the diagnosing of doubtful cases, it is advisable to employ several methods of assessment, with simultaneous individual appraisal of caries risk factors, such as oral hygiene, frequency of eating sweets or the number of active caries foci.

Dental caries is one of the most prevalent diseases across the globe, affecting all age groups. Various public health measures like water fluoridation have been started to control the spread of caries, especially among children. But these intervention methods are inaccessible to people in some parts of the world. Thus, a product called Silver Diamine Fluoride was introduced, which can help arrest carious lesions at an early stage. It is a beneficial aid for caries arrest in both primary and permanent teeth. Also, can be used for arresting root caries. Thus, the objective of this review article is to discuss about silver diamine fluoride and its diverse uses from both clinician and public health point of view.

The objective of this descriptive study was to define the at-risk occlusal surface to guide the practitioner in the decision of whether to seal or not. Method: All dentists affiliated with the French Society of Pediatric Odontology (SFOP) and general practitioners (GP) registered in postgraduate courses in three French dental schools answered the same questionnaire illustrating four occlusal surfaces of permanent molars. It was focused on obtaining an optimal definition of an at-risk occlusal surface. The corresponding four molars were later sectioned to check the answers. Univariate logistic regression analyses and multivariate logistic regression models were tested to identify the factors associated with the at-risk occlusal surface. Results:Eighty-six SFOP dentists and 136 GP filled in the form. Multivariate logistic regression models stratified by type of practice demonstrated that stained fissures (p=0.001) were only associated with at-risk occlusal surface among GP and the morphology of primary fissure (p=0.001) when considering SFOP dentists alone. The multivariate analyses demonstrated that stained fissures, and primary and secondary fissures were linked to the perception of an at-risk occlusal surface. Conclusion: An at-risk occlusal surface has narrow and deep primary fissures. Numerous secondary fissures could increase the risk. The coloration of fissures should not be used in the definition because it depends on tooth integrity.

The aim of this study was to assess the accuracy of visual-tactile inspection (VTI) and laser fluorescence (LF) in detection of initial carious lesions on occlusal surfaces. Total of 120 premolars and molars, without any macroscopic cavitation on occlusal surface, planed to be extracted for orthodontic, prosthetic or surgical reasons, were included in this study. The agreement between histological diagnosis and diagnostic and treatment decisions of evaluated methods was assessed. The obtained results showed that specificities and sensitivities were for VTI=83 and 36%, and LF-77 and 96%. The percentage of correct treatment decisions were: VTI=54% and LF-80%. Diagnosing caries was significantly dependent on the fissure morphology and LF showed statistically better results than VTI. It can be concluded that laser fluorescence was significantly efficient and more accurate in diagnosing initial carious lesions on occlusal surfaces.

The aim of the present study was to evaluate the association between clinical parameters and the presence of active caries lesions on the occlusal surface of first permanent molars. Forty eight children (5.8-13.8 years-old) with at least one first permanent molar present were selected. The clinical parameters evaluated were gender, age, DMF-T and dmf-t, presence of active white spots in other teeth, general plaque index, tooth's dental arch (upper or lower), tooth's side (right or left), presence of visible plaque and eruption degree of the first permanent molars. The first permanent molars were evaluated through visual inspection by two examiners in order to assess the presence of active or inactive caries lesions on the occlusal surface. Univariate and multivariate analyses for determination of the association between clinical parameters and the presence of active caries lesions in these teeth were performed. The presence of active white spots in other teeth was associated with the presence of active caries lesions in the first permanent molars, in both univariate and multivariate analyses (Odds ratio = 8.8 and 1.9, respectively). The presence of abundant visible plaque on the occlusal surface of the first permanent molars (Odds ratio = 3.5 in the univariate analysis, and 3.9 in the multivariate one) also presented a significant association. In conclusion, the presence of active white spots in other teeth and the presence of considerable visible plaque were associated with the presence of active caries lesions on the occlusal surfaces of first permanent molars.

The Chemo-mechanical caries removal technique involves the application of chemical agents, to cause a selective softening of the carious dentine and facilitate removal by gentle excavation. Carisolv® is one such new chemical agent used in this minimal invasive technique of carious dentine removal. The present study was, therefore, undertaken to evaluate the efficacy of Carisolv® in the chemo-mechanical removal of carious dentine, the time taken for caries removal, the perception of the treatment by the patient, and to evaluate the restorations, radiographically. Accordingly, fifty primary and permanent molars with dentinal carious lesions were excavated using this technique of caries removal. The chemo-mechanical caries removal technique using Carisolv® proved to be an effective atraumatic treatment modality with potential interest for use in clinical pediatric dentistry.

Background: Molars and premolars are considered as the most vulnerable teeth of caries attack, which is related to the morphology of their occlusal surfaces along with the difficulty of plaque removal. different methods were used for early caries detection that provide sensitive, accurate preoperative diagnosis of caries depths to establish adequate preventive measures and avoid premature tooth treatment by restoration. The aim of the present study was to evaluate the clinical sensitivity and specificity rates of DIAGNOdent and visual inspection as opposed to the ICDAS for the detection of initial occlusal caries in noncavitated first permanent molars. Materials and Methods: This study examined 139 occlusal surface of the first permanent molar pooled from fifty patients aged 8-9 years by three methods. The selected criteria include one occlusal site per tooth (first permanent molars) with carious lesions range from 0 to 3 according to ICDASII (gold standard) visual criteria then the clinical sensitivity and specificity of visual inspection according to Ekstrand et al.in 1997 and DIAGNOdent were performed. . Results: the highest correlation was found between the ICDASII and DIAGNOdent. The sensitivity of the DIAGNOdent for the enamel caries detection (D1) was better than that of visual inspection. The sensitivity and the specificity for the DIAGNOdent at D3 threshold were better than the D1 threshold and the visual inspection method. Conclusion: DIAGNOden pen can be used as a tool for early caries detection in cases of difficult diagnosis that provide good additional sensitivity to the visual inspection.


Lung cancer is the foremost cause of cancer-related deaths world-wide [1]. It affects 100,000 Americans of the smoking population every year of all age groups, particularly those above 50 years of the smoking population [2]. In India, 51,000 lung cancer deaths were reported in 2012, which include 41,000 men and 10,000 women [3]. It is the leading cause of cancer deaths in men; however, in women, it ranked ninth among all cancerous deaths [4]. It is possible to detect the lung cancer at a very early stage, providing a much higher chance of survival for the patients.
