Breastfeeding is endorsed by the medical community as the optimal nutrition for infants during the first 6-12 months of life.1,2,3  Breastfeeding rates in the US and worldwide have varied over time and still vary geographically.4 There is robust literature addressing the physiology of lactation, composition of breast milk, and health advantages of breastfeeding for both the mother and infant as well as strategies for clinicians to promote and support breastfeeding. This chapter reviews breastfeeding history, how milk is made, why breastfeeding matters, and the somewhat controversial the World Health Organization’s Baby Friendly Hospital Initiative (BFHI)5 for successful initiation of lactation as well as some special situations.    Key words:  breastfeeding, infant nutrition, human milk composition, breastfeeding advantages, lactation, lactation support, Baby Friendly Hospital Initiative, skin to skin contact

Background: Adherence to the Ten Steps of the Baby-Friendly Hospital Initiative has been shown to have a protective role for the initiation and maintenance of breastfeeding. Research Aims: (1) To determine the breastfeeding rate during the first 6 months of life in children of mothers diagnosed with COVID-19 infection at the time of birth; and (2) to assess the possible influence of being born in a center with Baby-Friendly Hospital Initiative accreditation. Methods: This was a two-group comparative longitudinal observational study of infants born to mothers with COVID-19 at the time of birth, between March 13–May 31, 2020 (the first wave of the pandemic) in Spain. Fourteen Spanish hospitals participated, five (35.7%) were Baby-Friendly Hospital Initiative accredited. Type of feeding was assessed prospectively at discharge, 1, 3, and 6 months of age. A total of 248 newborns were included in the study. Results: A total of 117 (47.3%) newborns were born in Baby-Friendly Hospital Initiative (BFHI) accredited centers. These centers applied skin-to-skin contact with greater probability ( OR = 1.9; 95% CI [1.18, 3.29]) and separated the newborns from their mothers less frequently ( OR = 0.46; 95% CI [0.26, 0.81]) than non-accredited centers. No differences were observed in relation to the presence of a companion at the time of birth. At discharge, 49.1% ( n = 57) of newborns born in BFHI-accredited centers received exclusive breastfeeding versus 35.3% ( n = 46) in non-accredited centers ( p = .03). No differences were observed in breastfeeding rates throughout follow-up. Conclusions: The exclusive breastfeeding rate at discharge in children of mothers with COVID-19 infection at birth was higher in Baby-Friendly Hospital Initiative accredited centers, which most frequently applied skin-to-skin contact at birth as well as rooming-in.

Abstract Background Maternal social support has a critical impact on breastfeeding outcomes. Fathers in particular are known to be especially influential. However, little is known about what they know and how they feel about breastfeeding. We aimed to explore paternal knowledge and attitude toward breastfeeding and possible association with breastfeeding rates at discharge. Methods We enrolled fathers of healthy term neonates born at our hospital from March to May 2019. At discharge, a self-administered structured questionnaire was proposed to fathers. Fathers were required to rate their degree of agreement to 12 items on a 5-point Likert scale. A total score was obtained from their answers. Subjects’ basic characteristics and mode of feeding at discharge were collected. Results Fathers showed a generally solid knowledge of maternal (87%) and neonatal (98%) benefits of breastfeeding, skin-to-skin contact (99.5%), rooming-in practice (79%) and responsive feeding (67.5%); conversely, only 51% of them knew about the recommended use of pacifiers. Fathers felt personally involved in their babies’ feeding in 79% of cases, regardless of type of feeding. A positive correlation was found between total score and exclusive breastfeeding rates at discharge (p = 0.04, OR 1.07; 95%C.I 1.002–1.152). ROC analysis showed a trend toward statistical significance (AUC 0.58, p = 0.083, 95%C.I 0.485–0.683). Conclusions This study underlines the importance of including fathers in the promotion of breastfeeding, expanding the classic mother-baby dyad to a more modern mother-father-baby triad. This may ultimately impact breastfeeding outcomes, although further studies are needed to confirm our results.

Background The World Health Organization recommends exclusive breastfeeding for the first 6 months of life; however, UK breastfeeding rates are some of the lowest worldwide. As such, various interventions have been trialled, aiming to increase breastfeeding rates. Aims To evaluate the effectiveness of interventions to increase breastfeeding rates in the UK and determine the features of successful interventions. Methods A literature search was performed, using four databases. The results were refined by applying inclusion and exclusion criteria. Two additional articles were recognised by scanning the references sections of identified studies, resulting in 12 articles for review. Findings Support-based interventions had predominantly insignificant effects upon breastfeeding rates. Incentives were associated with increases in rates, while combined interventions had mixed success. The interventions were well received by mothers and clinicians and may help to normalise breastfeeding. Conclusions Future interventions should provide targeted, personalised support to overcome breastfeeding difficulties, and reward mothers for their efforts.

ABSTRACT Objective: To describe the experience of the 25-year-old trajectory of the Baby Friendly Hospital Initiative (BFHI) in Brazil. The first unit was implemented in 1992. Methods: Information and data were collected from publications on the World Health Organization (WHO), the United Nations International Children’s Emergency Fund (UNICEF) and the Ministry of Health websites and in national and international journals, about the period 1990-2017. The descriptors used were: “iniciativa hospital amigo da criança”, “hospital amigo da criança”, “baby friendly initiative hospital”, “aleitamento materno” and “breastfeeding”. The number of hospitals in the 25 years, the course of the BFHI and its repercussions on breastfeeding in Brazil were evaluated. Results: The BFHI is an intervention strategy in hospital care at birth focused on the implementation of practices that promote exclusive breastfeeding from the first hours of life and with the support, among other measures of positive impact on breastfeeding, of the International Code of Marketing of Breastmilk Substitutes. Currently, the initiative has been revised, updated and expanded to integrate care for newborns in neonatal units and care for women since prenatal care. It can be concluded that, during these 25 years, the quantity of hospitals varied greatly, with numbers still below the capacity of hospital beds. BFHI shows higher rates of breastfeeding than non-accredited hospitals. However, the number of hospitals are still few when compared to other countries. Conclusions: The BFHI has contributed to breastfeeding in Brazil in recent decades. Greater support for public policies is needed to expand the number of accredited institutions in the country.

The global threat of new infectious diseases first became widely recognized in the 1990s. The US government published a report on emerging and reemerging infectious diseases followed by the World Health Organization (WHO), which adopted the slogan "Emerging Infectious Diseases: Global Alert, Global Response" in 1997. Typical examples of the more than 30 infectious diseases emerging since 1970s are HIV/AIDS, Vibrio cholerae O139 infection, enterohemorrhagic Escherichia coli infection, severe acute respiratory syndrome (SARS), and avian influenza. The New Infectious Diseases Control Law enacted in Japan in 1999 was to control these emerging infectious diseases and the already existing ones.

AbstractThe 2015 Dietary Guidelines for Americans Advisory Committee has set recommendations to limit added sugars. This action was based on the association between dietary pattern quality scores and chronic disease risk, the results of meta-analyses conducted for the World Health Organization, and data from modeling of dietary patterns for establishing the US Department of Agriculture’s Healthy US-Style Eating Patterns. Recommendations provided by the 2015–2020 Dietary Guidelines for Americans were used by the US Food and Drug Administration to establish, for the first time, the mandatory declaration of added sugars and a Daily Value of added sugars for the Nutrition Facts label. This review provides an overview of the scientific evidence considered by the World Health Organization, the 2015–2020 Dietary Guidelines for Americans, and the US Food and Drug Administration for setting recent polices and regulations on added sugars and highlights important issues and inconsistencies in the evaluations and interpretations of the evidence.

Since its publication in 1955, the WHO Monograph, Infant Nutrition in the Subtropics and Tropics by Derrick Jelliffe, has been indispensable reading and reference for persons dealing with pediatric problems in the developing areas of the world. Its review of infant feeding practices in tile subtropics and tropics was unique, and its description of nutritional diseases among young children in these areas was concise and reliable. The chapters on improving infant feeding, preventing nutritional disease, and introducing nutrition education were by far the best published guidelines available.

To the Editor.— The article by Dr Snyder, "Use and Misuse of Oral Rehydration Therapy for Diarrhea,"1 led us to review our treatment of gastroenteritis, revise our telephone protocols, and seek out rehydration solutions. Our difficulty in obtaining adequate supplies mirrored that of Dr O'Banion.2 Expense to the patient is also an important factor: commercial premixed solutions sell for $4 to $6 per quart in our area, which many families cannot pay. Our solution was to obtain from the World Health Organization the name of the US supplier of oral rehydration solution (ORS) packets and order direct from the supplier (Jianas Bros Packaging Co, 2533 SW Blvd, Kansas City, MO 64108).
