Assessing the Appearance and Fluorescence of Resin-Infiltrated White Spot Lesions With Caries Detection Devices

SUMMARY Objective: This in vitro study examined the effectiveness of caries detector devices in assessing the ability of resin infiltration (RI) (Icon, DMG-Hamburg, Hamburg, Germany) to improve the optical properties of enamel white spot lesions (WSLs). Methods and Materials: Ten caries-free third molars were used. Photographs, a subjective visual assessment of the photographs, fluorescent camera (FC) images using the Spectra (Air Techniques, Melville, NY, USA), and laser fluorescent (LF) readings using the DIAGNOdent (KaVo, Biberach, Germany) were obtained from each tooth's buccal surface. Specimens were coated with nail polish leaving a rectangular window on the buccal surface and placed in pH 4.5 lactic acid gel for two weeks to create a WSL. The WSLs were analyzed by the same methods. RI was applied to half of each WSL; final photographs were then taken, and caries detector assessments were conducted. FC images were converted to grayscale, and the fluorescent image's brightness intensity was measured using ImageJ. Data were analyzed with analysis of variance and Tukey-Kramer honestly significant difference test. Significance was set at α=0.05. Results: Subjective assessment of the photographs showed that RI improved the appearance of the WSLs so that they resembled intact enamel. Mean FC-brightness intensities for intact, demineralized, and demineralized RI-treated areas were 159.6 ± 9.2, 123.4 ± 7.2, and 160.9 ± 11.5, respectively. There were no significant differences in fluorescent intensity between the intact and RI areas (p=0.58). The demineralized areas had significantly lower fluorescent intensity than both the RI-treated and intact areas (p<0.001). LF values did not differ significantly between intact, demineralized, or RI-treated areas. Conclusions: This study demonstrates the ability of RI to restore artificial WSLs to the esthetics and fluorescence of intact enamel. The FC can be used to assess the optical properties of WSLs and the impact of RI on these properties.

Abstract Aim of the study: This study evaluated the impact of two remineralizing agents containing casein phosphopeptideamorphous calcium phosphate (CPPACP), and Tricalcium phosphate (TCP) on white spot lesions. Materials and Methods: A total of ninety freshly extracted upper premolars were divided in three equal groups (30 each). Group A: Control group. Group B: Varnish containing CPP–ACP. Group C: Varnish containing TCP. The teeth were subjected to Diagnodent test after inducing white spot lesions to assess any surface changes present on the teeth. The readings were repeated post demineralization and post-remineralization. Results: Statistically significant difference was present in the mean values between the tested groups. CPP-ACP group recorded higher mean value at demineralization when compared to TCP group. Statistically significant difference (p < 0.0001) was present between the baseline and the demineralization values. CPP-ACP group presented higher remineralization results when compared to TCP group. T-test proved a significant difference between the tested groups. Conclusion: CPP-ACP containing varnish had higher remineralizing effect when compared to TCP containing varnish. Clinical Significance: Recently, the focus of restorative dentistry has been directed toward a conservative approach. Prevention and non-restorative treatment of initial enamel caries is the main concern. Remineralization procedures are the most preferred way of regeneration of lost tooth structure.

Aim: To evaluate the impact of environmental pH on intra-radicular dentin push-out bond strengths of MTA, MTA HP, and Biodentine. Materials and Methodology: Freshly extracted human mandibular single-rooted premolars or maxillary anterior incisors that were either intact or contained only small carious lesions were selected.120 mid root dentins is horizontally divided into 1.0 mm thick slices and divided into 3 MTA, MTA HP, BIODENTINE groups. The compressive load is applied at a speed of 0.5 mm/min by exerting a downward pressure on the outer surface of MTA using a 1.00 mm diameter cylindrical stainless-steel plunger. Maximum load to MTA was reported in newtons at the time of dislodgement and converted to megapascals. The 1-way analysis of variance test was used to compare the push-out bond strength of the groups with the same storage time (4 or 34 days), followed by the pair-wise comparison of the Tukey post hoc test. The Student's t-test was used to evaluate 3-group means. At P = .005, the degree of significance was set. Thus the clinical importance of the present study states that considering several factors like microhardness, composition by products, particle sizes and the environmental pH plays a critical role in selection of root end filling material. Results: There was significant difference between groups (P = .001) after 4 days of PBS and Acid condition, where Bio dentine had significantly the highest bond strength. Conclusion: The strength of MTA HP, BIODENTINE, MTA materials at dentine interface increases over 30 days in the storage of PBS solution at pH 7.4, after an initial acid challenge by acetic acid of pH 5.4, which decreases initial bond strength.

Introduction. An in vivo determination of bile acid hepatobiliary transport efficiency can be of use in liver disease and preclinical drug development. Given the increased interest in bile acid Positron Emission Tomography- (PET-) imaging, a further understanding of the impact of 18-fluorine substitution on bile acid handling in vitro and in vivo can be of significance. Methods. A number of bile acid analogues were conceived for nucleophilic substitution with [18F]fluoride: cholic acid analogues of which the 3-, 7-, or 12-OH function is substituted with a fluorine atom (3α-[18F]FCA; 7β-[18F]FCA; 12β-[18F]FCA); a glycocholic and chenodeoxycholic acid analogue, substituted on the 3-position (3β-[18F]FGCA and 3β-[18F]FCDCA, resp.). Uptake by the bile acid transporters NTCP and OATP1B1 was evaluated with competition assays in transfected CHO and HEK cell lines and efflux by BSEP in membrane vesicles. PET-scans with the tracers were performed in wild-type mice (n=3 per group): hepatobiliary transport was monitored and compared to a reference tracer, namely, 3β-[18F]FCA. Results. Compounds 3α-[18F]FCA, 3β-[18F]FGCA, and 3β-[18F]FCDCA were synthesized in moderate radiochemical yields (4–10% n.d.c.) and high radiochemical purity (>99%); 7β-[18F]FCA and 12β-[18F]FCA could not be synthesized and included further in this study. In vitro evaluation showed that 3α-FCA, 3β-FGCA, and 3β-FCDCA all had a low micromolar Ki-value for NTCP, OATP1B1, and BSEP. In vivo, 3α-[18F]FCA, 3β-[18F]FGCA, and 3β-[18F]FCDCA displayed hepatobiliary transport with varying efficiency. A slight yet significant difference in uptake and efflux rate was noticed between the 3α-[18F]FCA and 3β-[18F]FCA epimers. Conjugation of 3β-[18F]FCA with glycine had no significant effect in vivo. Compound 3β-[18F]FCDCA showed a significantly slower hepatic uptake and efflux towards gallbladder and intestines. Conclusion. A set of 18F labeled bile acids was synthesized that are substrates of the bile acid transporters in vitro and in vivo and can serve as PET-biomarkers for hepatobiliary transport of bile acids.

Abstract Background: Ophthalmic formulations of chloramphenicol have poor bioavailability of chloramphenicol in the ocular cavity. Aim: The present study aimed at exploring the impact of different oil mixtures in the form of emulsion on the permeability of chloramphenicol after ocular application. Materials and methods: Selection of oil mixture and ratio of the components was made by an equilibrium solubility method. An emulsifier was chosen according to its emulsification properties. A constrained simplex centroid design was used for the assessment of the emulsion development. Emulsions were evaluated for physicochemical properties; zone of inhibition, in-vitro diffusion and ex-vivo local accumulation of chloramphenicol. Validation of the design using check-point batch and reduced polynomial equations were also developed. Optimization of the emulsion was developed by software Design® expert 6.0.8. Assessment of the osmolarity, ocular irritation, sterility testing and isotonicity of optimized batch were also made. Results: Parker Neem®, olive and peppermint oils were selected as an oil phase in the ratio 63.64:20.2:16.16. PEG-400 was selected as an emulsifier according to a pseudo-ternary phase diagram. Constrained simplex-centroid design was applied in the range of 25-39% water, 55-69% PEG-400, 5-19% optimized oil mixture, and 1% chloramphenicol. Unpaired Student’s t-test showed for in-vitro and ex-vivo studies that there was a significant difference between the optimized batch of emulsion and Chloramphenicol eye caps (a commercial product) according to both were equally safe. Conclusion: The optimized batch of an emulsion of chloramphenicol was found to be as safe as and more effective than Chloramphenicol eye caps.

Abstract Aim This study evaluated the influence of mechanical loading and thermocycling on microleakage of class V resin-based composite restorations with and without enamel bevel. Methods and Materials Sixty class V cavity preparations measuring 3.0 mm wide (mesiogingivally) x 2.0 mm high (occluso-gingivally) x 1.5 mm deep with the occlusal margin in enamel and the gingival margin in cementum were prepared on the buccal surfaces of human premolars using a #12 diamond round bur (Drendel & Zweiling Diamant GmbH, Lemgo, Germany) in a high-speed, water-cooled handpiece. The specimens were then divided into two groups of 30 specimens each, based on the type of enamel cavosurface margin configuration as beveled or nonbeveled (butt joint). After restoring the preparations with a flowable resin-based composite (Tetric Flow, Ivoclar Vivadent-AG, Schaan, Liechtenstein) and finishing and polishing with sequential discs (Sof-Lex Pop-on, 3M-ESPE, St. Paul, MN, USA), the teeth were stored at 37°C and 100 percent humidity. Twenty-four hours later, half of the specimens in each group (nonbeveled “N” or beveled “B”) were exposed to a cycling loading for 250,000 cycles to simulate occlusal loading and assigned to two subgroups (NL+ or BL+), while the remainder of the specimens in each group were only maintained in a 100-percent-humidity environment, without any cyclical loading, until tested (NL– or BL–). The specimens were sealed with sticky wax (Kemdent, Associated Dental Products, Swindon, UK) and nail polish. The apical foramen of each tooth was sealed with sticky wax and the rest of the tooth was covered with nail varnish, except for an area within 1.0 mm around the composite restoration. To detect marginal leakage, all of the samples were stored in a 0.5 percent basic fuchsine solution for 24 hours. The specimens were then sectioned longitudinally using a low-speed diamond blade (IsoMet, Buehler Ltd., Lake Bluff, IL, USA), machined, and evaluated under 25X magnification using a stereomicroscope (M9, Wild Heerbrugg, Switzerland). The specimens were scored on a scale from 1 to 4 on the degree of dye penetration. The qualitative data were analyzed by the Mann- Whitney U test at a 5 percent significance level (p<0.05). The null hypothesis of this study was that there is no difference in microleakage between beveled and nonbeveled class V buccal preparations in premolar teeth restored with resinbased composite and subjected to simulated occlusal loading and thermocycling. Results In each group the gingival margin showed significantly more microleakage than the enamel margin (p<0.05). Load cycling did not result in an increase in microleakage in nonbeveled (p=0.259) or in beveled (p=0.053) occlusal margins. However, the gingival margins showed a statistically significant difference in microleakage after load cycling whether in cavities with enamel occlusal bevel (p=0.004) or in groups without a bevel. This means the enamel margin configuration of the enamel occlusal margin had no effect on decreasing microleakage in the gingival aspect of class V composite restorations. In general, the nonbeveled preparations in this study had significantly less microleakage than the bevel specimens whether they were loaded occlusally or not (p=0.001). Clinical Significance Within the limitations of this in vitro study, no benefit was derived from placing an enamel cavosurface bevel on the occlusal margin of a standardized class V composite restoration located at the cementoenamel junction.
