AbstrakTujuan jangka panjang pelaksanaan program Mutiara Home Care adalah menciptakan akses bagi terciptanya wirausaha baru, menunjang otonomi kampus perguruan tinggi melalui perolehan pendapatan mandiri. Bagi pasien program ini membantu meringankan biaya rawat inap yang makin mahal, karena mengurangi biaya akomodasi pasien, transportasi dan konsumsi keluarga. Dampak ekonomi nasional home care bagi pasien dan keluarga adalah semakin pendeknya hari rawat, sehingga jumlah klaim rumah sakit ke BPJS semakin rendah, sehingga berdampak bagi penurunan anggaran biaya perawatan masyarakat secara nasional. Produk Jasa Layanan Mutiara Home Care memiliki keunggulan dibandingkan home care lain. Pertama, Mutiara Home care akan dikelola dan diorganisir secara profesional oleh tenaga dosen profesional dibidang kesehatan dan keperawatan. Selama ini, home care dilakukan secara individu atau berkelompok tanpa wadah atau organisasi yang jelas. Tenaga kesehatan yang akan ditempatkan di komunitas atau di rumah telah terlatih dimulai sejak masa pendidikan dan tersertifikasi dari USM-Indonesia. Kedua, menyediakan layanan antar jemput pasien yang membutuhkan perawatan rumah sakit atau pemeriksaan khusus, seperti radiologi dan laboratorium. Layanan Mutiara Home Care pada tahap awal melayani pasien paska stroke dan perawatan lanjutan jantung, perawatan pasien DM dengan atau tanpa luka dan perawatan pasien lanjut usia. Secara bertahap akan dilakukan pengembangan serta layanan terhadap berbagai penyakit yang membutuhkan jasa perawatan profesional di rumah.Kata Kunci: Home care, MutiaraAbstractThe long-term goal of the Mutiara Home Care program is to create access to new entrepreneurs, to support college campus autonomy through the acquisition of independent income. For patients this program helps alleviate the cost of increasingly expensive hospitalization, as it reduces patient accommodation costs, transportation and family consumption. The national economic impact of home care for patients and families is the shortening of day care, so the number of hospital claims to BPJS is lower, thus impacting the reduction of national community maintenance budget. Products Care Services Pearl Home Care has advantages over other home care. First, Mutiara Home care will be managed and organized professionally by professional lecturers in the field of health and nursing. During this time, home care is done individually or in groups without a clear container or organization. Health workers who will be placed in the community or at home have been trained since the education and certified from USM-Indonesia. Secondly, it provides a shuttle service to patients who require hospital treatment or special examinations, such as radiology and laboratories. Pearl Home Care Services in the early stages of serving post-stroke patients and advanced heart care, treatment of DM patients with or without injuries and care of elderly patients. Background. With the release of the Romanow Commission report, Canadian governments are poised to consider the creation of a national home care program. If occupational and physical therapists are to have input in shaping such a program, they will need to learn from lost opportunities of the past. Purpose. This paper provides an overview of recent reforms to home care in Ontario with an emphasis on rehabilitation services. Method. Data were collected from documents and 28 key informant interviews with rehabilitation professionals. Results. Home care in Ontario has evolved in a piecemeal manner without rehabilitation professionals playing a prominent role in program design. Practice Implications. Rehabilitation services play a critical role in facilitating hospital discharges, minimizing readmissions, and improving the quality of peoples' lives. Canadians will benefit if occupational and physical therapists seize the unique opportunity before them to provide meaningful input into creating a national home care program.

To the Editor.— Dr Sterne's commentary (Pediatrics 1987;79:445-446) on day care for sick children presented a succinct summary of the dilemmas faced by working parents and the current options available to them when their children are ill. I am in full agreement that ideally there should be liberalized parental leave policies as well as improved understanding and acceptance of certain minor illnesses by existing day-care centers. However, because of the cost, it is doubtful that corporations can or will pay for home care services, which are twice as costly as an infirmary model, or will they be willing to grant additional paid days off to parents.

(1) Background: This paper investigates the distribution of work hours by activity, for the main staff categories in home care services in three rural Norwegian municipalities. In Norway these categories are registered nurses, assistant nurses and assistant health workers. (2) Methods: The three categories of home care staff recorded 20,964 eligible observations over 8 weeks. We identified 19 activities, which were recorded. The majority of staff used a smartphone application for the time measuring, while some staff used a manual form for reporting purposes. (3) Results: The registered nurses (RNs) spent 32% of their time on direct patient work, while driving accounted for 18%. Direct patient work and driving accounted for the majority of activities performed by assistant nurses (48% and 29%, respectively) and assistant health workers (70% and 17%, respectively). (4) Conclusions: The demand for home care services is increasing in terms of both size and complexity. Simultaneously, there is a growing shortage of skilled staff. RNs is the group with the least face-to-face time with patients. To meet the patients’ needs, it is necessary to discuss and modify existing home care service systems in order to use resources appropriately and effectively.

Objectives. To identify characteristics associated with the likelihood of a client receiving a referral to the Home and Community Care (HACC) program from various sources. Methods. Data were collected from 73 809 home care clients during 2007–08. Binary logistic and multinomial logistic regression were used to investigate the likelihood of a client being referred by health workers v. non-health workers. Results. Females and clients cared for by their parents were less likely to receive referrals from health workers than non-health workers after confounding variables were controlled for. While poorer functional ability of clients increased the probability of receiving a referral from a health worker, the opposite was true for those with behavioural problems. Over 43% of the sample either self-referred or was referred by family or friends. Conclusions. Eligible individuals may miss out on services unless they or their family take the initiative to refer. There is a need for improved methods and incentives to support and encourage health workers to refer eligible individuals to the program. What is known about the topic? The absence or inappropriate referral to a suitable home care program can place pressure on formalised institutions and increase burdens on family members and the community. Factors largely unrelated to healthcare needs carry significant weight in determining hospital discharge decisions and home care referrals by practitioners. What does this paper add? The effectiveness of the HACC program is dependent on the referrer who acts to inform and facilitate individuals to the program. The purpose of this study is to identify the characteristics associated with the likelihood of individuals receiving a referral to the HACC program from various sources. What are the implications for practitioners? This study will assist policy makers and practitioners in developing effective strategies that transition individuals to suitable home care services in a timely manner. An effective referral process would provide opportunities for implementing preventative strategies that reduce disability rates among individuals and the burden of care for the community. For instance, individuals with unmet needs may be at higher risk from injury at home through inadequate monitoring of nutrient and medication intake and inappropriate home surroundings. Improving knowledge about care options and providing appropriate incentives that encourage health workers to refer individuals would be an effective start in improving the health outcomes of an ageing population.

Abstrak: Desa Darul Amin merupakan desa yang berada di Kecamatan Lawe Alas Kabupaten Aceh Tenggara, Provinsi Aceh, Indonesia. Survey awal selama masa pandemi COVID-19 didapatkan beberapa permasalahan berupa masyarakat sering berkumpul bersama bila ada acara/kegiatan, masyarakat merasa khawatir ke tempat layanan kesehatan, prevalensi penyakit tertinggi seperti hipertensi, rematik, asam urat dan diabetes. Adapun hasil pertanian masyarakat berupa jagung, padi, jahe, kunyit, lengkuas dan minyak kelapa.  Oleh karena itu, pentingnya pengembangan inovasi teknologi Excellent Care sebagai sarana fasilitas pelayanan kesehatan dan bisnis online untuk mewujudkan masyarakat sehat, sejahtera dan bebas COVID-19. Kegiatan yang dilakukan terdiri dari pemberdayaan kader kesehatan, desiminasi ilmu tentang ramuan herbal, pengembangan inovasi teknologi Excellent Care dan kunjungan tim tenaga kesehatan untuk layanan homecare kepada masyarakat. Kegiatan tersebut bertujuan meningkatkan kesehatan melalui layanan konsultasi dan homecare, mendukung program pemerintah dalam penerapanan social distancing, memberikan peluang bisnis bagi masyarakat dalam mempromosikan ramuan herbal serta mengembangkan Desa Darul Amin sebagai desa percontohan yang memanfaatkan fasilitas pelayanan kesehatan online yang dapat memandirikan masyarakat untuk hidup sehat dan produktif. Kelompok sasaran dalam kegiatan ini adalah masyarakat dan kader di Desa Darul Amin. Adapun hasil capaian kegiatan yaitu masyarakat sudah memanfaatkan layanan kesehatan online, masyarakat sudah mempromosikan ramuan herbalnya melalui www.excellentcare.com, kader dan tenaga kesehatan sudah mendapatkan penghasilan dari layanan homecare.Abstract: Darul Amin Village is a village located in Lawe Alas District, Southeast Aceh Regency, Aceh Province, Indonesia. During the COVID-19 pandemic period, the initial survey found several community problems, often gathering together when there was an event/activity. The community was worried about health services, the highest prevalence of hypertension, rheumatism, gout, and diabetes. The community agricultural products are corn, rice, ginger, turmeric, galangal, and coconut oil. Therefore, the importance of developing Excellent Care technology innovation as a means of health service facilities and online businesses create a healthy, prosperous, and free of COVID-19 society. Activities carried out consisted of empowering health cadres, disseminating knowledge about herbal ingredients, developing Excellent Care technology innovations, and visiting a healthcare team for home care services to the community. This activity aims to improve health through consultation and home care services, support government programs in the application of social distancing, provide business opportunities for the community in promoting herbal concoctions and develop Darul Amin Village as a pilot village that utilizes online health service facilities that can empower people to live healthy and productive. The target groups in this activity are the community and cadres in Darul Amin Village. The results of the activity are that the community has made use of online health services, the community has promoted their herbal ingredients through www.excellentcare.com, cadres and health workers have earned income from homecare services.

Abstract Background: to analyze the process of development of the caregiver in a home care program, focusing on the attributes, feeling and needs of those individuals. Methods: regarding our method, we made use of a qualitative research, which has been carried out at the Home Care Service of Belo Horizonte. The data has been collected through interviews performed with all the 7 caregivers and 6 healthcare practitioners. All the info has been analyzed under the scope of the the discourse analysis research method. Results: the caregiver acts with his/her heart, he/she listens and looks very carefully and attentively. Care is characterized by an intense and permanent process of conflict between the duty due to the assingned functions and the moral desisre of taking care of other human beings. Those two dimensions can be expressed sometimes by affection, gratitude and solidarity, but, in other times, it is guided by duty, resulting in work overload and concerns that make the caregiver unique in his/her moral condition. Conclusion: the person in charge of being a caregiver finds himself/herself surrounded by feelings and responsibilities, in a heavy load of relationships that are shaped by the subjective and subjectfying experiences, which makes critical the implementation of supportive social networks to aid the caregivers. keywords: Caregiver; Home Care Services; Home Care.

RESUMOObjetivos: identificar características dos serviços de atenção domiciliar brasileiros. Método: revisão integrativa, que buscou artigos científicos publicados entre janeiro de 2012 a maio de 2017, nas bases de BDENF, LILACS e na biblioteca virtual SciELO. A avaliação dos estudos foi feita pela leitura dos resumos dos artigos que respondessem como é o cenário da assistência domiciliar no Brasil e os resultados foram apresentados em forma de tabela e sínteses. Resultados: a amostra foi composta por nove artigos dos quais foram possíveis identificar três categorias temáticas – “os grupos populacionais prevalentes na assistência domiciliar”, “o perfil dos profissionais e dos cuidadores familiares inseridos nesse contexto” e “características do serviço de atenção domiciliar”. Conclusão: a atenção domiciliar é um eficaz meio de desospitalização e redução de custo e humanização do cuidado, porém, de acesso escasso aos usuários da rede pública de saúde. Objetivo: caracterizar clínica e epidemiologicamente os pacientes atendidos por um programa público de atenção domiciliar na cidade de Montes Claros, Minas Gerais, Brasil. Métodos: Estudo transversal e exploratório, realizado entre junho de 2017 e janeiro de 2018, com 131 pacientes cadastrados e atendidos pelo Serviço de Atenção Domiciliar - Melhor em Casa. Para o levantamento dos dados utilizou-se um instrumento contendo variáveis sociodemográficas, clínicas e funcionais. Os dados foram analisados por estatística descritiva. Resultados: A maioria dos pacientes eram idosos (67,9%), do sexo feminino (55%), pardos (47,3%), casados (31,3%) e/ou solteiros (31,3%), restritos ao leito (71%) e alimentavam-se via oral (68,7%). As doenças vasculares (42%) foram as mais prevalentes, 45,8% dos pacientes apresentavam lesão por pressão e o atendimento domiciliar foi realizado principalmente pelo enfermeiro (83,2%) e médico (82,4%). Home care services at midnight many residents complained and there is no distribution of home care officers at the Bara-Baraya Public Health Center in Makassar. The focus of research in this study is the evaluation of the implementation of home care programs at the Bara-Baraya Public Health Center in Makassar City. This study aims to evaluate the implementation of home care programs at the Makassar City of Bara Baraya Health Center. The approach used in this study is a qualitative approach that describes and observes deeply the evaluation of the implementation of home care programs at the Makassar City of Bara Baraya Health Center. This research also uses applied research design. The data source in this study was taken from four home care officers at the Bara-Baraya Public Health Center in Makassar City and four people who use home care services. The results showed that the input sub-variables can be quite good. Medical devices such as thermometers, tensimeters, and stethoscopes function properly and are suitable for use in home care health services. Then operational funds such as staff salaries are paid according to the number of visits to the community. The doctor for Rp. 100,000 / visit and nurses 75,000 / visit. But in the aspect of home care officers is still inadequate. Home care workers have a dual role between serving the community at the puskesmas, as well as serving the community at the residence. In the process sub-variables, it can be quite good. In the call center aspect, people can easily call the call center because this service is toll free. Home visits and observations have been carried out carefully. But the division of duties of home care officers is not optimal. In the output sub-variable, it can be said to be good because the public can directly benefit from home care services.
