ABSTRACTIntroduction: Charm needles are metal-based pin inserted within the soft tissue. Orofacial pain in charm needles wearers could be caused by trauma and pain in full denture patients, foreign body neuralgia, and potential damage to vital organs and penetration of neurovascular structures. This case report was aimed to discuss orofacial pain in a woman with charm needles that has been occurred in 3 years. Case report: A 38-year woman, complaining of spontaneous stabbing and throbbing pain on the right side of the face for three years prior. The patient had examinations from the neurologist, otolaryngologist, and general dentist, but the complaint remained. Clinical examinations at the Oral Medicine Department showed a normal and pale mucosa which recognised as an overextended denture. Radiographic findings showed the remaining root of tooth #14 and a corpus alienum over the maxillary and mandibular region. Root extraction and adjustment of denture were performed. One year later, the patient came back with a complaint of reduced pain but sometimes emerged from the point of the maxillary corpus alienum (charm needles), then spread to the right lip and cheek. The remaining root of tooth #14 and the charm needles in the buccal region #16 were considered as foreign objects that can trigger neuralgia (foreign body neuralgia). Root extraction could reduce the pain, but the charm needles was not extracted. Existing pain was treated with anticonvulsants, analgesic, and multivitamins. Conclusion: Corpus alienum in the form of charm needles in the patient’s radiographic finding can be one source of orofacial pain. Comprehensive treatment from different field of disciplines is needed in the orofacial pain case. Keywords: Pain, orofacial, charmed needles.

Cystoid Macular Oedema or CME is a painless disorder that affects the central retina or macula. It refers to the accumulation of fluid in the outer plexiform and inner nuclear layer of the retina with the formation of a fluid-filled cyst. The primary symptom of macular oedema is blurry or wavy vision near or in the centre of your field of vision. Materials and Methods: A male patient of 48 yrs. presented in Shalakya OPD of GAMC Bengaluru with symp- toms of diminished vision in his right eye for one year. The patient was diagnosed with cystoid macular oedema in the right eye for which he was given photocoagulation therapy but did not find much relief. So, he approached our OPD. After thorough examination patient was started with Ayurvedic Medicines. Result: The subject showed marked improvement both subjectively and objectively. Discussion: Oedema which is the terminus of the pathology in this condition has to be understood as Ekanga Shopha. Though Kapha is the predominant dosha involved in forming Shopha here, the lakshanas manifesting are that of Vataja Timira. So, in this case study, Kapha Vata Hara followed by Shopha Hara line of treatment is adopted. Keywords: Cystoid macular oedema, Shopha, Vataja Timira, Nasya, Punarnavadi Kashaya.

We had a sixty years old male patient, who had one year history of foreign body sensation in throat and the history of odynophagia for the last ten days. The NCCT neck showed 3.08×2.28 cm homogenous calcified mass in left tonsillar fossa. The stone was removed and tonsillectomy was performed. Giant tonsillolith is a rare clinical entity. As per available literature, 54 cases of giant tonsilloliths have been reported and to the best of our knowledge, this is one of the largest tonsillolith in the world till date.
Keywords: Tonsillolith, Oral cavity

Abstract BackgroundUnclear retroperitoneal tumors still impose major challenges for clinicians. Tumors can originate primarily from retroperitoneal tissue or secondarily invade into the retroperitoneal space. While benign lesions also occur, malignant tumors are far more common. Clinical presentation often depends on replacement or invasion of other organs and is therefore highly variable. The heterogeneous tumor composition makes a definitive preoperative diagnosis difficult. Nevertheless, surgical resection is the gold standard for treatment but often proves challenging due to frequent involvement of large retroperitoneal vessels.Case presentationWe present a case report of a 70-year old woman diagnosed with a large, unclear retroperitoneal tumor. Initial clinical symptoms were increasing dyspnea and dysphagia in our clinic. Gastroenterologic and cardiologic workup was unremarkable. Computed Tomography (CT) revealed a large retroperitoneal mass in the right upper abdomen with severe displacement of the inferior vena cava and renal veins. Without signs of irresectability, the patient was scheduled for tumor resection. The procedure was challenging due to the vessel involvement and large blood pressure alterations during tumor mobilization. The post-op pathologic workup then revealed the rare finding of a completely resected paraganglioma. The post-surgical course was uneventful. One year after diagnosis, the patient is relapse-free.CconclusionThis case impressively shows the challenges of retroperitoneal tumors and the importance of interdisciplinary work in these cases.

Objective: Isolated salpingeal torsion (IST) is a rare cause of acute abdomen in women of the reproductive age group. The lack of pathognomonic clinical, laboratory or radiographic findings makes early diagnosis a challenge. We describe a case of IST in a 13-year-old who suffered from a repeat torsion following initial conservative management. Case Report: A 13-year-old girl presented with acute right lower quadrant abdominal pain of insidious onset. Her past medical history was non-contributory and her initial workup was unremarkable. Exploratory laparoscopy subsequently revealed isolated torsion of the right fallopian tube which was detorted. She presented 10 months later with similar complaints and further workup demonstrated right adnexal torsion which was confirmed by laparoscopy; salpingo-oophorectomy was necessary due to gangrenous necrosis. Conclusion: Conservative management of fallopian tube torsion confers the advantage of fertility preservation but the risk of repeat torsion remains clinically significant.

This case report is about a seven-year-old male neutered European Shorthair cat infected by Dirofilaria immitis as the first reported autochthonous Dirofilaria immitis infection in Austria. There was no history of periods abroad. Echocardiography showed suspected D. immitis in the right cardiac chamber with increased pulmonary pressure and ascites. Surgical removal of the heartworms was performed. Twenty adult heartworms were removed by transvenous jugular approach under general anesthesia and stored in 4% formalin. Five out of 20 specimens were examined via light and stereomicroscopy and feline heartworm infection was confirmed. Amplification of a 203 bp or 724 bp fragment of the cytochrome c oxidase subunit I gene was unsuccessful. After surgery the cat developed acute renal failure but recovered quickly. One year later, the cat underwent a control examination including echocardiography and blood work. There were no more D. immitis detectable at echocardiography. Lung pressure was mildly increased. Complete blood count and creatinine were unremarkable. The Knott’s test and Dirofilaria-Antigen-test produced negative results. The cat did not show any clinical signs during the follow-up period. The aim of this case report is to highlight the growing risk of acquiring infection with D. immitis not only for Austrian dogs, but also for cats. This case report represents the first report of autochthonous D. immitis infection in Austria. Moreover, even if the prognosis in cats with caval syndrome due to feline heartworm disease is guarded to poor, surgical removal of the filariae can be a successful treatment option.

Dental avulsion is the complete displacement of a tooth from the alveolar socket due to trauma. Reimplantation is the procedure by which an avulsed tooth is replaced. The prognosis of reimplantation depends on several factors, the most important of which are the length of time that elapses between the trauma and reimplantation and how the tooth is stored. The most preferable management procedure for an avulsion is immediate reimplantation within 20-30 min after injury or preservation in an appropriate storage medium until the procedure. If the tooth cannot be immediately replanted, it should be stored in Hank’s Balanced Salt Solution, milk, saliva, or saline solution. Milk is readily available and can protect periodontal ligament cells. In this case report, a 30-year-old male patient suffered a sports injury that resulted in avulsion of the right maxillary incisor. The avulsed tooth was stored in milk for 5 h from avulsion until reimplantation. This case report details reimplantation of an avulsed tooth.

A two-and-a-half-year-old giant lop-eared rabbit, weighing 5.1 kg, presented with a one-month history of intermittent right hind limb lameness. The limb locked in extension during hopping. On examination, a grade-2 medial patellar luxation of the right hind was diagnosed, with associated stifle joint swelling. Radiographic findings of the right stifle comprised periarticular osteophyte formation consistent with mild degenerative joint disease and joint effusion. Surgical correction involving right trochlear wedge recession sulcoplasty and lateral imbrication was carried out to stabilise the patella in the trochlear groove. The right hind limb lameness resolved, and the patella was stable at a 6-month postoperative examination. One year postoperatively, the right patella was luxating again concurrent with bilateral stifle effusions. Euthanasia was performed twenty months after surgery due to recurrent lameness in the right hind limb.
