Relevance. The available unique experience of elimination of medical and sanitary consequences of emergency situations of natural, technogenic and social nature proves that the effectiveness of rendering medical care within or on the border of emergency area depends on timely arrival and actions of non-staff healthcare professionals of the All-Russian service for disaster medicine – crews for specialized medical care, physician-and-nurse crews. Directly during training and practical activities non-staff healthcare professionals face problematic issues affecting their readiness and quick response.Intention. To define the most significant problematic issues influencing work of non-staff healthcare professionals of the Service for disaster medicine of the Ministry of Defense of the Russian Federation on elimination of medical and sanitary consequences of emergency situations.Methodology. Content analysis of the facts and trends in the documents regulating activities of non-staff healthcare professionals of emergency formations was performed. Non-staff physicians were surveyed on problematic issues of training, practical activities and professional competence and importance of these issues ranked: “not important” – 1, “important” – 2, “very important” – 3 with assigning respective points.Results and Discussion. In the military-medical organizations of the central subordination, including the Kirov Military Medical Academy, emergency formations of Service for disaster medicine of the Ministry of Defense of the Russian Federation exist to strengthen (special-purpose) medical groups and medical institutions responsible for admitting casualties from emergency areas. An “average portrait” of a non-staff healthcare professional was as follows: male surgeon of 38.7±1.1 after a clinical internship, with an academic degree and experience in subject domain of 13.4 years. He has served (worked) at the Kirov Military Medical Academy and carried out functional duties on a constant basis, with experience more than two years as a part of physician-nurse or specialized medical care (thoracoabdominal) crew, with sufficient experience in elimination of medical and sanitary consequences of emergencies. At the same time during training and practical activities he faces a number of issues, such as labor relations and quality assessment of performance during elimination of medical and sanitary consequences of emergency situations, unsatisfactory resource and organizational support and also low readiness of all the non-staff members of the crew.Conclusion. Besides problematic issues of labor relations in emergency formations, the factors influencing training, performance and professional competence of non-staff healthcare professionals were defined, i.e. special equipment for medical evacuation (sanitary aviation); unsatisfactory facilities intended for elimination of medical and sanitary consequences of emergencies; medical equipment in emergency areas. As a result of the analysis, all the problematic issues were limited to 3 fundamental internal (hidden) factors influencing training, performance and professional competence of healthcare professionals at the Kirov Military Medical Academy: “Basic resources” (22.05%), “Shortcomings in daily activities” (11.9%), “Readiness for actions” (10.4%). To minimize the specified problematic issues, various (alternative) options are suggested.

According to the State program of the Russian Federation «Health care development» till 2020 approved by the decree of the Russian Federation government dated 24.12.2012 N 2511-p, the principle of the cluster approach became the one of the directions of informatization of healthcare. Subsystem perinatal cluster is the continuous monitoring of pregnant women. Its main advantage is the formation of a unified information space of the hospitals in the region that allows you to reliably group out pregnant females according to the risk of complications of pregnancy and childbirth, to determine the routing for all the patients. The goal of the system is a significant improvement of the assistance within a cluster (group), the formation of a standardized approach to the medical care, management of emergency situations in the occurrence of life-threatening conditions, the reduction of maternal and perinatal mortality rate. Computerization of medical care in obstetrics was implemented in the automated system «Program monitoring for pregnant females».

The article presents the results of 25-old activity of the head agency of the Service for Disaster Medicine of the Ministry of Health of the Russian Federation – All Russian Centre for Disaster Medicine «Zaschita» (ARCDM «Zaschita») of the Ministry of Health of the Russian Federation in the sphere of medical support of population in case of radiation accidents. The Service is a functional subsystem of the Unified State system of prevention and liquidation of emergency situations, intended for elimination of medical and sanitary consequences of emergency situation. In Russia, it is legally established that in case of radiation accident the Federal Medical Biological Agency of Russia (the FMBA of Russia) is responsible for radiation safety of the population living in the NPP surveillance zone (approximately 25 km). Responsibility for the radiation safety of the rest of the population living outside the surveillance zone is assigned to the Service for disaster medicine. In accordance with the current document “Model content of the protection plan of the population in case of an accident at the radiation facility” developed by the Ministry of Emergency Situations, mandatory protective measures are provided in the territory that radius is 25 km around the radiation object (planning zones of preventive and emergency measures). Early planning in the restrictive planning zone is not envisaged, and protective measures outside the 100-kilometer zone are considered inappropriate. The article presents the concept developed by the specialists of ARCDM «Zaschita» for drafting protection of the subjects of the Russian Federation for the nuclear power plants operating on the territory of Russia. The concept is based on experience of consequences of the Chernobyl NPP accident. The concept argues that the outer boundary of the planning area for protective measures around nuclear power plants should have an outer radius of 1000 km. Radius of the emergency planning area is 100 km. The problems of preparedness for radiation safety of population support in case of radiation accidents are discussed. They are: the need for elaboration of regulatory and normative documents of the federal level on obligatory advance planning of protective measures against the possibility of radiation accidents on radiation-hazardous objects from the nuclear power plants on the territory of Russia for the population, not supervised by the FMBA of Russia (living both in the observation zone and abroad) and regulating the need and procedure of planning, organization and carrying out in case of necessity iodine prophylaxis for the population on territories up to 1000 km from operating NPPs of Russia. Same approaches to the solution of the considered problems are suggested.

Relevance. In accordance with the Decree N 254 of the President of the Russian Federation dated June 06, 2019 “About strategy of the public health development in the Russian Federation up to the year 2025”, one of the primary challenges of public health is providing access to medical care for every citizen irrespective of his/her location in the country. Patients with life-threatening acute conditions should get emergency medical care within so-called “golden hour”, with selecting optimal routes to the appropriate specialized medical centres as soon as possible. In Russia, this can be realized only via air medical evacuation.Intention. To assess an existing routing system based on the aero-medical evacuation in the regions of the Russian Federation, both under emergency and routine conditions.Methodology. Reports of the Territorial Centres for Disaster Medicine and health authorities of the entities of the Russian Federation were studied.Results and Analysis. Standard locations of the capitals of the Russian Federation entities are described, along with need to cooperate with adjacent regions for evacuating patients both routinely and in case of emergencies. Trends and options of air medical transportation are prioritized to minimize risks of health deterioration in patients and casualties. The study showed increase in the number of patients evacuated via air transportation. Control over routing during medical evacuation of injured in emergencies can reduce the mortality rate due to transporting them to specialized medical centers.Conclusion. Aero-medical transport is extremely demanded for evacuating the injured in emergencies to interregional and federal medical centres.

The article is devoted to the analysis of medico-social factors that determine the outlook Image of the system of medical care for victims in emergency situations in the Arctic Zone of the Russian Federation. The social and economic development of the Russian Arctic zone is a strategic priority of the Russian Federation’s state policy in the Arctic. The goals of Russia’s state policy in the Arctic are to expand the resource base of the Arctic zone, to support the necessary combat potential of groups of troops of the Armed forces of the Russian Federation in the region, to preserve the natural environment, to ensure an adequate level of fundamental and applied scientific research, etc. Strategic priorities of the state policy of the Russian Federation in the Arctic are determined to increase the efforts of the Arctic States in creating a unified regional system of search and rescue, as well as prevention of man-made disasters and elimination of their consequences, including coordination of rescue forces; improving the quality of life of indigenous people and social conditions of economic activity in the Arctic. The main areas of development of the Russian Arctic include hydrocarbon production, fisheries, improvement of transport infrastructure and tourism. Three directions of socio-economic transformation of the Arctic zone of Russia, in the near future, can affect the formation of the appearance of the system of medical care for victims of emergencies: spatial expansion and increase of the economic potential of the Arctic zone of Russia; orientation to the concept of Trinity « innovative economy of energy efficiency sustainable development»; implementation of the concept of the formation in the Arctic eight reference zones of socio-economic development, of which the Kola, Nenets and Yamalo-Nenets reference zones are defined as pilot.

The article provides an overview and analysis of the regulatory legal acts of the Russian Federation that regulate the possibility of using video recording in the work of mobile ambulance and disaster medicine teams based on the example of our own experience of Antonius Medvision —  Ambulance LLC, an organization of a non-governmental healthcare system that provides emergency medical care outside a medical organization in the Kaluga and Moscow regions.

Relevance. Currently, the solution to the problem of medical support for troops (forces) and the population during the liquidation of the health consequences of emergencies by forces and means of the medical service of the Armed Forces of the Russian Federation does not lose its relevance.Intention. The system-morphological method to create a multidimensional morphological matrix with the corresponding morphological features and develop solutions to the problem of medical support in responding to the health consequences of emergencies.Methodology. The methodology of system-morphological analysis is presented, which allows creating alternative solutions. Variants of medical support for troops (forces) and the population were developed during the liquidation of the health consequences of emergency situations of a natural, technogenic and social nature: “Departmental”, “Interdepartmental”, “International”. In relation to each option, 9 indicators (criteria) are proposed: normative legal regulation; system controllability; complexity of organizing the system; resource (material) cost; staffing; the adequacy of medical care; efficiency; experience and innovation; autonomy of action. To achieve the goal of the study, the original MPRIORITY 1.0 dialog system (MY PRIORITY) was used which implements a method for analyzing hierarchies through repetitions (iterations).Results and Discussion. The results characterize each of the considered alternative options with an appropriate weight priority, and their preference is determined. During the first iteration, priorities were identified for a system of indicators and criteria that are characteristic for achieving a result with an emphasis on normative legal regulation of activities with its organizational structure and management system. In the second one, the priorities for the system of indicators and criteria were changed with a focus on the organization of medical care with the existing system of material and technical support, medical supply, as well as the proper level of training of medical personnel.Conclusion Proposals have been developed for the medical service of the Armed Forces of the Russian Federation, in particular for the Service for Disaster Medicine of the Ministry of Defense of Russia, on the organization of a medical support system for troops (forces) and the population during mitigation of the health consequences of emergencies.

The purpose of the study is to analyze the current procedure for organizing medical rehabilitation of participants in the elimination of the consequences of emergency situations (ES) and to develop the main conceptual issues of the medical rehabilitation of rescuers in the environment of emergency situations. Materials and methods of research. The study of the state of the system of medical rehabilitation of rescuers in the course of liquidation of consequences of ES began with the study of: the existing procedure for the medical rehabilitation of participants of ES consequences liquidation and rescuers, regulated by the current regulatory legal documents of the Russian Federation; methodological and training manuals in the field of healthcare; official documents of the All-Russian Service for Disaster Medicine (VSMC) on the organization of medical rehabilitation of those involved in the operations and rescuers. The results of the study and their analysis. The state, problems and methods of improving the system of medical rehabilitation of rescuers and participants in the elimination of the consequences of emergencies are considered. The main conceptual provisions on the medical rehabilitation of rescuers in the emergency zone are presented.

In June 1993, the All-Army Scientific and Practical Conference of Endocrinologists was held in St. Petersburg, dedicated to the memory of Professor D. Ya. Shurygin, Honored Scientist of the Russian Federation, who for a long time headed the endocrinological service of the Armed Forces. The conference was organized by the medical service of the Armed Forces of Russia and the Military Medical Academy.The conference was attended by the chief therapist of the Ministry of Defense of the Russian Federation, corresponding member. RAMS V.T. Ivashkin, endocrinologists of the Military Medical Academy and central military hospitals, a number of leading scientists and endocrinologists of the country from Moscow, St. Petersburg, Saratov and Yekaterinburg, as well as some specialists from neighboring countries. The conference lasted 2 days and consisted of 3 plenary sessions, at which 22 reports were heard. Other participants who submitted abstracts of messages (more than 130 people) could present materials in the form of poster presentations. Conference proceedings published.
